    The Influence of Deceptive Advertising and Consumer Loyalty in Telecommunication Industry

    The purpose of this study is to measure the impact of deceptive advertising on customer loyalty in telecom Sector of Pakistan. Calls Charges, SMS Packages, Internet Charges and Network Coverage & Quality were used to measure the deception in ads. Customer loyalty is measure through the satisfaction, recommendation, and continuity of network usage. 215 respondents were approached to conclude the results. Data was analyzed by using SPSS 17. It has been found that more or less every service provider telecom sector is using deceptive advertising and customers are stick to their current networks because of other more influencing reasons and almost all network / services providers are providing more or less same services and they are using same style of advertising to compete with each other.     Sacrohysteropexy Using Mesh for Uterovaginal Prolapsed in Women Who Wish to Retain Uterus

    Objective: to study the ongoing results of sacrohysteropexy with Teflon mesh for treatment of uterovaginal prolapse in women who desire to preserve their uterus.Study Design:Cross sectional study.Place and Duration:Study was conducted in Gynacology department of Nishtar hospital, Multan, POF Hospital Rawalpindi and Holy Family Hospital Rawalpindi in one year duration from April 2016 to April 2017.Methodology: A total number of three hundred and eighty two(n=382) patients.Patients who desired to retain their uterus were evaluated by standard questionnaire, general physical examination and opturodynamic techniques. Six main outcome variables were assessed: Stress incontinence, urgency, urge incontinence, prolapse symptoms, constipation and dyspareunia. All these variables were assessed on basis of their severity. Data was analysed by using SPSS version 24. P value ≤ 0.05 was considered as significant.Results:in our study we observed outcome variables as stress incontinence, urgency, urge incontinence after sacrohysteropexy, observation was made on basis of relief from symptoms as mild and moderate 28.5% and 0.3%, 15.2% and 8.4%, 8.6% and 9.2% respectively. Constipation was noted as mild, moderate and severe 32.2%, 7.6% and 8.9% respectively.  Dyspareunia was noted as mild and moderate 24.1% and 0.5% respectively. All of these values are lessen when compared with pre-operative baseline values.Conclusion:Results of our study revealed that sacrohysteropexywith Teflon mesh is an effective and successful procedure for patients who were facing uterovaginal prolapse symptoms but desired to retain their uterus for fertility or childbearing.     Outcome of Lateral Internal Sphincterotomy in the Management of Chronic Fissure in Ano

    Objective: to assess the outcome of lateral internal Sphincterotomy in the management of chronic fissure in ano.Study Design: Cross sectional StudyStudy Place and Duration: Department of General Surgery Nishtar hospital, Multan, POF Hospital Rawalpindi and Holy Family Hospital Rawalpindi from 12th January 2018 to 15th July 2018.Material and methods: A total number of 140 patients were enrolled in the study. Data regarding age, sex, day of operation, duration of postoperative hospital stay, position of the fissure in ano and mean follow up period was measured and recorded in the form of a performed performa. A designed questionnaire was provided to the patients, which contained questions regarding the level of satisfaction, need of further treatment, leakage of gas, fluid or feces and development of symptoms consistent with the symptoms of the previous fissure in ano. Data analysis; the data thus obtained was subjected to statistical analysis using computer software SPSS version 23. Frequency and percentages were calculated for qualitative variables while mean and standard deviation was calculated for quantitative variables.Results: Postoperatively, 17 (12.1%) patients complained of painful defecation and 9 (6.4%) patients complained of bleeding and only 12 (8.6%) needed medical treatment for these complaints. Postoperative incontinence to flatus was seen in 11 (7.8%) patients and 8 (5.7%) patients complained of incontinence to flatus and fluids. Only 2 (1.4%) patients complained of incontinence to feces. Overall new incidence of incontinence was seen in 5 (3.6%) of the patients.Conclusion: Outcome of lateral internal sphincterotomy are excellent as it is associated with a very low rate of incontinence, bleeding or painful defecation postoperatively and thus it improves the quality of life among the patients of chronic fissure in ano. It can be suggested that lateral internal Sphincterotomy is a safe and effective procedure for chronic fissure in ano.     Factors leading to dyspepsia in renal transplant recipients

    Introduction: Renal transplantation is the definitive treatment for end stage renal disease. Patients subjected to transplantation require lifelong immunosuppression and are prone to several gastrointestinal disorders. Dyspepsia is a common disorder in these patients. The objective of this study was to determine factors leading to dyspepsia in renal (kidney) transplant recipients. Methods: it was a cross sectional study conducted at department of hepatogastroenterology and transplant sciences, SIUT Karachi, from 1-6-15 to 1-12-15 for six months. All renal transplanted patients having dyspeptic symptoms for more than 6 weeks. EGD was performed, biopsy specimens obtained from antrum and duodenum, these were sent for histopathological examination. Frequency and percentages were obtained for categorical variables, mean ± SD was calculated for continuous variables. Chi square test was used for categorical variable and student t-test for continuous variables. Results: Ninety patients were included in the study out of which 64 (71.1%) were males, mean age was 35.82 ± 10.04 years (range: 18-65 years). Gastritis (non H.pylori associated) in 78 (78.6%), duodenitis in 35 (38.9%) and H. pylori infection in 29 (32.2%), renal transplant recipients. Most of the patients belonged to Sindhi ethnicity, 27 (30%), followed by Punjabi. Hypertension was the most common co-morbid condition in our patients found in 29 (32.2%), while most of them don't have any co morbid condition. Duodenitis was found to be associated with tacrolimus use (p = 0.037). Conclusion: Gastritis is the most common factor accountable for this symptoms, followed by duodenitis and H. Pylori. Patients taking tacrolimus as immunosuppressant are more prone to develop duodenitis

    Tukhm Khashkhaash is the seeds of Papaver somniferum belonging to the family Papaveraceae. The description of Poppy seeds is mentioned in various classical Unani books. Renowned Unani scholar Avicenna had also described poppy seeds in his famous book “The Canon of Medicine”. He wrote that Tukhme Khashkhaash safaid are the small white seeds obtained from the capsule of white poppy plant. The seeds which are obtained before the incision of capsules for the extraction of opium are considered to be of best quality. They are used since centuries for treating various ailments like diarrhea, dysentery, cough, premature ejaculation, chronic cough, insomnia and skin disorders as well as a nutritive food. Their antibacterial, anti-carcinogenic and hypoglycemic potential have also proved on scientific basis.     Role of Serum Vitamin B12 and Glutathione Peroxidase during Third Trimester of Pregnancy with Gestational Diabetes Mellitus

    Objective: To investigate the role and relationship of serum Vitamin B12 and Glutathione peroxidase in pregnancy with gestational diabetes mellitus (GDM). Study Design: Comparative cross-sectional study. Place and Duration of Study: Physiology Department, University of Health Sciences, Lahore Pakistan, from Feb 2019 to Mar 2020. Methodology: Ninty pregnant women were recruited, fulfilling the inclusion and exclusion criteria. Women with GDM were the cases, and those without GDM were the controls. Serum Vitamin B12 and Glutathione peroxidase were measured by ELISA. Results: Serum Vitamin B12 was significantly lower in cases compared to controls (158.98±7.38 and 357.48±42.07mg/dL, respectively, at p<0.001. Glutathione peroxidase was also significantly lower in cases compared to controls (2.68±1.13U/mL and 5.53±1.05U/mL, respectively, at p<0.001. Correlation analysis showed that Vitamin B12 and Glutathione peroxidase had a significant positive correlation in cases (p<0.001 and rho=0.76) and a non-significant correlation in controls (p=0.87 and rho=-0.024). Linear regression analysis showed that Vitamin B12 positively affects Glutathione peroxidase (β=0.13, p<0.001).Vitamin B12 and Glutathione peroxidase have a significant, negative effect on fasting blood glucose (β =-0.06, p<0.001 and β=- 3.66, p<0.001, respectively). Conclusion: Deficient Vitamin B12 leads to reduced Glutathione levels in GDM pregnancy. A deficiency of these two predicts elevated blood sugar levels

    Topical cystic fibrosis transmembrane conductance regulator gene replacement for cystic fibrosis-related lung disease

    BACKGROUND: Cystic fibrosis is caused by a defective gene encoding a protein called the cystic fibrosis transmembrane conductance regulator (CFTR), and is characterised by chronic lung infection resulting in inflammation and progressive lung damage that results in a reduced life expectancy. OBJECTIVES: To determine whether topical CFTR gene replacement therapy to the lungs in people with cystic fibrosis is associated with improvements in clinical outcomes, and to assess any adverse effects. SEARCH METHODS: We searched the Cochrane Cystic Fibrosis and Genetic Disorders Group Trials Register comprising references identified from comprehensive electronic database searches, handsearching relevant journals and abstract books of conference proceedings. Date of most recent search: 05 May 2016. An additional search of the National Institutes for Health (NIH) Genetic Modification Clinical Research Information System (GeMCRIS) was also performed for the years 1992 to 2015. Date of most recent search: 20 April 2016. SELECTION CRITERIA: Randomised controlled studies comparing topical CFTR gene delivery to the lung, using either viral or non‐viral delivery systems, with placebo or an alternative delivery system in people with confirmed cystic fibrosis. DATA COLLECTION AND ANALYSIS: The authors independently extracted data and assessed study quality. Authors of included studies were contacted and asked for any available additional data. Meta‐analysis was limited due to differing study designs. MAIN RESULTS: Four randomised controlled studies met the inclusion criteria for this review, involving a total of 302 participants lasting from 29 days to 13 months; 14 studies were excluded. The included studies differed in terms of CFTR gene replacement agent and study design, which limited the meta‐analysis. One study only enrolled adult males, the remaining studies included both males and females aged 12 years and over. Risk of bias in the studies was moderate. Random sequence generation and allocation concealment was only described in the more recent study; the remaining three studies were judged to have an unclear risk of bias. All four studies documented double‐blinding to the intervention, but there is some uncertainty with regards to participant blinding in one study. Some outcome data were missing from all four studies. There were no differences in either the number of respiratory exacerbations or the number of participants with an exacerbation between replacement therapy or placebo groups at any time point. Meta‐analysis of most respiratory function tests showed no difference between treatment and placebo groups, but the smallest study (n = 16) reported forced vital capacity (litres) increased more in the placebo group at up to 24 hours. A further study reported a significant improvement in forced expiratory volume at one second (litres) at 30 days after participants had received their first dose of favouring the gene therapy agent, but this finding was not confirmed when combined with at second study in the meta‐analysis. The more recent study (n = 140) demonstrated a small improvement in forced vital capacity (per cent predicted) at two and three months and again at 11 and 12 months for participants receiving CFTR gene replacement therapy compared to those receiving placebo. The same study reported a significant difference in the relative change in forced expiratory volume at one second (per cent predicted) at two months, three months and 12 months. One small study reported significant concerns with "influenza‐like" symptoms in participants treated with CFTR gene replacement therapy; this was not reported on repeated use of the same agent in a larger recent study. There was no other evidence of positive impact on outcomes, in particular improved quality of life or reduced treatment burden. Two studies measured ion transport in the lower airways; one (n = 16) demonstrated significant changes toward normal values in the participants who received gene transfer agents (P < 0.0001), mean difference 6.86 (95% confidence interval 3.77 to 9.95). The second study (n = 140) also reported significant changes toward normal values (P = 0.032); however, aggregate data were not available for analysis. In the most recent study, there was also evidence of increased salt transport in cells obtained by brushing the lower airway. These outcomes, whilst important, are not of direct clinical relevance. AUTHORS' CONCLUSIONS: One study of liposome‐based CFTR gene transfer therapy demonstrated some improvements in respiratory function in people with CF, but this limited evidence of efficacy does not support this treatment as a routine therapy at present. There was no evidence of efficacy for viral‐mediated gene delivery. Future studies need to investigate clinically important outcome measures

    Anti-seizure activity of flower extracts of Nepeta bractaeta in Swiss albino mice

    Epilepsy is a neurological disorder characterized by unprovoked, recurring seizures that disrupts the nervous system and can cause mental and physical dysfunction. Based on the ethno pharmacological information of the plant, the methanolic and aqueous extracts of the flowers of Nepeta bractaeta was evaluated for its antiepileptic activity. The methanolic and aqueous extracts of the flowers of Nepeta bracteata were observed for their antiepileptic activity by increased current Electroshock seizures (ICES) test and Pentylenetetrazole (PTZ) test using Swiss albino mice. Both the extracts showed significant activity in ICES and PTZ induced convulsions in comparison to control. In ICES model, NBAE at higher dose showed 16.7 % and NBME at higher dose showed 33.3 % protection against seizure and in PTZ model, NBME at higher dose showed 33.3 % protection against seizure. From the experiments performed, it can be said that Nepeta bractaeta does possess anticonvulsant property
