    Prevalencia de marcadores serológicos de hepatitis virales en una población seleccionada. Experiencia de un servicio universitario. Años 2000-2007

    En la actualidad se describen varios agentes virales causantes de hepatitis, entre los más comunes están VHB, VHA, VHC, VHD, VHE, cada uno de ellos con diferentes modos de transmisión, tiempo de incubación y patrones clínicos. El objetivo de este estudio fuedeterminar la prevalencia de marcadores virales de hepatitis en una población con sospecha clínica de hepatitis durante el periodo 2000 a 2007. En este estudio observacional descriptivo de corte transverso, fueron estudiados 756 pacientes de 1 a 88 años de edad, con una edad promedio de 36±17 años, que concurrieron al Departamento de Inmunología del IICS del 2000 al 2007, a quienes se solicitó, en ocasiones, el dosaje de dos o más marcadores virales. Todas las determinaciones serológicas fueron realizadas por el método de ELISA (Human–Alemania, Adaltis–Italia). El 4,5 % (28/617) tenianHBsAg (+), el 23,4% (11/47) anticuerpos antiBHs (+), el 17,3% (9/52) Ac anti HBcore(+), 37,9% (52/137) Ac anti-VHA de tipo IgM(+) y el 6,6%(11/166) anti-VHC. Coinfección de VHB y VHA en 1 individuo y de VHC y VHA en 1 individuo.El marcador viral más solicitado fue el de HBsAg, los datos obtenidos indican una endemicidad intermedia. En esta población estudiada se destaca la elevada prevalencia de anticuerpos IgM contra el virus de la hepatitis A, (característicos de la etapa aguda), lo cual coincide con que nuestro país pertenece a una zona de alta endemicidad para esta infección viral. Se desea hacer notar la necesidad de monitorear las hepatitis virales, sobre todo aquellas inmunoprevenibles, de modo a evitar el desarrollo de enfermedades crónicas que afecten la calidad de vida del individuo

    Hepatitis B: antígeno de superficie y anticuerpos post vacunales en mujeres del Hospital Psiquiátrico de Asunción, Paraguay

    El virus de la Hepatitis B se transmite por diversas vías incluyendo la sanguínea, vertical y sexual. Pacientes con severas condiciones mentales están sometidos a mayor riesgo de adquirir cualquier infección viral como virus de hepatitis B, C y VIH. La duración de la estadía en las instituciones mentales tiene influencia en el contagio de la enfermedad y largos periodos de permanencia, incrementan la oportunidad para una transmisión viral. El objetivo fue conocer la frecuencia de antígeno de superficie de la hepatitis B y determinar los anticuerpos post vacunales contra la hepatitis en mujeres en edad reproductiva, internadas en un Hospital Psiquiátrico del Ministerio de Salud Pública y Bienestar Social de Asunción- Paraguay. Estudio observacional descriptivo llevado a cabo de mayo 2008 a mayo 2010. Se estudiaron 72 pacientes de sexo femenino, en edad fértil, de 18 a 49 años (promedio de 38 años ± 6). Los sueros fueron analizados para detectar: HBsAg basal y anti-HBs post vacunal por el método de ELISA. En las 72 pacientes estudiadas no se encontró evidencia serológica de la infección por virus de hepatitis B. De las 30 pacientes a quienes se les aplicó 3 dosis de vacuna, 29 presentaron valores protectivos adecuados (mayor a 20 mUI/mL) a los dos meses posteriores a la vacunación, una tuvo un valor inferior a 10 mUI/mL. Con la respuesta obtenida en las pacientes estudiadas en esta institución psiquiátrica, se estaría reduciendo las complicaciones de la hepatitis B e impidiendo la transmisión horizontal a las demás pacientes y al personal de salud de la institución y sobretodo se prevendrá la transmisión de hepatitis B al feto, en casos de embarazo

    Colonic Microbiota Profile Characterization of the Responsiveness to Dietary Fibre Treatment in Hypercholesterolemia

    This study aimed to determine how the microbiota profile might be predisposed to a better response in blood lipid profiles due to dietary fibre supplementation. A three-arm intervention study that included three different fibre types (mainly insoluble, soluble, and antioxidant fibre) supplemented (19.2 g/day) during 2 months in individuals with hypercholesterolemia was developed. Changes in faecal microbiota and blood lipid profile after fibre supplementation were determined. In all volunteers, regardless of fibre type, an increase in the abundance of Bifidobacterium was observed, and similarly, an inverse relationship between faecal propionic acid and blood LDL-cholesterol, LDL particle size, and LDL/HDL particle ratio (p-values 0.0067, 0.0002, and 0.0067, respectively) was observed. However, not all volunteers presented an improvement in lipid profile. The non-responders to fibre treatment showed a decrease in microbiota diversity (Shannon and Simpson diversity index p-values of 0.0110 and 0.0255, respectively) after the intervention; where the reduction in short-chain fatty acids (SCFAs) producing bacterial genera such as Clostridium XIVa and Ruminococcus after dietary fibre treatment was the main difference.     Supplementation with Two New Standardized Tea Extracts Prevents the Development of Hypertension in Mice with Metabolic Syndrome

    Hypertension is considered to be both a cardiovascular disease and a risk factor for other cardiovascular diseases, such as coronary ischemia or stroke. In many cases, hypertension occurs in the context of metabolic syndrome (MetS), a condition in which other circumstances such as abdominal obesity, dyslipidemia, and insulin resistance are also present. The high incidence of MetS makes necessary the search for new strategies, ideally of natural origin and with fewer side effects than conventional pharmacological treatments. Among them, the tea plant is a good candidate, as it contains several bioactive compounds such as caffeine, volatile terpenes, organic acids, and polyphenols with positive biological effects. The aim of this study was to assess whether two new standardized tea extracts, one of white tea (WTE) and the other of black and green tea (CTE), exert beneficial effects on the cardiovascular alterations associated with MetS. For this purpose, male C57/BL6J mice were fed a standard diet (Controls), a diet high in fats and sugars (HFHS), HFHS supplemented with 1.6% WTE, or HFHS supplemented with 1.6% CTE for 20 weeks. The chromatography results showed that CTE is more concentrated on gallic acid, xanthines and flavan-3-ols than WTE. In vivo, supplementation with WTE and CTE prevented the development of MetS-associated hypertension through improved endothelial function. This improvement was associated with a lower expression of proinflammatory and prooxidant markers, and—in the case of CTE supplementation—also with a higher expression of antioxidant enzymes in arterial tissue. In conclusion, supplementation with WTE and CTE prevents the development of hypertension in obese mice; as such, they could be an interesting strategy to prevent the cardiovascular disorders associated with Met

    FRET distance dependence from upconverting nanoparticles to quantum dots

    Förster resonant energy transfer (FRET) with upconverting nanoparticles (UCNPs) as donors and quantum dots (QDs) as acceptors has been regarded as a promising tool for biosensing applications. In this work, we use time-resolved fluorescence spectroscopy to analyze the UCNP-to-QD FRET and we focus on the most relevant parameter of the FRET phenomenon, UCNP-QD distance. This distance is controlled by a nanometric silica shell around the UCNP surface. We theoretically reproduce the experimental results applying FRET theory to the distribution of emitting erbium ions in the UCNP. This simple model allows us to estimate the contribution of every erbium ion to the final FRET response and to explore different strategies to improve FRET efficiency

    WHODAS 2.0 as a Measure of Severity of Illness: Results of a FLDA Analysis

    WHODAS 2.0 is the standard measure of disability promoted by World Health Organization whereas Clinical Global Impression (CGI) is a widely used scale for determining severity of mental illness. Although a close relationship between these two scales would be expected, there are no relevant studies on the topic. In this study, we explore if WHODAS 2.0 can be used for identifying severity of illness measured by CGI using the Fisher Linear Discriminant Analysis (FLDA) and for identifying which individual items of WHODAS 2.0 best predict CGI scores given by clinicians. One hundred and twenty-two patients were assessed with WHODAS 2.0 and CGI during three months in outpatient mental health facilities of four hospitals of Madrid, Spain. Compared with the traditional correction of WHODAS 2.0, FLDA improves accuracy in near 15%, and so, with FLDA WHODAS 2.0 classifying correctly 59.0% of the patients. Furthermore, FLDA identifies item 6.6 (illness effect on personal finances) and item 4.5 (damaged sexual life) as the most important items for clinicians to score the severity of illness

    Endothelial dysfunction is an early indicator of sepsis and neutrophil degranulation of septic shock in surgical patients

    Producción CientíficaBackground: Stratification of the severity of infection is currently based on the Sequential Organ Failure Assessment (SOFA) score, which is difficult to calculate outside the ICU. Biomarkers could help to stratify the severity of infection in surgical patients. Methods: Levels of ten biomarkers indicating endothelial dysfunction, 22 indicating emergency granulopoiesis, and six denoting neutrophil degranulation were compared in three groups of patients in the first 12 h after diagnosis at three Spanish hospitals. Results: There were 100 patients with infection, 95 with sepsis and 57 with septic shock. Seven biomarkers indicating endothelial dysfunction (mid-regional proadrenomedullin (MR-ProADM), syndecan 1, thrombomodulin, angiopoietin 2, endothelial cell-specific molecule 1, vascular cell adhesion molecule 1 and E-selectin) had stronger associations with sepsis than infection alone. MR-ProADM had the highest odds ratio (OR) in multivariable analysis (OR 11·53, 95 per cent c.i. 4·15 to 32·08; P = 0·006) and the best area under the curve (AUC) for detecting sepsis (0·86, 95 per cent c.i. 0·80 to 0·91; P < 0·001). In a comparison of sepsis with septic shock, two biomarkers of neutrophil degranulation, proteinase 3 (OR 8·09, 1·34 to 48·91; P = 0·028) and lipocalin 2 (OR 6·62, 2·47 to 17·77; P = 0·002), had the strongest association with septic shock, but lipocalin 2 exhibited the highest AUC (0·81, 0·73 to 0·90; P < 0·001). Conclusion: MR-ProADM and lipocalin 2 could be alternatives to the SOFA score in the detection of sepsis and septic shock respectively in surgical patients with infection.Instituto de Salud Carlos III (grants PI15/01959, PI15/01451 and PI16/01156
