    Cross talk between serum Kisspeptin-Leptin during assisted reproduction techniques

    Background & Objective: Leptin facilitates onset of puberty by impact on hypothalamic Kisspeptin, gonadotropin releasing hormone, follicle stimulating and luteinizing hormone. The link of peripheral Leptin-Kisspeptin in regulating the ovarian and endometrial tissue in relation to adiposity is unknown. Therefore, we wanted to identify Kisspeptin-Leptin association with body mass index (BMI) and success of assisted reproductive treatments (ART) in infertile females.Methods: A cross sectional study was carried from August 2014 till May 2016 after receiving ethical approval at Australian Concept Infertility Medical Centre, and Aga Khan University. The study group comprised of females with an age range of 25-37 year who had duration of unexplained infertility for more than two years. They were grouped as; underweight (/m2), normal weight (18-22.9 kg/m2), overweight 23-24.99 kg/m2 and obese (\u3e25 kg/m2). Kisspeptin and Leptin levels were measured by enzyme linked immune sorbent assay before down regulation of ovaries and initiation of treatment protocol of ART. Failure of procedure was detected by beta human chorionic gonadotropin \u3c25mIU/ml (non-pregnant) whereas females with levels \u3e25mIU/ml and cardiac activity on trans-vaginal scan were declared pregnant.Results: Highest Kisspeptin and Leptin levels were seen in normal weight group (374.80 ± 185.08ng/L; 12.78 ± 6.8 pg/ml) respectively, yet the highest number of clinical pregnancy was observed in overweight group (42%).A strong correlation of Kisspeptin with Leptin (r=0.794, p=0.001) was observed in the overweight females.Conclusion: Leptin-Kisspeptin-fertility link is expressed by maximum number of clinical pregnancies in the female group that showed strongest relationship between serum Leptin and Kisspeptin levels, irrespective of their BMI

In this paper we conduct the systematic mapping review to know more about security aspects in HIS. Our study provides a comprehensive review of the literature on the evaluation and implementation of HIS security, detailing the challenges and recommendations for implementers and adopters alike.     A RESEARCH STUDY TO DETERMINE THE CAUSE OF MEASLES AND RELATED PROTECTIVE MECHANISMS ALONG WITH RELATED CONSEQUENCES

    Background: Measles is a disorder commonly present in children. It can be protected by the proper use of vaccination. It can easily transfer from one person to another. It mostly occurred in progressing countries like Pakistan. It can even cause deaths because of its severity. Objective: The reasons causing the measles, protective mechanisms of the patients, its severe reactions and consequences were noticed. Material and Methods: We carried out this research at Jinnah Hospital, Lahore (March 2017 to February 2018). Total of 166 patients showing the symptoms of measles were admitted in the hospital. Different factors like age, gender, the standard of livelihood, the ability of self-protection and quality of food taken by the patients were completely observed. The severe conditions in measles can cause pneumonia, gastroenteritis, encephalitis, otitis media, condition after measles, dysentery and heart issues. The consequences of the patients were examined by assessing the conditions of liberation from the clinics, leave towards clinical prescription, referral and mortalities of patients. The data was examined by using SPSS. Results: Out of 166 patients 93 were men and 73 were women. The average age of the patients were (3.72 ± 2.7) years. 117 patients belong to village area and 59 belong to city areas. 150 patients were already immunized by vaccination however 9 patients were not undergone vaccination. 59 patients gained nutritious and high-quality food. But most of the patients about 107 gained a less nutritious food. Pneumonia was observed in 72 cases. Gastroenteritis was examined in 37 patients. Encephalitis was recognized in 5 patients. Post measles state was recorded in 11 cases. Most of the patients were cured completely and discharged from the hospital. While mortality was observed in 9 patients. Conclusion: Measles was mainly observed in children having the age of 1 to 5 years. These children were mostly not vaccinated. Most of the patients gaining poor nutrition were attacked by measles.     To study the various types of breastcancer presenting in local hospital to ensure betterfacilities, early diagnosis and better treatmentoptionsMethods: In this descriptive study all breast cancerpatients, over a period of two years were included.Triple assessment of patients done for diagnosis andtreatment given all were included in data.Results: Total number of patients who presentedwere 1982. Benign breast disease patients were 1746(88.0%). Breast cancer were 236(11.90%). Fivehundred and two mammographies were done, 197were BIRAD 5. Preferably confirmation was donethrough trucut biopsy (35), FNAC was done in 74and where needed incisional biopsy in fungatingtumors (n=15) and excisional biopsy where in spiteof all modalities diagnosis was not confirmed (n=11). Age varied from 24 to 75 years. Majority (126)were in stage 3. Forty two patients receivedneoadjuvant chemotherapy. Modified radicalmastectomy (n=114), Breast conservation ( n=13) andsimple mastectomy (n=17) were performed .Receptorstatus was dtermined.Conclusion: The most common stage ofpresentation is stage 3. Such patients need downstaging and then surgery. They required propercounseling and support for their treatment. Femalesusually present late due to domestic issues. Earlydiagnosis, treatment and full support is required totreat breast cancer. This study will help to providebetter facilities for early diagnosis and treatment

    Psychological Morbidity in patients with Idiopathic Hirsutism presenting for Laser Treatment at a Tertiary Care Hospital

    Objective: The objective of our study was to determine the frequency of depression, anxiety, and stress in females presenting for laser treatment of hirsutism at a dermatology clinic. Materials and Methods: This was a cross-sectional descriptive study. 152 female participants of all ages presenting for laser treatment of hirsutism to the dermatology department –removed for blind review--from June 2019 to Dec 2019, were included using a consecutive convenient sampling technique. Patients having an FG (Ferriman and Gallwey) score of 8 or more, were included whereas those having any underlying cause (as assessed by history and examination and laboratory investigations where required) for both hirsutism and psychological disorder were excluded. The participants were given an Urdu version of the DASS 21 (depression, anxiety, and stress scale) to complete. Results: The participants had a mean age of 30.22 (SD = 9.17). The prevalence of depression, anxiety, and stress in the sample was found to be 13.2%, 23.7%, and 17.8%, respectively. Mean depression, anxiety, and stress scores on DASS 21 were 4.43(SD=4.87, Cut-off= 10), 4.34(SD=5.12, Cut-off= 8), and 8.21(SD=6.26, Cut-off=15), respectively, indicating that the scores were within the normal range. Anxiety was the most common illness in the sample and both depression and stress showed a decline with advancing age up till 45years of age. No statistically significant association was found between the severity of hirsutism and depression, stress, and anxiety scores. Conclusion: Psychological morbidity was found to be lower in patients of idiopathic hirsutism presenting for laser treatment. The long-term efficacy of laser treatment needs further evaluation in future research

    Impact Computation of electric and magnetic field on farmers working near high voltage transmission lines in Pakistan by developing software

    In this revolutionized era, electricity has become an essential necessity of humans. People all over the world are concerned about the effects of HVTL (high voltage transmission lines) and their severe effects impacting on their health. Over the last two decades, developed countries have become increasingly concerned about an increase in electromagnetic fields, which poses health risks. The electromagnetic field emitted by HVTL will cause a low current to pass through the bodies. The electricity system produces low frequency electromagnetic field, which lead to the health hazards. The critical problem to those who are living near HVTL, therefore our fundamental task is to let them aware of the risk. Due to harmful exposures of electric and magnetic field, farmers working under HVTL or having their residences, suffering through severe effects in the form of different diseases. The main aim behind this research is to design GUI based software having two parts which helps to bring output of the electromagnetic field exposures with the help of various parameters includes different weather conditions affecting electric field, how people of different ages are being affected through it, identification of the regions need further protection by developing a proper mathematical model. Two approaches were used, one is to interface GUI with different sensors and second is user define function. On the basis of output EMF it will be able to find out the possible disease by using the data of WHO, ICNIRP and the data which is collected from surveys of different regions
