Relationship between usual physical, cognitive and social activities and functional recovery at hospital discharge after acute stroke
Objective: To assess the relationship between lifestyle prior to the event and functional recovery at hospital discharge after acute stroke. Design: Cohort study. Patients: A sample of 191 patients with first stroke episode (87.4% ischaemic). Methods: Severity of the event at admission was measured by the Modified National Institutes of Health Stroke Scale. The Frenchay Activity Index was used to evaluate the patients’ previous lifestyles. Functional recovery was assessed using the Modified Rankin scale on discharge from hospital. A Rankin score ≤ 2 was the main outcome. Results: At discharge, 37.2% of the patients were functionally independent. A receiver operating characteristic curve analysis established a value of ≥ 18 on the Frenchay Activity Index scale as the best cut-off point to predict favourable outcome (specificity 62%; 95% confidence interval (CI) 54–69% and a sensibility of 60%; 95% CI 49–69%) with an area under the curve of 0.65 (95% CI 0.57–0.71). There was a positive association between Frenchay Activity Index ≥ 18 and a Rankin score ≤ 2, after controlling for potential confounders (odds ratio 2.62; 95% CI 1.21–5.66; p = 0.001). Conclusion: This result emphasizes the protective effect of mental, physical and social activity for the prevention of functional damage after an acute cerebrovascular event
Development, manufacturing and evaluation of effects and safety of a hip orthosis prototype used in the functional rehabilitation of a patient undergoing Girdlestone arthroplasty
A Artroplastia de Ressecção tipo Girdlestone (ARG) é um procedimento que altera o estilo de vida do indivíduo gerando fadiga precoce, instabilidade articular, distúrbio da marcha e discrepância de membros. Como medida auxiliar no processo de reabilitação podem ser utilizadas órteses. Contudo, não há no mercado nacional dispositivos adequados, sendo necessária a importação com custo elevado. Assim, este estudo desenvolveu e avaliou os efeitos e a segurança de um protótipo de órtese de quadril em uma paciente feminina submetida à ARG. O modelo proposto melhorou o desempenho físico e a funcionalidade e promoveu o alívio de dores osteomusculares.Girdlestone resection arthroplasty (GRA) is a procedure that changes one’s lifestyle, leading to early fatigue, joint instability, gait disorder and limb discrepancy. Orthoses can be used as an aid in the rehabilitation process. However, there are no adequate devices available in Brazil and the imported ones are high-cost. Thus, this study developed and evaluated the effects and safety of a hip orthosis prototype in a woman who underwent GRA. The proposed model improved physical performance and functionality and provided relief of musculoskeletal pain
Efeito potencializador do Cymbopogon citratus-Stapf sobre a ação da acetilcolina na contratilidade de músculo liso
Authors searched actions of Cymbopogon citratus-Stapf aquous extract, 2% concentration, on montility of smooth muscle. It were performed 10 (ten) experiments, each of them consisted of the motility record of a segment of duodenum from male Wistar rats maintained in a Tyrode solution water-bath to isolated organ. Physiological parameters (temperature, pH, oxigen supply) were controled to obtain the functional nature of organ motility. It were realized three records to each experiment in this sequency: First record: control record (standard) from motility of duodenum smooth muscle in Tyrode bath. Second record: after addition of 0,5 ml physiological solution before the third record, the bath liquid was entirely substituted and the organ was washed tith Tyrode solution. Third record: after 15 (fifteen) minutes from the addition of Cymbopogon citratus-Stapf tea 2% concentration, it was add 0,5ml acetylcholine solution (11,8165 Mg in 50 ml physiological solution).It was verified that it shows a significant difference between 2nd and third record groups compared whit control records (first record groups) on smooth muscle motility of isolated duodenum from Wistar male rats.Os autores pesquisaram a ação do extrato aquoso à 2% de Cymbopogon citratus-Stapf sobre a contratilidade de músculo liso (duodeno) de rato Wistar, machos. Foram realizados 10 experimentos; cada um constou de registro de contratilidade de um fragmento de duodeno de rato Wistar, mantido em banho-maria para órgão isolado, com solução de Tyrode, controlando-se os padrões fisiológicos de temperatura, pH e oxigenação do banho, a fim de garantir a natureza funcional da contratilidade do mesmo. Para cada experimento realizavam-se três registros, na seguinte sequência:Registro padrão (controle) de contratilidade do órgão no banho (Tyrode);Registro teste: após acrescentar ao banho acetilcolina. Decorridos 15 minutos de substituição do banho e lavagem do órgão;Registro experimental: após colocação do chá mais acetilcolina no banho.Verificou-se que houve diferença significativa entre os grupos 2 e 3 em relação ao grupo controle na motricidade de músculo liso de duodeno isolado, concluindo-se assim que Cymbopogon citratus produziu efeito acetilcolina símilis
