    Stereotactic radiosurgery for brain metastases: analysis of outcome and risk of brain radionecrosis

    <p>Abstract</p> <p>Purpose</p> <p>to investigate the factors affecting survival and toxicity in patients treated with stereotactic radiosurgery (SRS), with special attention to volumes of brain receiving a specific dose (V10 - V16 Gy) as predictors for brain radionecrosis.</p> <p>Patients and Methods</p> <p>Two hundred six consecutive patients with 310 cerebral metastases less than 3.5 cm were treated with SRS as primary treatment and followed prospectively at University of Rome La Sapienza Sant'Andrea Hospital. Overall survival, brain control, and local control were estimated using the Kaplan-Meier method calculated from the time of SRS. Univariate and multivariate analysis using a Cox proportional hazards regression model were performed to determine the predictive value of prognostic factors for treatment outcome and SRS-related complications.</p> <p>Results</p> <p>Median overall survival and brain control were 14.1 months and 10 months, respectively. The 1-year and 2-year survival rates were 58% and 24%, and respective brain control were 43% and 22%. Sixteen patients recurred locally after SRS, with 1-year and 2-year local control rates of 92% and 84%, respectively. On multivariate analysis, stable extracranial disease and KPS >70 were associated with the most significant survival benefit. Neurological complications were recorded in 27 (13%) patients. Severe neurological complications (RTOG Grade 3 and 4) occurred in 5.8% of patients. Brain radionecrosis occurred in 24% of treated lesions, being symptomatic in 10% and asymptomatic in 14%. On multivariate analysis, V10 through V16 Gy were independent risk factors for radionecrosis, with V10 Gy and V12 Gy being the most predictive (p = 0.0001). For V10 Gy >12.6 cm<sup>3 </sup>and V12 Gy >10.9 cm<sup>3 </sup>the risk of radionecrosis was 47%.</p> <p>Conclusions</p> <p>SRS alone represents a feasible option as initial treatment for patients with brain metastases, however a significant subset of patients may develop neurological complications. Lesions with V12 Gy >8.5 cm<sup>3 </sup>carries a risk of radionecrosis >10% and should be considered for hypofractionated stereotactic radiotherapy especially when located in/near eloquent areas.</p

    Mesoporous silicon carbide: Via nanocasting of Ludox® xerogel

    Porous SiC with uniformly sized 12 nm and 22 nm spherical mesopores was synthesized from nanocomposites of polycarbosilane (PCS) preceramic polymer and xerogels of Ludox® SiO2 nanoparticles as templates. The influence of PCS type (Mw 800 and 2000 Da), PCS:SiO2 ratio, pyrolysis temperature 1200-1400 °C, and addition of Ni complex to the preceramic composite was studied with respect to the SiC porous morphology, crystalline structure and chemical properties. We found that the pore walls of Ni-free por-SiC are composed of relatively large (20 nm) crystallites embedded inside a poorly crystalline SiC/SiC1+x phase. Increasing the pyrolysis temperature resulted in an increase of the large crystallites fraction, as well as of the stability with regard to air oxidation; however, some degradation of the porous morphology was noted too. The presence of Ni (1.5% wt relatively to PCS) noticeably improved the crystallinity of por-SiC prepared at 1200-1300 °C, with no degradation of the porous morphology occurring. On the other hand, higher Ni loadings and temperatures led to the transformation of the porous morphology into aggregates of irregularly packed large crystallites.SCOPUS: ar.jinfo:eu-repo/semantics/publishe

Пропонується методика побудови програми експерименту, що забезпечує виконання ряду загальних і специфічних вимог. Методика заснована на моделюванні спрощеного динамічного процесу зміни внутрішньої температури приладу в залежності від зовнішньої. Результати експерименту підтверджують ефективність розробленої методики.The problem of the planning of the experiment devoted to definition of the heat drifting of the angular velocity sensor is considered. The methodology for experiment program construction is proposed, with providing a number of general and specific requirements. The methodology is based on the modeling of the simplified dynamic process that describes varying internal device temperature depending on the value of the external temperature. Experimental results confirm efficiency of the developed methodology.Рассматривается задача планирования эксперимента по определению теплового дрейфа датчика угловой скорости.     Acute decrease in circulating T3 levels enhances, but does not normalise, the GH response to GHRP-6 plus GHRH in thyrotoxicosis

    In thyrotoxicosis there is an impaired GH response to GHRH, normal GH responsiveness to GHRP-6 and lack of synergistic GH response after simultaneous administration of both peptides. We have previously shown that the GHRH-induced GH release in these patients increases after an acute reduction of circulating T-3 values with administration of iopanoic acid, a compound that inhibits peripheral conversion of T-4 to T3. We have now studied the effect of a decrease in serum T3 levels on the GH response to GHRP-6 (1 mug/kg) plus GHRH (100 mug) in 9 hyperthyroid patients before and after 15 days of treatment with iopanoic acid (3 g every 3 days) and propylthiouracil (600 mg/day). Nine normal subjects were also studied. In all hyperthyroid patients iopanoic acid induced a rapid decrease and normalisation of serum T-3 levels. In these subjects peak GH (mug/l; mean+/-SE) and AUC (pg/l(.)120 min) values after GHRP-6 plus GHRH were significantly higher on day 15 compared to pretreatment values (peak, 18.3+/-3.0 vs 13.4+/-1.9; AUC, 1227.9+/-212.9 vs 968.5+/-160.4; p<0.05). Despite the significant enhancement of the GH responsiveness to GHRP-6 plus GHRH after treatment with iopanoic acid, this response remained significantly blunted when compared to controls both in terms of peak GH (18.3+/-3.0 vs 83.7+/-15.2; p<0.05) and AUC values (1227.9+/-212.9 vs 4956.5+/-889.3; p<0.05). In conclusion, our results show that an acute decrease of circulating T-3 levels enhances, but does not normalise, the GH response to GHRP-6 plus GHRH in thyrotoxicosis. This could suggest that circulating T-3 does not have a major role in the mechanisms involved in the synergistic effect of these peptides.
