Low-energy fractures — a new complication of type 2 diabetes?
W świetle wzrastającego zagrożenia złamaniami wśród kobiet po menopauzie wskazana jest kalkulacja ryzyka złamania także wśród starszych chorych z cukrzycą typu 2.Low bone mineral density has traditionally been linked with type 1 diabetes. Latest research has confirmed that type 2 diabetics have increased fracture risk, whereas their bone mineral density is normal or increased when compared to healthy peers. It seems the assessment of bone mineral density using DXA (dual X-ray absorptiometry) does not reflect the complexity of bone pathology in diabetes, which has recently been the subject of debate. Increase in fracture risk is probably related to i.a. disease duration, metabolic disease, presence of complications, type of therapy, the increased susceptibility to falls. The underlying mechanism whereby diabetes type 2 exerts its detrimental effect on bone health is still not clear and often described as poor bone quality. This might be due to the accumulation of advanced glycation end products in bone, abnormal collagen cross-linking, accompanied by low rate of bone turnover. It was also shown that patients with diabetes have impaired structural and geometrical bone parameters. In light of increasing fracture incidence in postmenopausal women, fracture risk calculation is indicated also among older patients with type 2 diabetes
Metabolic syndrome and bone
mineralną kości a dużym ryzykiem złamania niskoenergetycznego u chorych z cukrzycą typu 2. W pracy omówiono doniesienia i kontrowersje
związane ze współistnieniem i wzajemnymi interakcjami między elementami zespołu metabolicznego a osteoporozą i jej powikłaniami. In recent years an increasing socioeconomic burden of two pressing problems has been observed: enormous prevalence of obesity and
- along with aging of populations - great number of people affected with osteoporosis. Visceral obesity, expressed by increased waist
circumference, is according to the latest guidelines a crucial component of metabolic syndrome (MS). So far the two entities have not been
linked, as studies have proven beneficial effect of increased body weight on bone mineral density (BMD). It has been shown however, that
adipose tissue is actually an endocrine tissue, secreting numerous biologically active substances that influence also bone. Moreover, an
adipocyte and osteoblast derive from a common precursor cell. There is also increasing number of evidence showing common genetic
determination of the diseases. Data from experimental and epidemiological studies proves that obesity itself as well as remaining components
of MS might have negative influence on bone. BMD as well as fracture risk have been shown to be raised in patients diagnosed with
MS. The same has been observed for high blood pressure. What’s more, contradiction between high BMD and - simultaneously - high
fracture risk in subjects with type 2 diabetes has not been convincingly explained. The paper discusses reports and controversies on
coexistence and interactions between MS and osteoporosis and its complications
Pregnancy, lactation and bone mineral density
Abstract Evidence shows that pregnancy and lactation lead to the decrease of bone mineral density (BMD), which is related to mobilization of skeletal calcium. Numeorous mechanisms are involved in maternal adaptation to the increased demand for calcium. BMD has been proved to return to the baseline level after weaning. Pregnancies and long or repeated periods of feeding do not seem to determine the diagnosis of osteoporosis in later life. In fact, women with multiple pregnancies have been shown to have the same or higher BMD and lower fracture risk compared with nulliparous women. Pregnancy and lactation-associated osteoporosis (PLO) is a rare disease entity. It manifests itself as low back pain, and often leads to a number of vertebral compression fractures and, therefore, to potentially serious health consequences
Stimulating preventive procedures in primary care : effect of PIUPOZ program on the delivery of preventive procedures
Introduction: Educational meetings are one of the most frequently used strategies to change doctors' professional behavior; however, their effectiveness as a single intervention is limited. This study evaluated the effect of a multifactorial intervention, based on interactive workshops, on the GPs' knowledge and the delivery rates of preventive procedures in primary care.
Material and methods: The study population comprised 106 GPs working in the Wielkopolska region recruited to the PIUPOZ program (Improving Quality in Primary Care). The intervention in the program consisted of lectures, interactive workshops and an audit, before and three months after the training. Trained medical students directly observed GPs to register which of 12 studied preventive procedures were performed during the consultation in patients aged 40+.
Results: A total of 1060 consultations were recorded, during which 4899 preventive procedures were delivered: 2115 before and 2784 after workshops. The mean number of preventive procedures per patient before and after workshops was 3.84 and 5.25 respectively (p < 0.0001). The most commonly performed preventive procedures were blood pressure, blood glucose and lipid profile measurement. Mean number of correct answers for 16 questions in the initial knowledge test was 8.7 and 12.7 in the final test (p < 0.0001).
Conclusions: The observed number of delivered preventive procedures was below the recommended range. Preventive procedures based on laboratory tests were performed more often than lifestyle counseling
A discussion of the intervention thresholds in osteoporosis treatment in Poland
Introduction: Epidemiological prognoses regarding the global spread of post-menopausal osteoporosis can prove somewhat nebulous.
But it is clear that low-energy fractures and their consequences will become an increasingly serious health problem. Therefore it is crucial
to implement prognostic procedures which could more effectively predict the incidence of osteoporosis and its complications.
Material and methods: The study involved 378 female patients aged 40–86 years for whom clinical risk factors of osteoporotic fracture
were analysed. Densitometry (DPX) was performed at femoral neck. The 10–year risk of fracture was assessed according to the British
model of FRAX calculator.
Results: The study group was divided into two, depending on the history of low-energy fractures. Previous osteoporotic fractures were
confirmed in 128 patients. In this group, the mean bone mineral density (BMD) values (0.717 g/cm2) were lower than in the group without
fracture history (0.735 g/cm2). In 33.3% of patients aged 50-59 years and 17% of women aged 60-79 who required medical treatment for
their clinical status (previous fracture), the FRAX value did not meet the criterion of pharmacotherapy administration. Considering BMD
in the calculation of FRAX produced an even higher underestimation of the fracture risk. Of women aged 40-49, 25% were qualified for
pharmacotherapy of osteoporosis. In that particular age category, BMD did not affect the FRAX value. BMD measurement had a higher
discriminatory value among patients aged 50-79, increasing the number of patients requiring therapy by more than 50%.
Conclusions:
1. The FRAX calculator does not always consider the history of low-energy fractures as a criterion sufficient for therapy implementation.
2. Designing a FRAX calculator specifically for the Polish population would be advisable.
(Pol J Endocrinol 2011; 62 (1): 30-36)Wstęp: Prognozy epidemiologiczne dotyczące rozprzestrzenienia osteoporozy pomenopauzalnej na świecie są niepokojące. W związku
z tym konsekwencje złamań niskoenergetycznych będą stanowiły coraz większy problem zdrowotny społeczeństw. Konieczne staje się
zatem wdrożenie postępowań, które będą skuteczniej przewidywały występowanie osteoporozy pomenopauzalnej i powikłań choroby.
Materiał i metody: Występowanie klinicznych czynników złamania oceniano w grupie 378 pacjentek w wieku 40-86 lat. Dodatkowo
u wszystkich kobiet przeprowadzono badanie densytometryczne bliższego końca kości udowej. W celu obliczenia 10-letniego ryzyka

Evaluation of plasminogen activator inhibitor 1 concentration in patients with simple obesity
WSTĘP. Związek otyłości z rozwojem miażdżycy i jej klinicznymi
antropometrycznymi i biochemicznymi.
MATERIAŁ I METODY. Do badania włączono 88 pacjentów z otyłością
prostą oraz 25 zdrowych ochotników jako grupę kontrolną.
U wszystkich wykonano pomiary antropometryczne, oceniono stężenia:
glukozy na czczo, parametrów gospodarki lipidowej, insuliny
(metodą radioimmunometryczną), czynnika martwicy nowotworów
alfa (TNFa) oraz PAI-1 (metodą immunoenzymatyczną).
Stopień insulinooporności wyliczono na podstawie wskaźnika
HOMA-IR.
WYNIKI. Stężenia TNFa, PAI-1 oraz wartości wskaźnika HOMA-IR
były znamiennie większe w grupie pacjentów z otyłością niż w grupie
kontrolnej. Stwierdzono dodatnią korelację między stężeniami
insuliny oraz wartościami HOMA-IR a PAI-1.
WNIOSKI.
1. Otyłych pacjentów charakteryzują zwiększona aktywacja procesu
zapalnego, predyspozycja prozakrzepowa oraz insulinooporność.
2. W patogenezie zwiększonej aktywności prozakrzepowej osocza
u otyłych pacjentów, wyrażonej w badaniu autorów zwiększonymi
stężeniami PAI-1, należy rozważyć zjawisko insulinooporności.INTRODUCTION. Relationship between obesity and atherosclerosis
development is well-documented. Among risk factors of atherosclerosis
progression are both „old” and „new” — which clinical
value is still intensively studied. In the pathogenesis of increased
procoagulat state observed in obese patients potential role of plasminogen
activator inhibitor 1 (PAI-1) is analysed.
The aim of the study was to evaluate PAI-1 concentration and its
relationship with selected anthropometric and biochemical parameters.
MATERIAL AND METHODS. 88 obese patients and 25 healthy subjects
as a control were studied. Anthropometric parameters, concentrations
of: fasting glucose, lipids, insulin with use of radioimmunomethric method, tumor necrosis factor alpha (TNFa) and PAI-1
with use of immunoenzymatic method were assessed. Insulin resistance
in the participants was evaluated according to the homeostasis
model assessment–insulin resistance (HOMA-IR) protocol.
RESULTS. Serum concentrations TNFa, PAI-1 and HOMA-IR index
were significantly higher in the studied group as compared to
the control. Positive correlation between insulin concentration,
HOMA-IR and PAI-1 were found.
CONCLUSIONS.
1. Obese patients characterise increased activation of inflammatory
process, procoagulant predisposition and insulin resistance.
2. In the pathogenesis of increased procoagulant activity in obese
patients, observed in our study as elevated PAI-1 concentration,
insulin resistance role should be considered
Ghrelin and its relationship with insulinemia during OGTT in healthy people
i insulinemią. Celem pracy była ocena stężenia greliny całkowitej
oraz poszukiwanie związku między insulinemią a grelinemią
w odpowiedzi na doustne podanie glukozy u osób zdrowych.
MATERIAŁ I METODY. W badaniu udział wzięło 25 zdrowych ochotników
(16 K/9 M), w średnim wieku 40,9 ± 14,3 roku. Wykonano
u nich doustny test obciążenia glukozą, podczas którego w 0., 60.
i 120. minucie pobierano krew w celu oznaczenia glikemii, grelinemii
oraz insulinemii. Stężenia greliny w osoczu krwi żylnej oznaczono
metodą radioimmunologiczną, podobnie jak stężenia insuliny.
WYNIKI.
a stężeniem greliny. Wykazano istnienie kilku istotnych korelacji między insulinemią a grelinemią w poszczególnych etapach OGTT.
WNIOSEK.
decyduje insulinemia.INTRODUCTION. Ghrelin is a newly discovered gastric-derived
hormon-peptide, regulating energy homeostasis and numerous
processes in human organism. Its influence on apetite is indirect,
resulting probably from interactions between ghrelin, prandial glycemia
and insulinaemia. The aim of the study was to establish
concentration of total ghrelin and influence of orally ingested glucose
on ghrelin and insulin levels in healthy people.
MATERIALS AND METHODS. The study group comprised
25 healthy volunteers (16 F/9 M), mean age 40.9 ± 14.3 years.
During OGTT (in 0, 60th, and 120th minute) blood was collected in
order to evaluate glycamia, ghrelinaemia and insulinaemia. Plasma
ghrelin was established by means of RIA, as well as insulin.
RESULTS. Ghrelin level fell after 60. minutes of OGTT by 23.1%,
being significantly different from initial concentration. Total reduction
on ghrelin concentration during OGTT was 29.4%. Correlations
between glycemia and ghrelinemia were proven to be
not statistically significant. However, few significant correlations
between insulin and ghrelin in particular stages of OGTT were
proven. CONCLUSION. In healthy people insulin is probably the most important
factor contributing to lowering ghrelin levels after glucose
intake and also - most probably - postprandially
Efficacy of densitometry and fracture risk assessment tool FRAX in making therapeutic decisions in osteoporosis – a study on female patients of University of Medical Sciences Endocrinology Outpatient Clinic
Abstract Introduction: : Contemporary understanding of osteoporosis is based on the assessment of fracture risk. Evaluation of clinical risk factors of fracture with or without densitometry (DEXA) allows to identify patients requiring pharmacological treatment. Aim: The aim of the study was to estimate the usefulness of DEXA in assessment of fracture risk in women >50 years old. Materials and methods: In 296 previously untreated for osteoporosis women of Endocrinology Outpatient Clinic aged 50 to 85 years (mean 68,8+/-7,8 ) 10-year fracture risk using FRAX tool was computed from clinical risk factors alone (FRAX, FRAX hip) and after measurement of BMD (FRAX BMD). Then FRAX parameters were compared in 4 age categories. Fracture risk was confronted with therapeutic thresholds proposed in Poland. Results: 10-year fracture risk by FRAX increased with age. The most frequent risk factors were: previous fracture and family history of fractures. FRAX and FRAX BMD were significantly different in the 50-59 year-olds and 60-69 year-olds. Statistically significant difference was found for FRAX hip and FRAX hip BMD in 50-59 year old women. FRAX and FRAXhip were better predictors of fractures than FRAX BMD in patients >80 years old. In 50-79 year old women qualification for treatment was more effective when risk was assessed according to FRAX BMD. DEXA performance did not change the number of women over 80 who were eligible for treatment according to FRAX. Conclusions: BMD is crucial for the 10-year risk assessment in 50-69 year-olds without previous fracture, as an increasing number of patients need therapy. In >80 year old women clinical risk factors alone are sufficient to make therapeutic decisions. DEXA in these women has no influence on the risk of future fractures, including hip fracture. In 60-69 women with previous fracture DEXA is a good predictor for future fractures but has no value as far as therapeutic decisions are concerned
Assessment of tumor necrosis factor soluble receptor type 2 (sTNFR2) and soluble intercellular adhesion molecule 1 (sICAM-1) in obese patients with metabolic syndrome. Inflammatory process in metabolic syndrome
WSTĘP. Wzrastająca częstość otyłości w nowoczesnych społeczeństwach
przyjęła rozmiary epidemii. Bardzo rzadko nadmierna
masa ciała występuje niezależnie. Najczęściej otyłość współistnieje
z innymi chorobami i zaburzeniami w postaci zespołu metabolicznego.
Wykazano, że zespołowi oporności na insulinę towarzyszy
stan zapalny, który prawdopodobnie odpowiada za powstawanie
i rozwój miażdżycy u tych chorych. Celem pracy było zbadanie
wybranych wskaźników procesu zapalnego u otyłych chorych
z zespołem metabolicznym.
MATERIAŁ I METODY. Badaniem objęto 38 otyłych chorych z zespołem
metabolicznym rozpoznanym według kryteriów Adult Treatment
Panel III (ATP III). Grupa kontrolna składała się z 12 zdrowych
ochotników. W obu grupach oznaczono stężenia rozpuszczalnego
receptora typu 2 dla czynnika martwicy nowotworów
(sTNFR2, tumor necrosis factor soluble receptor type 2) oraz rozpuszczalnej
formy cząsteczki adhezyjnej 1 (sICAM-1, soluble intercellular
adhesion molecule 1 ), dodatkowo wykonano pomiary
antropometryczne oraz pomiar zawartości tkanki tłuszczowej metodą
bioimpedancji.
WYNIKI. Badane osoby charakteryzowały się podwyższonymi stężeniami
sTNFR2 oraz ICAM-1 w porównaniu z grupą kontrolną.
W grupie otyłych z nadciśnieniem tętniczym, dyslipidemią oraz cukrzycą
stwierdzono znamiennie wyższe stężenia badanych cytokin niż u chorych z takim samym profilem metabolicznym bez współistniejącej cukrzycy. W badanej grupie wykazano dodatnią korelację
między sTNFR2 a %FAT.
WNIOSKI. 1. Otyłych chorych z zespołem metabolicznym cechuje
zwiększona aktywacja procesu zapalnego. 2. Cukrzyca jest istotnym
czynnikiem zwiększającym proces zapalny w tej grupie pacjentów.
3. Przewlekły proces zapalny powinien być rozpatrywany
jako istotny element w rozwoju zmian miażdżycowych u otyłych
chorych z zespołem metabolicznym.INTRODUCTION. Obesity is an important public health problem.
Its prevalence has been steadily increasing during recent years,
especially in industrialised societies. In many patients obesity is
accompanied by other diseases and metabolic disorders forming
metabolic syndrome. There are more and more evidences that
metabolic syndrome lead to an inflammatory state which is probably
responsible for the initiation and progression of atherogenesis
in these groups of patients. The aim of the study was to evaluate
certain markers of inflammatory reaction in obese patients with
metabolic syndrome.
MATERIAL AND METHODS. The study group comprised 38 obese
patients with metabolic syndrome definable according to ATP III. The
control group consisted of 12 healthy volunteers. Serum concentration
of soluble tumor necrosis factor receptor type 2 (sTNFR2), soluble
intercellular adhesion molecule (sICAM-1) were assessed. All
patients underwent antropometric measurements. Fat content was
evaluated using bioimpedance method.
RESULTS. Higher concentrations of sTNFR2 and sICAM-1 were observed
in the study group when compared to the control group. The
obese hypertensives with dyslipidemia and type 2 diabetes mellitus
presented higher concentrations of the studied cytokines than the
obese non-diabetic hypertensives with dyslipidemia. In the study group
positive correlations between sTNFR2 and %FAT was proven.
CONCLUSIONS. 1. Obesity in metabolic syndrome is associated
with aggravation of an inflammatory process. 2. Diabetes mellitus
is an important factor contributing to aggravation of an inflammatory
reaction 3. Chronic inflammation should be considered as an
important factor in development of atherogenesis in patients with
metabolic syndrome
Do the family doctors on the verge of career have reasons to worry?
Cel pracy. Celem pracy była ocena źródeł problemów u młodych lekarzy w trakcie specjalizacji z medycyny rodzinnej.
Materiał i metody. Grupie 17 lekarzy w trakcie specjalizacji z medycyny rodzinnej postawiono trzy pytania otwarte: o ostatnie, najczęstsze i najpoważniejsze powody do zmartwień w pracy.
2. Szkolenia z zakresu komunikacji, zapobiegania wypaleniu zawodowemu, umiejętności radzenia sobie ze stresem, przekazywania trudnych wiadomości stanowią ważną część kształcenia przed- i podyplomowego.Introduction. At the beginning of their professional path, family physicians experience a lot of frustration every day. Here are just a few reasons for the problem: lack of self confidence in the new role, high expectations from patients, constantly changing family care system. We have focused our attention on potential sources of stress for the young family doctors.
Aim of the study. Assessment of the family medicine residents’ work related problems during the specialization course in family medicine.
Material and methods. A group of 17 family medicine residents was asked 3 open questions: about latest, most common and most vital problems related to their work. The answers were collected and analyzed regarding quantity and variety of problems.
Results. 1. For young GPs, the main reported source of stress are conflict situations between doctors and patients. 2. Many problems are also caused by tension between family physicians and doctors of another specialty or other members of medical stuff. 3. A significant part of problems are inflicted by system of public health care: excess of management work and insufficiency of family care system and difficulties of specialization process itself.
Conclusions. 1. There is a need of prevention of burnout syndrome from the beginning of professional path. 2. T raining of young doctors that includes counteracting burnout syndrome, coping with stress, passing on a difficult messages, seems to be an important part of training during studies and postgraduate studies
