    Impacts of fertilization on grassland productivity and water quality across the European Alps under current and warming climate: Insights from a mechanistic model

    Alpine grasslands sustain local economy by providing fodder for livestock. Intensive fertilization is common to enhance their yields, thus creating negative externalities on water quality that are difficult to evaluate without reliable estimates of nutrient fluxes. We apply a mechanistic ecosystem model, seamlessly integrating land-surface energy balance, soil hydrology, vegetation dynamics, and soil biogeochemistry, aiming at assessing the grassland response to fertilization. We simulate the major water, carbon, nutrient, and energy fluxes of nine grassland plots across the broad European Alpine region. We provide an interdisciplinary model evaluation by confirming its performance against observed variables from different datasets. Subsequently, we apply the model to test the influence of fertilization practices on grassland yields and nitrate (NO3_{3}^{-}) losses through leaching under both current and modified climate scenarios. Despite the generally low NO3_{3}^{-} concentration in groundwater recharge, the variability across sites is remarkable, which is mostly (but not exclusively) dictated by elevation. In high-Alpine sites, short growing seasons lead to less efficient nitrogen (N) uptake for biomass production. This combined with lower evapotranspiration rates results in higher amounts of drainage and NO3_{3}^{-} leaching to groundwater. Scenarios with increased temperature lead to a longer growing season characterized by higher biomass production and, consequently, to a reduction of water leakage and N leaching. While the intersite variability is maintained, climate change impacts are stronger on sites at higher elevations. The local soil hydrology has a crucial role in driving the NO3_{3}^{-} use efficiency. The commonly applied fixed threshold limit on fertilizer N input is suboptimal. We suggest that major hydrological and soil property differences across sites should be considered in the delineation of best practices or regulations for management. Using distributed maps informed with key soil and climatic attributes or systematically implementing integrated ecosystem models as shown here can contribute to achieving more sustainable practices

    Investigation of the weather conditions during the collapse of the Morandi Bridge in Genoa on 14 August 2018

    On 14 August 2018, Morandi Bridge in Genoa, Italy, collapsed sending vehicles and tons of rubble to the ground about 40\u2009m below and killing 43 people. Preliminary investigations indicated poor design, questionable building practices and insufficient maintenance or a combination of these factors as a possible cause of collapse. However, at the time of collapse, a thunderstorm associated with strong winds, lightning and rain was developed over the city. While it is still not clear whether or not it played a role in this disaster, the present paper documents the weather conditions during the collapse and analyzes in detail a downburst that occurred at the time of the collapse a few kilometers from the bridge. The thunderstorm is analyzed using direct and remote measurements in an attempt to describe the evolution of the cumulonimbus cloud as it approached the coast from the sea. The detected downburst is investigated using a lidar scanner and the anemometric network in the Port of Genoa. The paper shows that the unique lidar measurements enabled a partial reconstruction of the gust front shape and displacement velocity. The Weather Research and Forecasting (WRF) simulations, carried out with three different forcing conditions, forecasted the cumuliform convection at larger scales but did not accurately replicate the downburst signature at the surface that was measured by radar, lidar, and anemometers. This result demonstrates that the localized wind conditions during the collapse time could not be operationally forecasted

    Results of COVID-19 screening in a dermatologic clinic in northern Italy

    Introduction The COVID-19 pandemic has been a social, economic and sanitary challenge, which caused a great slowdown in most clinical activities. At the beginning of 2021, the gradual reopening of outpatient services was threatened by the risk of COVID-19 outbreaks in healthcare facilities. Methods Between January and March 2021, our dermatologic clinic promoted a screening campaign based on rapid antigen-testing: adhering patients were tested for COVID-19 and were visited only after getting a negative result. Results Among 635 recruited subjects, 514 agreed to be enrolled in the study, while 121 refused and were not tested. Only 1 of the 514 tests was positive for COVID-19, thus the incidence of COVID-19 infections was very low (0,002%). A significant percentage of patients (19,1%) refused to be tested. Among those who did not give consent for COVID-19 testing, 52,9% were male, although the total recruited population was prevalently female (56,1%). Discussion and conclusions Screening for COVID-19 in outpatient clinics is a promising tool to prevent virus outbreaks, despite the limitations posed by testing hesitancy. Moreover, the very low incidence of COVID-19 infection we detected could be seen as a sign of hope for the resumption of non-essential clinical activities

    On 14 August 2018, Morandi Bridge in Genoa, Italy, collapsed to the ground that was 40 m below. This tragedy killed 43 people. Preliminary investigations indicated poor design, questionable building practices, and insufficient maintenance\u2014or a combination of these factors\u2014as a possible cause of the collapse. However, around the collapse time, a thunderstorm associated with strong winds, lightning, and rain also developed over the city. While it is unclear if this thunderstorm played a role in the collapse, the present study examines the weather conditions before and during the bridge collapse. The study particularly focuses on the analysis of a downburst that was observed around the collapse time and a few kilometers away from the bridge. Direct and remote sensing measurements are used to describe the evolution of the thunderstorm during its approached from the sea to the city. The Doppler lidar measurements allowed the reconstruction of the gust front shape and the evaluation of its displacement velocity of 6.6 m s 121 towards the lidar. The Weather Research and Forecasting simulations highlighted that it is still challenging to forecast localized thunderstorms with operational setups. The study has shown that assimilation of radar reflectivity improves the timing and reconstruction of the gust front observed by local measurements

    Characteristics of Patients Experiencing a Flare of Generalized Pustular Psoriasis: A Multicenter Observational Study

    Background: Generalized pustular psoriasis (GPP) is a rare, severe inflammatory skin disease characterized by recurrent episodes of flares. Characteristics of patients experiencing a flare are hardly described in a real-life setting. The aim of the study is to investigate the clinical characteristics of patients experiencing a flare of GPP. Methods: Multicenter retrospective observational study on consecutive patients experiencing a flare of GPP between 2018 and 2022. Disease severity and quality of life were assessed by Generalized Pustular Psoriasis Area, Body Surface Area (BSA), and Severity Index (GPPASI), and Dermatology life quality index (DLQI) questionnaire, respectively. Visual analogue scale (VAS) of itch and pain, triggers, complications, comorbidities, pharmacological therapies, and outcome were collected. Results: A total of 66 patients, 45 (68.2%) females, mean age 58.1 ± 14.9 years, were included. The GPPASI, BSA, and DLQI were 22.9 ± 13.5 (mean ± standard deviation), 47.9 ± 29.1, and 21.0 ± 5.0, respectively. The VAS of itch and pain were 6.2 ± 3.3 and 6.2 ± 3.0, respectively. Fever (>38 ◦C) and leukocytosis (WBC > 12 × 109/L) were found in 26 (39.4%) and 39 (59.1%) patients, respectively. Precipitating triggers were identified in 24 (36.3%) and included infections (15.9%), drugs (10.6%), stressful life events (7.6%), and corticosteroids withdrawal (3.0%). Fourteen (21.2%) patients were hospitalized because of complications including infections in 9 (13.6%)leading to death in one case and hepatitis in 3 (4.5%). Conclusions: GPP flares can be severe and cause severe pain and itch with significant impact on the quality of life. In about one-third of patients the flare may have a persistent course and, with complications, lead to hospitalization

    Real-life effectiveness of tildrakizumab in chronic plaque psoriasis: A 52-week multicentre retrospective study—IL PSO (Italian landscape psoriasis)

    Background: Tildrakizumab is a humanized monoclonal antibody that binds selectively the p19 subunit of interleukin-23. It is approved for treatment of moderate– severe chronic plaque psoriasis. Objectives: We conducted a 52-week retrospective study to assess the effectiveness and safety of tildrakizumab in a real-life setting. Methods: Our retrospective study included 237 consecutive adults with moderateto-severe plaque psoriasis, enrolled in 10 different Italian centres, treated with tildrakizumab up to Week 52. Patient characteristics, comorbidities, previous treatmentsand the PASI (Psoriasis Area and Severity Index) score at each visit (baseline, Week 16, Week 28 and Week 52) were retrieved from the electronic medical records. The percentages of patients achieving 75%, 90% and 100% (PASI 75, PASI 90 and PASI 100) improvement in PASI with respect to baseline PASI were registered. Results: At Week 52, 90.91%, 73.55% and 58.68% of patients achieved a PASI reduction ≥75% (PASI 75), PASI 90 and PASI 100, respectively. An absolute PASI≤2 was reached by 85.95% at Week 52. Compared with Phase 3 clinical trials, we observed similar rates of PASI 75/90 responses and higher percentages of patients achieving PASI 100. Patients who had not responded to previous biologic treatments and patients with cardio-metabolic comorbidities were significantly more likely to achieve PASI 100 at Week 28 and PASI 90 at Week 52. The higher body mass index did not interfere with the odds of reaching PASI 75/90/100 at each time point. No significant safety findings were recorded throughout the study, and none of the patients had to interrupt the treatment because of adverse events. Conclusion: Our data suggest that the efficacy of tildrakizumab for plaque psoriasis in ‘real-life’ clinical practice is comparable with Phase 3 clinical trials with higher percentages of patients achieving complete skin clearance (PASI 100) at Weeks 16, 28 and 52

    Effectiveness, Tolerability, and Drug Survival of Risankizumab in a Real-World Setting: A Three-Year Retrospective Multicenter Study—IL PSO (ITALIAN LANDSCAPE PSORIASIS)

    Background: Risankizumab is a humanized monoclonal antibody that selectively inhibits interleukin-23. It has been approved for moderate-to-severe plaque psoriasis and has shown efficacy and safety in clinical trials and real-world experiences. This study aimed to evaluate the long-term effectiveness, safety, and drug survival of risankizumab in a real-life setting. Materials and Methods: We included patients treated with risankizumab from January 2019 to February 2023. A Psoriasis Area and Severity Index score (PASI) was collected at weeks 0, 16, 28, 52, 104, and 156, when available. The occurrence of any adverse events was recorded at each visit. Results: We enrolled 1047 patients. At week 52, a ≥90% improvement in PASI was observed in 81.44% of patients, with a continuous improvement throughout the study (88.99% and 99.07% at weeks 104 and 156, respectively). After three years of treatment, all patients involving the scalp, palms/soles, and genitalia and 95% of patients with nail psoriasis achieved a complete or almost complete skin clearance. No significant safety findings were observed, and 90.73% of the patients were still on treatment after 36 months. Conclusions: This study supports the long-term effectiveness and safety of risankizumab in a real- world setting, even in patients involving difficult-to-treat areas

    Anthropogenic and catchment characteristic signatures in the water quality of Swiss rivers: a quantitative assessment

    The hydrological and biogeochemical response of rivers carries information about solute sources, pathways, and transformations in the catchment. We investigate long-term water quality data of 11 Swiss catchments with the objective to discern the influence of major catchment characteristics and anthropic activities on delivery of solutes in stream water. Magnitude, trends, and seasonality of water quality samplings of different solutes are evaluated and compared across catchments. Subsequently, the empirical dependence between concentration and discharge is used to classify the solute behaviors. While the anthropogenic impacts are clearly detectable in the concentration of certain solutes (i.e., Na+, Cl−, NO3, DRP), the influence of single catchment characteristics such as geology (e.g., on Ca2+ and H4SiO4), topography (e.g., on DOC, TOC, and TP), and size (e.g., on DOC and TOC) is only sometimes visible, which is also because of the limited sample size and the spatial heterogeneity within catchments. Solute variability in time is generally smaller than discharge variability and the most significant trends in time are due to temporal variations of anthropogenic rather than natural forcing. The majority of solutes show dilution with increasing discharge, especially geogenic species, while sediment-bonded solutes (e.g., total phosphorous and organic carbon species) show higher concentrations with increasing discharge. Both natural and anthropogenic factors affect the biogeochemical response of streams, and, while the majority of solutes show identifiable behaviors in individual catchments, only a minority of behaviors can be generalized across the 11 catchments that exhibit different natural, climatic, and anthropogenic features.ISSN:1027-5606ISSN:1607-793
