    Jovian Auroral Ion Precipitation: X‐Ray Production From Oxygen and Sulfur Precipitation

    Many attempts have been made to model X‐ray emission from both bremsstrahlung and ion precipitation into Jupiter's polar caps. Electron bremsstrahlung modeling has fallen short of producing the total overall power output observed by Earth‐orbit‐based X‐ray observatories. Heavy ion precipitation was able to reproduce strong X‐ray fluxes, but the proposed incident ion energies were very high ( urn:x-wiley:jgra:media:jgra55396:jgra55396-math-00011 MeV per nucleon). Now with the Juno spacecraft at Jupiter, there have been many measurements of heavy ion populations above the polar cap with energies up to 300–400 keV per nucleon (keV/u), well below the ion energies required by earlier models. Recent work has provided a new outlook on how ion‐neutral collisions in the Jovian atmosphere are occurring, providing us with an entirely new set of impact cross sections. The model presented here simulates oxygen and sulfur precipitation, taking into account the new cross sections, every collision process, the measured ion fluxes above Jupiter's polar aurora, and synthetic X‐ray spectra. We predict X‐ray fluxes, efficiencies, and spectra for various initial ion energies considering opacity effects from two different atmospheres. We demonstrate that an in situ measured heavy ion flux above Jupiter's polar cap is capable of producing over 1 GW of X‐ray emission when some assumptions are made. Comparison of our approximated synthetic X‐ray spectrum produced from in situ particle data with a simultaneous X‐ray spectrum observed by XMM‐Newton shows good agreement for the oxygen part of the spectrum but not for the sulfur part

    Patient Access to U.S. Physicians Who Conduct Internet or E-mail Consults

    BACKGROUND: E-mail communication has the potential to improve communication between patients and doctors. OBJECTIVE: The objective of the study is to describe the access of patients to physicians who conduct e-mail consults. METHODS: We analyzed data from the National Ambulatory Medical Care Survey (NAMCS), a nationally representative cross-sectional survey of office-based physician visits, in 2001, 2002, and 2003. The main outcome measure was the percentage of visits to a provider who reported doing internet or e-mail consults. RESULTS: There was fewer than 1 in 10 outpatient visits in 2001 (9.2%) to physicians who reported doing internet or e-mail consults, and this did not increase in 2002 (5.8%) or 2003 (5.5%). Access to these physicians was greater among patients who were male, nonminority, lived in the Western United States, seen for pre-/postoperative care, seen by a primary care provider, and not seen by a nurse during their visit. Access to physicians who conducted internet or e-mail consults was independent of other patient (e.g., chronic conditions), provider (e.g., office setting), and visit (e.g., medications prescribed) characteristics. CONCLUSIONS: Access to physicians who do internet or e-mail consults is generally low and did not increase between 2001 and 2003, despite growth in internet access and in other internet-related activities

Cutting-edge observations and simulations are providing insights into the underlying wave generation2, configuration3,4 and damping5 mechanisms found in sunspot atmospheres. However, the in situ amplification of magnetohydrodynamic waves6, rising from a few hundreds of metres per second in the photosphere to several kilometres per second in the chromosphere7, has, until now, proved difficult to explain. Theory predicts that the enhanced umbral wave power found at chromospheric heights may come from the existence of an acoustic resonator8–10, which is created due to the substantial temperature gradients experienced at photospheric and transition region heights11. Here, we provide strong observational evidence of a resonance cavity existing above a highly magnetic sunspot. Through a combination of spectropolarimetric inversions and comparisons with high-resolution numerical simulations, we provide a new seismological approach to mapping the geometry of the inherent temperature stratifications across the diameter of the underlying sunspot, with the upper boundaries of the chromosphere ranging between 1,300 ± 200 km and 2,300 ± 250 km. Our findings will allow the three-dimensional structure of solar active regions to be conclusively determined from relatively commonplace two-dimensional Fourier power spectra.     All clinically-relevant blood components transmit prion disease following a single blood transfusion: a sheep model of vCJD

    Variant CJD (vCJD) is an incurable, infectious human disease, likely arising from the consumption of BSE-contaminated meat products. Whilst the epidemic appears to be waning, there is much concern that vCJD infection may be perpetuated in humans by the transfusion of contaminated blood products. Since 2004, several cases of transfusion-associated vCJD transmission have been reported and linked to blood collected from pre-clinically affected donors. Using an animal model in which the disease manifested resembles that of humans affected with vCJD, we examined which blood components used in human medicine are likely to pose the greatest risk of transmitting vCJD via transfusion. We collected two full units of blood from BSE-infected donor animals during the pre-clinical phase of infection. Using methods employed by transfusion services we prepared red cell concentrates, plasma and platelets units (including leucoreduced equivalents). Following transfusion, we showed that all components contain sufficient levels of infectivity to cause disease following only a single transfusion and also that leucoreduction did not prevent disease transmission. These data suggest that all blood components are vectors for prion disease transmission, and highlight the importance of multiple control measures to minimise the risk of human to human transmission of vCJD by blood transfusion

    Pathogenesis of bovine spongiform encephalopathy in sheep

    The pathogenesis of bovine spongiform encephalopathy (BSE) in sheep was studied by immunohistochemical detection of scrapie-associated prion protein (PrPSc) in the gastrointestinal, lymphoid and neural tissues following oral inoculation with BSE brain homogenate. First accumulation of PrPSc was detected after 6 months in the tonsil and the ileal Peyer’s patches. At 9 months postinfection, PrPSc accumulation involved all gut-associated lymphoid tissues and lymph nodes as well as the spleen. At this time point, PrPSc accumulation in the peripheral neural tissues was first seen in the enteric nervous system of the caudal jejunum and ileum and in the coeliac-mesenteric ganglion. In the central nervous system, PrPSc was first detected in the dorsal motor nucleus of the nervus Vagus in the medulla oblongata and in the intermediolateral column in the spinal cord segments T7–L1. At subsequent time points, PrPSc was seen to spread within the lymphoid system to also involve all non-gut-associated lymphoid tissues. In the enteric nervous system, further spread of PrPSc involved the neural plexi along the entire gastrointestinal tract and in the CNS the complete neuraxis. These findings indicate a spread of the BSE agent in sheep from the enteric nervous system through parasympathetic and sympathetic nerves to the medulla oblongata and the spinal cord

    Photo-antagonism of the GABAA receptor

    Neurotransmitter receptor trafficking is fundamentally important for synaptic transmission and neural network activity. GABAA receptors and inhibitory synapses are vital components of brain function, yet much of our knowledge regarding receptor mobility and function at inhibitory synapses is derived indirectly from using recombinant receptors, antibody-tagged native receptors and pharmacological treatments. Here we describe the use of a set of research tools that can irreversibly bind to and affect the function of recombinant and neuronal GABAA receptors following ultraviolet photoactivation. These compounds are based on the competitive antagonist gabazine and incorporate a variety of photoactive groups. By using site-directed mutagenesis and ligand-docking studies, they reveal new areas of the GABA binding site at the interface between receptor β and α subunits. These compounds enable the selected inactivation of native GABAA receptor populations providing new insight into the function of inhibitory synapses and extrasynaptic receptors in controlling neuronal excitation

    Renal artery stenosis-when to screen, what to stent?

    Renal artery stensosis (RAS) continues to be a problem for clinicians, with no clear consensus on how to investigate and assess the clinical significance of stenotic lesions and manage the findings. RAS caused by fibromuscular dysplasia is probably commoner than previously appreciated, should be actively looked for in younger hypertensive patients and can be managed successfully with angioplasty. Atheromatous RAS is associated with increased incidence of cardiovascular events and increased cardiovascular mortality, and is likely to be seen with increasing frequency. Evidence from large clinical trials has led clinicians away from recommending interventional revascularisation towards aggressive medical management. There is now interest in looking more closely at patient selection for intervention, with focus on intervening only in patients with the highest-risk presentations such as flash pulmonary oedema, rapidly declining renal function and severe resistant hypertension. The potential benefits in terms of improving hard cardiovascular outcomes may outweigh the risks of intervention in this group, and further research is needed

    Behavioural syndrome in a solitary predator is independent of body size and growth rate.

    Models explaining behavioural syndromes often focus on state-dependency, linking behavioural variation to individual differences in other phenotypic features. Empirical studies are, however, rare. Here, we tested for a size and growth-dependent stable behavioural syndrome in the juvenile-stages of a solitary apex predator (pike, Esox lucius), shown as repeatable foraging behaviour across risk. Pike swimming activity, latency to prey attack, number of successful and unsuccessful prey attacks was measured during the presence/absence of visual contact with a competitor or predator. Foraging behaviour across risks was considered an appropriate indicator of boldness in this solitary predator where a trade-off between foraging behaviour and threat avoidance has been reported. Support was found for a behavioural syndrome, where the rank order differences in the foraging behaviour between individuals were maintained across time and risk situation. However, individual behaviour was independent of body size and growth in conditions of high food availability, showing no evidence to support the state-dependent personality hypothesis. The importance of a combination of spatial and temporal environmental variation for generating growth differences is highlighted
