Evidence-based planning and costing palliative care services for children : novel multi-method epidemiological and economic exemplar
Background:
Children’s palliative care is a relatively new clinical specialty. Its nature is multi-dimensional and its delivery necessarily multi-professional. Numerous diverse public and not-for-profit organisations typically provide services and support. Because services are not centrally coordinated, they are provided in a manner that is inconsistent and incoherent. Since the first children’s hospice opened in 1982, the epidemiology of life-limiting conditions has changed with more children living longer, and many requiring transfer to adult services. Very little is known about the number of children living within any given geographical locality, costs of care, or experiences of children with ongoing palliative care needs and their families. We integrated evidence, and undertook and used novel methodological epidemiological work to develop the first evidence-based and costed commissioning exemplar.
Methods:
Multi-method epidemiological and economic exemplar from a health and not-for-profit organisation perspective, to estimate numbers of children under 19 years with life-limiting conditions, cost current services, determine child/parent care preferences, and cost choice of end-of-life care at home.
Results:
The exemplar locality (North Wales) had important gaps in service provision and the clinical network. The estimated annual total cost of current children’s palliative care was about £5.5 million; average annual care cost per child was £22,771 using 2007 prevalence estimates and £2,437- £11,045 using new 2012/13 population-based prevalence estimates. Using population-based prevalence, we estimate 2271 children with a life-limiting condition in the general exemplar population and around 501 children per year with ongoing palliative care needs in contact with hospital services. Around 24 children with a wide range of life-limiting conditions require end-of-life care per year. Choice of end-of-life care at home was requested, which is not currently universally available. We estimated a minimum (based on 1 week of end-of-life care) additional cost of £336,000 per year to provide end-of-life support at home. Were end-of-life care to span 4 weeks, the total annual additional costs increases to £536,500 (2010/11 prices).
Conclusions:
Findings make a significant contribution to population-based needs assessment and commissioning methodology in children’s palliative care. Further work is needed to determine with greater precision which children in the total population require access to services and when. Half of children who died 2002-7 did not have conditions that met the globally used children's palliative care condition categories, which need revision in light of findings

The impact of atmospheric pCO2 on carbon isotope ratios of the atmosphere and ocean
It is well known that the equilibration timescale for the isotopic ratios 13C/12C and 14C/12C in the ocean mixed layer is on the order of a decade, 2 orders of magnitude slower than for oxygen. Less widely appreciated is the fact that the equilibration timescale is quite sensitive to the speciation of dissolved inorganic carbon (DIC) in the mixed layer, scaling linearly with the ratio DIC/CO2, which varies inversely with atmospheric pCO2. Although this effect is included in models that resolve the role of carbon speciation in air-sea exchange, its role is often unrecognized, and it is not commonly considered in the interpretation of carbon isotope observations. Here we use a global three-dimensional ocean model to estimate the redistribution of the carbon isotopic ratios between the atmosphere and ocean due solely to variations in atmospheric pCO2. Under Last Glacial Maximum (LGM) pCO2, atmospheric Δ14C is increased by ~30‰ due to the speciation change, all else being equal, raising the surface reservoir age by about 250 years throughout most of the ocean. For 13C, enhanced surface disequilibrium under LGM pCO2 causes the upper ocean, atmosphere, and North Atlantic Deep Water δ13C to become at least 0.2‰ higher relative to deep waters ventilated by the Southern Ocean. Conversely, under high pCO2, rapid equilibration greatly decreases isotopic disequilibrium. As a result, during geological periods of high pCO2, vertical δ13C gradients may have been greatly weakened as a direct chemical consequence of the high pCO2, masquerading as very well ventilated or biologically dead Strangelove Oceans. The ongoing anthropogenic rise of pCO2 is accelerating the equilibration of the carbon isotopes in the ocean, lowering atmospheric Δ14C and weakening δ13C gradients within the ocean to a degree that is similar to the traditional fossil fuel “Suess” effect
Superpotentials for M-theory on a G_2 holonomy manifold and Triality symmetry
For -theory on the holonomy manifold given by the cone on {\bf
S^3}\x {\bf S^3} we consider the superpotential generated by membrane
instantons and study its transformations properties, especially under monodromy
transformations and triality symmetry. We find that the latter symmetry is,
essentially, even a symmetry of the superpotential. As in Seiberg/Witten
theory, where a flat bundle given by the periods of an universal elliptic curve
over the -plane occurs, here a flat bundle related to the Heisenberg group
appears and the relevant universal object over the moduli space is related to

Enterobacter bugandensis: a novel enterobacterial species associated with severe clinical infection
Nosocomial pathogens can cause life-threatening infections in neonates and immunocompromised patients. E. bugandensis (EB-247) is a recently described species of Enterobacter, associated with neonatal sepsis. Here we demonstrate that the extended spectrum betha-lactam (ESBL) producing isolate EB-247 is highly virulent in both Galleria mellonella and mouse models of infection. Infection studies in a streptomycin-treated mouse model showed that EB-247 is as efficient as Salmonella Typhimurium in inducing systemic infection and release of proinflammatory cytokines. Sequencing and analysis of the complete genome and plasmid revealed that virulence properties are associated with the chromosome, while antibiotic-resistance genes are exclusively present on a 299 kb IncHI plasmid. EB-247 grew in high concentrations of human serum indicating septicemic potential. Using whole genome-based transcriptome analysis we found 7% of the genome was mobilized for growth in serum. Upregulated genes include those involved in the iron uptake and storage as well as metabolism. The lasso peptide microcin J25 (MccJ25), an inhibitor of iron-uptake and RNA polymerase activity, inhibited EB-247 growth. Our studies indicate that Enterobacter bugandensis is a highly pathogenic species of the genus Enterobacter. Further studies on the colonization and virulence potential of E. bugandensis and its association with septicemic infection is now warranted
Kanamycin resistance during in vitro development of pollen from transgenic tomato plants
Effects of kanamycin on pollen germination and tube growth of pollen from non-transformed plants and from transgenic tomato plants containing a chimaeric kanamycin resistance gene were determined. Germination of pollen was not affected by the addition of kanamycin to the medium in both genotypes. Kanamycin, however, severely affected tube growth of pollen from non-transformed plants, while pollen from plants containing the chimaeric gene were less sensitive and produced significantly longer tubes at kanamycin concentrations between 200-400 mg l-1. Apparently, this resistance for kanamycin correlates with the expression of the chimaeric gene during male gametophytic development.
The Chandra Source Catalog
The Chandra Source Catalog (CSC) is a general purpose virtual X-ray
astrophysics facility that provides access to a carefully selected set of
generally useful quantities for individual X-ray sources, and is designed to
satisfy the needs of a broad-based group of scientists, including those who may
be less familiar with astronomical data analysis in the X-ray regime. The first
release of the CSC includes information about 94,676 distinct X-ray sources
detected in a subset of public ACIS imaging observations from roughly the first
eight years of the Chandra mission. This release of the catalog includes point
and compact sources with observed spatial extents <~ 30''. The catalog (1)
provides access to the best estimates of the X-ray source properties for
detected sources, with good scientific fidelity, and directly supports
scientific analysis using the individual source data; (2) facilitates analysis
of a wide range of statistical properties for classes of X-ray sources; and (3)
provides efficient access to calibrated observational data and ancillary data
products for individual X-ray sources, so that users can perform detailed
further analysis using existing tools. The catalog includes real X-ray sources
detected with flux estimates that are at least 3 times their estimated 1 sigma
uncertainties in at least one energy band, while maintaining the number of
spurious sources at a level of <~ 1 false source per field for a 100 ks
observation. For each detected source, the CSC provides commonly tabulated
quantities, including source position, extent, multi-band fluxes, hardness
ratios, and variability statistics, derived from the observations in which the
source is detected. In addition to these traditional catalog elements, for each
X-ray source the CSC includes an extensive set of file-based data products that

Statistical Characterization of the Chandra Source Catalog
The first release of the Chandra Source Catalog (CSC) contains ~95,000 X-ray
sources in a total area of ~0.75% of the entire sky, using data from ~3,900
separate ACIS observations of a multitude of different types of X-ray sources.
In order to maximize the scientific benefit of such a large, heterogeneous
data-set, careful characterization of the statistical properties of the
catalog, i.e., completeness, sensitivity, false source rate, and accuracy of
source properties, is required. Characterization efforts of other, large
Chandra catalogs, such as the ChaMP Point Source Catalog (Kim et al. 2007) or
the 2 Mega-second Deep Field Surveys (Alexander et al. 2003), while
informative, cannot serve this purpose, since the CSC analysis procedures are
significantly different and the range of allowable data is much less
restrictive. We describe here the characterization process for the CSC. This
process includes both a comparison of real CSC results with those of other,
deeper Chandra catalogs of the same targets and extensive simulations of

The class of the locus of intermediate Jacobians of cubic threefolds
We study the locus of intermediate Jacobians of cubic threefolds within the
moduli space of complex principally polarized abelian fivefolds, and its
generalization to arbitrary genus - the locus of abelian varieties with a
singular odd two-torsion point on the theta divisor. Assuming that this locus
has expected codimension (which we show to be true for genus up to 5), we
compute the class of this locus, and of is closure in the perfect cone toroidal
compactification, in the Chow, homology, and the tautological ring.
We work out the cases of genus up to 5 in detail, obtaining explicit
expressions for the classes of the closures of the locus of products of an
elliptic curve and a hyperelliptic genus 3 curve, in moduli of principally
polarized abelian fourfolds, and of the locus of intermediate Jacobians in
genus 5. In the course of our computation we also deal with various
intersections of boundary divisors of a level toroidal compactification, which
is of independent interest in understanding the cohomology and Chow rings of

Phase diagram and critical properties in the Polyakov--Nambu--Jona-Lasinio model
We investigate the phase diagram of the so-called
Polyakov--Nambu--Jona-Lasinio model at finite temperature and nonzero chemical
potential with three quark flavours. Chiral and deconfinement phase transitions
are discussed, and the relevant order-like parameters are analyzed. The results
are compared with simple thermodynamic expectations and lattice data. A special
attention is payed to the critical end point: as the strength of the
flavour-mixing interaction becomes weaker, the critical end point moves to low
