SÃndrome metabólica em adolescentes obesos: comparação entre três diferentes critérios diagnósticos
Objective: To investigate the difference in the proportion of adolescents with metabolic syndrome diagnosed based on three different criteria, as well as the use of insulin resistance instead of fasting glucose.Methods: Cross-sectional study with 121 obese adolescents, between 10 and 14 years old, from public schools of the city of Porto Alegre, Brazil, in 2011. Anthropometric, blood pressure, and biochemical variables were assessed. Metabolic syndrome was defined using three different diagnostic criteria: the International Diabetes Federation (IDF), Cook and de Ferranti. All of them include five components: waist circumference, blood pressure, high-density lipoprotein (HDL) cholesterol, triglycerides and fasting glucose, and there should be at least three abnormal results for the diagnosis of the syndrome. the Homeostasis Model Assessment - Insuline Resistance (HOMA-IR) was used for the characterization of insulin resistance. the analysis of agreement among the criteria was performed using Kappa statistics.Results: Metabolic syndrome Was diagnosed in 39.7, 51.2, and 74.4% of adolescents, according to the IDF, Cook and de Ferranti criteria, respectively. There was agreement for the three diagnostic criteria in 60.3% of the sample. Waist circumference was the most prevalent component (81.0, 81.0, and 96.7%), whereas high fasting glucose was the least prevalent (7.4, 1.7, and 1.7%). the use of HOMA-IR significantly increased the proportion of positive diagnoses for the syndrome.Conclusion: the results showed significant differences between the three diagnostic criteria. EquilÃbrio corporal de idosos caidores e não caidores : influência de um programa fÃsico para prevenção de quedas
Com o processo de envelhecimento humano muitas alterações fisiológicas, psicológicas e anatômicas ocorrem no corpo humano. Entre essas mudanças destacam-se as alterações no equilÃbrio corporal, que se relacionam com o risco de quedas em idosos. O objetivo desse estudo foi verificar a influência do exercÃcio fÃsico no equilÃbrio corporal relacionado à prevenção de quedas em idosos com e sem histórico de quedas. Fizeram parte da amostra 23 idosos integrantes do projeto de extensão Centro de Estudos de Lazer e Atividade FÃsica do Idoso – CELARI na modalidade de equilÃbrio da Escola de Educação FÃsica da UFRGS, voluntários, de ambos os sexos, com idade entre 60 e 90 anos. Os participantes foram divididos entre dois grupos: a) idosos com histórico de quedas (caidores); b) idosos sem histórico de quedas (não caidores). O estudo foi do tipo quase-experimental e a análise de dados foi a estatÃstica descritiva (frequência e percentual da classificação dos testes) com auxÃlio do software SPSS 18.0. Para avaliação de equilÃbrio foram aplicados três testes: Alcance Funcional (AF); ´Timed Up and Go´ (TUG); Apoio Unipodal Anterior (AU). Os resultados indicaram que a média de idade foi de 75,4±6,68 anos. Verificou-se alteração no teste de Alcance Funcional nos dois grupos analisados: AF pré intervenção: caidores 55,6% baixo risco de queda; não caidores 69,2% sem risco de queda); pós intervenção: caidores 77,8% sem risco de queda, não caidores 92,3% sem risco de queda. No Apoio Unipodal indicou alteração somente para o grupo caidores na pré intervenção: caidores 44,4% baixo risco de quedas; não caidores 76,9% baixo risco de quedas; e, pós intervenção: caidores 55,6% baixo risco de quedas; não caidores 76,9% baixo risco de quedas. No TUG ocorreu uma redução do número de idosos caidores na classificação para baixo risco de quedas: TUG pré intervenção: caidores 100%; não caidores 100%; pós intervenção: caidores 88,9%; não caidores 100%. Constatou-se assim que, a intervenção proposta produziu melhora nos Ãndices de equilÃbrio corporal dos idosos, o que pode influenciar na diminuição no risco de quedas.With the process of human aging, many physiological, anatomical and psychological changes occur in the human body. Among these changes, there are the ones in body balance, which are related to the risk of fallings in elderly people. The aim of this study was to investigate the influence of exercises on body balance related to the prevention of fallings in elderly people with and without history of fallings. The sample included 23 elderly participants from the extension project called "Centro de Estudos de Lazer e Atividade FÃsica do Idoso" - CELARI in balance mode of the School of Physical Education from UFRGS, the participants were also volunteers, from both sexes, aged between 60 and 90 years. Participants were divided into two groups: a) elderly people with a history of fallings (fallers); b) no history of fallings elderly people (non-fallers). The study was quasi-experimental and data analysis was done under the descriptive statistics (frequency and percentage of classification test) made with the SPSS 18.0 software. As to the balance evaluation, three tests were applied: Functional Reach (FR); Timed Up and Go (TUG); and Single Leg Support (SLS). The results indicated that the overall age among the participants was 75.4 ± 6.68 years. It was verified that a change occurred in the Functional Range Test in the two analyzed groups: FR pre intervention (55.6% faller low risk of falling, non-faller 69.2% with no risk of falling). In addition, a post intervention (77.8% faller without risk of falling, 92.3 without risk of falling). In the Single Leg Support, there was change only for the faller group: pre intervention FR (faller 44.4% lower risk of fallings, non-faller 76.9% lower risk of fallings) and post intervention (faller 55.6% lower risk of fallings; non-faller 76.9% lower risk of fallings). At TUG, there was a reduction in the number of fallers in the classification down risk of fallings: TUG pre intervention (fallers 100%, non-fallers 100%) post intervention (fallers 88.9%; non-fallers 100%). Thus, it was found that the proposed intervention produced better improvements in rates of body balance of the elderly people, which may influence the decrease in the risk of fallings

Objetivo: Verificar a diferença na proporção de adolescentes com sÃndrome metabólica, diagnosticados por três diferentes critérios, bem como com a utilização da resistência à insulina no lugar da glicemia de jejum. Métodos: Estudo transversal com 121 adolescentes obesos, de 10 a 14 anos de idade, da rede municipal de ensino de Porto Alegre, em 2011. Foram realizadas avaliações antropométricas, de pressão arterial e bioquÃmicas. A sÃndrome metabólica foi definida por três critérios diagnósticos diferentes: International Diabetes Federation, Cook e de Ferranti. Todos eles incluem cinco componentes: perÃmetro abdominal, pressão arterial, lipoproteÃna de alta densidade (HDL) colesterol, triglicerÃdeos e glicemia em jejum, sendo necessária a alteração de pelo menos três para o diagnóstico da sÃndrome. Para a caracterização da resistência à insulina, foi utilizado o Ãndice Homeostasis Model Assessment - Insulin Resistance. A análise de concordância entre os critérios foi realizada pela estatÃstica de Kappa. Resultados: A sÃndrome metabólica foi observada em 39,7, 51,2 e 74,4% dos adolescentes, de acordo com as definições da International Diabetes Federation, Cook e de Ferranti, respectivamente. Houve concordância do diagnóstico para os três critérios, simultaneamente, em 60,3% da amostra. O componente mais prevalente foi o perÃmetro abdominal elevado (81,0, 81,0 e 96,7%), e o menos prevalente foi a glicemia em jejum elevada (7,4, 1,7 e 1,7%). A utilização do Homeostasis Model Assessment - Insulin Resistance aumentou significativamente a proporção de diagnósticos positivos para a sÃndrome. Conclusão: Os resultados mostraram considerável diferença entre os três critérios diagnósticos.
