This paper reports the synthesis and structure of one of them, 2-cyclohexyl-6-(2-oxo-2H-chromen-3-yl)imidazo[2,1-b][1,3,4]thiadiazole-5-carbaldehyde. Tumor response and endogenous immune reactivity after administration of HER2 CAR T cells in a child with metastatic rhabdomyosarcoma
Refractory metastatic rhabdomyosarcoma is largely incurable. Here we analyze the response of a child with refractory bone marrow metastatic rhabdomyosarcoma to autologous HER2 CAR T cells. Three cycles of HER2 CAR T cells given after lymphodepleting chemotherapy induces remission which is consolidated with four more CAR T-cell infusions without lymphodepletion. Longitudinal immune-monitoring reveals remodeling of the T-cell receptor repertoire with immunodominant clones and serum autoantibodies reactive to oncogenic signaling pathway proteins. The disease relapses in the bone marrow at six months off-therapy. A second remission is achieved after one cycle of lymphodepletion and HER2 CAR T cells. Response consolidation with additional CAR T-cell infusions includes pembrolizumab to improve their efficacy. The patient described here is a participant in an ongoing phase I trial (NCT00902044; active, not recruiting), and is 20 months off T-cell infusions with no detectable disease at the time of this report
Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries
Background
Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres.
Methods
This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and lowâmiddle-income countries.
Results
In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of âsingle-useâ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for lowâmiddle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia.
Conclusion
This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both highâ and lowâmiddleâincome countries
UâPb systematics of the western Dharwar Craton - Glimpses of a billion year history of crustal evolution and relations to ancient supercratons
The Dharwar Craton developed progressively over a billion years, through two main stages of crustal growth separated by a few-hundred million year long period of relative quiescence. The first stage between 3.4 and 3.0 Ga developed a proto-craton, which was considerably amplified during the second main stage between 2.7 and 2.4 Ga, through extensive magmatism, tectonism, and crustal consolidation. This paper reports UâPb dating results obtained in four specific areas of the craton, with the data encompassing key moments in this long development. Rocks formed during the proto-craton stage include a 3089 Ma augen gneiss and a 2973 Ma evolved granite, the latter of which marks the final cratonization event of the proto-craton. The beginning of the second main stage is recorded in this study by 2650 Ma tonalite and trondhjemite, a 2623 Ma granite dyke cutting augen gneiss, and 2614, 2602 and 2588 Ma volcanic rocks. Titanite responded differently to the long evolution, as a function of location and type of overprint. It preserved an original 2973 Ma magmatic age in the west, but was reset and/or crystallized during secondary events in central domains of the craton, yielding ages between 2590 and 2360 Ma. A diorite stock intruded at 2207 Ma in the consolidated crust. It is correlated with the Anantapur-Kunigal mafic dyke swarm, one of a series of such events in the Dharwar Craton between 2.35 and 1.79 Ma. In terms of its overall evolution the Dharwar Craton has an affinity with the Slave clan, which includes the Wyoming and Zimbabwe cratons. It also matches many features in the evolution of the SĂŁo Francisco Craton, a probable other member of Sclavia. This is in contrast to the Amazonian Craton, which has more affinity with the Superior clan
A note on the hydrographic conditions in the upper halocline region off Bhatkal and Kundapur, Karnataka, during the late phase of southwest monsoon.
572-577Hydrographic conditions in the upper halocline region of the
continental shelf off Bhatkal and Kundapur is studied utilizing the data
collected on board ORV Sagar Kanya during 28th and 29th
of Aug. 2008. Salinity, temperature, and dissolved oxygen profiles are acquired
for seven stations from an area of 300 sq Km between the depths 35 and 20 m and
also from two stations along the 45 m bathymetry. Variation in salinity is
observed (34.7 to 36.2 PSU) in the upper halocline region and salinity maxima
occurred between depths of 5â15 m. Sea surface temperature was between 24 to 27° C. Drastic decrease in dissolved
oxygen concentrations with depth also observed in the upper halocline. These
features reveal stratified nature of the water column at > 40 m bathymetry and
significant upwelling and buoyant mixing of water masses between
40 and 20 m depth (bathymetry) during the late phase of monsoon
Morphology and sediment movement in a monsoon influenced open beach at Gangavali,near Gokarn (central west coast of India)
31-36Morphological and grain size studies have been carried out at
monthly intervals (February 1993 to February 1994) to understand sedimentation
pattern. Eight study sites were selected for morphological studies and five for
grain size studies. It is observed that the morphological changes are cyclic
i.e. erosion during monsoon and deposition during fair-weather season. The
beach has experienced a loss of about 951 m3/m-1, a gain
of about 1677 m3/m-1 and hence a net gain of about 725 m3/m-1
of sediments along the line of transect (all the 8 stations together) over a
period of one year. A secondary cycle of erosion is observed between December
and February. In general, the sediments of the study area are fine grained,
well sorted, and symmetrical and platykurtic in nature. Mean grain size
decreases, sorting improves and percentage of fine sand, positively skewed
grains increases from north to south (G1 to G8). Beach experiences moderate
turbulent energy conditions and the sediment movement is towards south in this
beach .The prevailing conditions in this beach are ideal for recreational development
