    Physician-patient Communication In The Prevention Of Female Reproductive Tract Infections: Some Limitations.

    The objective of this study was to describe the information that women with tubal infertility recalled having received from physicians when they consulted for a reproductive tract infection (RTI) and the information physicians reported having provided to women consulting for RTIs. Interviews were conducted with 16 women presenting primary tubal infertility selected on the basis of purposeful sampling criteria. Also, 15 physicians working in the public health network in the city of Campinas, Brazil, were selected at random and interviewed. Analysis of the women's statements concerning previous RTI-related medical consultations showed that they did not recall having received any information on either prevention or the impact of RTI on their reproductive future. Analysis of interviews with physicians showed that the information they provided to women consulting for RTIs was incomplete and unclear. The information women recalled having received and that which physicians remembered having provided at the time of treating a patient with RTI was similar. In conclusion, these women lacked adequate or complete information and that it was probably not possible for them to adopt measures to avoid repetition of RTI and minimize risk of tubal infertility.16124925

    Social Inequality And Common Mental Disorders.

    OBJECTIVE: To analyze the association between the socioeconomic characteristics of individuals and common mental disorders. METHOD: A cross-sectional survey of a representative sample of the urban population, 14 years and older, in Campinas (Brazil) (n=515) was conducted using a multipurpose instrument that included the Self-Reporting Questionnaire (SRQ-20) to assess common mental disorders in the previous 3 months. Weighted prevalence of common mental disorders was calculated for each independent variable. Crude and adjusted prevalence ratios were estimated using Poisson regression. RESULTS: The overall prevalence was 17% (95% CI 12.8-22.3), 8.9% in males and 24.4% in females. An inverse association was found between common mental disorders and the socioeconomic characteristics (schooling and employment) even after controlling for all the other variables. Higher common mental disorders prevalence was observed in those with less than 5 years of schooling (PR=5.5) and unemployed or underemployed (PR=2.0). CONCLUSIONS: As in other studies, common mental disorders were unevenly distributed; it was significantly more frequent in socially disadvantaged individuals. Specific actions to reduce inequalities in the general and mental health system should be studied.29325025

    Effectiveness of brief intervention and contact for suicide attempters: A randomized controlled trial in five countries

    Objective: To determine whether brief intervention and contact is effective in reducing subsequent suicide mortality among suicide attempters in low and middle-income countries. Methods: Suicide attempters (n = 1867) identified by medical staff in the emergency units of eight collaborating hospitals in five culturally different sites (Campinas, Brazil; Chennai, India; Colombo, Sri Lanka; Karaj, Islamic Republic of Iran; and Yuncheng, China) participated, from January 2002 to October 2005, in a randomized controlled trial to receive either treatment as usual, or treatment as usual plus brief intervention and contact (BIC), which included patient education and follow-up. Overall, 91 completed the study. The primary study outcome measurement was death from suicide at 18-month follow-up. Findings: Significantly fewer deaths from suicide occurred in the BIC than in the treatment-as-usual group (0.2 versus 2.2, respectively; �2 = 13.83, P < 0.001). Conclusion: This low-cost brief intervention may be an important part of suicide prevention programmes for underresourced low-and middle-income countries

    Consultation-liaison Psychiatry In Brazil. Psychiatric Residency Training

    Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP)A mailing survey assessed consultation-liaison (C-L) service delivery and training among all 23 approved psychiatric residency training programs throughout the country. Twenty questionnaires were returned. Outpatient, inpatient, emergency, and consultation services were present together in 35% of the academic hospitals where residency training took place. Consultation service was provided by 18 (90%) responding programs. Eleven of those reported C-L training as part of their general psychiatry rotation. Nevertheless, educational objectives, weekly case conferences, and didactic seminars were provided by only five of them. The training time varied from 40 to 560 hours, 190 hours on average. The same five programs offered an elective third-year residency training, including liaison with specific units.     Introduction. It is not clear as to the process by which the psychiatrist attracts referrals and the reasons and motivations that doctors have for referring or not. This study describes the modus operandi of referrals to the psychiatrist in the context of a university general hospital. Method. A semi-structured interview was carried out on 50 doctors (35 male, 15 female) from the teaching hospital of the Universidade Estadual de Campinas, Brazil. Data was analized qualitatively, by means of content analysis, according to the psychodynamic approach. Results. Some referrals need to be worked through in order for the doctor to overcome his resistance. Others are automatic: because something moved him deeply, the doctor directs his anguish and responsability outside himself, without realizing the motives that lead him to do so. Conclusion.     Brazil is among the ten countries that record the highest absolute numbers of suicides, with 9,852 deaths in 2011. The average coefficient for the period of 2004-2010 was 5.7%. This figure increased 29.5% in the last two decades and is higher for men, elders, indigenous and small and mid-sized population towns. Mental disorders are present in most cases of suicide, especially depression, bipolar disorder, and dependence of alcohol and other psychoactive drugs. A population study revealed that, throughout life, 17.1% of people had suicidal ideation, 4.8% developed a suicide plan, and 2.8% actually attempted it.     Aim: To asses the support network of people that attempted suicide and were being seen at the emergency room of a general hospital, and compare it to the people who accompanied other kind of patients in the same place. Methods: Twenty-nine people (14 males and 15 females) were assessed consecutively in each group. The instruments used were: TS-PS (a questionnaire on suicide attempts) which included demographic and clinical data, the Social Support Questionnaire (SSQ), the Social Support Scale (self-rating), the GAS and the Garf, Results: The mean age of the individuals of the suicide attempt (SA) group was 37,4. Most attempts (11) involved drug overdosis, self-poisoning (ten) and cutting (eight). This group had a low suicide intention and lethality. 34,5% were married and in control-group 51.7% were too. The individuals of the AS group were more frequently receiving psychiatric treatment (41.4% vs 17.2%) and psychotropia drug (55.2% vs 13.8%). The number of people perceived as supportive by the AS was on average one and trhee by the control-group. The AS group report more dissatisfaction towards their relationships. These findings are in line with the worse figures of the AS group on the Garf and GAS. Conclusions: Our finding confirms the hypothesys that people who attempt suicide have a far weaker social support as to compared to the control-group.     Attempted Suicide: A Descriptive Study With Patients Admitted To A General Hospital [tentativas De Suicídio Envolvendo Risco De Vida: Internações Em Um Hospital Geral]

    This study was carried out at the Hospital das Clínicas of the Universidade Estadual de Campinas, Brazil. It describes the profile of 53 patients consecutively admitted to a general hospital after attempted suicide. A standardized instrument comprising clinical and demographic date was used. Most patients were male (66%) adolescents and young adults. The patients' mean age was 29 years old. Five (9%) patients died; all of them were male with a mean age of 23 years old. Male patients presented higher suicidal intent. They asked less frequently for help after the attempt. Among men the act had been more frequently planned and was believed to be lethal, revealing lesser degrees of impulsivity as compared to the observed in the female group. Poisoning was used in 77% of the cases. Shooting was used by three of the five who died. The other two used poisoning. Seventy-three per cent of the patients had from moderate to severe physical danger.
