Self-help Group Participation and Empowerment in Hong Kong
This paper reports on the first comprehensive study of self-help groups in Hong Kong. Initial findings from the quantative and qualitative data suggest that self-help group participation has an impact on intrapersonal, interpersonal and community/political empowerment. Based on existing data, this study has resulted in the development of a hypothetical model encompassing the interrelationships among self-help group participation, social support, social learning, leadership and empowerment, for testing in future research
In the Service of Market Socialism: The Quest for a Welfare Model in China
The socialist market economy in China has brought about impressive economic growth. It has also resulted in serious social problems such as unemployment, deteriorating family relations, prostitution, and poverty among the disadvantaged groups. This paper discusses a welfare model proposed by the Chinese Government to contain the destabilizing effects of the social problems and to serve the newly adopted market socialism. The authors argue that although this welfare model is still very much residual by western standards, it is a big step forward in building a modern Chinese welfare system
Validation and application of health utilities index in Chinese subjects with down syndrome
Objectives:
The objectives of the study were (1) to validate the Chinese version of Health Utilities Index (HUI-Ch); (2) to examine the Health-related Quality of Life (HRQoL) of Chinese subjects with Down syndrome (DS); and (3) to study the impact of chronic health conditions on HRQoL of Chinese with DS.
Methods:
The multiple choice questionnaire for scoring Health Utilities Index Mark 2 (HUI2) and Health Utilities Index Mark 3 (HUI3) was translated and validated. In addition to the HRQoL scores from HUI2 and HUI3, proxy-data on socio-demographics, and 10 common chronic health conditions for people with DS were collected and analyzed. Data analysis involves multiple imputation and multiple regression analysis to predict variations in HRQoL in relation to different factors. Lastly, a gradient interval was constructed on the number of chronic health conditions in relation to HRQoL.
Results:
HUI-Ch was validated according to standard guidelines. People with DS were found to have a lower HRQoL as compared to the general population, with the majority categorized as moderate or severe on the scale. Behavioral and hearing problems on HUI2, and hearing problems on HUI3 were found to be statistically significant predictors of a lower HRQoL score. A significant gradient relationship existed showing when the number of health problems increased, the HRQoL scores decreased.
Conclusions:
HUI-Ch is a valid instrument to assess HRQoL. It can have broad application in Chinese subjects with DS including the study of the impact of different chronic health conditions on their quality of life. The quantifiable nature of HUI-Ch will facilitate longitudinal study on the well-being of subjects with DS and evaluation of effectiveness of intervention programs in the near future
The development and validation of a scoring tool to predict the operative duration of elective laparoscopic cholecystectomy
Background: The ability to accurately predict operative duration has the potential to optimise theatre efficiency and utilisation, thus reducing costs and increasing staff and patient satisfaction. With laparoscopic cholecystectomy being one of the most commonly performed procedures worldwide, a tool to predict operative duration could be extremely beneficial to healthcare organisations.
Methods: Data collected from the CholeS study on patients undergoing cholecystectomy in UK and Irish hospitals between 04/2014 and 05/2014 were used to study operative duration. A multivariable binary logistic regression model was produced in order to identify significant independent predictors of long (> 90 min) operations. The resulting model was converted to a risk score, which was subsequently validated on second cohort of patients using ROC curves.
Results: After exclusions, data were available for 7227 patients in the derivation (CholeS) cohort. The median operative duration was 60 min (interquartile range 45–85), with 17.7% of operations lasting longer than 90 min. Ten factors were found to be significant independent predictors of operative durations > 90 min, including ASA, age, previous surgical admissions, BMI, gallbladder wall thickness and CBD diameter. A risk score was then produced from these factors, and applied to a cohort of 2405 patients from a tertiary centre for external validation. This returned an area under the ROC curve of 0.708 (SE = 0.013, p 90 min increasing more than eightfold from 5.1 to 41.8% in the extremes of the score.
Conclusion: The scoring tool produced in this study was found to be significantly predictive of long operative durations on validation in an external cohort. As such, the tool may have the potential to enable organisations to better organise theatre lists and deliver greater efficiencies in care
Charging Effect in Au Nanoparticle Memory Device with Biomolecule Binding Mechanism
Organic memory device having gold nanoparticle (Au NPs) has been introduced in the structure of metal-pentacene-insulator-silicon (MPIS) capacitor device, where the Au NPs layer was formed by a new bonding method. Biomolecule binding mechanism between streptavidin and biotin was used as a strong binding method for the formation of monolayered Au NPs on polymeric dielectric of poly vinyl alcohol (PVA). The self-assembled Au NPs was functioned to show storages of charge in the MPIS device. The binding by streptavidin and biotin was confirmed by AFM and UV-VIS. The UV-VIS absorption of the Au NPs was varied at 515 nm and 525 nm depending on the coating of streptavidin. The AFM image showed no formation of multi-stacked layers of the streptavidin-capped Au NPs on biotin-NHS layer. Capacitance voltage (C-V) performance of the memory device was measured to investigate the charging effect from Au NPs. In addition, charge retention by the Au NPs storage was tested to show 10,000 s in the C-V curve
Electrical charging of Au nanoparticles embedded by streptavidin-biotin biomolecular binding in organic memory device
In this study, electrical charging phenomena in an organic memory structure using Au nanoparticles (NPs) conjugated with a specific binding mechanism were demonstrated. Monolayer of streptavidin-passivated Au NPs was incorporated on biotin-coated SiO(2) in structure of metal-pentacene-insulator-silicon (MPIS) device. Clockwise and counter-clockwise capacitance-voltage (C-V) hysteresis loops were measured depending on the oxide thickness. For 30 nm, a clockwise C-V hysteresis having memory window of 0.68 V was obtained under (+/-)12 V sweep range, while a counter-clockwise C-V hysteresis having memory window of 6.47 V was obtained under (+/-)7 V sweep range for 10 nm thick oxide.
