Youth and the Millennium Development Goals (MDGs): Challenges and Opportunities for Implementation
Young people ages 15 to 24 are 1.2 billion of the worldâs human capital. Around the world, many of them are already making contributions to the Millennium Development Goals (MDGs), and their work should be further acknowledged and strengthened. Increasingly, youth are recognized as key participants in decision-making and development, as reflected in the growing presence of non-governmental youth organizations and the upsurge of youth advisory boards and committees to international institutions and programmes. Yet building the capacity of and creating sustained partnerships with young people are crucial strategies to achieving the MDGs that have not been fully realized by the international community. This paper aims to provide an overview of youth participation as it currently exists, to outline the ways in which youth are directly involved and affected by each Goal, to demonstrate the ways in which young people are contributing to the MDGs, and to provide âOptions for Actionâ that governments, the United Nations system, donors and other actors can harness, support, and scale-up in order to support young people in making significant contributions to achieving the MDGs. Part I outlines the existing mechanisms for youth participation in development policy. These channels can be used by governments and institutions to strengthen and mobilize young people as partners in policy formulation. Successful modes of participation should be recognized and replicated, and also adapted to the challenging political and socio-economic realities facing many youth-led and youth-serving organizations. Part II presents youth participation as it relates directly to the MDGs. Each goal is analyzed with respect to its effect on young peoples lives as well as how young people can play â and indeed are playing â a role in its implementation. Under each goal are a number of âOptions for Actionâ that governments, the UN and multilateral organizations can use to fully harness the contributions that youth can make to achieving the MDGs. Part III outlines the synergies between the Options for Action presented in this report and the Quick Wins proposed by the Millennium Project. The Options for Action are complimentary and provide a process to implement the Quick Win actions, using young people as key implementing agents and service providers. Part III also outlines a number of youth-focused Quick Wins that can make a significant and measurable difference to the state of young people in target countries. Part IV elaborates on how youth can participate in achieving the MDGs and contains cross-cutting recommendations on youth engagement in all 8 Goals. Overall, the report demonstrates that investing in youth will provide the longest and most effective dividend towards meeting the Millennium Development Goals (MDGs) by building the social capital needed to foster pragmatic development. Indeed, without the involvement of young people, a demographic that comprises one fifth of the worldâs total population, the full achievement of the MDGs will remain elusive and their long-term sustainability will be compromised. Youth participation is currently quite varied, ranging from effective, to sometimes tokenistic, to often non-existent. There are specific ways in which youth and youth organizations can contribute to the design and implementation of MDG-based strategies, some of which are outlined in this document. Evaluating techniques for metagenome annotation using simulated sequence data
The advent of next-generation sequencing has allowed huge amounts of DNA sequence data to be produced, advancing the capabilities of microbial ecosystem studies. The current challenge is identifying from which microorganisms and genes the DNA originated. Several tools and databases are available for annotating DNA sequences. The tools, databases and parameters used can have a significant impact on the results: naĂŻve choice of these factors can result in a false representation of community composition and function. We use a simulated metagenome to show how different parameters affect annotation accuracy by evaluating the sequence annotation performances of MEGAN, MG-RAST, One Codex and Megablast. This simulated metagenome allowed the recovery of known organism and function abundances to be quantitatively evaluated, which is not possible for environmental metagenomes. The performance of each program and database varied, e.g. One Codex correctly annotated many sequences at the genus level, whereas MG-RAST RefSeq produced many false positive annotations. This effect decreased as the taxonomic level investigated increased. Selecting more stringent parameters decreases the annotation sensitivity, but increases precision. Ultimately, there is a trade-off between taxonomic resolution and annotation accuracy. These results should be considered when annotating metagenomes and interpreting results from previous studies

The Therapeutic Potential of Mangosteen Pericarp as an Adjunctive Therapy for Bipolar Disorder and Schizophrenia
New treatments are urgently needed for serious mental illnesses including bipolar disorder and schizophrenia. This review proposes that Garcinia mangostana Linn. (mangosteen) pericarp is a possible adjunctive therapeutic agent for these disorders. Research to date demonstrates that neurobiological properties of the mangosteen pericarp are well aligned with the current understanding of the pathophysiology of bipolar disorder and schizophrenia. Mangosteen pericarp has antioxidant, putative neuroprotective, anti-inflammatory, and putative mitochondrial enhancing properties, with animal studies demonstrating favorable pharmacotherapeutic benefits with respect to these disorders. This review summarizes evidence of its properties and supports the case for future studies to assess the utility of mangosteen pericarp as an adjunctive treatment option for mood and psychotic disorders
Validation of the Bluebelle Wound Healing questionnaire (WHQ) for assessment of surgical site infection in primary surgical wounds after hospital discharge
Background
Accurate assessment of surgicalâsite infection (SSI) is crucial for surveillance and research. Selfâreporting patient measures are needed because current SSI tools are limited for assessing patients after leaving hospital. The Bluebelle Wound Healing Questionnaire (WHQ) was developed for patient or observer completion; this study tested its acceptability, scale structure, reliability and validity in patients with closed primary wounds after abdominal surgery.
Methods
Patients completed the WHQ (selfâassessment) within 30 days after leaving hospital and returned it by post. Healthcare professionals completed the WHQ (observer assessment) by telephone or faceâtoâface. Questionnaire response rates and patient acceptability were assessed. Factor analysis and Cronbach's α examined scale structure and internal consistency. Testâretest and selfâ versus observer reliability assessments were performed. Sensitivity and specificity for SSI discrimination against a faceâtoâface reference diagnosis (using Centers for Disease Control and Prevention criteria) were examined.
Results
Some 561 of 792 selfâassessments (70·8 per cent) and 597 of 791 observer assessments (75·5 per cent) were completed, with few missing data or problems reported. Data supported a singleâscale structure with strong internal consistency (α greater than 0·8). Reliability between testâretest and selfâ versus observer assessments was good (Îș 0·6 or above for the majority of items). Sensitivity and specificity for SSI discrimination was high (area under the receiver operating characteristic (ROC) curve 0·91).
Conclusion
The Bluebelle WHQ is acceptable, reliable and valid with a singleâscale structure for postdischarge patient or observer assessment of SSI in closed primary wounds
Pan-cancer Alterations of the MYC Oncogene and Its Proximal Network across the Cancer Genome Atlas
Although theMYConcogene has been implicated incancer, a systematic assessment of alterations ofMYC, related transcription factors, and co-regulatoryproteins, forming the proximal MYC network (PMN),across human cancers is lacking. Using computa-tional approaches, we define genomic and proteo-mic features associated with MYC and the PMNacross the 33 cancers of The Cancer Genome Atlas.Pan-cancer, 28% of all samples had at least one ofthe MYC paralogs amplified. In contrast, the MYCantagonists MGA and MNT were the most frequentlymutated or deleted members, proposing a roleas tumor suppressors.MYCalterations were mutu-ally exclusive withPIK3CA,PTEN,APC,orBRAFalterations, suggesting that MYC is a distinct onco-genic driver. Expression analysis revealed MYC-associated pathways in tumor subtypes, such asimmune response and growth factor signaling; chro-matin, translation, and DNA replication/repair wereconserved pan-cancer. This analysis reveals insightsinto MYC biology and is a reference for biomarkersand therapeutics for cancers with alterations ofMYC or the PMN
Pan-Cancer Analysis of lncRNA Regulation Supports Their Targeting of Cancer Genes in Each Tumor Context
Long noncoding RNAs (lncRNAs) are commonly dys-regulated in tumors, but only a handful are known toplay pathophysiological roles in cancer. We inferredlncRNAs that dysregulate cancer pathways, onco-genes, and tumor suppressors (cancer genes) bymodeling their effects on the activity of transcriptionfactors, RNA-binding proteins, and microRNAs in5,185 TCGA tumors and 1,019 ENCODE assays.Our predictions included hundreds of candidateonco- and tumor-suppressor lncRNAs (cancerlncRNAs) whose somatic alterations account for thedysregulation of dozens of cancer genes and path-ways in each of 14 tumor contexts. To demonstrateproof of concept, we showed that perturbations tar-geting OIP5-AS1 (an inferred tumor suppressor) andTUG1 and WT1-AS (inferred onco-lncRNAs) dysre-gulated cancer genes and altered proliferation ofbreast and gynecologic cancer cells. Our analysis in-dicates that, although most lncRNAs are dysregu-lated in a tumor-specific manner, some, includingOIP5-AS1, TUG1, NEAT1, MEG3, and TSIX, synergis-tically dysregulate cancer pathways in multiple tumorcontexts

Spatial Organization and Molecular Correlation of Tumor-Infiltrating Lymphocytes Using Deep Learning on Pathology Images
Beyond sample curation and basic pathologic characterization, the digitized H&E-stained images
of TCGA samples remain underutilized. To highlight this resource, we present mappings of tumorinfiltrating lymphocytes (TILs) based on H&E images from 13 TCGA tumor types. These TIL
maps are derived through computational staining using a convolutional neural network trained to
classify patches of images. Affinity propagation revealed local spatial structure in TIL patterns and
correlation with overall survival. TIL map structural patterns were grouped using standard
histopathological parameters. These patterns are enriched in particular T cell subpopulations
derived from molecular measures. TIL densities and spatial structure were differentially enriched
among tumor types, immune subtypes, and tumor molecular subtypes, implying that spatial
infiltrate state could reflect particular tumor cell aberration states. Obtaining spatial lymphocytic
patterns linked to the rich genomic characterization of TCGA samples demonstrates one use for
the TCGA image archives with insights into the tumor-immune microenvironment
