Deformación plástica del antebrazo: revisión y caso clínico
La deformidad plástica de un hueso es una rara entidad, que en ocasiones pasa desapercibida. Se trata de una lesión típica del niño, con un hueso más elástico que los adultos. La mayoría de las deformidades plásticas del antebrazo se producen por caídas con el brazo en extensión, causando deformidad, dolor y una limitación de la pronosupinación. Para reducir la curvatura del hueso se precisan fuerzas contrarias a las que produjeron la lesión, mantenidas en el tiempo. En niños menores de 4 años, con enorme capacidad de remodelación, sólo grandes deformidades >20º deberían ser reducidas. En los mayores de 10 años, con escaso potencial remodelador, deben manipularse aquellas angulaciones >10-15º. La mayor complicación derivada de una mala reducción es la limitación de la pronosupinación, que en la mayoría de las ocasiones no altera la funcionalidad de la extremidad.Plastic bowing of a bone is a rare entity that some-
times goes unnoticed. It’s a typical injury in children, who
have more elastic bones than adults. Most of the plastic
bowing of the forearm take place by falls with an outstretched
arm, causing deformities, pain and a limitation of pronation
and supination. In order to reduce the curvature of the bone,
opposing forces to those that produced the injury are needed,
maintained for a long time. In children younger than 4 years,
with an enormous remodelling capacity, only great deformi-
ties >20º should be reduced. For those older than 10 years,
with little remodelling capacity, angulations >10-15º must be
manipulated. The greatest complication derived from a bad
reduction is the limitation of the pronation and supination,
which in most cases does not modify the functionality of the
extremity

Hypothalamyc hamartomas: Different ways of clinical debut. Cases report
Los hamartomas hipotalámicos son malformaciones no neoplásicas de sustancia gris compuestas por neuronas hiperplásicas. Suelen ser lesiones pequeñas localizadas en la base del cerebro, en el piso del tercer ventrículo y, generalmente, asintomáticas. Sin embargo, pueden ocurrir con alteraciones conductuales-cognitivas, crisis epilépticas y/o signos de pubertad precoz central en función de la localización en la que se encuentren. Se presentan dos pacientes de 2 años 8 meses y 7 años, con presencia de hamartomas hipotalámicos diagnosticados tras el estudio de pubertad precoz central. La paciente de menor edad presenta, además, crisis gelásticas, típicamente asociadas a hamartomas hipotalámicos. Tras los hallazgos clínicos y radiológicos, se trataron con análogos de gonadotropinas, y se observó una regresión de los signos puberales y una no progresión del tamaño de los hamartomas.
Hypothalamic hamartomas are benign tumors of gray substance composed by hyperplasic neurons. They are usually asymptomatic small masses with extensions into the third ventricular cavity. In some instances they can cause cognitive behavioral alterations, seizures and/or central precocious puberty depending on the location. Here we present two cases of central precocious puberty due to hypothalamic hamartomas at 28/12 and 7 years of age. The younger patient also presents gelastic seizures, typically associated with hypothalamic hamartomas. After the clinical and radiological findings, they started treatment with GnRH analogues and a regression of the puberty signs without progression in the hamartomas size was observed
Chemical pressure effects on the spectroscopic properties of Nd3+-doped gallium nano-garnets
[EN] Nd3+-doped RE3Ga5O12 (RE = Gd, Y, and Lu) nano-crystalline
garnets of 40-45 nm in size have been synthesized by a sol-gel method.
With the decrease of the RE atom size, the chemical pressure related to the
decreasing volumes of the GaO4 tetrahedral, GaO6 octahedral and REO8
dodecahedral units drive the nano-garnets toward a more compacted
structure, which is evidenced by the change of the vibrational phonon mode
frequencies. The chemical pressure also increases the crystal-field strength
felt by the RE3+ ions while decreases the orthorhombic distortion of the
REO8 local environment. These effects alter the absorption and emission
properties of the Nd3+ ion measured in the near-infrared luminescence range
from 0.87 to 1.43 ¿m associated with the 4
F3/2¿4
IJ (J = 9/2, 11/2, 13/2)
transitions. The 4
F3/2 luminescence decay curves show non-exponential
behavior due to dipole-dipole energy transfer interactions among Nd3+ ions

Extracellular NK histones promote immune cell anti-tumor activity by inducing cell clusters through binding to CD138 receptor.
BACKGROUND: Natural killer (NK) cells are important anti-tumor cells of our innate immune system. Their anti-cancer activity is mediated through interaction of a wide array of activating and inhibitory receptors with their ligands on tumor cells. After activation, NK cells also secrete a variety of pro-inflammatory molecules that contribute to the final immune response by modulating other innate and adaptive immune cells. In this regard, external proteins from NK cell secretome and the mechanisms by which they mediate these responses are poorly defined. METHODS: TRANS-stable-isotope labeling of amino acids in cell culture (TRANS-SILAC) combined with proteomic was undertaken to identify early materials transferred between cord blood-derived NK cells (CB-NK) and multiple myeloma (MM) cells. Further in vitro and in vivo studies with knock-down of histones and CD138, overexpression of histones and addition of exogenous histones were undertaken to confirm TRANS-SILAC results and to determine functional roles of this material transferred. RESULTS: We describe a novel mechanism by which histones are actively released by NK cells early after contact with MM cells. We show that extracellular histones bind to the heparan sulfate proteoglycan CD138 on the surface of MM cells to promote the creation of immune-tumor cell clusters bringing immune and MM cells into close proximity, and thus facilitating not only NK but also T lymphocyte anti-MM activity. CONCLUSION: This study demonstrates a novel immunoregulatory role of NK cells against MM cells mediated by histones, and an additional role of NK cells modulating T lymphocytes activity that will open up new avenues to design future immunotherapy clinical strategies
Hypogonadotropic hypogonadism in Klinefelter syndrome and hypothalamic-pituitary tumor
El síndrome de Klinefelter es la causa más frecuente de hipogonadismo hipergonadotropo en el varón. La supresión en la respuesta al estímulo con hormona liberadora de la hormona luteinizante en estos pacientes debe hacer sospechar como posible etiología una tumoración a nivel hipotalámico. Se presenta el caso de un paciente diagnosticado a los 4 meses con síndrome de Klinefelter mediante cribado neonatal, con cariotipo 47 XXY, en el que se realizan controles clínicos y analíticos seriados y se encuentran, a los 17 años, valores suprimidos de hormona luteinizante y hormona folículo estimulante. Inicia, posteriormente, cefalea y amaurosis de ojo izquierdo, y se encuentra, en una resonancia magnética cerebral, un tumor germinal mixto a nivel hipotalámico, que precisa tratamiento quirúrgico, quimioterapia y radioterapia, con respuesta favorable.
Klinefelter Syndrome is the most frequent cause of hypergonadotropic hypogonadism in men. A flat response at luteinizing hormone releasing hormone stimulation test could be the first sign of hypothalamic tumor in these patients. We report the case of a patient diagnosed by neonatal screening with Klinefelter Syndrome, 47 XXY, that at 17 years follow-up presents analytical modification of the response to luteinizing hormone releasing hormone stimulation test with suppressed luteinizing hormone and follicle-stimulating hormone values; lately he presents with headache and loss of left eye vision. A magnetic resonance imaging of the brain showed a mixed germ cell hypothalamus tumor, requiring surgery, chemotherapy and radiotherapy with optimal response
Costs and benefits of automation for astronomical facilities
The Observatorio Astrof\'isico de Javalambre (OAJ{\dag}1) in Spain is a young
astronomical facility, conceived and developed from the beginning as a fully
automated observatory with the main goal of optimizing the processes in the
scientific and general operation of the Observatory. The OAJ has been
particularly conceived for carrying out large sky surveys with two
unprecedented telescopes of unusually large fields of view (FoV): the JST/T250,
a 2.55m telescope of 3deg field of view, and the JAST/T80, an 83cm telescope of
2deg field of view. The most immediate objective of the two telescopes for the
next years is carrying out two unique photometric surveys of several thousands
square degrees, J-PAS{\dag}2 and J-PLUS{\dag}3, each of them with a wide range
of scientific applications, like e.g. large structure cosmology and Dark
Energy, galaxy evolution, supernovae, Milky Way structure, exoplanets, among
many others. To do that, JST and JAST are equipped with panoramic cameras under
development within the J-PAS collaboration, JPCam and T80Cam respectively,
which make use of large format (~ 10k x 10k) CCDs covering the entire focal
plane. This paper describes in detail, from operations point of view, a
comparison between the detailed cost of the global automation of the
Observatory and the standard automation cost for astronomical facilities, in
reference to the total investment and highlighting all benefits obtained from
this approach and difficulties encountered. The paper also describes the
engineering development of the overall facilities and infrastructures for the
fully automated observatory and a global overview of current status,
pinpointing lessons learned in order to boost observatory operations
performance, achieving scientific targets, maintaining quality requirements,

Measurement of the Crab Nebula spectrum over three decades in energy with the MAGIC telescopes
The MAGIC stereoscopic system collected 69 hours of Crab Nebula data between
October 2009 and April 2011. Analysis of this data sample using the latest
improvements in the MAGIC stereoscopic software provided an unprecedented
precision of spectral and night-by-night light curve determination at gamma
rays. We derived a differential spectrum with a single instrument from 50 GeV
up to almost 30 TeV with 5 bins per energy decade. At low energies, MAGIC
results, combined with Fermi-LAT data, show a flat and broad Inverse Compton
peak. The overall fit to the data between 1 GeV and 30 TeV is not well
described by a log-parabola function. We find that a modified log-parabola
function with an exponent of 2.5 instead of 2 provides a good description of
the data (). Using systematic uncertainties of red the MAGIC and
Fermi-LAT measurements we determine the position of the Inverse Compton peak to
be at (53 3stat + 31syst -13syst) GeV, which is the most precise
estimation up to date and is dominated by the systematic effects. There is no
hint of the integral flux variability on daily scales at energies above 300 GeV
when systematic uncertainties are included in the flux measurement. We consider
three state- of-the-art theoretical models to describe the overall spectral
energy distribution of the Crab Nebula. The constant B-field model cannot
satisfactorily reproduce the VHE spectral measurements presented in this work,
having particular difficulty reproducing the broadness of the observed IC peak.
Most probably this implies that the assumption of the homogeneity of the
magnetic field inside the nebula is incorrect. On the other hand, the
time-dependent 1D spectral model provides a good fit of the new VHE results
when considering a 80 {\mu}G magnetic field. However, it fails to match the
data when including the morphology of the nebula at lower wavelengths.Comment: accepted by JHEAp, 9 pages, 6 figure
Detection of bridge emission above 50 GeV from the Crab pulsar with the MAGIC telescopes
The Crab pulsar is the only astronomical pulsed source detected at very high
energy (VHE, E>100GeV) gamma-rays. The emission mechanism of VHE pulsation is
not yet fully understood, although several theoretical models have been
proposed. In order to test the new models, we measured the light curve and the
spectra of the Crab pulsar with high precision by means of deep observations.
We analyzed 135 hours of selected MAGIC data taken between 2009 and 2013 in
stereoscopic mode. In order to discuss the spectral shape in connection with
lower energies, 4.6 years of {\it Fermi}-LAT data were also analyzed. The known
two pulses per period were detected with a significance of and
. In addition, significant emission was found between the two
pulses with . We discovered the bridge emission above 50 GeV
between the two main pulses. This emission can not be explained with the
