A Novel Factorized Sparse Approximate Inverse Preconditioner with Supernodes
AbstractKrylov methods preconditioned by Factorized Sparse Approximate Inverses (FSAI) are an efficient approach for the solution of symmetric positive definite linear systems on massively parallel computers. However, FSAI often suffers from a high set-up cost, especially in ill-conditioned problems. In this communication we propose a novel algorithm for the FSAI computation that makes use of the concept of supernode borrowed from sparse LU factorizations and direct methods
Finite element analysis of land subsidence above depleted reservoirs with pore pressure gradient and total stress formulations
SUMMARY The solution of the poroelastic equations for predicting land subsidence above productive gas/oil "elds may be addressed by the principle of virtual works using either the e!ective intergranular stress, with the pore pressure gradient regarded as a distributed body force, or the total stress incorporating the pore pressure. In the "nite element (FE) method both approaches prove equivalent at the global assembled level. However, at the element level apparently the equivalence does not hold, and the strength source related to the pore pressure seems to generate di!erent local forces on the element nodes. The two formulations are brie#y reviewed and discussed for triangular and tetrahedral "nite elements. They are shown to yield di!erent results at the global level as well in a three-dimensional axisymmetric porous medium if the FE integration is performed using the average element-wise radius. A modi"cation to both formulations is suggested which allows to correctly solve the problem of a "nite reservoir with an in"nite pressure gradient, i.e. with a pore pressure discontinuity on its boundary
Modelling ground rupture due to groundwater withdrawal: applications to test cases in China and Mexico
The stress variation induced by aquifer overdraft in sedimentary basins with shallow bedrock may
cause rupture in the form of pre-existing fault activation or earth fissure generation. The process is causing major
detrimental effects on a many areas in China and Mexico. Ruptures yield discontinuity in both displacement
and stress field that classic continuous finite element (FE) models cannot address. Interface finite elements (IE),
typically used in contact mechanics, may be of great help and are implemented herein to simulate the fault geomechanical
behaviour. Two main approaches, i.e. Penalty and Lagrangian, are developed to enforce the contact
condition on the element interface. The incorporation of IE incorporation into a three-dimensional (3-D) FE
geomechanical simulator shows that the Lagrangian approach is numerically more robust and stable than the
Penalty, thus providing more reliable solutions. Furthermore, the use of a Newton-Raphson scheme to deal with
the non-linear elasto-plastic fault behaviour allows for quadratic convergence. The FE – IE model is applied to
investigate the likely ground rupture in realistic 3-D geologic settings. The case studies are representative of the
City of Wuxi in the Jiangsu Province (China), and of the City of Queretaro, Mexico, where significant land subsidence
has been accompanied by the generation of several earth fissures jeopardizing the stability and integrity
of the overland structures and infrastructure
Osteomalacia: The Missing Link in the Pathogenesis of Bisphosphonate-Related Osteonecrosis of the Jaws?
Background. Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is a well-documented adverse event from treatment with nitrogen-containing bisphosphonates (NBPs). During a preliminary histomorphometric study aimed at assessing the rate of bone remodeling in the jaws of patient with surgically resected BRONJ, we found a defect of bone mineralization (unpublished data). We hypothesized that osteomalacia could be a risk factor for BRONJ in patients taking NBPs. Therefore, we looked for static and dynamic histomorphometric evidence of osteomalacia in biopsies from subjects with and without BRONJ.Methods. This case-control study used histomorphometric analysis of bone specimens of patients using NBPs (22 patients with BRONJ and 21 patients without BRONJ) who required oral surgical interventions for the treatment/prevention of osteonecrosis. Patients were given tetracycline hydrochloride according to a standardized protocol before taking bone biopsies from their jaws. Biopsies with evidence of osteomyelitis or necrosis at histology were excluded from the study. Osteomalacia was defined as a mineralization lag time >100 days, a corrected mean osteoid thickness >12.5 mm, and an osteoid volume >10%.Results. In all, 77% of patients with BRONJ were osteomalacic compared with 5% of patients without BRONJ, according to histomorphometry. Because osteomalacia was found almost exclusively in NBP users with BRONJ, this is likely to be a generalized process in which the use of NBPs further deteriorates mechanisms of bone repair.Conclusions. Osteomalacia represents a new and previously unreported risk factor for disease development. This finding may contribute to a better understanding of the pathogenesis of this disease and help with the development of strategies to increase the safety of NBP administration
Social cognition in people with schizophrenia: A cluster-analytic approach
Background The study aimed to subtype patients with schizophrenia on the basis of social cognition (SC), and to identify cut-offs that best discriminate among subtypes in 809 out-patients recruited in the context of the Italian Network for Research on Psychoses. Method A two-step cluster analysis of The Awareness of Social Inference Test (TASIT), the Facial Emotion Identification Test and Mayer-Salovey-Caruso Emotional Intelligence Test scores was performed. Classification and regression tree analysis was used to identify the cut-offs of variables that best discriminated among clusters. Results We identified three clusters, characterized by unimpaired (42%), impaired (50.4%) and very impaired (7.5%) SC. Three theory-of-mind domains were more important for the cluster definition as compared with emotion perception and emotional intelligence. Patients more able to understand simple sarcasm (14 for TASIT-SS) were very likely to belong to the unimpaired SC cluster. Compared with patients in the impaired SC cluster, those in the very impaired SC cluster performed significantly worse in lie scenes (TASIT-LI <10), but not in simple sarcasm. Moreover, functioning, neurocognition, disorganization and SC had a linear relationship across the three clusters, while positive symptoms were significantly lower in patients with unimpaired SC as compared with patients with impaired and very impaired SC. On the other hand, negative symptoms were highest in patients with impaired levels of SC. Conclusions If replicated, the identification of such subtypes in clinical practice may help in tailoring rehabilitation efforts to the person's strengths to gain more benefit to the person

Long-term, Real-life, Observational Study in Treating Outpatient Ulcerative Colitis with Golimumab
Background and Aims: Several studies have found Golimumab (GOL) effective and safe in the short-term treatment of ulcerative colitis (UC), but few long-term data are currently available from real world. Our aim was to assess the long-term real-life efficacy and safety of GOL in managing UC outpatients in Italy.
Methods: A retrospective multicenter study assessing consecutive UC outpatients treated with GOL for at least 3-month of follow-up was made. Primary endpoints were the induction and maintenance of remission in UC, defined as Mayo score ≤2. Several secondary endpoints, including clinical response, colectomy rate, steroid free remission and mucosal healing, were also assessed during the follow-up.
Results: One hundred and seventy-eight patients were enrolled and followed up for a median (IQR) time of 9 (3-18) months (mean time follow-up: 33.1±13 months). Clinical remission was achieved in 57 (32.1%) patients: these patients continued with GOL, but only 6 patients (3.4%) were still under clinical remission with GOL at the 42nd month of follow-up. Clinical response occurred in 64 (36.4%) patients; colectomy was performed in 8 (7.8%) patients, all of them having primary failure. Steroid-free remission occurred in 23 (12.9%) patients, and mucosal healing was achieved in 29/89 (32.6%) patients. Adverse events occurred in 14 (7.9%) patients.
Conclusions: Golimumab does not seem able to maintain long-term remission in UC in real life. The safety profile was good
