    The Zika virus crisis, like of all diseases, is an indicator of the inequality that persists in Brazil, even after decades of democracy. Zika illustrates disparity not only in terms of class, and a variety of other issues linked to class, but also of gender and race. Ethical issues related to the Zika virus also have different impacts in terms of reproductive choices, the use of chemicals for spraying, and the development of genetically-modified mosquitoes. By using a multi-disciplinary set of methods based on history, anthropology, and eco-epidemiology, the authors show how the current Zika crisis is part of the long history of health in Brazil, with often-tense relations between agents of the State and the population at-large.A crise do vírus do zika, como todas as doenças, é um indicador da desigualdade que persiste no Brasil mesmo após décadas de democracia. O zika ilustra disparidade não apenas em termos de classe e com a variedade de questões que se conectam com classe, como gênero e raça. Questões éticas relacionadas ao vírus do zika também têm impactos diferenciais em termos de escolhas reprodutivas, no uso de produtos químicos para a pulverização e no desenvolvimento de mosquitos geneticamente modificados. Ao utilizarem um conjunto multidisciplinar de métodos baseados em história, antropologia e ecoepidemiologia, os autores mostram como a crise atual do zika é parte do histórico de saúde no Brasil com as relações muitas vezes tensas entre atores estatais e população em geral

    Reinfestation Sources for Chagas Disease Vector, Triatoma infestans, Argentina

    Reinfestation by Triatoma infestans after insecticide spraying has caused elimination efforts in the dry Chaco region to fail repeatedly. The sources and spatial extent that need to be considered to understand the reinfestation pattern and to plan a comprehensive control program were studied in 2 adjacent rural communities in northwestern Argentina from 1993 to 1997. The effects of external, residual, and primary sources on the reinfestation pattern were evaluated by using geographic information systems, satellite imagery, spatial statistics, and 5-year retrospective data for 1,881 sites. The reinfestation process depended on primary internal sources and on surrounding infested communities. In the dry Chaco, successfully reducing the risk for reinfestation in a community depends on treating all communities and isolated sites within 1,500 m of the target community. In addition, during the surveillance phase, spraying all sites within 500 m of new foci will delay reinfestation

    Environmental and social determinants of human risk during a West Nile virus outbreak in the greater Chicago area, 2002

    BACKGROUND: The outbreak of West Nile Virus (WNV) in and around Chicago in 2002 included over 680 cases of human illness caused by the virus within this region. The notable clustering of the cases in two well-defined areas suggests the existence of specific environmental and social factors that increase the risk for WNV infection and/or illness in these locations. This investigation sought to create an empirically based model to account for these factors and to assess their importance in explaining the possible processes that may have led to this pattern. RESULTS: The cluster pattern of high incidence of cases was statistically significant. The risk factors that were found to be important included the presence of vegetation, age, income, and race of the human population, distance to a WNV positive dead bird specimen, age of housing, mosquito abatement and geological factors. The effect of different mosquito abatement efforts was particularly notable. About 53 percent of the variation of the location of WNV clusters was explained by these factors. CONCLUSION: The models developed indicate that differential mosquito abatement efforts are especially important risk factors, even when controlling for key environmental factors. Human population characteristics play a role in risk that is measurable in this ecological study but would require further research to associate causality with risk. The analysis of spatial clusters of case incidence indicates that this approach provides more insight into the focal nature of differential risk factors that tend to be associated with WNV than an analysis of all individual cases

    Blood feeding patterns of mosquitoes: random or structured?

    <p>Abstract</p> <p>Background</p> <p>The foraging behavior of blood-sucking arthropods is the defining biological event shaping the transmission cycle of vector-borne parasites. It is also a phenomenon that pertains to the realm of community ecology, since blood-feeding patterns of vectors can occur across a community of vertebrate hosts. Although great advances in knowledge of the genetic basis for blood-feeding choices have been reported for selected vector species, little is known about the role of community composition of vertebrate hosts in determining such patterns.</p> <p>Methods & Results</p> <p>Here, we present an analysis of feeding patterns of vectors across a variety of locations, looking at foraging patterns of communities of mosquitoes, across communities of hosts primarily comprised of mammals and birds. Using null models of species co-occurrence, which do not require ancillary information about host abundance, we found that blood-feeding patterns were aggregated in studies from multiple sites, but random in studies from a single site. This combination of results supports the idea that mosquito species in a community may rely primarily on host availability in a given landscape, and that contacts with specific hosts will be influenced more by the presence/absence of hosts than by innate mosquito choices. This observation stresses the importance of blood-feeding plasticity as a key trait explaining the emergence of many zoonotic mosquito transmitted diseases.</p> <p>Discussion</p> <p>From an epidemiological perspective our observations support the idea that phenomena promoting synchronization of vectors and hosts can promote the emergence of vector-borne zoonotic diseases, as suggested by observations on the linkages between deforestation and the emergence of several human diseases.</p

    Association of West Nile virus illness and urban landscapes in Chicago and Detroit

    BACKGROUND: West Nile virus infection in humans in urban areas of the Midwestern United States has exhibited strong spatial clustering during epidemic years. We derived urban landscape classes from the physical and socio-economic factors hypothesized to be associated with West Nile Virus (WNV) transmission and compared those to human cases of illness in 2002 in Chicago and Detroit. The objectives were to improve understanding of human exposure to virus-infected mosquitoes in the urban context, and to assess the degree to which environmental factors found to be important in Chicago were also found in Detroit. RESULTS: Five urban classes that partitioned the urban space were developed for each city region. The classes had many similarities in the two settings. In both regions, the WNV case rate was considerably higher in the urban class associated with the Inner Suburbs, where 1940–1960 era housing dominates, vegetation cover is moderate, and population density is moderate. The land cover mapping approach played an important role in the successful and consistent classification of the urban areas. CONCLUSION: The analysis demonstrates how urban form and past land use decisions can influence transmission of a vector-borne virus. In addition, the results are helpful to develop hypotheses regarding urban landscape features and WNV transmission, they provide a structured method to stratify the urban areas to locate representative field study sites specifically for WNV, and this analysis contributes to the question of how the urban environment affects human health

    Intensified surveillance and insecticide-based control of the Chagas disease vector Triatoma infestans in the Argentinean Chaco.

    Background: The elimination of Triatoma infestans, the main Chagas disease vector in the Gran Chaco region, remains elusive. We implemented an intensified control strategy based on full-coverage pyrethroid spraying, followed by frequent vector surveillance and immediate selective insecticide treatment of detected foci in a well-defined rural area in northeastern Argentina with moderate pyrethroid resistance. We assessed long-term impacts, and identified factors and procedures affecting spray effectiveness. Methods and Findings: After initial control interventions, timed-manual searches were performed by skilled personnel in 4,053 sites of 353?411 houses inspected every 4?7 months over a 35-month period. Residual insecticide spraying was less effective than expected throughout the three-year period, mainly because of the occurrence of moderate pyrethroid resistance and the limited effectiveness of selective treatment of infested sites only. After initial interventions, peridomestic infestation prevalence always exceeded domestic infestation, and timed-manual searches consistently outperformed householders? bug detection, except in domiciles. Most of the infestations occurred in houses infested at baseline, and were restricted to four main ecotopes. Houses with an early persistent infestation were spatially aggregated up to a distance of 2.5 km. An Akaike-based multi-model inference approach showed that new site-level infestations increased substantially with the local availability of appropriate refugia for triatomine bugs, and with proximity to the nearest site found infested at one or two preceding surveys. Conclusions and Significance: Current vector control procedures have limited effectiveness in the Gran Chaco. Selective insecticide sprays must include all sites within the infested house compound. The suppression of T. infestans in rural areas with moderate pyrethroid resistance requires increased efforts and appropriate management actions. In addition to careful, systematic insecticide applications, housing improvement and development policies that improve material conditions of rural villagers and reduce habitat suitability for bugs will contribute substantially to sustainable vector and disease control in the Gran Chaco.Fil: Gurevitz, Juan Manuel.
