Factors associated with medication adherence in school-aged children with asthma
Adherence to preventive asthma treatment is poor, particularly in children, yet the factors associated with adherence in this age group are not well understood. Adherence was monitored electronically over 6 months in school-aged children who attended a regional emergency department in New Zealand for an asthma exacerbation and were prescribed twice-daily inhaled corticosteroids. Participants completed questionnaires including assessment of family demographics, asthma responsibility and learning style. Multivariable analysis of factors associated with adherence was conducted. 101 children (mean (range) age 8.9 (6-15) years, 51% male) participated. Median (interquartile range) preventer adherence was 30% (17-48%) of prescribed. Four explanatory factors were identified: female sex (+12% adherence), Asian ethnicity (+19% adherence), living in a smaller household (-3.0% adherence per person in the household), and younger age at diagnosis (+2.7% for every younger year of diagnosis) (all p<0.02). In school-aged children attending the emergency department for asthma, males and non-Asian ethnic groups were at high risk for poor inhaled corticosteroid adherence and may benefit most from intervention. Four factors explained a small proportion of adherence behaviour indicating the difficulty in identifying adherence barriers. Further research is recommended in other similar populations
Chromatic Illumination Discrimination Ability Reveals that Human Colour Constancy Is Optimised for Blue Daylight Illuminations
The phenomenon of colour constancy in human visual perception keeps surface colours constant, despite changes in their reflected light due to changing illumination. Although colour constancy has evolved under a constrained subset of illuminations, it is unknown whether its underlying mechanisms, thought to involve multiple components from retina to cortex, are optimised for particular environmental variations. Here we demonstrate a new method for investigating colour constancy using illumination matching in real scenes which, unlike previous methods using surface matching and simulated scenes, allows testing of multiple, real illuminations. We use real scenes consisting of solid familiar or unfamiliar objects against uniform or variegated backgrounds and compare discrimination performance for typical illuminations from the daylight chromaticity locus (approximately blue-yellow) and atypical spectra from an orthogonal locus (approximately red-green, at correlated colour temperature 6700 K), all produced in real time by a 10-channel LED illuminator. We find that discrimination of illumination changes is poorer along the daylight locus than the atypical locus, and is poorest particularly for bluer illumination changes, demonstrating conversely that surface colour constancy is best for blue daylight illuminations. Illumination discrimination is also enhanced, and therefore colour constancy diminished, for uniform backgrounds, irrespective of the object type. These results are not explained by statistical properties of the scene signal changes at the retinal level. We conclude that high-level mechanisms of colour constancy are biased for the blue daylight illuminations and variegated backgrounds to which the human visual system has typically been exposed
Curriculum Infusion Through Case Studies: Engaging Undergraduate Students In Course Subject Material and Influencing Behavior Change
This study investigated infusing health promotion topics into an engineering course via problem-based case studies and lecture to assess student learning and self-reported behavior. Junior-level systems engineering students in two sections participated: one section with 52 students and one with 36. One section received a celebratory drinking case; one received distracted driving case and a lecture about hazardous drinking. Student ability ratings related to the course subject matter generally improved with both cases. The lecture appeared to enhance health promotion knowledge. Students self-reported behavior change with both cases. Case studies as a form of curriculum infusion for health promotion topics show promise. The use of case studies overall was well-received by students and coupled with lecture material can increase student health promotion knowledge and behavior change
Optical Coherence Tomography in the UK Biobank Study - Rapid Automated Analysis of Retinal Thickness for Large Population-Based Studies
PURPOSE:
To describe an approach to the use of optical coherence tomography (OCT) imaging in large, population-based studies, including methods for OCT image acquisition, storage, and the remote, rapid, automated analysis of retinal thickness.
METHODS:
In UK Biobank, OCT images were acquired between 2009 and 2010 using a commercially available “spectral domain” OCT device (3D OCT-1000, Topcon). Images were obtained using a raster scan protocol, 6 mm x 6 mm in area, and consisting of 128 B-scans. OCT image sets were stored on UK Biobank servers in a central repository, adjacent to high performance computers. Rapid, automated analysis of retinal thickness was performed using custom image segmentation software developed by the Topcon Advanced Biomedical Imaging Laboratory (TABIL). This software employs dual-scale gradient information to allow for automated segmentation of nine intraretinal boundaries in a rapid fashion.
RESULTS:
67,321 participants (134,642 eyes) in UK Biobank underwent OCT imaging of both eyes as part of the ocular module. 134,611 images were successfully processed with 31 images failing segmentation analysis due to corrupted OCT files or withdrawal of subject consent for UKBB study participation. Average time taken to call up an image from the database and complete segmentation analysis was approximately 120 seconds per data set per login, and analysis of the entire dataset was completed in approximately 28 days.
CONCLUSIONS:
We report an approach to the rapid, automated measurement of retinal thickness from nearly 140,000 OCT image sets from the UK Biobank. In the near future, these measurements will be publically available for utilization by researchers around the world, and thus for correlation with the wealth of other data collected in UK Biobank. The automated analysis approaches we describe may be of utility for future large population-based epidemiological studies, clinical trials, and screening programs that employ OCT imaging
Asymptomatic Clostridium difficile colonization in two Australian tertiary hospitals, 2012-2014: prospective, repeated cross-sectional study.
OBJECTIVES: To investigate the prevalence and risk factors for asymptomatic toxigenic (TCD) and nontoxigenic Clostridium difficile (NTCD) colonization in a broad cross section of the general hospital population over a 3-year period. METHODS: Patients without diarrhoea admitted to two Australian tertiary hospitals were randomly selected through six repeated cross-sectional surveys conducted between 2012 and 2014. Stool specimens were cultured under anaerobic conditions, and C. difficile isolates were tested for the presence of toxin genes and ribotyped. Patients were then grouped into noncolonized, TCD colonized or NTCD colonized for identifying risk factors using multinomial logistic regression models. RESULTS: A total of 1380 asymptomatic patients were enrolled; 76 patients (5.5%) were TCD colonized and 28 (2.0%) were NTCD colonized. There was a decreasing annual trend in TCD colonization, and asymptomatic colonization was more prevalent during the summer than winter months. TCD colonization was associated with gastro-oesophageal reflux disease (relative risk ratio (RRR) = 2.20; 95% confidence interval (CI) 1.17-4.14), higher number of admissions in the previous year (RRR = 1.24; 95% CI 1.10-1.39) and antimicrobial exposure during the current admission (RRR = 2.78; 95% CI 1.23-6.28). NTCD colonization was associated with chronic obstructive pulmonary disease (RRR = 3.88; 95% CI 1.66-9.07) and chronic kidney failure (RRR = 5.78; 95% CI 2.29-14.59). Forty-eight different ribotypes were identified, with 014/020 (n = 23), 018 (n = 10) and 056 (n = 6) being the most commonly isolated. CONCLUSIONS: Risk factors differ between patients with asymptomatic colonization by toxigenic and nontoxigenic strains. Given that morbidity is largely driven by toxigenic strains, this novel finding has important implications for disease control and prevention
Large CP Violation in B_s Meson Mixing with EDM constraint in Supersymmetry
Motivated by the recent measurement of the like-sign dimuon charge asymmetry,
we investigate anomalous CP violation in the B_s- bar{B}_s mixing within the
supersymmetry. We show that when gluino diagrams dominate supersymmetry
contributions, it is very difficult to realize a large B_s- bar{B}_s mixing
phase under the constraint from electric dipole moments barring cancellations.
This constraint can be ameliorated by supposing superparticles decoupled. In
this limit, we find that it is possible to achieve the large CP asymmetry, and
the branching ratio of B_s -> mu^+ mu^- tends to become sizable.Comment: 20 pages, 5 figure

Modeling brittle fracture, slip weakening, and variable friction in geomaterials with an embedded strong discontinuity finite element.
Localized shear deformation plays an important role in a number of geotechnical and geological processes. Slope failures, the formation and propagation of faults, cracking in concrete dams, and shear fractures in subsiding hydrocarbon reservoirs are examples of important effects of shear localization. Traditional engineering analyses of these phenomena, such as limit equilibrium techniques, make certain assumptions on the shape of the failure surface as well as other simplifications. While these methods may be adequate for the applications for which they were designed, it is difficult to extrapolate the results to more general scenarios. An alternative approach is to use a numerical modeling technique, such as the finite element method, to predict localization. While standard finite elements can model a wide variety of loading situations and geometries quite well, for numerical reasons they have difficulty capturing the softening and anisotropic damage that accompanies localization. By introducing an enhancement to the element in the form of a fracture surface at an arbitrary position and orientation in the element, we can regularize the solution, model the weakening response, and track the relative motion of the surfaces. To properly model the slip along these surfaces, the traction-displacement response must be properly captured. This report focuses on the development of a constitutive model appropriate to localizing geomaterials, and the embedding of this model into the enhanced finite element framework. This modeling covers two distinct phases. The first, usually brief, phase is the weakening response as the material transitions from intact continuum to a body with a cohesionless fractured surface. Once the cohesion has been eliminated, the response along the surface is completely frictional. We have focused on a rate- and state-dependent frictional model that captures stable and unstable slip along the surface. This model is embedded numerically into the element using a generalized trapezoidal formulation. While the focus is on the constitutive model of interest, the framework is also developed for a general surface response. This report summarizes the major research and development accomplishments for the LDRD project titled 'Cohesive Zone Modeling of Failure in Geomaterials: Formulation and Implementation of a Strong Discontinuity Model Incorporating the Effect of Slip Speed on Frictional Resistance'. This project supported a strategic partnership between Sandia National Laboratories and Stanford University by providing funding for the lead author, Craig Foster, during his doctoral research
