    The Efficacy of Tele-practice on Expressive Language Outcomes for Adults with Aphasia

    Access to skilled speech and language intervention can be difficult for individuals residing in rural areas as well as for individuals with complex health and mobility issues. Telehealth (including therapy and rehabilitation) can provide effective services in the context of one’s home, allowing clinicians to reach a wider population of individuals. Purpose: To determine whether tele-practice service delivery produces positive expressive language outcomes that are comparable to direct service delivery for adults with aphasia. Method: A variety of databases were searched utilizing systematic inclusionary and exclusionary criteria. Research focused on adults over the age of 18 with a formal diagnosis of aphasia who engaged in telehealth intervention. Various research designs were identified and analyzed. Identified articles included a total of 235 participants. Results: The identified studies supported the implementation of tele-practice as a means of providing individuals with aphasia access to services that produce positive expressive language outcomes. Several studies indicated that tele-practice produces similar outcomes when compared to traditional direct therapy. Several studies also included qualitative data regarding patient satisfaction and quality of life, much of which produced positive outcomes. Conclusion: The chosen studies were found to largely support the inclusion of tele-practice as an effective option for producing positive expressive language outcomes for individuals with aphasia. Potential limitations include variability in treatment times and programs, assessment tools used, clinical training of individuals providing treatment, small sample sizes, and variable patient characteristics.     Protections for All? Attitudes Toward Civil Rights Protections for Trans and Gender Non-Binary Individuals

    Despite the increase in visibility and recognition, the rights of LGBTQ+ people continue to be debated and politically challenged. Transgender and gender non-binary people specifically face barriers to healthcare, government documents, and public accommodations. Due to the lack of literature connecting attitudes and expansive systemic protections for this population, this study aims to examine the impact of several constructs and identify what may be critical to explore in future research as components of attitudinal change. Empathy, contact, salience of religious identity, gender role belief, political affiliation, and gender were all identified in previous literature as having a relationship with attitudes toward LGBTQ+ people. Thus, they were utilized in this study to assess their relationship with attitudes toward expansive civil rights protections for TGNB individuals. 99 college students in the United States were participants. A hierarchical multiple regression indicated that empathy and gender role belief remained related to attitudes toward civil rights protections for TGNB individuals when accounting for the other variables. Participants were more likely to support expansive civil rights protections if they experience higher levels of empathy and have less strict gender role beliefs. Further, findings indicate empathy acts as a mechanism in the relationship between contact and attitudes toward civil rights protections. The impact of empathy and gender role beliefs should be considered when exploring how to increase support for expansive civil rights protections for TGNB people even when controlling for other factors known to be related to attitudes. Discussion and implications for future research are provided

    The spectrum of hot methane in astronomical objects using a comprehensive computed line list

    Hot methane spectra are important in environments ranging from flames to the atmospheres of cool stars and exoplanets. A new spectroscopic line list, 10to10, for 12^{12}CH4_4 containing almost 10 billion transitions is presented. This comprehensive line list covers a broad spectroscopic range and is applicable for temperatures up to 1 500 K. Previous methane data are incomplete leading to underestimated opacities at short wavelengths and elevated temperatures. Use of 10to10 in models of the bright T4.5 brown dwarf 2MASS 0559-14 leads to significantly better agreement with observations and in studies of the hot Jupiter exoplanet HD 189733b leads to up to a twentifold increase in methane abundance. It is demonstrated that proper inclusion of the huge increase in hot transitions which are important at elevated temperatures is crucial for accurate characterizations of atmospheres of brown dwarfs and exoplanets, especially when observed in the near-infrared.Comment: PNAS, Early Edition, June 16, 201

    Inflammation in the avian spleen: timing is everything

    <p>Abstract</p> <p>Background</p> <p>The synchrony of an organism with both its external and internal environment is critical to well-being and survival. As a result, organisms display daily cycles of physiology and behavior termed circadian rhythms. At the cellular level, circadian rhythms originate via interlocked autoregulatory feedback loops consisting of circadian clock genes and their proteins. These regulatory loops provide the molecular framework that enables the intracellular circadian timing system necessary to generate and maintain subsequent 24 hr rhythms. In the present study we examine the daily control of circadian clock genes and regulation of the inflammatory response by the circadian clock in the spleen.</p> <p>Results</p> <p>Our results reveal that circadian clock genes as well as proinflammatory cytokines, including Tnfά and IL-1β, display rhythmic oscillations of mRNA abundance over a 24 hr cycle. LPS-induced systemic inflammation applied at midday vs. midnight reveals a differential response of proinflammatory cytokine induction in the spleen, suggesting a daily rhythm of inflammation. Exogenous melatonin administration at midday prior to LPS stimulation conveys pleiotropic effects, enhancing and repressing inflammatory cytokines, indicating melatonin functions as both a pro- and anti-inflammatory molecule in the spleen.</p> <p>Conclusion</p> <p>In summary, a daily oscillation of circadian clock genes and inflammatory cytokines as well as the ability of melatonin to function as a daily mediator of inflammation provides valuable information to aid in deciphering how the circadian timing system regulates immune function at the molecular level. However, further research is needed to clarify the precise mechanisms by which the circadian clock and melatonin have an impact upon daily immune functions in the periphery.</p

    Acute acetaminophen ingestion improves performance and muscle activation during maximal intermittent knee extensor exercise

The final version is available from the publisher via the DOI in this record.AIM: Acetaminophen is a commonly used medicine for pain relief and emerging evidence suggests that it may improve endurance exercise performance. This study investigated some of the physiological mechanisms by which acute acetaminophen ingestion might blunt muscle fatigue development. METHODS: Thirteen active males completed 60 × 3 s maximum voluntary contractions (MVC) of the knee extensors with each contraction separated by a 2 s passive recovery period. This protocol was completed 60 min after ingesting 1 g of maltodextrin (placebo) or 1 g of acetaminophen on two separate visits. Peripheral nerve stimulation was administered every 6th contraction for assessment of neuromuscular fatigue development, with the critical torque (CT), which reflects the maximal sustainable rate of oxidative metabolism, taken as the mean torque over the last 12 contractions. Surface electromyography was recorded continuously as a measure of muscle activation. RESULTS: Mean torque (61 ± 11 vs. 58 ± 14% pre-exercise MVC) and CT (44 ± 13 vs. 40 ± 15% pre-exercise MVC) were greater in the acetaminophen trial compared to placebo (both P  0.05). However, the decline in electromyography amplitude was attenuated in the acetaminophen trial, with electromyography amplitude being greater compared to placebo from 210 s onwards (P < 0.05). CONCLUSION: These findings indicate that acute acetaminophen ingestion might be ergogenic by increasing CT and preserving muscle activation during high-intensity exercise.This research was not sponsored by any funding body external to University of Exete

    A Rare Cause of Thrombotic Thrombocytopenia Purpura- (TTP-) Like Syndrome, Vitamin B12 Deficiency: Interpretation of Significant Pathological Findings

    Thrombotic thrombocytopenia purpura (TTP) is a hematological emergency that requires rapid assessment followed by prompt initiation of therapy due to high mortality associated with delayed treatment. TTP has many causes including heritable syndromes, ADAMTS13 deficiency, and drugs-related etiologies. Profound vitamin B12 deficiency can, in rare cases, mimic TTP in presentation, and since plasmapheresis can be of limited benefit, prompt diagnosis is necessary for accurate treatment with B12. Therefore, careful analysis of all clinical signs, symptoms, and labs must be assessed.
