    Comparison of Achilles tendon loading between male and female recreational runners

    Recreational running is an activity with multiple reported health benefits for both sexes, however, chronic injuries caused by excessive and/or repetitive loading of the Achilles tendon are common. Males have been identified as being at an increased risk of suffering an injury to the Achilles tendon and as such, knowledge of differences in loading between the sexes may provide further information to better understand why this is the case. The aim of the current investigation was to determine whether gender differences in the Achilles tendon load exist in recreational runners. Fifteen male (age 26.74 Ā± 5.52 years, body height 1.80 Ā± 0.11 m and body mass 74.22 Ā± 7.27 kg) and fifteen female (age 25.13 Ā± 6.39 years, body height 1.68 Ā± 0.12 m and body mass 67.12 Ā± 9.11 kg) recreational runners volunteered to take part in the current investigation. Participants completed 10 trials running at 4.0 mā€¢s-1 Ā±5% striking a force platform (1000 Hz) with their right foot. Ankle joint kinematics were synchronously recorded (250 Hz) using an optoelectric motion capture system. Ankle joint kinetics were computed using Newton-Euler inverse-dynamics. Net external ankle joint moments were then calculated. To estimate Achilles tendon kinetics the plantarflexion moment calculated was divided by an estimated Achilles tendon moment arm of 0.05 m. Differences in Achilles tendon kinetics were examined using independent sample t-tests (p<0.05). The results indicate that males were associated with significantly (p<0.05) greater Achilles tendon loads than females. The findings from this study support the notion that male recreational runners may be at greater risk of Achilles tendon pathology

    2.5-D/3-D resistivity modelling in anisotropic media using Gaussian quadrature grids

    We present a new numerical scheme for 2.5-D/3-D direct current resistivity modelling in heterogeneous, anisotropic media. This method, named the ā€˜Gaussian quadrature grid' (GQG) method, cooperatively combines the solution of the Variational Principle of the partial differential equation, Gaussian quadrature abscissae and local cardinal functions so that it has the main advantages of the spectral element method. The formulation shows that the GQG method is a modification of the spectral element method but does not employ the constant elements or require the mesh generator to match the Earth's surface. This makes it much easier to deal with geological models having a 2-D/3-D complex topography than using traditional numerical methods. The GQG technique can achieve a similar convergence rate to the spectral element method. We show it transforms the 2.5-D/3-D resistivity modelling problem into a sparse and symmetric linear equation system that can be solved by an iterative or matrix inversion method. Comparison with analytic solutions for homogeneous isotropic and anisotropic models shows that the error depends on the Gaussian quadrature order (abscissa number) and the subdomain size. The higher the order or the smaller the subdomain size that is employed, the more accurate are the results obtained. Several other synthetic examples, both homogeneous and inhomogeneous, incorporating sloping, undulating and severe topography, are presented and found to yield results comparable to finite element solutions involving a dense mes

    3-D frequency-domain seismic wave modelling in heterogeneous, anisotropic media using a Gaussian quadrature grid approach

    We present an extension of the 3-D spectral element method (SEM), called the Gaussian quadrature grid (GQG) approach, to simulate in the frequency-domain seismic waves in 3-D heterogeneous anisotropic media involving a complex free-surface topography and/or sub-surface geometry. It differs from the conventional SEM in two ways. The first is the replacement of the hexahedral element mesh with 3-D Gaussian quadrature abscissae to directly sample the physical properties or model parameters. This gives a point-gridded model which more exactly and easily matches the free-surface topography and/or any sub-surface interfaces. It does not require that the topography be highly smooth, a condition required in the curved finite difference method and the spectral method. The second is the derivation of a complex-valued elastic tensor expression for the perfectly matched layer (PML) model parameters for a general anisotropic medium, whose imaginary parts are determined by the PML formulation rather than having to choose a specific class of viscoelastic material. Furthermore, the new formulation is much simpler than the time-domain-oriented PML implementation. The specified imaginary parts of the density and elastic moduli are valid for arbitrary anisotropic media. We give two numerical solutions in full-space homogeneous, isotropic and anisotropic media, respectively, and compare them with the analytical solutions, as well as show the excellent effectiveness of the PML model parameters. In addition, we perform numerical simulations for 3-D seismic waves in a heterogeneous, anisotropic model incorporating a free-surface ridge topography and validate the results against the 2.5-D modelling solution, and demonstrate the capability of the approach to handle realistic situation

    Designing assisted living technologies 'in the wild' : preliminary experiences with cultural probe methodology

    Background There is growing interest in assisted living technologies to support independence at home. Such technologies should ideally be designed ā€˜in the wildā€™ i.e. taking account of how real people live in real homes and communities. The ATHENE (Assistive Technologies for Healthy Living in Elders: Needs Assessment by Ethnography) project seeks to illuminate the living needs of older people and facilitate the co-production with older people of technologies and services. This paper describes the development of a cultural probe tool produced as part of the ATHENE project and how it was used to support home visit interviews with elders with a range of ethnic and social backgrounds, family circumstances, health conditions and assisted living needs. Method Thirty one people aged 60 to 98 were visited in their homes on three occasions. Following an initial interview, participants were given a set of cultural probe materials, including a digital camera and the ā€˜Home and Life Scrapbookā€™ to complete in their own time for one week. Activities within the Home and Life Scrapbook included maps (indicating their relationships to people, places and objects), lists (e.g. likes, dislikes, things they were concerned about, things they were comfortable with), wishes (things they wanted to change or improve), body outline (indicating symptoms or impairments), home plan (room layouts of their homes to indicate spaces and objects used) and a diary. After one week, the researcher and participant reviewed any digital photos taken and the content of the Home and Life Scrapbook as part of the home visit interview. Findings The cultural probe facilitated collection of visual, narrative and material data by older people, and appeared to generate high levels of engagement from some participants. However, others used the probe minimally or not at all for various reasons including limited literacy, physical problems (e.g. holding a pen), lack of time or energy, limited emotional or psychological resources, life events, and acute illness. Discussions between researchers and participants about the materials collected (and sometimes about what had prevented them completing the tasks) helped elicit further information relevant to assisted living technology design. The probe materials were particularly helpful when having conversations with non-English speaking participants through an interpreter. Conclusions Cultural probe methods can help build a rich picture of the lives and experiences of older people to facilitate the co-production of assisted living technologies. But their application may be constrained by the participantā€™s physical, mental and emotional capacity. They are most effective when used as a tool to facilitate communication and development of a deeper understanding of older peopleā€™s needs

    Gender differences in the kinetics and kinematics of distance running: implications for footwear design

    Interest in distance running amongst females has expanded rapidly. Although there are numerous health benefits associated with running, the occurrence of injury is well documented. Given the relative susceptibility of females to overuse running injuries, a key issue within the discipline of footwear biomechanics that has yet to be appropriately addressed is the specific demands of athletic footwear for females. The aim of this study was therefore to provide both a kinetic and 3-D kinematic comparison of male and female runners in order to determine the relative susceptibility of females to the proposed mechanisms of overuse injuries and whether based on this information, females require more specific footwear designs to meet their needs. Twelve male participants and twelve female participants completed five successful trials running at 4.0ms-1+5%. 3-D angular joint kinematics from the hip, knee and ankle were collected using an eight camera motion analysis system. In addition simultaneous tibial acceleration and ground reaction forces were obtained. Differences in impact parameters and joint kinematics were subsequently compared using independent samples t-tests. Females were found to be associated with significantly greater knee abduction, knee internal rotation and ankle eversion, whilst males were associated with significantly greater hip flexion. Based on these findings it is recommended that females select running footwear with design characteristics aimed towards the reduction of coronal plane ankle eversion in order to reduce the incidence of injury

    On the sensitivity of surface NMR in the presence of electrical conductivity anomalies

    The surface-NMR tomography technique is based on the principles of electromagnetic induction and proton spin dynamics. Electromagnetic fields emitted by large surface current-driven loops are employed to locate and quantify groundwater reservoirs. The oscillating magnetic fields interact with proton spins of water molecules in the electrically conductive subsurface. To study the influence of changing subsurface electrical properties on the nuclear spin response, we consider the spin magnetization as a virtual magnetic dipole receiver. The numerical solutions for the electric and magnetic fields of the transmitter and the virtual receiver in 3-D heterogeneous ground are based on the finite-element method. We explicitly compute the frequency-domain electromagnetic sensitivities for separate spin magnetizations in a groundwater aquifer to study the distortion of the NMR response because of electrical heterogeneities in the medium. Analyses of entire pulse moment sequences yield the cumulative sensitivities to electrical conductivity and water-content variations in the subsurface. We illustrate the influence of conductivity on NMR responses using a limited number of models. From these models we found that electrical conductivity anomalies in the shallow subsurface (<50 m) having values =0.1 S m-1 and volumes with linear dimensions in the order of our loop size (i.e. edge length 100 m) can have a strong influence on the NMR response and ought to be taken into account in the inversion of surface-NMR data. The effect increases non-linearly with increased body size, increased conductivity contrast and decreased anomaly dept

    An examination of business occupier relocation decision making : distinguishing small and large firm behaviour

    This paper explores how business occupiers decide whether and where to relocate. It captures the experience and behaviour of a range of sizes and types of business occupier and subjects their decision-making processes to detailed scrutiny. A linear three-stage decision model is used to sequence and structure interviews with individuals who have intimate involvement with the relocation of 28 firms and organizations in Tyne and Wear, in the north-east of England. The 'constant comparative' method is used to analyse the interview data, from which emerges 18 key concepts, comprising 51 characteristic components. Using an axial approach, these are organized into 10 cross-cutting themes that represent the main areas of consideration or influence on the thinking of the people involved in determining whether a firm or organization should relocate and, if so, where to. The resulting analysis finds that organizations adopt varying degrees of sophistication when making relocation decisions; small firms are more inclined to make decisions based on constrained information; larger organizations adopt a more complex approach. Regardless of firm size, key individuals exert considerable influence over the decision-making process and its outcome

    Models and applications for measuring the impact of health research: Update of a systematic review for the health technology assessment programme

    This report reviews approaches and tools for measuring the impact of research programmes, building on, and extending, a 2007 review. Objectives: (1) To identify the range of theoretical models and empirical approaches for measuring the impact of health research programmes; (2) to develop a taxonomy of models and approaches; (3) to summarise the evidence on the application and use of these models; and (4) to evaluate the different options for the Health Technology Assessment (HTA) programme. Data sources: We searched databases including Ovid MEDLINE, EMBASE, Cumulative Index to Nursing and Allied Health Literature and The Cochrane Library from January 2005 to August 2014. Review methods: This narrative systematic literature review comprised an update, extension and analysis/discussion. We systematically searched eight databases, supplemented by personal knowledge, in August 2014 through to March 2015. Results: The literature on impact assessment has much expanded. The Payback Framework, with adaptations, remains the most widely used approach. It draws on different philosophical traditions, enhancing an underlying logic model with an interpretative case study element and attention to context. Besides the logic model, other ideal type approaches included constructionist, realist, critical and performative. Most models in practice drew pragmatically on elements of several ideal types. Monetisation of impact, an increasingly popular approach, shows a high return from research but relies heavily on assumptions about the extent to which health gains depend on research. Despite usually requiring systematic reviews before funding trials, the HTA programme does not routinely examine the impact of those trials on subsequent systematic reviews. The York/Patient-Centered Outcomes Research Institute and the Grading of Recommendations Assessment, Development and Evaluation toolkits provide ways of assessing such impact, but need to be evaluated. The literature, as reviewed here, provides very few instances of a randomised trial playing a major role in stopping the use of a new technology. The few trials funded by the HTA programme that may have played such a role were outliers. Discussion: The findings of this review support the continued use of the Payback Framework by the HTA programme. Changes in the structure of the NHS, the development of NHS England and changes in the National Institute for Health and Care Excellenceā€™s remit pose new challenges for identifying and meeting current and future research needs. Future assessments of the impact of the HTA programme will have to take account of wider changes, especially as the Research Excellence Framework (REF), which assesses the quality of universitiesā€™ research, seems likely to continue to rely on case studies to measure impact. The HTA programme should consider how the format and selection of case studies might be improved to aid more systematic assessment. The selection of case studies, such as in the REF, but also more generally, tends to be biased towards high-impact rather than low-impact stories. Experience for other industries indicate that much can be learnt from the latter. The adoption of researchfishĀ® (researchfish Ltd, Cambridge, UK) by most major UK research funders has implications for future assessments of impact. Although the routine capture of indexed research publications has merit, the degree to which researchfish will succeed in collecting other, non-indexed outputs and activities remains to be established. Limitations: There were limitations in how far we could address challenges that faced us as we extended the focus beyond that of the 2007 review, and well beyond a narrow focus just on the HTA programme. Conclusions: Research funders can benefit from continuing to monitor and evaluate the impacts of the studies they fund. They should also review the contribution of case studies and expand work on linking trials to meta-analyses and to guidelines. Funding: The National Institute for Health Research HTA programme

    Tissue-selective expression of a conditionally-active ROCK2-estrogen receptor fusion protein

    The serine/threonine kinases ROCK1 and ROCK2 are central mediators of actomyosin contractile force generation that act downstream of the RhoA small GTP-binding protein. As a result, they have key roles in regulating cell morphology and proliferation, and have been implicated in numerous pathological conditions and diseases including hypertension and cancer. Here we describe the generation of a gene-targeted mouse line that enables CRE-inducible expression of a conditionally-active fusion between the ROCK2 kinase domain and the hormone-binding domain of a mutated estrogen receptor (ROCK2:ER). This two-stage system of regulation allows for tissue-selective expression of the ROCK2:ER fusion protein, which then requires administration of estrogen analogues such as tamoxifen or 4-hydroxytamoxifen to elicit kinase activity. This conditional gain-of-function system was validated in multiple tissues by crossing with mice expressing CRE recombinase under the transcriptional control of cytokeratin14 (K14), murine mammary tumor virus (MMTV) or cytochrome P450 Cyp1A1 (Ah) promoters, driving appropriate expression in the epidermis, mammary or intestinal epithelia respectively. Given the interest in ROCK signaling in normal physiology and disease, this mouse line will facilitate research into the consequences of ROCK activation that could be used to complement conditional knockout models
