    Headache is common. Up to 5% of attendances to emergency departments and acute medical units are due to headache. Headache is classified as either primary (eg migraine, cluster headache) or secondary to another cause (eg meningitis, subarachnoid haemorrhage). Even in the acute setting the majority of cases are due to primary causes. The role of the attending physician is to take a comprehensive history to diagnose and treat benign headache syndromes while ruling out sinister aetiologies.     Protest Music and Survival in the Basque Country During the Franco Regime

    This thesis is an examination of the origins of the Basque Nationalist movement that originated in the late nineteenth century, the Spanish Civil War that followed, and how a dictatorial regime damaged a minority autonomous region, the Basque Country. Within this scope, a singular, unique musical and artistic collective contributed efforts in preserving and growing Basque culture, a culture that had been refined to place focus on nationalistic ideals, unifying traditions, Euskera, and the nation of Euskadi. Ez Dok Amairu, the focus of this thesis, was a short-lived artistic group that concentrated on producing art, literature, and music (among other mediums) in Euskera. With clear inspirations from previous and existing Basque cultural institutions, movements, and history, Ez Dok Amairu nurtured a renewed interest and use of Euskera in the Basque country, even with the mitigating factors of the Franco regime, which included violence, censorship, and erasure. Through the analysis of some of their musical compositions and the development of the movement itself, this thesis argues that Ez Dok Amairu was successful in establishing linguistic foundations for Euskera through its music, while also the artistic works it produced acted as a tool for collectivization and celebration of Basque people during one of the most brutal periods of history in Spain and the Basque Country

    Use of Most Bothersome Symptom as a Coprimary Endpoint in Migraine Clinical Trials: A Post-Hoc Analysis of the Pivotal ZOTRIP Randomized, Controlled Trial.

    ObjectiveTo better understand the utility of using pain freedom and most bothersome headache-associated symptom (MBS) freedom as co-primary endpoints in clinical trials of acute migraine interventions.BackgroundAdhesive dermally applied microarray (ADAM) is an investigational system for intracutaneous drug administration. The recently completed pivotal Phase 2b/3 study (ZOTRIP), evaluating ADAM zolmitriptan for the treatment of acute moderate to severe migraine, was one of the first large studies to incorporate MBS freedom and pain freedom as co-primary endpoints per recently issued guidance by the US Food and Drug Administration. In this trial, the proportion of patients treated with ADAM zolmitriptan 3.8 mg, who were pain-free and MBS-free at 2 hours post-dose, was significantly higher than for placebo.MethodsWe undertook a post-hoc analysis of data from the ZOTRIP trial to examine how the outcomes from this trial compare to what might have been achieved using the conventional co-primary endpoints of pain relief, nausea, photophobia, and phonophobia.ResultsOf the 159 patients treated with ADAM zolmitriptan 3.8 mg or placebo, prospectively designated MBS were photophobia (n = 79), phonophobia (n = 43), and nausea (n = 37). Two-hour pain free rates in those with photophobia as the MBS were 36% for ADAM zolmitriptan 3.8 mg and 14% for placebo (P = .02). Corresponding rates for those with phonophobia as the MBS were 14% and 41% (P = .05). For those whose MBS was nausea, corresponding values were 56% and 16%, respectively (P = .01). Two-hour freedom from the MBS for active drug vs placebo were 67% vs 35% (P < .01) for photophobia, 55% vs 43% (P = .45) for phonophobia, and 89% vs 58% for nausea (P = .04). MBS freedom but not pain freedom was achieved in 28%. Only 1 patient (1%) achieved pain freedom, but not MBS freedom. The proportion with both pain and MBS freedom was highest (56%) among those whose MBS was nausea.ConclusionIn this study, the use of MBS was feasible and seemed to compare favorably to the previously required 4 co-primary endpoints

    Perkembangan teknologi informasi telah merambah dalam berbagai bidang, salah satunya bidang permainan seperti puzzle atau teka-teki. Sudoku adalah sebuah permainan teka-teki logika yang cukup menarik untuk dimainkan. Hingga saat ini permainan Sudoku telah populer di kalangan masyarakat. Berbagai jenis variasi puzzle dan tingkat kesulitan yang terdapat dalam Sudoku membuat para ilmuwan berusaha untuk melakukan penelitian terhadap permainan ini. Penggunaan algoritma backtracking dalam penyelesaian puzzle Sudoku merupakan salah satu penelitian yang telah dilakukan sebelumnya. Hanya saja, algoritma ini masih membutuhkan waktu yang cukup lama dalam melakukan komputasi penyelesaian puzzle Sudoku. Dalam penelitian ini, dibangun sebuah aplikasi untuk mengoptimalkan algoritma backtracking dalam menyelesaikan puzzle Sudoku menggunakan sebuah teknik optimasi yang disebut teknikhidden single.Aplikasi ini telah berhasil mengimplementasikan teknik hidden single ke dalam algoritma backtracking yang digunakan.     Fremanezumab for the Preventive Treatment of Chronic Migraine.

    BACKGROUND: Fremanezumab, a humanized monoclonal antibody targeting calcitonin gene-related peptide (CGRP), is being investigated as a preventive treatment for migraine. We compared two fremanezumab dose regimens with placebo for the prevention of chronic migraine. METHODS: In this phase 3 trial, we randomly assigned patients with chronic migraine (defined as headache of any duration or severity on ≥15 days per month and migraine on ≥8 days per month) in a 1:1:1 ratio to receive fremanezumab quarterly (a single dose of 675 mg at baseline and placebo at weeks 4 and 8), fremanezumab monthly (675 mg at baseline and 225 mg at weeks 4 and 8), or matching placebo. Both fremanezumab and placebo were administered by means of subcutaneous injection. The primary end point was the mean change from baseline in the average number of headache days (defined as days in which headache pain lasted ≥4 consecutive hours and had a peak severity of at least a moderate level or days in which acute migraine-specific medication [triptans or ergots] was used to treat a headache of any severity or duration) per month during the 12 weeks after the first dose. RESULTS: Of 1130 patients enrolled, 376 were randomly assigned to fremanezumab quarterly, 379 to fremanezumab monthly, and 375 to placebo. The mean number of baseline headache days (as defined above) per month was 13.2, 12.8, and 13.3, respectively. The least-squares mean (±SE) reduction in the average number of headache days per month was 4.3±0.3 with fremanezumab quarterly, 4.6±0.3 with fremanezumab monthly, and 2.5±0.3 with placebo (P CONCLUSIONS: Fremanezumab as a preventive treatment for chronic migraine resulted in a lower frequency of headache than placebo in this 12-week trial.
