Characterization of microplastics in water bottled in different packaging by Raman spectroscopy
Raman spectroscopy was applied to quantitatively and qualitatively characterize microplastics (MPs) in bottled water packaged in three different materials: polyethylene terephthalate (PET), recycled PET (rPET) and glass. The results showed a low mean concentration of MPs in all samples, with higher values in glass bottles (8.65 +/- 5.39 p L-1) than in PET (5.09 +/- 3.28 p L-1) or rPET (3.33 +/- 1.34 p L-1) bottles. Through the use of a method capable of detecting smaller particles, MPs of 20-100 mu m were dominant and fragments were the most abundant particle shape. PET was the prevalent polymer in PET bottles underlying the possible contribution of packaging in MP contamination, while polyethylene (PE) with additives prevailed in rPET and glass bottles, suggesting a contribution from the cap. A standardized protocol would allow comparable data to be obtained and allow an objective assessment of exposure, in view of plans to monitor contaminants of emerging concern (including MPs) under recent European legislation.MPs were detected by Raman spectroscopy in bottled water from different packaging
Colorectal Cancer Stage at Diagnosis Before vs During the COVID-19 Pandemic in Italy
IMPORTANCE Delays in screening programs and the reluctance of patients to seek medical
attention because of the outbreak of SARS-CoV-2 could be associated with the risk of more advanced
colorectal cancers at diagnosis.
OBJECTIVE To evaluate whether the SARS-CoV-2 pandemic was associated with more advanced
oncologic stage and change in clinical presentation for patients with colorectal cancer.
DESIGN, SETTING, AND PARTICIPANTS This retrospective, multicenter cohort study included all
17 938 adult patients who underwent surgery for colorectal cancer from March 1, 2020, to December
31, 2021 (pandemic period), and from January 1, 2018, to February 29, 2020 (prepandemic period),
in 81 participating centers in Italy, including tertiary centers and community hospitals. Follow-up was
30 days from surgery.
EXPOSURES Any type of surgical procedure for colorectal cancer, including explorative surgery,
palliative procedures, and atypical or segmental resections.
MAIN OUTCOMES AND MEASURES The primary outcome was advanced stage of colorectal cancer
at diagnosis. Secondary outcomes were distant metastasis, T4 stage, aggressive biology (defined as
cancer with at least 1 of the following characteristics: signet ring cells, mucinous tumor, budding,
lymphovascular invasion, perineural invasion, and lymphangitis), stenotic lesion, emergency surgery,
and palliative surgery. The independent association between the pandemic period and the outcomes
was assessed using multivariate random-effects logistic regression, with hospital as the cluster
variable.
RESULTS A total of 17 938 patients (10 007 men [55.8%]; mean [SD] age, 70.6 [12.2] years)
underwent surgery for colorectal cancer: 7796 (43.5%) during the pandemic period and 10 142
(56.5%) during the prepandemic period. Logistic regression indicated that the pandemic period was
significantly associated with an increased rate of advanced-stage colorectal cancer (odds ratio [OR],
1.07; 95%CI, 1.01-1.13; P = .03), aggressive biology (OR, 1.32; 95%CI, 1.15-1.53; P < .001), and stenotic
lesions (OR, 1.15; 95%CI, 1.01-1.31; P = .03).
CONCLUSIONS AND RELEVANCE This cohort study suggests a significant association between the
SARS-CoV-2 pandemic and the risk of a more advanced oncologic stage at diagnosis among patients
undergoing surgery for colorectal cancer and might indicate a potential reduction of survival for
these patients
Goodbye Hartmann trial: a prospective, international, multicenter, observational study on the current use of a surgical procedure developed a century ago
Background: Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann's procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods: This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results: 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). Severe complications (Clavien-Dindo ≥ 3b) were higher in the HP group (P < 0.001). 30-day mortality was higher in HP patients (13.7%), especially in case of bowel perforation and diffused peritonitis. 1-year follow-up showed no differences on ostomy reversal rate between HP and RPA. (P = 0.127). A backward likelihood logistic regression model showed that RPA was preferred in younger patients, having low ASA score (≤ 3), in case of large bowel obstruction, absence of colonic ischemia, longer time from admission to surgery, operating early at the day working hours, by a surgeon who performed more than 50 colorectal resections. Conclusions: After 100 years since the first Hartmann's procedure, HP remains the most common treatment for left-sided colorectal emergencies. Treatment's choice depends on patient characteristics, the time of surgery and the experience of the surgeon. RPA should be considered as the gold standard for surgery, with HP being an exception
Reproductive biology of Ophelia barquii (Annelida, Opheliidae) along the Salento Peninsula (Mediterranean Sea, South Italy)
This study investigates two populations of a polychaete identified as Ophelia barquii Fauvel, 1927, collected in two beaches from
the Adriatic (Alimini beach) and Ionian (Gallipoli beach) coasts from the Salento Peninsula. Each population was analyzed
monthly from April 2017 to March 2018. In both beaches, populational density decreased from April to August with a complete
absence of worms from August to the following January. We hypothesize that this trend results from horizontal migration of
individuals for reproductive purposes. Sexual maturation was asynchronous between the two populations, with individuals
becoming ripe earlier in Alimini than in Gallipoli. Significant differences in body size were recorded between the two populations,
with specimens larger in Gallipoli than in Alimini, suggesting that they could belong to different species. However, lifecycle
studies performed under laboratory conditions showed that both populations follow a similar developmental path and can
cross-fertilize. The larval development featured a very short period of pelagic life, the shortest known so far for any species of
Ophelia
PET-Bottled Water Consumption in View of a Circular Economy: The Case Study of Salento (South Italy)
Polyethylenterephtalate (PET) is the preferred packaging material in the bottled water industry and represents the main cause of waste production. This work investigated the drinking water consumption habits, with particular reference to PET-bottled water, of people living in the province of Lecce (Apulia Region, Southern Italy) by age and geographical groups. Their perception about the quality of tap water was also explored. The survey was performed by the administration of a questionnaire to 4137 citizens. Bottled mineral water was consumed by about the 90.4% of respondents with an average consumption of 387.7 L/year per capita (375.2 L/year in PET-bottles, 12.5 L/year in glass bottles). Public supply system was used by the 61.5% of respondents with an average consumption of 169.4 L/year per capita. The consumption of tap water was negatively related to the perception of its bad quality and the residence in a big city (Lecce); while it was positively associated with the age group ≤16 years old. Effective communications strategies must be developed in order to promote the consumption of tap water and implement good practice of circular economy
Direct detection of free vitamin D as a tool to assess risk conditions associated with chronic plaque psoriasis: Free vitamin D in chronic plaque psoriasis patients
Introduction.Psoriasis is a major public health problem that results in high social and health costs. New approaches and methods are required to identify any conditions related to the disease and comorbidity development. The vitamin D deficiency is associated to psoriasis and could play an important role in its pathogenesis. However, the serum level of vitamin D is currently measured as total vitamin D, which is affected by wide variability. Therefore, the determination of the free form could be more significant, since it is independent of confounding factors. A cross-sectional study was performed to assess the association between chronic plaque psoriasis and serum level of free vitamin D, detected by a direct analytical method.
Methods.The levels of bioavailable vitamin D, total vitamin D and other metabolic parameters whose homeostasis is regulated by vitamin D were evaluated in 72 psoriasis patients and in 48 healthy controls. A direct immunoassay method was used to directly measure serum free vitamin D level. Analysis of covariance was performed to calculate estimated marginal means (EMM) and 95% confidence interval (CI), after adjustment for age, sex and BMI, within the two groups.
Results.Patients showed an EMM of 5.526±0.271pg/ml, 95% CI 4.989-6.063; while controls an EMM of 6.776±0.271 pg/ml, 95% CI 6.115-7.437.
Conclusions.Chronic plaque psoriasis patients exhibited a serum level of free vitamin D lower than controls. The direct immunoassay method could represent a useful tool to assess vitamin D status and identify a risk condition associated with the onset of the pathology

FANCM c.5791C > T nonsense mutation (rs144567652) induces exon skipping, affects DNA repair activity and is a familial breast cancer risk factor
Numerous genetic factors that influence breast cancer risk are known. However, approximately two-thirds of the overall familial risk remain unexplained. To determine whether some of the missing heritability is due to rare variants conferring high to moderate risk, we tested for an association between the c.5791C>T nonsense mutation (p.Arg1931*; rs144567652) in exon 22 of FANCM gene and breast cancer. An analysis of genotyping data from 8635 familial breast cancer cases and 6625 controls from different countries yielded an association between the c.5791C>T mutation and breast cancer risk [ odds ratio (OR) = 3.93 (95% confidence interval (CI) = 1.28-12.11; P = 0.017)]. Moreover, we performed two meta-analyses of studies from countries with carriers in both cases and controls and of all available data. These analyses showed breast cancer associations with OR = 3.67 (95% CI = 1.04-12.87; P = 0.043) and OR = 3.33 (95% CI = 1.09-13.62; P = 0.032), respectively. Based on information theory-based prediction, we established that the mutation caused an out-of-frame deletion of exon 22, due to the creation of a binding site for the pre-mRNA processing protein hnRNP A1. Furthermore, genetic complementation analyses showed that the mutation influenced the DNA repair activity of the FANCM protein. In summary, we provide evidence for the first time showing that the common p.Arg1931* loss-of-function variant in FANCM is a risk factor for familial breast cancer
