    To the Editor, We would like to thank the authors for their interest in our paper and their comments regarding our study entitled “A review of the fixed dose use of new oral anticoagulants in obese patients: Is it really enough?” published in Anatol J Cardiol 2015; 15: 1020-9 (1). Under-representation of obese patients in the subgroups of relevant studies raises concerns about the efficacy and safety of new oral anticoagulants (NOACs). The number of patients with high body weights is quite low in studies investigating the pharmacodynamics and pharmacokinetics of NOACs. In the context of data obtained from these studies, a fixed dose use of NOACs is recommended for obese or morbidly obese patients with no distinction from other patients

    Tricuspid stenosis occurs after the implantation of a ventricular pacemaker lead and is a rare complication. An inflammatory response is stimulated when the pacemaker leads are passing through the tricuspid valve which leads to fibrosis in the long-term. In our case report, we aim to present a patient with asymptomatic multiple pacemaker lead- related tricuspid stenosis and with a history of severe COVID-19 pneumonia.Triküspit stenozu ventriküler pacemaker lead implantasyonu sonrası gelişen ve nadir görülen bir komplikasyondur.     The association of left atrial mechanics with left ventricular morphology in patients with hypertrophic cardiomyopathy: A cardiac magnetic resonance study

    Purpose: Hypertrophic cardiomyopathy (HCM) is related with structural and pathologic changes in the left atrium (LA) and left ventricle (LV). The aim of this study was to explore the association between LA mechanics and LV charac-teristics in patients with HCM using cardiac magnetic resonance feature tracking (CMR-FT). Material and methods: A total of 76 patients with HCM and 26 healthy controls were included in the study. The pa-rameters including the extent of LV late gadolinium enhancement (LGE-%) and the LV early diastolic longitudinal strain rate (edLSR) were assessed for LV. LA conduit, booster, and reservoir functions were assessed by LA fractional volumes and strain analyses using CMR-FT. HCM patients were classified as HCM patients without LGE, with mild LGE-% (0% = 10%), and prominent LGE-% (10% < LGE-%).Results: HCM patients had worse LA functions compared with the controls (p < 0.05). The majority of LA functional indices were more impaired in HCM patients with regard to LGE. LA volumes were higher in HCM patients with prominent LGE-% compared with HCM patients with mild LGE-% (p < 0.05). However, only a minority of LA functional parameters differed between the 2 groups. LA strain parameters showed weak to modest correlations with LV LGE-% and LV edLSR.Conclusions: LV characteristics, to some extent, influence LA mechanics, but they might not be the only factor induc-ing LA dysfunction in patients with HCM

    Background: The aim of the study was to examine the frequency of factor XIII polymorphism among patients with cardiac syndrome X (CSX).Methods: This study was designed as a cross-sectional and observational study. Forty-eight female patients with CSX and 36 controls matched by age, gender, diabetes, and hypertension were studied. CSX was defined as typical chest pain during rest or effort, abnormal test result for exercise ECG, and presence of angiographically normal epicardial coronary arteries after ruling out inducible spasm. Factor XIII gene polymorphism was investigated by using CVD Strip Assay (ViennaLab Diagnostic GmbH) commercial kit.Results: The frequency of factor XIII (Val/Leu + Leu/Leu) mutation was significantly higher in patients with CSX (43%) than in controls (19%) (p = 0.02). Frequency of the Leu allele was significantly higher in the patient group (23.5% vs. 11.1%, p = 0.04). Factor XIII (Val/Leu + Leu/Leu) mutation (p = 0.01, OR = 3.42; 95% CI 1.22–9.58) and smoking (p = 0.04, OR = 3.33, 95% CI 1.05–10.58) were identified as independent predictors of the disease in multivariate regression analysis.Conclusions: This study indicates that there is an evidence for association between factor XIII Val34Leu polymorphism and CSX

    DR-70: A promising biomarker for the detection of lung cancer

    Objective: Lung cancer (LC) is characterized by an aggressive phenotype with a high mortality rate, early metastasis, and proliferation rate. Treatment options and prognosis differ significantly at each stage. Despite the availability of multiple imaging studies and invasive procedures, the disorder is diagnosed at an advanced stage. Therefore, it is essential to find biomarkers for the early detection of LC. Patients and Methods: Between 2018 and 2020, 73 LC and 71 control with the same demographic characteristics were included in our study. DR-70 level was measured by a photometric method in serum samples taken from all subjects. Results: A total of 144 subjects (110 male, 34 female) was included in the study. DR-70 levels in the LC group (2.53 +/- 2.64 mu g/mL) were found to be statistically significantly higher than the control group (0.56 +/- 1.23 mu g/mL). Clinical sensitivity and specificity of DR-70 for LC were found to be 87.67% and 88.73%. Conclusions: The high sensitivity and specificity of DR-70 can be used as a biomarker for rapid diagnosis in patients with LC. Compared with other tumor biomarkers, DR-70 seems to have a better sensitivity and specificity in the diagnosis of LC

    Changes in electrocardiographic p wave parameters after cryoballoon ablation and their association with atrial fibrillation recurrence

    Background: Changes in P wave parameters after circumferential pulmonary vein isolation (CPVI) have been previously identified. In this study, we aimed to determine the changes in P wave parameters surface electrocardiogram (ECG) after cryoballoon ablation (CBA) for atrial fibrillation (AF) and evaluate their relationship with AF recurrence. Methods: Sixty-one patients (mean age 53 ± 11 years, 50.8% male) with paroxysmal AF who underwent CBA were enrolled. A surface ECG was obtained from all patients immediately before the procedure, and repeated 12 hours after the procedure. P wave amplitude (Pamp), P wave duration (Pwd), and P wave dispersion (Pdis) values in preprocedural and postprocedural ECGs were measured and compared. Recurrence rates of AF in 3, 6, and 9 months following ablation were recorded for all patients. Changes in P wave parameters were compared between patients with and without AF recurrence. Results: Compared to preprocedural measurements, Pamp (from 0.58 ± 0.18 mV at baseline to 0.48 ± 0.17 mV, P 0.05). Conclusion: Pamp, Pwd, and Pdis parameters exhibited significant decrease after CBA compared to preprocedural measurements. Decreased Pamp was shown to be a predictor for good clinical outcomes following CBA
