    Contralateral delayed endolymphatic hydrops: clinical features and long term outcome

    BACKGROUND: Contralateral delayed endolymphatic hydrops (CDEH) is a clinical entity characterized by fluctuating low frequency hearing loss and/or vertigo, mimicking Ménière’s disease (MD), that manifests after the appearance of severe non-hydropic hearing loss (NHHL) at the other ear. OBJECTIVES: to describe the clinical features and the course of 57 patients affected by CDEH. METHOD: this is a retrospective study; 57 patients affected by CDEH, out of 1065 patients seen in the same period and affected by MD, were subjected to otoscopy, PTA threshold evaluation, impedance testing, ABR, research of positioning nystagmus, vestibular function evaluated by means of bithermal caloric test under video-oculographic, and MRI with gadolinium. RESULTS: the CDEH was definite in 24 cases (42%), probable in 2 (4%) and possible in 31 (54%). The mean PTA threshold at the hydropic ear was 41 dB. At the last follow-up, 40 patients (70%) did not report vertigo or fluctuating hearing loss. Among the 17 patients who still reported symptomatology, 11 (64%) were affected by fluctuating hearing loss alone, 4 (23%) reported a subjective worsening of hearing loss and 2 (12%) an acute vertigo crisis. CONCLUSIONS: contralateral delayed endolymphatic hydrops is a relatively rare form of Ménière disease that manifests more frequently as a definite form or with fluctuating low-frequency hearing loss. The prognosis at a long term follow-up is relatively good in terms of vertigo resolution. Contralateral delayed endolymphatic hydrops rarely determines a severe hearing loss in the better ear

    Spectral multiplexing using stacked volume-phase holographic gratings - I

    Many focal-reducer spectrographs, currently available at state-of-the-art telescopes facilities, would benefit from a simple refurbishing that could increase both the resolution and spectral range in order to cope with the progressively challenging scientific requirements, but, in order to make this update appealing, it should minimize the changes in the existing structure of the instrument. In the past, many authors proposed solutions based on stacking subsequently layers of dispersive elements and recording multiple spectra in one shot (multiplexing). Although this idea is promising, it brings several drawbacks and complexities that prevent the straightforward integration of such a device in a spectrograph. Fortunately, nowadays, the situation has changed dramatically, thanks to the successful experience achieved through photopolymeric holographic films, used to fabricate common volume-phase holographic gratings (VPHGs). Thanks to the various advantages made available by these materials in this context, we propose an innovative solution to design a stacked multiplexed VPHG that is able to secure efficiently different spectra in a single shot. This allows us to increase resolution and spectral range enabling astronomers to greatly economize their awarded time at the telescope. In this paper, we demonstrate the applicability of our solution, both in terms of expected performance and feasibility, supposing the upgrade of the Gran Telescopio CANARIAS (GTC) Optical System for Imaging and low-Intermediate-Resolution Integrated Spectroscopy (OSIRIS)

    Stress-Induced Changes of Hippocampal NMDA Receptors: Modulation by Duloxetine Treatment

    It is now well established that the glutamatergic system contributes to the pathophysiology of depression. Exposure to stress, a major precipitating factor for depression, enhances glutamate release that can contribute to structural abnormalities observed in the brain of depressed subjects. On the other hand, it has been demonstrated that NMDA antagonists, like ketamine, exert an antidepressant effect at preclinical and clinical levels. On these bases, the purpose of our study was to investigate whether chronic mild stress is associated with specific alterations of the NMDA receptor complex, in adult rats, and to establish whether concomitant antidepressant treatment could normalize such deficits. We found that chronic stress increases the expression of the obligatory GluN1 subunit, as well as of the accessory subunits GluN2A and GluN2B at transcriptional and translational levels, particularly in the ventral hippocampus. Concomitant treatment with the antidepressant duloxetine was able to normalize the increase of glutamatergic receptor subunit expression, and correct the changes in receptor phosphorylation produced by stress exposure. Our data suggest that prolonged stress, a condition that has etiologic relevance for depression, may enhance glutamate activity through post-synaptic mechanisms, by regulating NMDA receptors, and that antidepressants may in part normalize such changes. Our results provide support to the notion that antidepressants may exert their activity in the long-term also via modulation of the glutamatergic synapse

    Bendamustine plus rituximab is an effective first-line treatment in hairy cell leukemia variant: A report of three cases

    Hairy cell leukemia variant (HCLv) is a chronic lymphoproliferative disorder classified as a provisional entity in the 2016 WHO Classification of Lymphoid Tumors. HCLv is characterized by unfavorable prognosis, low complete remission rates and limited disease control following classical hairy cell leukemia-based regimens. In this study, we report 3 cases of elderly patients with treatment-naive, TP53 un-mutated HCLv, who were effectively treated with four cycles of bendamustine plus rituximab. The regimen was completed in all the patients with acceptable toxicity. All patients achieved a complete clinical response with no evidence of residual disease at bone marrow biopsy and flow-cytometry examination. After a median follow-up of 19 months, the 3 subjects are still in complete remission. In this work, bendamustine plus rituximab proved to be an effective and feasible first-line treatment strategy for elderly patients with TP53 un-mutated HCLv

    An adaptive interferometer for optical testing

    Interferometry is a well-established technique to test optical elements. However, its use is challenging in the case of free-form and aspheric elements, due to the lack of the reference optics. The proposed idea concerns the development of a versatile interferometer, where its reference arm is equipped with a reprogrammable Computer Generated Hologram. This principle takes advantage from our study on photochromic materials for optical applications, which shows a strong and reversible modulation of transparency in the visible region. The encoding of the desired hologram can be done off-line, or directly into the interferometer, and different patterns may be realized sequentially after the erasing of the previous hologram.     BDNF Val66Met polymorphism and protein levels in Amniotic Fluid

    Brain-Derived Neurotrophic Factor (BDNF) is a neurotrophin which plays survival- and growth-promoting activity in neuronal cells and it is involved in cellular plasticity mechanisms as it controls activity dependent synaptic transmission. A functional polymorphism (Val66Met) in the pro-region of BDNF, which affects the intracellular trafficking of proBDNF has been associated with memory and cognitive deficits as well as to an increased susceptibility for several psychiatric disorders especially those with a neurodevelopmental origin. To date, no study has evaluated the influence of the Val66Met polymorphism on BDNF levels in a peripheral system that may reflect fetal neurodevelopment. Therefore we investigated in amniotic fluids (AF) obtained from 139 healthy women during 15-17 week of pregnancy, BDNF protein levels in correlation with the Val66Met polymorphism
