    Hepatoid carcinoma of the ovary (HCO) was first reported in 1987 in 5 cases of malignant ovarian tumors which were similar to hepatocarcinoma in the histological analysis. We report the first case in the literature of a patient diagnosed with HCO treated using HIPEC after extensive cytoreductive surgery, and we discuss the value of this therapeutical option in patients with HCO

    Presentamos un resumen de las actividades que algunos de los profesores de la Facultad de Medicina de Murcia han llevado a cabo durante las 3 semanas previas a las vacaciones de primavera. En éstas, la docencia presencial tuvo que ser sustituída por actividades en línea o virtuales, a causa de la implantación del estado de alarma en España, que motivó el cierre completo de las Universidades desde el 13 de marzo de 2020.     La aplicación de terapias novedosas en el ámbito de la oncología ha permitido mejorar la esperanza de vida en patologías antes consideradas como intratables. Un ejemplo de ello es el abordaje multidisciplinar de la diseminación peritoneal de tumores digestivos y ginecológicos, con la aplicación de cirugías extensas con procedimientos de peritonectomía y la administración de quimioterapia intraperitoneal intraoperatoria hipertérmica (HIPEC). En la presente tesis doctoral se exponen los resultados obtenidos tras la aplicación de dicho esquema terapéutico en pacientes con carcinomatosis peritoneal de origen ovárico, primario y recurrente, explorando los factores que se relacionaron con la morbilidad y las recurrencias en el grupo de pacientes estudiadas. Estos resultados permiten concluir que la terapia es aplicable con una morbilidad postoperatoria controlada y genera unos resultados de supervivencia esperanzadores a la espera de los estudios prospectivos y aleatorizados que se desarrollan en la actualidad.The use of novel therapies in oncology has improved life expectancy in diseases previously considered untreatable. An example of this is the multidisciplinary approach to the peritoneal dissemination of gastrointestinal and gynecological tumors, with the application of extensive surgery with peritonectomy procedures and administration of hyperthermic intraoperative intraperitoneal chemotherapy (HIPEC). In this thesis we presents the results obtained after the treatment with this therapeutic scheme of patients with peritoneal carcinomatosis of primary and recurrent ovarian cancer, analizing the factors associated with morbidity and recurrences in the group of patients studied. These results suggest that HIPEC therapy can be applied with a controlled postoperative morbidity rates and generates encouraging survival results until the results of prospective randomized studies which are developing in ovarian cance

    Hepatocellular Carcinoma (HCC) is an aggressive tumor entity, with the only curative options being surgical resection or orthotopic liver transplantation (OLT). The presence of one single tumor nodule of less than 5 centimeters diameter or a maximum of 3 nodules, with the largest of these not exceeding 3 centimeters (Milan criteria) constitute the clinical situation in which the best results for OLT in patients with HCC have been achieved. The survival of patients fulfilling the Milan criteria after transplantation is comparable to patients with similar tumor stages without cirrhosis, undergoing hepatic resection. The application of PET in oncology has become increasingly common in the last decade as it is a non-invasive tool that also gathers information about the degree of the biological aggressiveness of the tumor.     Selecting the Appropriate Downstaging and Bridging Therapies for Hepatocellular Carcinoma: What Is the Role of Transarterial Radioembolization? A Pooled Analysis

    Background: Transarterial radioembolization in HCC for LT as downstaging/bridging has been increasing in recent years but some indication criteria are still unclear. Methods: We conducted a systematic literature search of primary research publications conducted in PubMed, Scopus and ScienceDirect databases until November 2022. Relevant data about patient selection, HCC features and oncological outcomes after TARE for downstaging or bridging in LT were analyzed. Results: A total of 14 studies were included (7 downstaging, 3 bridging and 4 mixed downstaging and bridging). The proportion of whole liver TARE was between 0 and 1.6%. Multiple TARE interventions were necessary for 16.7% up to 28% of the patients. A total of 55 of 204 patients across all included studies undergoing TARE for downstaging were finally transplanted. The only RCT included presents a higher tumor response with the downstaging rate for LT of TARE than TACE (9/32 vs. 4/34, respectively). Grade 3 or 4 adverse effects rate were detected between 15 and 30% of patients. Conclusions: TARE is a safe therapeutic option with potential advantages in its capacity to necrotize and reduce the size of the HCC for downstaging or bridging in LT
