    XMM Follow-Up Observations of Three Swift BAT-Selected Active Galactic Nuclei

    We present XMM-Newton observations of three AGN taken as part of a hunt to find very heavily obscured Compton-thick AGN. For obscuring columns greater than 10^25 cm^-2, AGN are only visible at energies below 10 keV via reflected/scattered radiation, characterized by a flat power-law. We therefore selected three objects (ESO 417-G006, IRAS 05218-1212, and MCG -01-05-047) from the Swift BAT hard X-ray survey catalog with Swift X-ray Telescope XRT 0.5-10 keV spectra with flat power-law indices as candidate Compton-thick sources for follow-up observations with the more sensitive instruments on XMM-Newton. The XMM spectra, however, rule out reflection-dominated models based on the weakness of the observed Fe K-alpha lines. Instead, the spectra are well-fit by a model of a power-law continuum obscured by a Compton-thin absorber, plus a soft excess. This result is consistent with previous follow-up observations of two other flat-spectrum BAT-detected AGN. Thus, out of the six AGN in the 22-month BAT catalog with apparently flat Swift XRT spectra, all five that have had follow-up observations are not likely Compton-thick. We also present new optical spectra of two of these objects, IRAS 05218-1212 and MCG -01-05-047. Interestingly, though both these AGN have similar X-ray spectra, their optical spectra are completely different, adding evidence against the simplest form of the geometric unified model of AGN. IRAS 05218-1212 appears in the optical as a Seyfert 1, despite the ~8.5x10^22 cm^-2 line-of-sight absorbing column indicated by its X-ray spectrum.     Investigating the Effect of a Visual Search Task for Simulator-Based Driver Training

    Novice drivers tend to direct their gaze to the road ahead and not scan the environment properly. This study investigated the training effectiveness of a visual search task in a driving simulator, aimed at increasing young drivers\u27 spread of visual search. Two groups of inexperienced drivers were instructed to drive as accurately as possible in the center of the right lane in a self-paced driving task of four 6-min sessions in a rural environment. While driving, one group performed a visual search task, consisting of detecting and fixating on visual stimuli in the peripheral area. The stimuli were purple dots that faded in slowly and disappeared when fixated by the participant. After training, both groups drove a transfer session in an urban environment, in which various hazardous situations occurred. Results showed that both groups improved their lane keeping performance, whereas the training group became more proficient in the visual search task. However, in the transfer session no group differences were detected. In conclusion, despite improvements in visual search performance during a relatively short training period, the visual search training did not detectibly influence the spread of visual search of novice drivers during a post training transfer session

    Stability of Executive Functioning of Moderately-Late Preterm and Full-Term Born Children at Ages 11 and 19:The TRAILS Cohort Study

    Moderately-late preterm-born children (MLPs, 32-36 weeks gestational age, GA) have poorer executive functioning (EF) at primary school age than full-term children (FTs). Evidence is lacking on their EF in adolescence, but for early preterm-born children, this has been shown to be much poorer. We, therefore, compared EF of MLPs and FTs at ages 11 and 19 and assessed development between these ages. We obtained data from TRAILS, a community-based prospective cohort study in the northern Netherlands, on 98 MLPs and 1832 FTs. We assessed EF by the Amsterdam Neuropsychological Tasks (ANT) at ages 11 and 19 years and computed gender-specific z-scores on reaction time and accuracy. We compared baseline speed, pattern search, working memory, sustained attention, inhibition, and attentional flexibility of MLPs and FTs crude, and adjusted for small-for-GA status, socioeconomic status, and estimated intelligence. MLPs and FTs performed similarly on all EF components at ages 11 and 19, except for the speed, but not the accuracy measure of attentional flexibility. This was slightly poorer for MLPs than FTs at age 19 (adjusted B 0.25; 95% confidence interval: 0.00 to 0.50; p = 0.047), but not at age 11 (adjusted B -0.02; -0.19 to 0.22; p = 0.87). Differences in EF between MLPs and FTs did not change significantly from age 11 to 19. MLPs had comparable EF on most components as FTs, with only attentional flexibility at age 19 developing slightly poorer for MLPs than for FTs. These findings suggest the effects of MLP birth on long-term EF to be small

    Higher rates of behavioural and emotional problems at preschool age in children born moderately preterm

    Objective To compare preschool children born moderately preterm (MP; 32-35 weeks' gestation) and children born at term (38-41 weeks' gestation) regarding the occurrence of behavioural and emotional problems, overall, for separate types of problems and by gender. Design Prospective cohort study consisting of a community-based sample of MP and a random sample of term-born children in 13 Preventive Child Healthcare centres throughout the Netherlands. Patients 995 MP and 577 term-born children just under age 4 were included. Main outcome measures Behavioural and emotional problems were measured using the Child Behavior Checklist 1.5-5 years. Seven syndrome scales, internalising, externalising and total problems were determined. Higher scores indicate worse outcomes. Results MP children had higher scores on all syndrome scales, internalising, externalising and total problems than term-born controls. The mean difference on total problems was 4.04 (95% CI 2.08 to 6.00). Prevalence rates of elevated externalising problem scores were highest in boys (10.5%) and internalising problems were highest in girls (9.9%). MP children were at greater risk for somatic complaints (OR 1.92, 95% CI 1.09 to 3.38), internalising (OR 2.40, 95% CI 1.48 to 3.87), externalising (OR 1.69, 95% CI 1.07 to 2.67) and total problems (OR 1.84, 95% CI 1.12 to 3.00). Conclusions Moderate preterm birth affects all domains of behavioural and emotional problems, particularly for girls. MP children should be targeted for the prevention of mental health problems as they have a great impact on developmental and social competencies at school and in the community

    Stripe-hexagon competition in forced pattern forming systems with broken up-down symmetry

    We investigate the response of two-dimensional pattern forming systems with a broken up-down symmetry, such as chemical reactions, to spatially resonant forcing and propose related experiments. The nonlinear behavior immediately above threshold is analyzed in terms of amplitude equations suggested for a 1:21:2 and 1:11:1 ratio between the wavelength of the spatial periodic forcing and the wavelength of the pattern of the respective system. Both sets of coupled amplitude equations are derived by a perturbative method from the Lengyel-Epstein model describing a chemical reaction showing Turing patterns, which gives us the opportunity to relate the generic response scenarios to a specific pattern forming system. The nonlinear competition between stripe patterns and distorted hexagons is explored and their range of existence, stability and coexistence is determined. Whereas without modulations hexagonal patterns are always preferred near onset of pattern formation, single mode solutions (stripes) are favored close to threshold for modulation amplitudes beyond some critical value. Hence distorted hexagons only occur in a finite range of the control parameter and their interval of existence shrinks to zero with increasing values of the modulation amplitude. Furthermore depending on the modulation amplitude the transition between stripes and distorted hexagons is either sub- or supercritical.Comment: 10 pages, 12 figures, submitted to Physical Review

    How Do Patients Understand Questions about Lower Urinary Tract Symptoms?:A Qualitative Study of Problems in Completing Urological Questionnaires

    Lower urinary tract symptoms are common complaints in ageing people. For a urological evaluation of such complaints in men, the International Prostate Symptom Score (IPSS) is used worldwide. Previous quantitative studies have revealed serious problems in completing this questionnaire. In order to gain insight into the nature and causes of these problems, we conducted a qualitative study. Not only the purely verbal IPSS was studied but also two alternatives, including pictograms: the Visual Prostate Symptom Score (VPSS) and the Score Visuel Prostatique en Image (SVPI). Men aged 40 years and over with an inadequate level of health literacy (IHL; n = 18) or an adequate level of health literacy (AHL; n = 47) participated. Each participant filled out one of the three questionnaires while thinking aloud. The analysis of their utterances revealed problems in both health literacy groups with form-filling tasks and subtasks for all three questionnaires. Most noticeable were the problems with the IPSS; the terminology and layout of this form led to difficulties. In the VPSS and SVPI, the pictograms sometimes raised problems. As in previous research on form-filling behavior, an overestimation by form designers of form fillers' knowledge and skills seems to be an important explanation for the problems observed

    Surfactant protein-D and exposure to bioaerosols in wastewater and garbage workers

    Purpose: Bioaerosols and their constituents, such as endotoxins, are capable of causing an inflammatory reaction at the level of the lung-blood barrier, which becomes more permeable. Thus, it was hypothesized that occupational exposure to bioaerosols can increase leakage of surfactant protein-D (SP-D), a lung-specific protein, into the bloodstream. Methods: SP-D was determined by ELISA in 316 wastewater workers, 67 garbage collectors, and 395 control subjects. Exposure was assessed with four interview-based indicators and by preliminary endotoxin measurements using the Limulus amoebocyte lysate assay. Influence of exposure on serum SP-D was assessed by multiple linear regression considering smoking, glomerular function, lung diseases, obesity, and other confounders. Results: Overall, mean exposure levels to endotoxins were below 100EU/m3. However, special tasks of wastewater workers caused higher endotoxin exposure. SP-D concentration was slightly increased in this occupational group and associated with the occurrence of splashes and contact to raw sewage. No effect was found in garbage collectors. Smoking increased serum SP-D. No clinically relevant correlation between spirometry results and SP-D concentrations appeared. Conclusions: These results support the hypothesis that inhalation of bioaerosols, even at low concentrations, has a subclinical effect on the lung-blood barrier, the permeability of which increases without associated spirometric change

    Interactions of Ar(9+) and metastable Ar(8+) with a Si(100) surface at velocities near the image acceleration limit

    Auger LMM spectra and preliminary model simulations of Ar(9+) and metastable Ar(8+) ions interacting with a clean monocrystalline n-doped Si(100) surface are presented. By varying the experimental parameters, several yet undiscovered spectroscopic features have been observed providing valuable hints for the development of an adequate interaction model. On our apparatus the ion beam energy can be lowered to almost mere image charge attraction.     Learning democracy in social work

    In this contribution, we discuss the role of social work in processes of democracy. A key question in this discussion concerns the meaning of ‘the social’ in social work. This question has often been answered in a self-referential way, referring to a methodological identity of social work. This defines the educational role of social work as socialisation (be it socialisation into obedience or into an empowered citizen). However, the idea of democracy as ‘ongoing experiment’ and ‘beyond order’ challenges this methodological identity of social work. From the perspective of democracy as an ‘ongoing experiment’, the social is to be regarded as a platform for dissensus, for ongoing discussions on the relation between private and public issues in the light of human rights and social justice. Hence, the identity of social work cannot be defined in a methodological way; social work is a complex of (institutionalized) welfare practices, to be studied on their underlying views on the ‘social’ as a political and educational concept, and on the way they influence the situation of children, young people and adults in society

    Effectiveness of a Comprehensive Health Literacy Consultation Skills Training for Undergraduate Medical Students:A Randomized Controlled Trial

    Comprehensible communication by itself is not sufficient to overcome health literacy related problems. Future doctors need a larger scope of capacities in order to strengthen a patient's autonomy, participation, and self-management abilities. To date, such comprehensive training-interventions are rarely embedded in curricula, nor systematically evaluated. We assessed whether comprehensive training increased these health literacy competencies, in a randomized controlled trial (RCT), with a waiting list condition. Participants were international undergraduate medical students of a Dutch medical faculty (intervention: 39; control: 40). The 11-h-training-intervention encompassed a health literacy lecture and five interactive small-group sessions to practise gathering information and providing comprehensible information, shared decision-making, and enabling of self-management using role-play and videotaped conversations. We assessed self-reported competencies (knowledge and awareness of health literacy, attitude, self-efficacy, and ability to use patient-centred communication techniques) at baseline, after a five and ten-week follow-up. We compared students' competencies using multi-level analysis, adjusted for baseline. As validation, we evaluated demonstrated skills in videotaped consultations for a subsample. The group of students who received the training intervention reported significantly greater health literacy competencies, which persisted up to five weeks afterwards. Increase was greatest for providing comprehensible information (B: 1.50; 95% confidence interval, CI 1.15 to 1.84), shared decision-making (B: 1.08; 95% CI 0.60 to 1.55), and self-management (B: 1.21; 95% CI 0.61 to 1.80). Effects regarding demonstrated skills confirmed self-rated competency improvement. This training enhanced a larger scope of health literacy competences and was well received by medical students. Implementation and further evaluation of this training in education and clinical practice can support sustainable health literacy capacity building of future doctors and contribute to better patient empowerment and outcomes of consultations
