Using discrete event simulation (DES) to manage theatre operations in healthcare: An audit-based case study
This paper discusses the application of Discrete Event Simulation (DES) in modelling the complex relationship between patient types, case-mix and operating theatre allocation in a large National Health Service (NHS) Trust in London. The simulation model that was constructed described the main features of nine theatres, focusing on operational processes and patient throughput times. The model was used to test three scenarios of case-mix and to demonstrate the potential of using simulation modelling as a cost effective method for understanding the issues of healthcare operations management and the role of simulation techniques in problem solving. The results indicated that removing all day cases will reduce patient throughput by 23.3% and the utilization of the orthopaedic theatre in particular by 6.5%. This represents a case example of how DES can be used by healthcare managers to inform decision making

A review of historical developments of quality assessment in industry and healthcare
Purpose: This study reviewed the literature on the historical development of quality assessment methods in industry and in healthcare. A comparative analysis of quality methods in industry and healthcare was conducted to examine the gap between methods in the two sectors. An attempt was then made to examine the latest approaches to quality assessment in healthcare and finally a proposal has been offered for a more effective approach to tackling the problem of quality in healthcare.
Design/methodology/approach:
Firstly, a review of the evolution of quality assessment in industry and healthcare was conducted. This was based on books written by prominent experts in the field of quality. secondly, a study of the current approaches in healthcare was undertaken. Publications from varied sources were selected and reviewed. The literature consulted includes worldwide operations research and healthcare sources including dissertations, the internet and reference lists of relevant articles.
The journal papers and conference proceedings were selected according to the following criteria: Objective: the study must be aimed at measuring or improving quality both. It could also be aimed at developing new ways of measuring the quality of health care; Method: observational studies, experimental trials or systematic reviews; Setting: study should be in a hospital setting and not narrowed to quality of clinical cares.
Findings: This study showed that the concept of quality management and its control in healthcare is not as advanced as it is in industry. Moreover, it seemed that most researchers, who set out to assess quality of care in one way or the other, have had differing views of quality and the factors that contribute to its assessment. It was also deduced that the way forward in healthcare quality is the development of systems that give staff ownership and pride in a way that is akin to the era of the craftsmen
Reconstructing 3D x-ray CT images of polymer gel dosimeters using the zero-scan method
In this study x-ray CT has been used to produce a 3D image of an irradiated PAGAT gel sample, with noise-reduction achieved using the âzero-scanâ method. The gel was repeatedly CT scanned and a linear fit to the varying Hounsfield unit of each pixel in the 3D volume was evaluated across the repeated scans, allowing a zero-scan extrapolation of the image to be obtained. To minimise heating of the CT scannerâs x-ray tube, this study used a large slice thickness (1 cm), to provide image slices across the irradiated region of the gel, and a relatively small number of CT scans (63), to extrapolate the zero-scan image. The resulting set of transverse images shows reduced noise compared to images from the initial CT scan of the gel, without being degraded by the additional radiation dose delivered to the gel during the repeated scanning. The full, 3D image of the gel has a low spatial resolution in the longitudinal direction, due to the selected scan parameters. Nonetheless, important features of the dose distribution are apparent in the 3D x-ray CT scan of the gel. The results of this study demonstrate that the zero-scan extrapolation method can be applied to the reconstruction of multiple x-ray CT slices, to provide useful 2D and 3D images of irradiated dosimetry gels
Accounting for decarbonisation and reducing capital at risk in the S&P500

The moral technical imaginaries of internet convergence in an American television network
How emergent technologies are imagined, discussed, and implemented reveals social morality about how society, politics, and economics should be organized. For the television industry in the United States, for instance, the development of internet âconvergenceâ provoked the rise of a new discourse about participatory democracy as well as the hopes for lucrative business opportunities. The simultaneity of technical, moral, and social ordering defines the âmoral technical imaginary.â Populating this concept with ethnographic and historical detail, this article expands the theory of the moral technical imaginary with information from six years of participant observation, interviews, and employment with Current TV, an American-based television news network founded by Vice President Al Gore to democratize television production. An immunotherapy survivor population: health-related quality of life and toxicity in patients with metastatic melanoma treated with immune checkpoint inhibitors

Metastatic melanoma survivors are a new population in the era of cancer survivorship. The aimofthis study was to evaluate metastatic melanoma survivors in terms of health-related quality of life (HRQoL), immune-relatedadverse events (irAEs) and exposure to immunosuppressive agents in a large single centre in the UK.Methods We defined the survivor population as patients with a diagnosis of metastatic melanoma who achieved a durableresponse to an ICI and had been followed-up for a minimum of 12 months from initiation of ICI without disease progression.HRQoL was assessed using SF-36. Electronic health records were accessed to collect data on demographics, treatments, irAEsand survival. HRQoL data was compared with two norm-based datasets.Results Eighty-four metastatic melanoma survivors were eligible and 87% (N = 73) completed the SF-36. ICI-related toxicity ofany grade occurred in 92%of patients and 43%had experienced a grade 3 or 4 toxicity. Almost half (49%) of the patients requiredsteroids for the treatment of ICI-related toxicity, whilst 14% required treatment with an immunosuppressive agent beyondsteroids.Melanoma survivors had statistically significant lower HRQoL scores with regard to physical, social and physical rolefunctioning and general health compared with the normative population. There was a trend towards inferior scores in patientswith previous exposure to ipilimumab compared with those never exposed to ipilimumab.Conclusions Our results show that metastatic melanoma survivors have potentially experienced significant ICI-related toxicityand experience significant impairments in specific HRQoL domains. Future service planning is required to meet this populationâsunique survivorship needs.Peer reviewe
Novel Bayesian Networks for Genomic Prediction of Developmental Traits in Biomass Sorghum.
The ability to connect genetic information between traits over time allow Bayesian networks to offer a powerful probabilistic framework to construct genomic prediction models. In this study, we phenotyped a diversity panel of 869 biomass sorghum (Sorghum bicolor (L.) Moench) lines, which had been genotyped with 100,435 SNP markers, for plant height (PH) with biweekly measurements from 30 to 120 days after planting (DAP) and for end-of-season dry biomass yield (DBY) in four environments. We evaluated five genomic prediction models: Bayesian network (BN), Pleiotropic Bayesian network (PBN), Dynamic Bayesian network (DBN), multi-trait GBLUP (MTr-GBLUP), and multi-time GBLUP (MTi-GBLUP) models. In fivefold cross-validation, prediction accuracies ranged from 0.46 (PBN) to 0.49 (MTr-GBLUP) for DBY and from 0.47 (DBN, DAP120) to 0.75 (MTi-GBLUP, DAP60) for PH. Forward-chaining cross-validation further improved prediction accuracies of the DBN, MTi-GBLUP and MTr-GBLUP models for PH (training slice: 30-45 DAP) by 36.4-52.4% relative to the BN and PBN models. Coincidence indices (target: biomass, secondary: PH) and a coincidence index based on lines (PH time series) showed that the ranking of lines by PH changed minimally after 45 DAP. These results suggest a two-level indirect selection method for PH at harvest (first-level target trait) and DBY (second-level target trait) could be conducted earlier in the season based on ranking of lines by PH at 45 DAP (secondary trait). With the advance of high-throughput phenotyping technologies, our proposed two-level indirect selection framework could be valuable for enhancing genetic gain per unit of time when selecting on developmental traits
Controlling Light Through Optical Disordered Media : Transmission Matrix Approach
We experimentally measure the monochromatic transmission matrix (TM) of an
optical multiple scattering medium using a spatial light modulator together
with a phase-shifting interferometry measurement method. The TM contains all
information needed to shape the scattered output field at will or to detect an
image through the medium. We confront theory and experiment for these
applications and we study the effect of noise on the reconstruction method. We
also extracted from the TM informations about the statistical properties of the
medium and the light transport whitin it. In particular, we are able to isolate
the contributions of the Memory Effect (ME) and measure its attenuation length
Pre-Altitude Serum Ferritin Levels and Daily Oral Iron Supplement Dose Mediate Iron Parameter and Hemoglobin Mass Responses to Altitude Exposure
Purpose : To investigate the influence of daily oral iron supplementation on changes in hemoglobin mass (Hbmass) and iron parameters after 2â4 weeks of moderate altitude exposure.Methods :Hematological data collected from 178 athletes (98 males, 80 females) exposed to moderate altitude (1,350â3,000 m) were analysed using linear regression to determine how altitude exposure combined with oral iron supplementation influenced Hbmass, total iron incorporation (TII) and blood iron parameters [ferritin and transferrin saturation (TSAT)]. Results :Altitude exposure (mean ± s: 21 ± 3 days) increased Hbmass by 1.1% [-0.4, 2.6], 3.3% [1.7, 4.8], and 4.0% [2.0, 6.1] from pre-altitude levels in athletes who ingested nil, 105 mg and 210 mg respectively, of oral iron supplement daily. Serum ferritin levels decreased by -33.2% [-46.9, -15.9] and 13.8% [-32.2, 9.7] from pre-altitude levels in athletes who supplemented with nil and 105 mg of oral iron supplement daily, but increased by 36.8% [1.3, 84.8] in athletes supplemented with 210 mg of oral iron daily. Finally, athletes who ingested either 105 mg or 210 mg of oral iron supplement daily had a greater TII compared with non-supplemented athletes (0 versus 105 mg: effect size (d) = -1.88 [-2.56, -1.17]; 0 versus 210 mg: effect size (d) = -2.87 [-3.88, -1.66]). Conclusion :Oral iron supplementation during 2â4 weeks of moderate altitude exposure may enhance Hbmass production and assist the maintenance of iron balance in some athletes with low pre-altitude iron stores
