Use of laboratory-developed assays in global HIV-1 treatment-monitoring and research
There has been a surge in the emergence of HIV-1 drug resistance in Low and Middle-Income Countries (LMICs) due to poor drug-adherence and limited access to viral load testing, the current standard for treatment-monitoring. It is estimated that only 75% of people living with HIV (PLWH) worldwide have access to viral load testing. In LMICs, this figure is below 50%. In a recent WHO survey in mostly LMICs, 21 out of 30 countries surveyed found HIV-1 first-line pre-treatment drug resistance in over 10% of study participants. In the worst-affected regions, up to 68% of infants born to HIV-1 positive mothers were found to harbour first-line HIV-1 treatment resistance. This is a huge public health concern. Greater access to treatment-monitoring is required in LMICs if the UNAIDS "third 95" targets are to be achieved by 2030. Here, we review the current challenges of viral load testing and present the case for greater utilization of Laboratory-based assays that quantify intracellular HIV-1 RNA and/or DNA to provide broader worldwide access to HIV-1 surveillance, drug-resistance monitoring, and cure-research
Corrosion and Electrochemical Properties of Laser-Shock-Peening-Treated Stainless Steel AISI 304L in VVER Primary Water Environment
Laser Shock Peening (LSP) is a surface treatment technique for metallic materials. It induces plastic deformation at the surface of up to around 1 mm in depth. This process introduces residual stresses that lead to strain hardening, and potentially improvements in fatigue, stress corrosion cracking (SCC) and general corrosion behaviour in many, but not all, corrosive media. In this paper, two specimens made of AISI 304L stainless steel, one LSP-treated and one un-treated, were tested at 280 °C and 8 MPa in VVER (or PWR) primary circuit water chemistry using in situ Electrochemical Impedance Spectroscopy (EIS). This experiment serves to qualify the influence of LSP on the changes in corrosion behaviour in high-temperature, high-density water. The residual stress (RS) measurement of the surface showed a compression RS. Before LSP treatment, RS at the surface was 52.2 MPa in the rolling direction 0°RD and 10.42 MPa in the transverse rolling direction 90°RD. After the treatment, surface RS was −175.27 MPa and −183.51 MPa for Scan and TScan directions, respectively. The effect of compressive RS at the surface was studied and showed an increase in corrosion rate. The analysis of oxide layer by SEM revealed differences between LSP-treated and untreated AISI 304L specimens and their connection to corrosion rates
Rural palliative care in North India: Rapid evaluation of a program using a realist mixed method approach
Context: Palliative care has not developed widely in rural North India. Since 2010, the Emmanuel Hospitals Association (EHA) has been developing a model of palliative care appropriate for this setting, based on teams undertaking home visits with the backup of outpatient and inpatient services. A project to further develop the model operated from 2012 to 2015 supported by funding from the UK.
Aims: This study aims to evaluate the EHA palliative care project.
Settings and Design: Rapid evaluation method using a mixed method realist approach at the five project hospital sites. Methods: An overview of the project was obtained by analyzing project documents and key informant interviews. Questionnaire data from each hospital were collected, followed by interviews with staff, patients, and relatives and observations of home visits and other activities at each site.
Analysis: Descriptive analysis of quantitative and thematic analysis of qualitative data was undertaken. Each site was measured against the Indian Minimum Standards Tool for Palliative Care (IMSTPC).
Results: Each team followed the EHA model, with local modifications. Services were nurse led with medical support. Eighty percent of patients had cancer. Staff demonstrated good palliative care skills and patients and families appreciated the care. Most essential IMSTPC markers were achieved but morphine licenses were available to only two teams. Remarkable synergy was emerging between palliative care and community health. Hospitals planned to fund palliative care through income from surgical services. Conclusions: Excellent palliative care appropriate for rural north India is delivered through the EHA model. It could be extended to other similar sites.sch_nur24pub5356pub
Established non-union of an operatively managed trans-scaphoid perilunate fracture dislocation progressing to spontaneous union
Perilunate dislocations and fracture dislocations represent uncommon and unusual injuries that are often missed at initial presentation and diagnosed late in up to 25% of cases. Prompt open reduction, carpal stabilisation and ligamentous repair is required to reduce the risk of complications. We report a case of an established scaphoid non-union in an operatively managed perilunate fracture dislocation that spontaneously united almost 2 years after the initial injury, just before a planned revision scaphoid fixation with bone grafting. This case highlights the importance of initial clinical assessment together with appropriate radiographs and follow-up of these injuries post-operatively, especially when complications such as non-union arise

Nucleonic resonance excitations with linearly polarized photon in
In this work, an improved quark model approach to the meson
photo-production with an effective Lagrangian is presented. The {\it t}-channel
{\it natural}-parity exchange is taken into account through the Pomeron
exchange, while the {\it unnatural}-parity exchange is described by the
exchange. With a very limited number of parameters, the available experimental
data in the low energy regime can be consistently accounted for. We find that
the beam polarization observables show sensitivities to some {\it s}-channel
individual resonances in the quark model symmetry limit.
Especially, the two resonances and , which belong
to the representation , have dominant contributions
over other excited states. Concerning the essential motivation of searching for
"missing resonances" in meson photo-production, this approach provides a

Properties of and Mesons at Finite Temperature and Density as Inferred from Experiment
The mass shift, width broadening, and spectral density for the and
mesons in a heat bath of nucleons and pions are calculated using a
general formula which relates the self-energy to the real and imaginary parts
of the forward scattering amplitude. We use experimental data to saturate the
scattering amplitude at low energies with resonances and include a background
Pomeron term, while at high energies a Regge parameterization is used. The real
part obtained directly is compared with the result of a dispersion integral
over the imaginary part. The peaks of the spectral densities are little shifted
from their vacuum positions, but the widths are considerably increased due to
collisional broadening. Where possible we compare with the UrQMD model and find
quite good agreement. At normal nuclear matter density and a temperature of 150
MeV the spectral density of the meson has a width of 345 MeV, while that
for the is in the range 90--150 MeV.Comment: 21 pages revtex + 9 postscript figure
Conservation and divergence of chemical defense system in the tunicate Oikopleura dioica revealed by genome wide response to two xenobiotics
<p>Abstract</p> <p>Background</p> <p>Animals have developed extensive mechanisms of response to xenobiotic chemical attacks. Although recent genome surveys have suggested a broad conservation of the chemical defensome across metazoans, global gene expression responses to xenobiotics have not been well investigated in most invertebrates. Here, we performed genome survey for key defensome genes in <it>Oikopleura dioica </it>genome, and explored genome-wide gene expression using high density tiling arrays with over 2 million probes, in response to two model xenobiotic chemicals - the carcinogenic polycyclic aromatic hydrocarbon benzo[a]pyrene (BaP) the pharmaceutical compound Clofibrate (Clo).</p> <p>Results</p> <p><it>Oikopleura </it>genome surveys for key genes of the chemical defensome suggested a reduced repertoire. Not more than 23 cytochrome P450 (CYP) genes could be identified, and neither CYP1 family genes nor their transcriptional activator AhR was detected. These two genes were present in deuterostome ancestors. As in vertebrates, the genotoxic compound BaP induced xenobiotic biotransformation and oxidative stress responsive genes. Notable exceptions were genes of the aryl hydrocarbon receptor (AhR) signaling pathway. Clo also affected the expression of many biotransformation genes and markedly repressed genes involved in energy metabolism and muscle contraction pathways.</p> <p>Conclusions</p> <p><it>Oikopleura </it>has the smallest number of CYP genes among sequenced animal genomes and lacks the AhR signaling pathway. However it appears to have basic xenobiotic inducible biotransformation genes such as a conserved genotoxic stress response gene set. Our genome survey and expression study does not support a role of AhR signaling pathway in the chemical defense of metazoans prior to the emergence of vertebrates.</p
Observation of Pseudoscalar and Axial Vector Resonances in pi- p -> K+ K- pi0 n at 18 GeV
A new measurement of the reaction pi- p -> K+ K- pi0 n has been made at a
beam energy of 18 GeV. A partial wave analysis of the K+ K- pi0 system shows
evidence for three pseudoscalar resonances, eta(1295), eta(1416), and
eta(1485), as well as two axial vectors, f1(1285), and f1(1420). Their observed
masses, widths and decay properties are reported. No signal was observed for
C(1480), an IG J{PC} = 1+ 1{--} state previously reported in phi pi0 decay.Comment: 7 pages, 6 figs, to be submitted to Phys. Let
