    Effects of Evidence-Based Fall Reduction Programing on the Functional Wellness of Older Adults in a Senior Living Community: A Clinical Case Study.

    BACKGROUND: Older adults at a high risk of falls may be referred to a physical therapist. A physical therapy episode of care is designed for the transition of an older adult from a high fall risk to a moderate to low fall risk. However, these episodes of care are limited in time and duration. There is compelling evidence for the efficacy of group-based exercise classes to address risk, and transitioning an older adult from physical therapy to a group-based program may be an effective way to manage risk through the continuum of care. OBJECTIVES: The purpose of this study was to translate research findings into a real world setting, and demonstrate the efficacy of integrating evidence-based fall prevention exercises into pre-existing exercise classes at a senior living facility as a proof of concept model for future programing. METHODS: Twenty-four participants aged 65 years and older living in a senior living community and the community were stratified into group-based exercise classes. Cutoff scores from functional outcome measures were used to stratify participants. Exercises from The Otago Exercise Program were implemented into the classes. Functional outcome measures collected included the 10-Meter Walk Test, 30-Second Sit to Stand, and Timed Up and Go (TUG). Number of falls, hospitalizations, and physical therapy episodes of care were also tracked. Data were compared to a control group in a different senior living community that offered classes with similar exercises aimed at improving strength and mobility. The classes were taught by an exercise physiologist and were of equal duration and frequency. RESULTS: Participants demonstrated significant improvements in all functional outcome measures. TUG mean improved from 13.5 to 10.4 s (p = 0.034). The 30-Second Sit to Stand mean improved from 10.5 to 13.4 (p = 0.002). The 10-Meter Walk Test improved from 0.81 to 0.98 m/s (p \u3c 0.0001). Participants did not experience any falls or hospitalizations, and two participants required physical therapy episodes of care. CONCLUSION: Implementing an evidence-based fall reduction program into a senior living program has a positive effect on strength, balance, fall risk, gait speed, fall rate, hospitalizations, and amount of physical therapy intervention

    It is commonly assumed that solar hot water systems save energy and reduce greenhouse gas emissions. Very rarely has the life-cycle energy requirements of solar hot water systems been analysed, including their embodied energy. The extent to which solar hot water systems save energy compared to conventional systems in Melbourne, Australia, is shown through a comparative net energy analysis. The solar systems provided a net energy saving compared to the conventional systems after 0.5 to 2 years, for electricity and gas systems respectively.<br /

    A phylogeny of birds based on over 1,500 loci collected by target enrichment and high-throughput sequencing

    Evolutionary relationships among birds in Neoaves, the clade comprising the vast majority of avian diversity, have vexed systematists due to the ancient, rapid radiation of numerous lineages. We applied a new phylogenomic approach to resolve relationships in Neoaves using target enrichment (sequence capture) and high-throughput sequencing of ultraconserved elements (UCEs) in avian genomes. We collected sequence data from UCE loci for 32 members of Neoaves and one outgroup (chicken) and analyzed data sets that differed in their amount of missing data. An alignment of 1,541 loci that allowed missing data was 87% complete and resulted in a highly resolved phylogeny with broad agreement between the Bayesian and maximum-likelihood (ML) trees. Although results from the 100% complete matrix of 416 UCE loci were similar, the Bayesian and ML trees differed to a greater extent in this analysis, suggesting that increasing from 416 to 1,541 loci led to increased stability and resolution of the tree. Novel results of our study include surprisingly close relationships between phenotypically divergent bird families, such as tropicbirds (Phaethontidae) and the sunbittern (Eurypygidae) as well as between bustards (Otididae) and turacos (Musophagidae). This phylogeny bolsters support for monophyletic waterbird and landbird clades and also strongly supports controversial results from previous studies, including the sister relationship between passerines and parrots and the non-monophyly of raptorial birds in the hawk and falcon families. Although significant challenges remain to fully resolving some of the deep relationships in Neoaves, especially among lineages outside the waterbirds and landbirds, this study suggests that increased data will yield an increasingly resolved avian phylogeny.Comment: 30 pages, 1 table, 4 figures, 1 supplementary table, 3 supplementary figure

    British digital game studies

    This paper provides a short and potted recent history of digital games research in Great Britain. We begin this story in 2001. Though a substantial amount of research and writing on digital games was taking in Britain since at least the 1980s, for us the turn of the new millennium makes a logical starting point for our recent history. This is because this was not only the year that Aarseth (2001) marked as 'year one' for 'computer game research', but it was also the year that the first major international conference on digital games took place in the UK (in Bristol), and the first time a major British government grant was awarded to undertake research on digital gaming. The paper then charts the significant role Britain played in hosting major early international gatherings of (now leading) games researchers, such as those in Bristol and also Manchester. As well as the important crop of early British authored (text)books that helped shape the direction of this new emerging discipline. What we then see is a significant growth in British digital games studies focused around a number of key events, research clusters, and publications, and the development of a particular framing of digital games within a wider social, cultural, and political context. It is this, we would suggest, which has given British digital game studies its particular flavour and also its important global role in pushing forward research, theory, and key debates

    The Velocity Distribution of the Nearest Interstellar Gas

    The bulk flow velocity for the cluster of interstellar cloudlets within about 30 pc of the Sun is determined from optical and ultraviolet absorption line data, after omitting from the sample stars with circumstellar disks or variable emission lines and the active variable HR 1099. Ninety-six velocity components towards the remaining 60 stars yield a streaming velocity through the local standard of rest of -17.0+/-4.6 km/s, with an upstream direction of l=2.3 deg, b=-5.2 deg (using Hipparcos values for the solar apex motion). The velocity dispersion of the interstellar matter (ISM) within 30 pc is consistent with that of nearby diffuse clouds, but present statistics are inadequate to distinguish between a Gaussian or exponential distribution about the bulk flow velocity. The upstream direction of the bulk flow vector suggests an origin associated with the Loop I supernova remnant. Groupings of component velocities by region are seen, indicating regional departures from the bulk flow velocity or possibly separate clouds. The absorption components from the cloudlet feeding ISM into the solar system form one of the regional features. The nominal gradient between the velocities of upstream and downstream gas may be an artifact of the Sun's location near the edge of the local cloud complex.     Losing Weight: A KECK Spectroscopic Survey of the Massive Cluster of Galaxies RX J1347-1145

    We present a sample of 47 spectroscopically confirmed members of RX J1347-1145, the most luminous X-ray cluster of galaxies discovered to date. With two exceptions, all the galaxies in this sample have red B-R colors and red spectral indices, with spectra similar to old local ellipticals. Using all 47 cluster members, we derive a mean redshift of 0.4509\pm 0.003, and a velocity dispersion of 910\pm130 km/sec, which corresponds to a virial mass of 4.4 x 10^{14} h^{-1} Solar masses with an harmonic radius of 380 h^{-1} kpc. The derived total dynamical mass is marginally consistent with that deduced from the cluster's X-ray emission based on the analysis of ROSAT/ASCA images (Schindler et al. 1997), but not consistent with the more recent X-ray analyses of Allen (2000), Ettori, Allen & Fabian (2001) and Allen, Schmidt & Fabian (2002). Furthermore, the dynamical mass is significantly smaller than that derived from weak lensing (Fischer & Tyson 1997) and from strong lensing (Sahu et al. 1998). We propose that these various discrepant mass estimates may be understood if RX J1347-1145 is the product of two clusters caught in the act of merging in a direction perpendicular to the line of sight, although there is no evidence from the galaxy redshift distribution supporting this hypothesis. Even with this hypothesis, a significant part of the extremely high X-ray luminosity must still arise from non-virialized, presumably shocked, gas. Finally, we report the serendipitous discovery of a lensed background galaxy at z=4.083 which will put strong constraints on the lensing mass determination once its counter-image is securely identified.Comment: Minor changes to conform to version accepted by Ap

    Discovery of CH and OH in the -513 km s-1 Ejecta of Eta Carinae

    The very massive star, Eta Carinae, is enshrouded in an unusual complex of stellar ejecta, which is highly depleted in C and O, and enriched in He and N. This circumstellar gas gives rise to distinct absorption components corresponding to at least 20 different velocities along the line-of-sight. The velocity component at -513 kms-1 exhibits very low ionization with predominantly neutral species of iron-peak elements. Our statistical equilibrium/photoionization modeling indicates that the low temperature (T = 760 K) and high density (n_H=10^7 cm^-3) of the -513 kms-1 component is conducive to molecule formation including those with the elements C and O. Examination of echelle spectra obtained with the Space Telescope Imaging Spectrograph (STIS) aboard the confirms the model's predictions. The molecules, H_2, CH, and most likely OH, have been identified in the -513 kms-1 absorption spectrum. This paper presents the analysis of the HST/STIS spectra with the deduced column densities for CH, OH and C I, and upper limit for CO. It is quite extraordinary to see molecular species in a cool environment at such a high velocity. The sharp molecular and ionic absorptions in this extensively CNO- processed material offers us a unique environment for studying the chemistry, dust formation processes, and nucleosynthesis in the ejected layers of a highly evolved massive star.Comment: tentatively scheduled for the ApJ 1 September 2005, v630, 1 issu

    Diplomatic Metonymy and Antithesis in 3 Henry VI

    This essay takes as its starting point the resemblance between the historical practice of diplomatic representation and the rhetorical practice of metonymy. The early modern ambassador acted as a substitute abroad for the sovereign who sent him and metonymy describes a comparable replacement – in words – of one thing by another associated thing. Yet metonyms can all too easily become confused with their referents or even come to replace them, as the sign is taken too literally for its signified, creating a kind of rivalry between representative and represented, as competing sources of authority, in a shift from relations of likeness to opposition. As 3 Henry VI points out – and as this essay argues – the metonymic characteristics of early modern ambassadorial representation made it vulnerable to this drift towards antithesis. Antithesis, the figure of opposition, governs the contentious disorder of 3 Henry VI, from the rhetorical patterning of its speeches to its structure and subject matter and politics. The Earl of Warwick’s embassy in Act 3 is no exception: it is the pivotal point around which the play’s oppositions turn. As Warwick moves from representing to replacing Edward IV, the figures that express his migration from substitution to subversion reflect on a comparable instability in European diplomatic culture.     20 cm VLA Radio-Continuum Study of M31 - Images and Point Source Catalogues

    We present a series of new high-sensitivity and high-resolution radio-continuum images of M31 at \lambda=20 cm (\nu=1.4 GHz). These new images were produced by merging archived 20 cm radio-continuum observations from the Very Large Array (VLA) telescope. Images presented here are sensitive to rms=60 \mu Jy and feature high angular resolution (<10"). A complete sample of discrete radio sources have been catalogued and analysed across 17 individual VLA projects. We identified a total of 864 unique discrete radio sources across the field of M31. One of the most prominent regions in M31 is the ring feature for which we estimated total integrated flux of 706 mJy at \lambda=20 cm. We compare here, detected sources to those listed in Gelfand et al. (2004) at \lambda=92 cm and find 118 sources in common to both surveys. The majority (61%) of these sources exhibit a spectral index of \alpha <-0.6 indicating that their emission is predominantly non-thermal in nature.
