    A Prospective Pilot Study to Validate the Management Protocol for Patients Presenting with Acute Urinary Retention: A Community-Based, Nonhospitalised Protocol

    Acute urinary retention (AUR) in males is managed conventionally by hospital admission, alpha-adrenergic therapy, and trial without catheter. To reduce inpatient bed pressures, we set up a protocol to manage such patients in the community. We review our results in this paper. We performed a prospective study of male patients presenting to our acute admissions ward and Accident and Emergency department over 6 months. Patients with chronic urinary retention, macroscopic haematuria, sepsis, urinary tract infection, and/or serum creatinine >130 mmol/l were excluded from the study. Those enrolled were catheterised, commenced on alfuzosin (10 mg nocte), and discharged to the community. A trial without catheter (TWOC) was performed 5—7 days later. QoL/IPSS, peak flow rate, and residual volume assessment were performed following successful TWOC 3 months later.Thirty-one male patients with a median age of 69 years were studied and the median residual volume following catheterisation was 900 ml. The aetiology of AUR was benign prostatic hyperplasia (BPH) in 29 patients and constipation in the remaining 2 patients. TWOC was successful in 19 patients (61.3%) following first TWOC, 26 (83.9%) following second trial of voiding. The mean peak flow rate was 6.5 ml/sec and postvoid scan 165 ml, following an immediate TWOC. At 3 months follow-up, mean peak flow rate was 13.2 ml/sec, postvoid scan 26.5 ml, IPSS 4.5, and QoL score was 2. This study has shown that AUR can be managed safely and effectively in the community. Effective communication with the nurse urology specialist, general practitioner, and emergency department are crucial for the successful implementation of the protocol

    Investigation of Bearing Fatigue Damage Life Prediction Using Oil Debris Monitoring

    Research was performed to determine if a diagnostic tool for detecting fatigue damage of helicopter tapered roller bearings can be used to determine remaining useful life (RUL). The taper roller bearings under study were installed on the tail gearbox (TGB) output shaft of UH- 60M helicopters, removed from the helicopters and subsequently installed in a bearing spall propagation test rig. The diagnostic tool was developed and evaluated experimentally by collecting oil debris data during spall progression tests on four bearings. During each test, data from an on-line, in-line, inductance type oil debris sensor was monitored and recorded for the occurrence of pitting damage. Results from the four bearings tested indicate that measuring the debris generated when a bearing outer race begins to spall can be used to indicate bearing damage progression and remaining bearing life

    A Vehicle for Research: Using Street Sweepers to Explore the Landscape of Environmental Community Action

    Researchers are developing mobile sensing platforms to facilitate public awareness of environmental conditions. However, turning such awareness into practical community action and political change requires more than just collecting and presenting data. To inform research on mobile environmental sensing, we conducted design fieldwork with government, private, and public interest stakeholders. In parallel, we built an environmental air quality sensing system and deployed it on street sweeping vehicles in a major U.S. city; this served as a "research vehicle" by grounding our interviews and affording us status as environmental action researchers. In this paper, we present a qualitative analysis of the landscape of environmental action, focusing on insights that will help researchers frame meaningful technological interventions.Comment: 10 page

    Vaporous marketing: Uncovering pervasive electronic cigarette advertisements on twitter

    Background Twitter has become the wild-west of marketing and promotional strategies for advertisement agencies. Electronic cigarettes have been heavily marketed across Twitter feeds, offering discounts, kid-friendly flavors, algorithmically generated false testimonials, and free samples. Methods All electronic cigarette keyword related tweets from a 10% sample of Twitter spanning January 2012 through December 2014 (approximately 850,000 total tweets) were identified and categorized as Automated or Organic by combining a keyword classification and a machine trained Human Detection algorithm. A sentiment analysis using Hedonometrics was performed on Organic tweets to quantify the change in consumer sentiments over time. Commercialized tweets were topically categorized with key phrasal pattern matching. Results The overwhelming majority (80%) of tweets were classified as automated or promotional in nature. The majority of these tweets were coded as commercialized (83.65% in 2013), up to 33% of which offered discounts or free samples and appeared on over a billion twitter feeds as impressions. The positivity of Organic (human) classified tweets has decreased over time (5.84 in 2013 to 5.77 in 2014) due to a relative increase in the negative words \u27ban\u27, \u27tobacco\u27, \u27doesn\u27t\u27, \u27drug\u27, \u27against\u27, \u27poison\u27, \u27tax\u27 and a relative decrease in the positive words like \u27haha\u27, \u27good\u27, \u27cool\u27. Automated tweets are more positive than organic (6.17 versus 5.84) due to a relative increase in the marketing words like \u27best\u27, \u27win\u27, \u27buy\u27, \u27sale\u27, \u27health\u27, \u27discount\u27 and a relative decrease in negative words like \u27bad\u27, \u27hate\u27, \u27stupid\u27, \u27don\u27t\u27. Conclusions Due to the youth presence on Twitter and the clinical uncertainty of the long term health complications of electronic cigarette consumption, the protection of public health warrants scrutiny and potential regulation of social media marketing

    The distribution and properties of DLAs at z \leq 2 in the EAGLE simulations

    Determining the spatial distribution and intrinsic physical properties of neutral hydrogen on cosmological scales is one of the key goals of next-generation radio surveys. We use the EAGLE galaxy formation simulations to assess the properties of damped Lyman-alpha absorbers (DLAs) that are associated with galaxies and their underlying dark matter haloes between 0 \leq z \leq 2. We find that the covering fraction of DLAs increases at higher redshift; a significant fraction of neutral atomic hydrogen (HI) resides in the outskirts of galaxies with stellar mass greater than or equal to 1010^{10} M_\odot; and the covering fraction of DLAs in the circumgalactic medium (CGM) is enhanced relative to that of the interstellar medium (ISM) with increasing halo mass. Moreover, we find that the mean density of the HI in galaxies increases with increasing stellar mass, while the DLAs in high- and low-halo-mass systems have higher column densities than those in galaxies with intermediate halo masses (~ 1012^{12} M_\odot at z = 0). These high-impact CGM DLAs in high-stellar-mass systems tend to be metal-poor, likely tracing smooth accretion. Overall, our results point to the CGM playing an important role in DLA studies at high redshift (z \geq 1). However, their properties are impacted both by numerical resolution and the detailed feedback prescriptions employed in cosmological simulations, particularly that of AGN.Comment: 25 pages. Other readers will be made aware of trends that are outside of their experience. This is the nature of trends in our current technological and educational environments: change is continual, but it affects different libraries at different rates.     <p>Abstract</p> <p>Background</p> <p>Comparisons of functionally important changes at the molecular level in model systems have identified key adaptations driving isolation and speciation. In cichlids, for example, long wavelength-sensitive (LWS) opsins appear to play a role in mate choice and male color variation within and among species. To test the hypothesis that the evolution of elaborate coloration in male guppies (<it>Poecilia reticulata</it>) is also associated with opsin gene diversity, we sequenced long wavelength-sensitive (LWS) opsin genes in six species of the family Poeciliidae.</p> <p>Results</p> <p>Sequences of four LWS opsin genes were amplified from the guppy genome and from mRNA isolated from adult guppy eyes. Variation in expression was quantified using qPCR. Three of the four genes encode opsins predicted to be most sensitive to different wavelengths of light because they vary at key amino acid positions. This family of LWS opsin genes was produced by a diversity of duplication events. One, an intronless gene, was produced prior to the divergence of families Fundulidae and Poeciliidae. Between-gene PCR and DNA sequencing show that two of the guppy LWS opsins are linked in an inverted orientation. This inverted tandem duplication event occurred near the base of the poeciliid tree in the common ancestor of <it>Poecilia </it>and <it>Xiphophorus</it>. The fourth sequence has been uncovered only in the genus <it>Poecilia</it>. In the guppies surveyed here, this sequence is a hybrid, with the 5' end most similar to one of the tandem duplicates and the 3' end identical to the other.</p> <p>Conclusion</p> <p>Enhanced wavelength discrimination, a possible consequence of opsin gene duplication and divergence, might have been an evolutionary prerequisite for color-based sexual selection and have led to the extraordinary coloration now observed in male guppies and in many other poeciliids.</p

    Remote home monitoring (virtual wards) for confirmed or suspected COVID-19 patients:a rapid systematic review

    Background: the aim of this review was to analyze the implementation and impact of remote home monitoring models (virtual wards) for confirmed or suspected COVID-19 patients, identifying their main components, processes of implementation, target patient populations, impact on outcomes, costs and lessons learnt. Methods: we carried out a rapid systematic review on models led by primary and secondary care across seven countries (US, Australia, Canada, The Netherlands, Ireland, China, UK). The main outcomes included in the review were: impact of remote home monitoring on virtual length of stay, escalation, emergency department attendance/reattendance, admission/readmission and mortality. The search was updated on February 2021. We used the PRISMA statement and the review was registered on PROSPERO (CRD: 42020202888). Findings: the review included 27 articles. The aim of the models was to maintain patients safe in the appropriate setting. Most models were led by secondary care and confirmation of COVID-19 was not required (in most cases). Monitoring was carried via online platforms, paper-based systems with telephone calls or (less frequently) through wearable sensors. Models based on phone calls were considered more inclusive. Patient/career training was identified as a determining factor of success. We could not reach substantive conclusions regarding patient safety and the identification of early deterioration due to lack of standardized reporting and missing data. Economic analysis was not reported for most of the models and did not go beyond reporting resources used and the amount spent per patient monitored. Interpretation: future research should focus on staff and patient experiences of care and inequalities in patients' access to care. Attention needs to be paid to the cost-effectiveness of the models and their sustainability, evaluation of their impact on patient outcomes by using comparators, and the use of risk-stratification tools

    Characterisation of the Fusarium graminearum-Wheat Floral Interaction

    Fusarium Ear Blight is a destructive fungal disease of cereals including wheat and can contaminate the crop with various trichothecene mycotoxins. This investigation has produced a new β-glucuronidase (GUS) reporter strain that facilitates the quick and easy assessment of plant infection. The constitutively expressed gpdA:GUS strain of Fusarium graminearum was used to quantify the overall colonisation pattern. Histochemical and biochemical approaches confirmed, in susceptible wheat ear infections, the presence of a substantial phase of symptomless fungal growth. Separate analyses demonstrated that there was a reduction in the quantity of physiologically active hyphae as the wheat ear infection proceeded. A simplified linear system of rachis infection was then utilised to evaluate the expression of several TRI genes by RT-qPCR. Fungal gene expression at the advancing front of symptomless infection was compared with the origin of infection in the rachis. This revealed that TRI gene expression was maximal at the advancing front and supports the hypothesis that the mycotoxin deoxynivalenol plays a role in inhibiting plant defences in advance of the invading intercellular hyphae. This study has also demonstrated that there are transcriptional differences between the various phases of fungal infection and that these differences are maintained as the infection proceeds
