    Plasma Cell Granuloma of the Thyroid: A Conservative Approach to a Rare Condition and Review of the Literature

    Introduction. We present a case of an 89-year-old female who attended our surgical endocrine clinic with a 3-month history of a left-sided neck lump. There was no past medical history of thyroid disease. Methods. Following examination and further investigation, including core biopsy, a diagnosis of plasma cell granuloma of the thyroid was made. Biochemical testing of thyroid function and Thyroid Peroxidase Antibody was in-keeping with an associated Hashimoto's thyroiditis. Results. The patient was treated conservatively with thyroxine and regularly seen in clinic. TSH levels improved and the lump showed signs of regression. Conclusion. Plasma cell granuloma of the thyroid is rare with only 16 previously reported cases. We present a new approach to management without the use of surgery or steroids. The literature is reviewed comparing clinico-pathological features and management of other reported cases

    How to break access control in a controlled manner

    The Electronic Medical Record (EMR) integrates heterogeneous information within a Healthcare Institution stressing the need for security and access control. The Biostatistics and Medical Informatics Department from Porto Faculty of Medicine has recently implemented a Virtual EMR (VEMR) in order to integrate patient information and clinical reports within a university hospital. With more than 500 medical doctors using the system on a daily basis, an access control policy and model were implemented. However, the healthcare environment has unanticipated situations (i.e. emergency situations) where access to information is essential. Most traditional policies do not allow for overriding. A policy that allows for Break-The-Glass (BTG) was implemented in order to override access control whilst providing for non-repudiation mechanisms for its usage. The policy was easily integrated within the model confirming its modularity and the fact that user intervention in defining security procedures is crucial to its successful implementation and use

    Biologic Cycling of Silica across a Grassland Bioclimosequence

    The dynamics of biologic Si cycling in grassland ecosystems are largely unknown and likely to impact mineral weathering rates regionally and diatom productivity globally; key regulatory processes in the global Si cycle are closely tied to the global carbon cycle. Across a bioclimatic sequence spanning major grassland ecosystems in the Great Plains, soil biogenic silica depth distributions are similar to that of soil organic carbon; however, unlike soil organic carbon, quantities of soil biogenic silica decrease with increasing precipitation, despite an increase in annual biogenic inputs through litterfall across the same gradient. Though comprising only 1–3% of the total Si pool, faster turnover of biogenic Si and annual cycling by grasses should positively impact mineral dissolution. Our results suggest that the largest reservoir of biogenic Si in terrestrial ecosystems resides in soils, and emphasize the potential significance of grasslands in the global biogeochemical cycle of Si

    Following the discovery of Her X-1 as a source of pulsed 1000 Gev X-rays, a search for emission from an X-ray binary containing a pulsar with similar values of period, period derivative and luminosity was successful. The sporadic X-ray binary 4U 0115-63 has been observed, with probability 2.5 x 10 to the minus 6 power ergs/s to emit 1000 GeV gamma-rays with a time averaged energy flux of 6 to 10 to the 35th power

    The Complex Wind Torus and Jets of PSR B1706-44

    We report on Chandra ACIS imaging of the pulsar wind nebula (PWN) of the young Vela-like PSR B1706-44, which shows the now common pattern of an equatorial wind and polar jets. The structure is particularly rich, showing a relativistically boosted termination shock, jets with strong confinement, a surrounding radio/X-ray PWN and evidence for a quasi-static `bubble nebula'. The structures trace the pulsar spin geometry and illuminate its possible relation to SNR G343.1-2.3. We also obtain improved estimates of the pulsar flux and nebular spectrum, constraining the system age and energetics.Comment: To appear in the Astrophysical Journal. 15pp, 4 figures in 7 file

    Study of the nucleon-induced preequilibrium reactions in terms of the Quantum Molecular Dynamics

    The preequilibrium (nucleon-in, nucleon-out) angular distributions of 27^{27}Al, 58^{58}Ni and 90^{90}Zr have been analyzed in the energy region from 90 to 200 MeV in terms of the Quantum Moleculear Dynamics (QMD) theory. First, we show that the present approach can reproduce the measured (p,xp') and (p,xn) angular distributions leading to continuous final states without adjusing any parameters. Second, we show the results of the detailed study of the preequilibrium reaction processes; the step-wise contribution to the angular distribution, comparison with the quantum-mechanical Feshbach-Kerman-Koonin theory, the effects of momentum distribution and surface refraction/reflection to the quasifree scattering. Finally, the present method was used to assess the importance of multiple preequilibrium particle emission as a function of projectile energy up to 1 GeV.Comment: 22pages, Revex is used, 10 Postscript figures are available by request from [email protected]
