    Emergency care research priorities in South Africa

    Background: Emergency care research is rarely undertaken in low- and middle-income countries. A manageable ‘road map’ for research in South African (SA) emergency care is needed to address research gaps.Objective: To identify, collate and prioritise research topics from identified knowledge gaps in emergency care in SA.Methods: Seventy-six individuals were invited to participate in a modified Delphi study. Participants were requested to suggest important research topics before rating them. Consensus was achieved when >75% of participants strongly agreed or disagreed. Participants then ranked the agreed statements before selecting the most appropriate methodology relating to study design, funding and collaboration.Results: Three hundred and fifty topics were suggested by 31 participants. Topics were collated into 123 statements before participants rated them. Consensus was achieved for 39 statements. The highest-ranked priority in the prehospital group was to determine which prehospital interventions improve outcomes in critically ill patients. The competence of emergency care providers in performing common lifesaving skills was deemed the most important in clinical emergency care. Implementing and reviewing quality improvement systems scored the highest under general systems and safety management. Only 22 statements achieved consensus regarding study design. The National Department of Health was the preferred funding source, while private organisations and emergency care societies were identified as possible collaborative partners.Conclusion: This study provides expert consensus on priority research areas in emergency care in SA as a guide for emergency care providers to ensure evidence-based care that is relevant to the SA population

    Extension of Yeast Chronological Lifespan by Methylamine

    Background: Chronological aging of yeast cells is commonly used as a model for aging of human post-mitotic cells. The yeast Saccharomyces cerevisiae grown on glucose in the presence of ammonium sulphate is mainly used in yeast aging research. We have analyzed chronological aging of the yeast Hansenula polymorpha grown at conditions that require primary peroxisome metabolism for growth. Methodology/Principal Findings: The chronological lifespan of H. polymorpha is strongly enhanced when cells are grown on methanol or ethanol, metabolized by peroxisome enzymes, relative to growth on glucose that does not require peroxisomes. The short lifespan of H. polymorpha on glucose is mainly due to medium acidification, whereas most likely ROS do not play an important role. Growth of cells on methanol/methylamine instead of methanol/ammonium sulphate resulted in further lifespan enhancement. This was unrelated to medium acidification. We show that oxidation of methylamine by peroxisomal amine oxidase at carbon starvation conditions is responsible for lifespan extension. The methylamine oxidation product formaldehyde is further oxidized resulting in NADH generation, which contributes to increased ATP generation and reduction of ROS levels in the stationary phase. Conclusion/Significance: We conclude that primary peroxisome metabolism enhanced chronological lifespan of H. polymorpha. Moreover, the possibility to generate NADH at carbon starvation conditions by an organic nitrogen source supports further extension of the lifespan of the cell. Consequently, the interpretation of CLS analyses in yeast should include possible effects on the energy status of the cell.

    Are Bone and Muscle Changes from POWER PE, an 8-month In-school Jumping Intervention, Maintained at Three Years?

    Our aim was to determine if the musculoskeletal benefits of a twice-weekly, school-based, jumping regime in healthy adolescent boys and girls were maintained three years later. Subjects of the original POWER PE trial (n = 99) were contacted and asked to undergo retesting three years after cessation of the intervention. All original measures were completed including: sitting height, standing height, weight, calcaneal broadband ultrasound attenuation (BUA), whole body, hip and spine bone mineral content (BMC), lean tissue mass, and fat mass. Physical activity was recorded with the bone-specific physical activity questionnaire (BPAQ) and calcium intake was estimated with a calcium-focussed food questionnaire. Maturity was determined by Tanner staging and estimation of the age of peak height velocity (PHV). Twenty-nine adolescents aged 17.3±0.4 years agreed to participate. Three years after the intervention, there were no differences in subject characteristics between control and intervention groups (p>0.05). Three-year change in weight, lean mass, and fat mass were similar between groups (p>0.05). There were no significant group differences in three-year change in BUA or BMC at any site (p>0.05), although the between-group difference in femoral neck BMC at follow-up exceeded the least significant change. While significant group differences were not observed three years after cessation of the intervention, changes in bone parameters occurred in parallel for intervention and control groups such that the original benefits of the intervention observed within the treatment group were sustained

    How large should whales be?

    The evolution and distribution of species body sizes for terrestrial mammals is well-explained by a macroevolutionary tradeoff between short-term selective advantages and long-term extinction risks from increased species body size, unfolding above the 2g minimum size induced by thermoregulation in air. Here, we consider whether this same tradeoff, formalized as a constrained convection-reaction-diffusion system, can also explain the sizes of fully aquatic mammals, which have not previously been considered. By replacing the terrestrial minimum with a pelagic one, at roughly 7000g, the terrestrial mammal tradeoff model accurately predicts, with no tunable parameters, the observed body masses of all extant cetacean species, including the 175,000,000g Blue Whale. This strong agreement between theory and data suggests that a universal macroevolutionary tradeoff governs body size evolution for all mammals, regardless of their habitat. The dramatic sizes of cetaceans can thus be attributed mainly to the increased convective heat loss is water, which shifts the species size distribution upward and pushes its right tail into ranges inaccessible to terrestrial mammals. Under this macroevolutionary tradeoff, the largest expected species occurs where the rate at which smaller-bodied species move up into large-bodied niches approximately equals the rate at which extinction removes them.Comment: 7 pages, 3 figures, 2 data table

    A head-to-head comparison of speckle tracking echocardiography and feature tracking cardiovascular magnetic resonance imaging in right ventricular deformation

    Aims: Speckle tracking echocardiography (STE) and feature tracking cardiovascular magnetic resonance imaging (FT-CMR) are advanced imaging techniques which are both used for quantification of global and regional myocardial strain. Direct comparisons of STE and FT-CMR regarding right ventricular (RV) strain analysis are limited. We aimed to study clinical performance, correlation and agreement of RV strain by these techniques, using arrhythmogenic right ventricular cardiomyopathy (ARVC) as a model for RV disease. // Methods and results: We enrolled 110 subjects, including 34 patients with definite ARVC, 30 preclinical relatives of ARVC patients, and 46 healthy control subjects. Global and regional RV longitudinal peak strain (PS) were measured by STE and FT-CMR. Both modalities showed reduced strain values in ARVC patients compared to ARVC relatives (STE global PS: P < 0.001; FT-CMR global PS: P < 0.001) and reduced strain values in ARVC relatives compared to healthy control subjects (STE global PS: P = 0.042; FT-CMR global PS: P = 0.084). There was a moderate, albeit significant correlation between RV strain values obtained by STE and FT-CMR [global PS r = 0.578 (95% confidence interval 0.427–0.697), P < 0.001]. Agreement between the techniques was weak (limits of agreement for global PS: ±11.8%). Correlation and agreement both deteriorated when regional strain was studied. // Conclusion: RV STE and FT-CMR show a similar trend within the spectrum of ARVC and have significant correlation, but inter-modality agreement is weak. STE and FT-CMR may therefore both individually have added value for assessment of RV function, but RV PS values obtained by these techniques currently cannot be used interchangeably in clinical practice

    Deposition and preservation of fluvio-tidal shallow-marine sandstones: A re-evaluation of the Neoproterozoic Jura Quartzite (western Scotland)

    The 2 to 5 km thick, sandstone‐dominated (>90%) Jura Quartzite is an extreme example of a mature Neoproterozoic sandstone, previously interpreted as a tide‐influenced shelf deposit and herein re‐interpreted within a fluvio‐tidal deltaic depositional model. Three issues are addressed: (i) evidence for the re‐interpretation from tidal shelf to tidal delta; (ii) reasons for vertical facies uniformity; and (iii) sand supply mechanisms to form thick tidal‐shelf sandstones. The predominant facies (compound cross‐bedded, coarse‐grained sandstones) represents the lower parts of metres to tens of metres high, transverse fluvio‐tidal bedforms with superimposed smaller bedforms. Ubiquitous erosional surfaces, some with granule–pebble lags, record erosion of the upper parts of those bedforms. There was selective preservation of the higher energy, topographically‐lower, parts of channel‐bar systems. Strongly asymmetrical, bimodal, palaeocurrents are interpreted as due to associated selective preservation of fluvially‐enhanced ebb tidal currents. Finer‐grained facies are scarce, due largely to suspended sediment bypass. They record deposition in lower‐energy environments, including channel mouth bars, between and down depositional‐dip of higher energy fluvio‐ebb tidal bars. The lack of wave‐formed sedimentary structures and low continuity of mudstone and sandstone interbeds, support deposition in a non‐shelf setting. Hence, a sand‐rich, fluvial–tidal, current‐dominated, largely sub‐tidal, delta setting is proposed. This new interpretation avoids the problem of transporting large amounts of coarse sand to a shelf. Facies uniformity and vertical stacking are likely due to sediment oversupply and bypass rather than balanced sediment supply and subsidence rates. However, facies evidence of relative sea level changes is difficult to recognise, which is attributed to: (i) the areally extensive and polygenetic nature of the preserved facies, and (ii) a large stored sediment buffer that dampened response to relative sea‐level and/or sediment supply changes. Consideration of preservation bias towards high‐energy deposits may be more generally relevant, especially to thick Neoproterozoic and Lower Palaeozoic marine sandstones

    A Compromise between Neutrino Masses and Collider Signatures in the Type-II Seesaw Model

    A natural extension of the standard SU(2)L×U(1)YSU(2)_{\rm L} \times U(1)_{\rm Y} gauge model to accommodate massive neutrinos is to introduce one Higgs triplet and three right-handed Majorana neutrinos, leading to a 6×66\times 6 neutrino mass matrix which contains three 3×33\times 3 sub-matrices MLM_{\rm L}, MDM_{\rm D} and MRM_{\rm R}. We show that three light Majorana neutrinos (i.e., the mass eigenstates of Îœe\nu_e, ΜΌ\nu_\mu and Μτ\nu_\tau) are exactly massless in this model, if and only if ML=MDMR−1MDTM_{\rm L} = M_{\rm D} M_{\rm R}^{-1} M_{\rm D}^T exactly holds. This no-go theorem implies that small but non-vanishing neutrino masses may result from a significant but incomplete cancellation between MLM_{\rm L} and MDMR−1MDTM_{\rm D} M_{\rm R}^{-1} M_{\rm D}^T terms in the Type-II seesaw formula, provided three right-handed Majorana neutrinos are of O(1){\cal O}(1) TeV and experimentally detectable at the LHC. We propose three simple Type-II seesaw scenarios with the A4×U(1)XA_4 \times U(1)_{\rm X} flavor symmetry to interpret the observed neutrino mass spectrum and neutrino mixing pattern. Such a TeV-scale neutrino model can be tested in two complementary ways: (1) searching for possible collider signatures of lepton number violation induced by the right-handed Majorana neutrinos and doubly-charged Higgs particles; and (2) searching for possible consequences of unitarity violation of the 3×33\times 3 neutrino mixing matrix in the future long-baseline neutrino oscillation experiments.Comment: RevTeX 19 pages, no figure

    Brief communication: Impacts of a developing polynya off Commonwealth Bay, East Antarctica, triggered by grounding of iceberg B09B

    The dramatic calving of the Mertz Glacier tongue in 2010, precipitated by the movement of iceberg B09B, reshaped the oceanographic regime across the Mertz Polynya and Commonwealth Bay, regions where high-salinity shelf water (HSSW) - the precursor to Antarctic bottom water (AABW) - is formed. Here we present post-calving observations that suggest that this reconfiguration and subsequent grounding of B09B have driven the development of a new polynya and associated HSSW production off Commonwealth Bay. Supported by satellite observations and modelling, our findings demonstrate how local icescape changes may impact the formation of HSSW, with potential implications for large-scale ocean circulation

    A systematic review of correlates of sedentary behaviour in adults aged 18–65 years: a socio-ecological approach

    Background: Recent research shows that sedentary behaviour is associated with adverse cardio-metabolic consequences even among those considered sufficiently physically active. In order to successfully develop interventions to address this unhealthy behaviour, factors that influence sedentariness need to be identified and fully understood. The aim of this review is to identify individual, social, environmental, and policy-related determinants or correlates of sedentary behaviours among adults aged 18-65 years. Methods: PubMed, Embase, CINAHL, PsycINFO and Web of Science were searched for articles published between January 2000 and September 2015. The search strategy was based on four key elements and their synonyms: (a) sedentary behaviour (b) correlates (c) types of sedentary behaviours (d) types of correlates. Articles were included if information relating to sedentary behaviour in adults (18-65 years) was reported. Studies on samples selected by disease were excluded. The full protocol is available from PROSPERO (PROSPERO 2014:CRD42014009823). Results: 74 original studies were identified out of 4041: 71 observational, two qualitative and one experimental study. Sedentary behaviour was primarily measured as self-reported screen leisure time and total sitting time. In 15 studies, objectively measured total sedentary time was reported: accelerometry (n = 14) and heart rate (n = 1). Individual level factors such as age, physical activity levels, body mass index, socio-economic status and mood were all significantly correlated with sedentariness. A trend towards increased amounts of leisure screen time was identified in those married or cohabiting while having children resulted in less total sitting time. Several environmental correlates were identified including proximity of green space, neighbourhood walkability and safety and weather. Conclusions: Results provide further evidence relating to several already recognised individual level factors and preliminary evidence relating to social and environmental factors that should be further investigated. Most studies relied upon cross-sectional design limiting causal inference and the heterogeneity of the sedentary measures prevented direct comparison of findings. Future research necessitates longitudinal study designs, exploration of policy-related factors, further exploration of environmental factors, analysis of inter-relationships between identified factors and better classification of sedentary behaviour domains
