    EVALUATION OF EARLY AND LONG TERM RESULTS OF THE TREATMENT OF MUCOCELE OF THE LIP USING 980 NM DIODE LASER

    The objective The objective of this study is to compare the results obtained from treatment of mucocele with cold scalpel versus 980 nm diode laser . Introduction : Mucoceles are common benign lesions of the oral cavity that develop following extra vacation or retention of mucous material from salivary glands in the subepithelial tissue.Treatment may be performed by conventional surgery, cryotherapy and more recently laser surgery.Material and methods : In this study we report 20 cases with mucoceles of the lower lip treated in the Dental Clinic of the University of Tirana from January 2007 to January 2011, 10 of them are resected with a scalpel and 10 with 980 nm diode laser.We have documented the operative treatment and postoperative follow-up for each patients for the evaluation of early and long term results. Results : Diode laser surgery was rapid and bloodless.Postoperative period was without complication,after one month from the treatment with laser no scar formation was reported versus the cases treated with scalpel.After 1-3 years of follow-up no recurrence was recorded after laser surgery compared with 3cases of recurrence among the cases of conventional surgical removal of mucocele. Conclusion: 980 nm diode laser is a good modality for the treatment of mucoceleΒ΄slip.This treatment provides satisfactory results and is also wellaccepted by all age groups

    Extrusion of Irrigant in Open Apex Teeth with Periapical Lesions Following Laser-Activated Irrigation and Passive Ultrasonic Irrigation

    Introduction: Sodium hypochlorite (NaOCl) irrigation is critical for the success of endodontic treatment and several agitation techniques have been developed to improve the efficacy of this irrigation. Using a combination of contrast medium and radiographic examination, this study evaluated NaOCl extrusion during agitation of irrigant. Development of pressure, which may result in apical extrusion of the irrigant, has been described during laser-activated irrigation (LAI) and passive ultrasonic irrigation (PUI). Methods and Materials: We examined 40 single root canals categorized as having open apices with apical lesions in 40 patients. For the final irrigation, the teeth were irrigated with a mixture of radiopaque contrast medium and 2.5% NaOCl in solution. The solution was activated for 60 sec in both groups [the Er, Cr: YSGG laser group (n=20) and the ultrasonic group (n=20)]. The teeth were imaged subsequently using radiography for the evaluation of contrast extrusion. Results: Radiopaque contrast medium was absent from the periapical tissues in all cases. Conclusion: Use of LAI or PUI appears to be safe as used currently in endodontic treatment.Keywords: Endodontics; Er, Cr: YSGG Laser; Open Apex; Periapical Lesion

    The Numerical Simulation Application for Fire-Tube Boiler Heating Surface Safety Evaluation

    The numerical simulation is applied for fire-tube boiler heating surface safety estimation. Thermal processes in an inflatable fire-tube furnace during its emergency operation were simulated using the finite volume method with Euler approximation and the implicit pressure based algorithm. Study results reproduce failures connected with increasing of impasse aerodynamic resistance. The method of these failures prediction is suggested. Simulation has shown that entering the amount of coolant into combustion volume results in burner fan incapability to overcome the impasse resistance of the furnace. The simulation results are visually confirmed during the inspection of emergency boilers

    TREATMENT OF VASCULAR LESION OF LIP WITH 980NM DIODE LASER COMPARED WITH CONVENTIONAL METHOD

    Introduction: Mulliken and Glowacki introduced a simple classification in 1982 that was based on the clinical, histochemical and cellular criteria to distinguish between the various vascular anomalies. They described two distinct entities which are haemangiomas and vascular malformations. Hence, they can be situated at different area of the oromaxillofacial region: tongue, lip, palate, buccal mucosa, and gingiva. Numerous method of treatment has been used such as cryotherapy, embolization, sclerotisation, cold scalpel and combination of these. Management of these lesions with laser is an option, and several laser systems have been developed using principles of selective photothermolysis. Thus, the clinical cases presented in this study are treated with 980 nm diode laser. Material and methods: In this study, we reported 60 cases with vascular lesions of lip treated in the Dental Clinic of the University of Tirana from May 2007 to May 2012. 30 cases were treated with 980 nm diode laser and 30 cases with scalpel. Thus, the operative treatment and post-operative follow-up for the evaluation of early and long term results was documented. All statistical test and graphics were created using SPSS for Medical Calculation (MedCalc). Data were expressed as mean –standard deviation and 95 % confidence interval and the statistical significance was estimated by StudentΒ΄s t-test and p values< 0,05 were considered significant. Results: According the protocol research, the results of the two groups of population were compared as laser group and the control group. Diode laser surgery was rapid and bloodless. The wounds were healed without complications in two to three weeks depending on the size of the lesions. The parameters evaluated are: bleeding, pain, swelling, scar formation, functional disturbance, and recurrence. On the basis of the data collected from the laser group, no pain, or swelling were reported during the first week after intervention and from the control group, 22 of the patients reported pain and 20 referred swelling after the surgical excision. After one month of the treatment, no scar formation was reported from laser group and in the patients of control group, scarring was observed in the site of the performed excision. After 3 years of follow-up, no recurrence was recorded after laser surgery versus one case was treated with scalpel during the first year. No functional disturbance was recorded from the laser group versus 6 cases reported from the control group. Conclusion: 980 nm is a good modality for the treatment of vascular lesion of lip. This treatment provides satisfactory results, and is well accepted by all patients without compromising their health and functionality

    Comparative Evaluation of the Effects of CO2 and Er:YAG Lasers on Smear Layer Removal and Blood Cell Attachment to Tooth Root Surfaces

    Background: The tooth root surfaces are modified by different agents for better removal of the smear layer, formation of fibrin clots and attachment of blood cells. This in vitro study compared removal of the smear layer, formation of fibrin clots and attachment of blood cells after exposing periodontally compromised root surfaces to ER:YAG and CO2 laser beams.Methods: Eighteen dentin block samples were prepared from freshly extracted periodontally compromised teeth that were deemed hopeless, and divided into 3 groups; exposed to Er:YAG laser beams, exposed to CO2 laser beams and the control group. The samples were evaluated using scanning electron microscopy and micrographs were taken. Smear layer removal and blood cell attachment were scored. Data were analyzed with SPSS using Kruskal-Wallis and Mann-Whitney tests.Results: In the Er:YAG laser group, the smear layer was removed completely. In the specimens exposed to blood, better fibrin clot formation and blood cell attachment were observed in the Er:YAG laser group. In the CO2 laser group, the smear layer was also removed; however, there were no significant differences between the CO2 laser and control groups in fibrin clot formation and blood cell attachment.Conclusion: Application of Er:YAG laser to the root dentin appears to result in the formation of a suitable surface for fibrin clot formation and blood cell attachment. Further clinical studies are necessary to support these results

    Evaluation of Effectiveness of Photodynamic Therapy With Low-level Diode Laser in Nonsurgical Treatment of Peri-implantitis

    Introduction: Side effects related to antibiotic therapy for peri-implantitis are rare in laser therapy (LT); therefore, the aim of this study was to evaluate the effectiveness of LT and photodynamic therapy (PDT) on patients with primary peri-implantitis.Methods: In this randomized clinical trial, 40 implants presenting primary peri-implantitis in 20 patients with a mean age of 52.6 years old were included using the simple sampling technique. Periodontal treatment comprising scaling and root planing (SRP) was accomplished for the whole mouth while mechanical debridement with titanium curettes and air polishing with sodium bicarbonate powder was accomplished around the implants. The implants were randomly divided into two groups and treated with LT (control) and PDT (test). The clinical indices were measured at baseline, 6 weeks and 3 months after treatment. Real-time polymerase chain reaction (PCR) was used for analysis of microbial samples at baseline and 3-month follow-up. Data were analyzed with SPSS 20, using repeated-measures analysis of variance (ANOVA) and Friedman’s and Mann-Whitney tests (Ξ± = 0.05).Results: Both groups showed statistically significant improvements in terms of bleeding on probing (P < 0.001), probing pocket depth (PPD) (P = 0.006) and modified plaque index (P < 0.001), with no significant differences between the 2 groups (P > 0.05). The number of Aggregatibacter actinomycetemcomitans (P = 0.022), Tannerella forsythia (P = 0.038) and Porphyromonas gingivalis (P = 0.05) in the test group and Porphyromonas gingivalis (P = 0.015) in the control group significantly decreased.Conclusion: The results suggested that LT and PDT have significant short-term benefits in the treatment of primary peri-implantitis
