Epidermal growth factor stimulates rabbit achilles tendon histologically and biomechanically healing
Background. Epidermal Growth Factor (EGF) stimulates epidermis cell growth, proliferation and differentiation in skin regeneration. The aim of this study was to pre-clinically
investigation of the role of EGF in tendon healing.
Methods. One cm defects were created at the right Achilles tendons of 30 New Zealand
White rabbits. Ten rabbits were allocated to one of three groups: Group-1-(Sham) tendon
defect with a gap that was splinted with a non-absorbable suture; Group-2-(EGF +)
tendon defect with a gap that was splinted with a non-absorbable suture and a 25 µg/kg
EGF injection into the defect; Group-3-(Scaffold + EGF) tendon defect was grafted with
a biodegradable, porous Polycaprolactone (PCL) scaffold loaded with 25 µg/kg EGF and
stabilized with a non-absorbable suture. Animals were sacrificed at 8 weeks post-surgery
and Achilles tendon repair and healing status was investigated using histopathologic and
biomechanical analysis methods.
Results. Group-2-(EGF +) had greater adipocyte development (moderate) than Group1-(Sham) and Group-3-(Scaffold + EGF). Group-2-(EGF +) and Group-3-(Scaffold +
EGF) had greater peripheral nerve development (weak) than Group-1-(Sham). Group-2-
(EGF +) had greater vascularization (moderate) than Group-1-(Sham) and Group-3-(Scaffold + EGF). Group-2-(EGF +) had greater collagen Type-III development (moderate) than Group-1-(Sham) and Group-3-(Scaffold + EGF). Group-3-(Scaffold + EGF)
had greater collagen Type-I development (moderate) than Group-1-(Sham) and Group2-(EGF +). Groups did not display statistically significant differences for load to failure
or elongation at failure. Group-2-(EGF +) and Group-3-(Scaffold + EGF) displayed less
stiffness that the control (healthy contralateral Achilles tendon) (p 0.05).
Conclusions. The application of EGF and scaffold displayed superior histological tendon
healing evidence, but there was no significant difference in terms of biomechanics
Evaluation of preoperative model surgery and the use of a maxillary sinus surgical template in sinus floor augmentation surgery
Maxillary sinus augmentation is an accepted technique
for dental implant placement in presence of insufficient maxillary
bone. There are various techniques in the literature, either by crestal
or lateral approach in maxillary sinus augmentation that have high
percentage of success, while all have complications. Schneiderian
membrane perforation is the most common complication encountered
during surgery. The aim of this study was to evaluate the
benefits of preoperative model surgery and the ease of use of a
maxillary sinus surgical template (MSST) during maxillary sinus
augmentation surgery with a lateral approach. Ten patients included
in the study needed rehabilitation of a partially or totally edentulous
maxilla with an implant-supported fixed prosthesis and requiring
sinus augmentation. A questionnaire was asked to performing surgeons,
and study results showed the use of an MSSTwas found to be
effective in terms of adaptation (62.5%), window preparation
(87.5%), ease of elevation (95.9%), ease of grafting (95.9%), reduction
of perforation risk (91.7%), and achieving immobility during
the procedure (62.5%); however, the use of an MSST was also
found to prolong the surgical procedure (100%) and restrict the view
of the surgical area (79.2%). Maxillary sinus augmentation appears
to be a useful tool for locating an appropriate entrance to the sinus
cavity, allowing for safe elevation of the sinus membrane and effectively
grafting the sinus floor
The ımpact of handicap severity on oral and periodontal status of patients with mental retardation
Aim: Mental retardation (MR) subjects comprise a considerable amount of the community and are susceptible to oral and periodontal problems due to insufficient oral care. The aim of this cross-sectional study was to determine the oral health and periodontal status of MR patients in Turkey with regard to periodontal indices and Decay missed filling teeth (DMFT) scores and compare findings according to severity of the MR.
Materials and methods: One hundred and five MR patients
were included to the study and divided into 3 groups according to MR severity diagnoses. Demographic variables like age, gender, disabled sibling, BMI, living an institution and clinical parameters like plaque index (PI), gingival index (GI), bleeding on probing
(BOP), probing depth (PD), clinical attachment level (CAL), missing teeth and DMFT scores were recorded from all subjects. Appropriate statistical analyses were used to compare the findings.
Results: There were no statistically significant differences between groups according to age, gender, disabled sibling, living
status and DMFT scores. Clinical periodontal indices and the number of missing teeth were showed a statistically significant increasing trend with the severity of MR (p < 0.05). Correlation analysis showed significant positive correlations between PI
and periodontal disease measures like GI, PD, CAL and BOP and also between PI and DMFT (p < 0.05).
Conclusion: The deteriorated teeth condition and deprived periodontal health of MR patients may be most likely caused by the poor oral hygiene and may be worsen with the severity of the MR
Nadir bir sinüs enfeksiyonu etkeni olarak citrobacter koseri: bir olgu raporu
Maksiller posterior bölgede yapılan diş çekimleri sırasında maksiller sinüs ile ilişkili çok sayıda komplikasyon ile karşılaşılmaktadır. Diş kökenli maksiller sinüzitlerin %10’unu oluşturan sinüse kök kaçırılması dental kaynaklı nedenlerden birisidir. Literatürde dental kaynaklı sinüs enfeksiyonu oluşturabilen birçok bakteri türünden bahsedilmesine rağmen, enterik bir bakteri olan Citrobacter koseri (C. koseri) nadir görülen bir sinüs enfeksiyonu etkenidir. 34 yaşındaki, bayan hasta, kliniğimize üst çene sağ taraf büyükazı dişler bölgesinde ağrı şikayeti ile başvurdu. Yapılan radyografik inceleme sonucunda hastanın maksiller sinüsünde rezidüel kök tespit edildi. Mevcut residüel kök Caldwel Luc cerrahisi ile çıkarıldı. Hastanın şikayetleri ortadan kalktı. Yeşil renkli olan diş kökü mikrobiyoloji laboratuvarında anaerobik ve aerobik olarak kültüre edildi. 37 °C’taki 24 saatlik aerobik inkübasyonu takiben gram negatif basil morfolojisine sahip saf koloniler saptandı. Koloniler konvansiyonel yöntemlerin yanısıra VITEK2 Compact otomatik identifikasyon sistemi ile de C. koseri olarak tanımlandı
Bifosfanat kullanımına bağlı çenelerde gelişen osteonekrozlar: Çok merkezli retrospektif çalışma
Bifosfonatlar (BP), metastatik kemik hastalıkları ve
osteoporoz tedavisinde kullanılan kemik rezorbsiyonu
inhibitörleridir. Bifosfanat kullanımına bağlı çenelerde
gelişen osteonekroz (BBÇO), çenelerinde radyasyon
tedavisi görmemiş, bifosfanat kullanan ya da
kullanmış hastalarda mandibula ya da maksillada 8
haftadan daha uzun süren kemik ekspozu ile
karakterize bir durum olarak tanımlanmıştır. Bu
çalışmada farklı merkeze yönlendirilen BBÇO vakaları
retrospektif olarak sunulmuştur. 2012 ve 2013
yıllarında GATA Haydarpaşa Eğitim Hastanesi Diş
Hekimliği, Kocaeli Üniversitesi Diş Hekimliği Fakültesi,
ve Kırıkkale Üniversitesi Diş Hekimliği Fakültesi, Ağız,
Diş ve Çene Cerrahisi kliniklerine başvuran BBÇO
tanısı konulmuş 18 hastanın demografik özellikleri,
anamnez bulguları, klinik muayene bulguları,
bifosfanat çeşidi ve kullanım bilgileri (süre, uygulama
yolu) ve eğer varsa ilgili sebep açısından
değerlendirmeleri yapılmıştır. Hastaların 12’ sinde
mandibula (% 67), 5’ inde maksilla (%28) ve 1 hastada
ise hem mandibula hem maksillada (%5) BBÇO
izlendi. Hastaların 2 tanesi osteoporozis (% 11)
nedeniyle 16 hasta ise (% 89) onkolojik tedavi
nedeniyle bifosfanat grubu ilaç kullanmaktaydı. Tıp ve
diş hekimliğini ortak olarak ilgilendiren konu olması
itibariyle bifosfanat grubu ilaç tedavisine başlanacak
hasta gruplarının ortak klinikler tesis edilerek takip
edilmeleri ve bilgi paylaşımlarıyla ortaya çıkabilecek
olası komplikasyonların azaltılabileceğini

The efficacy of cinacalcet in the treatment of hyperparathyroidism in Turkish hemodialysis patient population
WOS: 000393291900012OBJECTIVE: Cinacalcet reduces parathyroid hormone levels by increasing the sensitivity of the parathyroid gland to calcium. in this study, we firstly aimed to evaluate the efficacy of cinacalcet in Turkish hemodialysis patients. MATERIAL and METHODS: 4483 hemodialysis patients were screened and 469 patients who had used cinacalcet were included in the study. the patients were divided into 4 groups according to drug usage durations (Group 1: 3 months, Group 2: 6 months, Group 3: 9 months and Group 4: 12 months). the patients' Parathormone, Ca, P and CaxP levels at the 3rd, 6th, 9th and 12th months were compared to the start of treatment and previous months. RESULTS: the levels of Parathormone, Ca, P and CaxP significantly decreased compared to their initial levels in all groups (from 1412 pg/ml to 1222 pg/mL for Parathormone, p< 0,001) in the 3rd month. However, this reduction was not continued in the subsequent months (Parathormone: 1381 pg/ml for the 12th month). CONCLUSION: Cinacalcet may not provide adequate benefit in control of hyperparathyroidism in Turkish hemodialysis patient population
Epidermal Growth Faktör (Egf) ve Egf Yüklenmiş Polikaprolakton Skafoldun Tendon Defekt İyileşmesine Etkileri
Introduction: In order to obtain the most accurate anatomical and histological tendon
regeneration, therapies including the administration of platelet rich plasma (PRP), bone
marrow aspiration, bone morphogenic protein, mesenchymal stem cells and growth factors
( bFGF, HGF, rhPDGF-BB, TGF-b, VEGF ) are being applied along with the conventional
treatment. Epidermal growth factor (EGF) is a growth factor that stimulates cell growth,
proliferation and differentiation through binding EGFR cell surface receptor. Its current
clinical application includes the bridging in between epidermis fragments in skin
regeneration. In this study, the aim was to investigate the potential role of EGF in tendon
repair and regenaration which has not been investigated previously.
Material and Methods: In this preclinical study, 1 cm size defects were created on one of
the Achilles tendon of 30 New Zealand white rabbits. 3 experimental groups of 10 tendons
were made including:(1) “Sham” group: 1 cm tendon defect was splinted leaving the gap
with non absorbable suture; (2) EGF(+) group: 1 cm tendon defect was splinted leaving the
gap with non absorbable suture and 25 µg/kg EGF injection was made in the defect; (3)
Scaffold+ EGF(+) group: 1 cm tendon defect was grafted with biodegradable, porous
polycaprolactone (PCL) scaffolds loaded with 25 µg/kg EGF and stabilized using non
absorbable suture. In groups 2 and 3, same dose EGF injections were made every other day
for 10 days. Animals were sacrificed at 8 weeks postop, and Achilles tendon repair and
regeneration was investigated by histopathological and biomechanics analysis. The tissues
were rapidly fixed in 10% formalin and processed for routine light microscopy. All
specimens were embedded in paraffin, 10 mm sections were cut and stained by
hematoxylin and eosin (H&E) and Masson’s trichrome. The biomechanical study was
performed immediately after the animal was sacrificed and the tendons were maintained
fresh. Pullout force test was performed for Achilles tendons and then was measured
breaking force, amount of elongation at break and young modulus. All procedures were
approved by Animal Research Ethical Committee.vii
Results: It was observed macroscopically that defects healed and bridged in all of the
experimental groups. Histologically, “Sham” group: Damar formation and Type 3 collagen
have been observed to be newly constructed. Rare adiposity was observed EGF(+) group:
vessel formation, peripheral nerve buds and more adipocytes were observed inside the
tendon. A high level of type III collagen was detected which was laid by fibroblasts to be
replaced by type I collagen eventually for complete tendon regeneration.
Scaffold +EGF(+)group: vessel formation and peripheral nerve buds were again observed,
but the amount of adipocytes and laid type III collagen were less compared to EGF(+)
group. Type Ⅰ / Type Ⅲ collagen ratio was highest in Skafold + EGF group and at least in
sham group. Biomechanically, there was no statistically significant difference between the
3 experimental groups and the control groups in terms of the breaking forces (p> 0,05).
There was no significant difference between the groups in terms of the amount of
elongation at break (p> 0,05). Young modulus was not significantly different between the
three experimental groups (p> 0,05) but it was found that there was a significant difference
between control group , EGF and Skafold + EGF groups (P <0,05).
Conclusion: EGF application together with the conventional therapy led to more
appropriate, advanced tendon regeneration histologically. Our hypothesis is; The
histological demonstration that EGF application, increased vascularity, accumulation of
"pericyte" cells (stem cells) adjacent to the endothelial cell in the vasculature, and
appearance of the development of fat cells in the tendon defect region in order to provide
the necessary energy for healing can provide high tissue repair. There was no significant
difference between the groups in terms of breaking forces. EGF application did not
negatively affect tendon healing in biomechanical aspects. It was observed that EGF
without biomechanical adverse effects improves histologically more appropriate and
advanced tendon healing by increasing the proportion of Type I / Type Ⅲ collagen.
Histologically effective tendon healing was achieved by the addition of EGF. Treatment
with EGF + scaffold was obtained the most effective tendon healing. Scaffold implantation
into the defect as the void filler and by increasing the effectiveness of the growth factor,
resulted in less type III collagen formation and thus faster tendon repair response.Genel Bilgi: En uygun histolojik ve anatomik tendon iyileşmesini sağlamak için,
Platelet’ten zengin plazma (PRP), mezenkimal kök hücre uygulamaları, kemik morfojenik
protein, kemik iliği aspiratı, büyüme faktörleri ( bFGF, HGF, rhPDGF-BB, TGF-b, VEGF )
geleneksel tedavi protokolüne eklenmektedir. Epidermal Growth Faktör (EGF), hücre
gelişmesini, farklılaşmasını EGFR reseptörüne bağlanarak sağlayan bir büyüme faktörüdür.
Günümüzde EGF’ in cilt rejenerasyonunda epidermisler arasında köprüleşmeyi içeren,
özellikle diyabetik ayak gibi kronik yara problemleri olan hastalarda klinik uygulanmaları
vardır. Bu çalışmada, daha önce araştırılmamış olan defektif tendon tamiri ve iyileşmesinde
EGF ‘in potansiyel rolünün araştırılması hedeflenmiştir.
Gereç ve Yöntemler: Hayvan deneyi çalışmasında 30 tane Yeni Zellanda beyaz tavşanının
aşil tendonunda 1 cm büyüklüğünde defekt oluşturuldu. Herbirinde 10 tendon bulunan 3
denek grubu oluşturuldu; (1)“Sham grubu: 1 cm lik tendon defekti gap bırakılarak
emilmeyen dikiş ile “splint”leme uygulandı. (2) EGF(+) grup: 1 cm lik tendon defektine
“gap” bırakılarak emilmeyen dikiş ile “splint”leme uygulandı ve defekt oluşturulan
bölgeye 25 µg/kg EGF enjekte edildi. (3) Skafold+EGF(+) grup: 1 cm lik tendon
defektine 25 µg/kg EGF yüklenmiş dönüşebilen, poroz polikaprolakton (PCL) “skafold”
yerleştirilerek emilmeyen dikiş ile tespit edildi. 2. ve 3. Gruplara aynı dozlarda EGF
enjeksiyonuna postoperatif gün aşırı 10 gün devam edildi. 8. haftada hayvanlar sakrifiye
edildikten sonra sonra aşil tendon tamiri ve rejenerasyonu histolojik ve biyomekanik olarak
incelendi. Histolojik incelemede ışık mikroskopi yöntemi kullanıldı. Işık mikroskopi
yöntemi için dokular %10 formalinde fikse edildi. Bütün parçalar parafine gömüldü, 10
mm kesitlere ayrıldı, Hematoksilen - Eosin (H&E) ve Masson’s trichrome ile boyandı.
Biyomekanik çalışma, hayvanlar sakrifiye edildikten hemen sonra tendonlar tazeliğini
korurken uygulandı. Çekme testi uygulanan aşil tendonlarında kopma kuvveti, kopma
anındaki uzama miktarı ve young modulus değerleri ölçüldü. Tüm prosedürler Hayvan
Araştırma Etik Kurulundan onay alınarak yapılmıştır.v
Bulgular: Bütün denek gruplarında, defektin iyileştiği ve köprüleşmenin olduğu
makroskopik olarak gözlemlendi. Histolojik olarak “Sham” grup: Damar formasyonu ve
Tip 3 kollojenin yeni yapılmaya başlanmış olduğu gözlemlendi. Nadir adipozit izlendi.
EGF (+) grup: Tendon içinde damar formasyonu, periferal sinir tomurcuklanması, daha
fazla adipoz dokular gözlemlendi. Tamamen tendon rejenerasyonu için sonuçta Tip 1
kollajene dönüşen ve fibroblastlar tarafından üretilen yüksek düzeyde Tip 3 kollajen
görüldü. Skafold + EGF(+) grup: damar formasyonu, periferal sinir tomurcaklanması yine
gözlemlendi fakat adipoz doku miktarı, ve üretilen Tip 3 kollajen mıktarı EGF(+) grup ile
karşılaştırıldığında daha az olduğu gözlemlendi. Tip Ⅰ / Tip Ⅲ kollajen oranının en fazla
Skafold + EGF grubunda olduğu, en az ise Sham grubunda olduğu saptandı. Biyomekanik
olarak, kopma kuvvetleri açısından 3 deney grubu ve kontrol grupları arasında istatiksel
olarak anlamlı fark saptanmadı(p>0,05). Kopma anındaki uzama miktarı açısından da
gruplar arasında anlamlı fark saptanmadı(p>0,05). Young modulus açısından 3 deney grubu
arasında anlamlı fark saptanmadı(p>0,05) ama kontrol grubuyla EGF ve Skafold + EGF
gruplarının ikili karşılaştırılması sonucunda anlamlı fark olduğu saptanmıştır(P<0,05).
Sonuçlar: Geleneksel tedavi ile beraber EGF uygulanması daha uygun ve histolojik olarak
ileri düzeyde tendon iyileşmesi ile sonuçlandı. Bizim hipotezimiz; EGF uygulanması,
damarlanmanın artması, damarda endotel hücreye komşu “ perisit” hücrelerin ( kök
hücrelerin) defekt boyunca toplanması, iyileşmede gerekli enerjinin sağlanabilmesi için
tendon defekt bölgesinde yağ hücrelerinin gelişiminin görülmesi yüksek doku onarımının
olabileceği histolojik olarak göstermiştir. Kopma kuvvetleri açısından gruplar arasında
anlamlı bir fark saptanmamıştır. EGF eklenmesi tendon iyileşmesini biyomekanik açıdan
negatif etkilememiştir. Biyomekanik açıdan olumsuz etkileri bulunmayan EGF’in, Tip Ⅰ /
Tip Ⅲ kollajen oranını artırarak histolojik olarak daha uygun ve daha ileri seviyede tendon
iyileşmesinini sağladığı gözlemlendi. Tedaviye EGF eklenmesiyle histolojik olarak etkin
tendon iyileşmesi elde edildi. Tedaviye EGF + skafold birlikte eklenmesiyle en etkin
tendon iyileşmesi elde edildi. Skafoldun boşluk doldurarak ve büyüme faktörünün
etkinliğini artırarak daha az Tip 3 kollajen formasyonu ve böylece daha hızlı tendon tamiri
olabileceği savı da ortaya çıkmıştır
