    Changes in quality of care and costs induced by implementation of a diabetes program in a social security entity of Argentina

    Purpose: To measure the impact of a Diabetes and Cardiovascular Risk Factors Program implemented in a social security institution (OSPERYH) upon short- and long-term clinical/metabolic outcomes and costs of care. Methods: Observational longitudinal cohort analysis of clinical/metabolic data and resource use of 300 adult male and female OSPERYH affiliates with diabetes before (Baseline) and one and three years after implementation of the program. Data were obtained from clinical records (Qualidiab) and the administration´s database. Results: The implementation of the program in "real world" conditions resulted in an immediate and sustainable improvement of the quality of care provided to people with diabetes incorporated therein. We also recorded a more appropriate oral therapy prescription for hyperglycemia and cardiovascular risk factors (CVRF) as well as a decrease of events related to chronic complications. This improvement was associated to an increased use of diagnostic and therapeutic resources, particularly those related to pharmacy prescriptions, not specifically used for the control of hyperglycemia and other CVRF. Conclusion: The implementation of a diabetes program in "real world" conditions results in a significant short- and long-term improvement of the quality of care provided to people with diabetes and other CVRF, but simultaneously increased the use of resources and the cost of diagnostic and therapeutic practices. Since controlled studies have shown improvement in quality of care without increasing its costs, our results suggest the need to include management control strategies in these programs for an appropriate medical and administrative feedback to ensure the simultaneous improvement of clinical outcomes and optimization of the use of resources.Fil: Gagliardino, Juan Jose.     Intramolecular [3+2] cycloaddition of azido-unsaturated esters derived of monosaccharides

    Iminosugars have been shown to be very potent inhibitors of glycosidases and glycolsyltransferases. Due to their ability to resemble the transition states of the sugars involved in these processes, a variety of m onocyclic and bicyclic iminosugars have been synthesized or isolated from natural sources over the years. As part of our ongoing work on the preparation of glycosidase inhibitors , we developed stereoselective methods for synthesizing iminosugars from 2,3-epoxyamides obtained from monosaccharides.Now, we are interested in the syntheses of novel bicyclic triazoles, by intramolecular cycloaddition, due to the possibility of combining azido group and unsaturated esters in the same molecule. The triazole system is broadly considered in syntheses of bioactive products and the option of fused iminosugar with triazole has been evaluated. Firstly, we started with a less complex azido derivative 2a to continue with azido compounds 2b obtained from our 2,3-epoxyamides 3, by regioselective introduction of an azido group. The fused triazolines 4a were formed by heating of 2a. Aromatization of 4a afforded byciclic triazoles. There are several possibilitie s in the cyclization process depending on the reaction conditions and products 5,6 and 7 can be formed. Moreover, we conducted a theoretical DFT based study of the cycloaddition to value the probability of formation of the triazoline 4a and subsequent aroma tization to triazole 1,versus elimination to the unsaturated ester 5Universidad de Málaga.     This document is the final report for the LoRa project made by three international Erasmus students in the Aarhus Universitet as their bachelor's project. The goal of this project is to take profit of the Internet of Things technology by developing a low consuming sensing device. This device will consist in one gateway and one node. Where the node will be the responsible of the data gathering and will send it to the gateway. Then, the gateway will save all the data in a database in order to make statistics in future applications

    Las actividades de exportaciones de carbón a través de los puertos situados en el norte del departamento del Magdalena han afectado directamente a la comunidad de Don Jaca, ubicada en el distrito de Santa Marta en sus límites con el municipio de Ciénaga. Estas actividades de cargue y descargue del mineral por parte de compañías multinacionales, sin el debido respeto de los protocolos de seguridad ambiental, han deteriorado gravemente la calidad de vida de los pobladores contaminando el aire, los suelos, las fuentes hídricas, las playas y el mar, lo que trae consigo la aparición de enfermedades físicas y mentales, pérdida de recursos agrícolas, pesqueros y turísticos y ocasionando el desplazamiento de sus pobladores a otras comunidades, vulnerando su derecho fundamental a vivir en un ambiente sano. 25 años después del inicio de estas actividades las vulneraciones persisten, pues no existe ni una verdadera conciencia en las empresas o en las autoridades de los impactos negativos que esta tiene en la población y en las responsabilidades que a ellas les incumben, ni una capacidad de la comunidad para defender y exigir sus derechos. Este proyecto pretende dotar, a través de la capacitación, a la comunidad de Don jaca de mecanismos jurídicos o no jurídicos para defender y exigir su derecho al ambiente sano y sus derechos conexos
