    Molecular dynamics simulations of aqueous urea solutions: Study of dimer stability and solution structure, and calculation of the total nitrogen radial distribution function GN(r

    Molecular dynamics simulations have been performed in order to study the structure of two molal urea solutions in D2O. Several initial dimer configurations were considered for an adequate sampling of phase space. Eventually all of them appeared to be unstable, when system size and periodic boundary conditions are chosen properly, even after a very careful equilibration. The total nitrogen scattering function GN(r), calculated from these simulations, is in good agreement with neutron scattering experiments when both intra- and intermolecular correlations are considered and the experimental truncation ripples are introduced by a Fourier transform of GN(r) back and forth. The simple pair potential model that we used gives results in good agreement with experiments and with a much more involved potential model, recently described in the literature [J. Chem.     A paper read at a WHO/UNICEF Seminar on "Anemia and the Use of Blood Transfusion in M.C.H. Work" 6th June, 1967. Kampala. Coagulation defects are only rarely met with in obstetrics. It is probable that the complication is nowadays being diagnosed with increasing frequency, owing mainly to a greater awareness of the condition. In Mulago Hospital where the author practises, it occurred 11 times in about 33,500 deliveries in the period 1964-66. This gives an incidence of about 0.03%. In cases of concealed accidental hemorrhage the incidence is however higher; it appears to occur in some 5% of cases. A historical background of this condition is presented, and a description of how the clotting mechanism takes place is also explained. Furthermore, a detailed diagram describes the mechanism by which coagulation failure is produced. The author analysed cases at Mulago Hospital, indicating the relative clinical diagnosis and treatment, the latter consisting of two forms, preventive and curative.peer-reviewe

    A rewriting grammar for heat exchanger network structure evolution with stream splitting

    The design of cost optimal heat exchanger networks is a difficult optimisation problem due both to the nonlinear models required and also the combinatorial size of the search space. When stream splitting is considered, the combinatorial aspects make the problem even harder. This paper describes the implementation of a two level evolutionary algorithm based on a string rewriting grammar for the evolution of the heat exchanger network structure. A biological analogue of genotypes and phenotypes is used to describe structures and specific solutions respectively. The top level algorithm evolves structures while the lower level optimises specific structures. The result is a hybrid optimisation procedure which can identify the best structures including stream splitting. Case studies from the literature are presented to demonstrate the capabilities of the novel procedure

    Perspective on the economic evaluation of deep brain stimulation

    Parkinson’s disease (PD) is an example of a disease area experiencing increasing use of deep brain stimulation (DBS) to treat symptoms. PD is a major cause of morbidity and has a substantial economic impact on the patients, their caregivers, the health service, and broader social and community services. The PDSURG Collaborators Group reported that DBS surgery for patients with advanced PD improves motor function and quality of life that medical therapy alone at 1 year but there are surgery related side effects in a minority (Williams et al., 2010). The aim of this paper however is to build upon the knowledge generated from evaluating DBS in PD and to provide a detailed perspective on the economic evaluation of DBS more generally with a view to providing a framework for informative design of DBS economic evaluations. This perspective will outline the key categories of resource use pertinent to DBS beyond the surgical scenario and into the broader aspects of follow-up care, adverse events, repeat procedures, social and community care, patient and carer costs, and will explore the importance of handling capital costs of DBS equipment appropriately as well as including costs occurring in the future. In addition, this perspective article will outline the importance of capturing broader aspects of “outcome” or benefits as compared to those traditional clinical measures used. The key message is the importance of employing a broad “perspective” on the measurement and valuation of costs and benefits as well as the importance of adopting the appropriate time horizon for evaluating the costs and benefits of DBS. In order to do this effectively it may be that alternative methods of economic evaluation in health care to the commonly used cost-effectiveness analysis may have to be used, such as cost-benefit analysis (McIntosh et al., 2010)
