The Angra dos Reis Nuclear Power Plant’s emergency plan, it´s the bunch of instructions that every citizen must adopt in case of an emergency situation. It’s highly important, that all the people living in the power plant´s surroundings truly understand every single step of the plan, because only in this way people will know how to react in case of a necessity. To hit this goals, the Brazil`s Electronuclear, made educational booklets, in the shape of comic books, trying to guide the population about the plan. On the other hand, we have an increasingly connected world, making possible that digital games, be very well accepted by the population. So this project has as an objective, developing a digital tool, in form of a mobile game that shows in a playful and interactive way for the user, the emergency plan, complementing the educational process and social actions made by many institutions. With the information taken from the booklets, objects and buildings were modeled in Autodesk 3ds Max, allied with the Unity 3D Game Engine, to make a city, inspired in Angra do Reis (RJ). The player has to follow all the security protocols giving by the Eletronuclear according with the rules provided by the National Nuclear Energy Commission. Is expected, with this game that will be available for the Eletronuclear that more people have the chance to know and believe in the efficiency of the emergency plan already established

Combined antibiotic therapy spacers either commercial or handmade are superior to monotherapy – a microbiological analysis at the second stage of revision

Background: Antibiotic-loaded spacers are often used during two-stage exchange for periprosthetic joint infections (PJIs) both for its mechanical properties and as a means of local antibiotic delivery. Purpose: The main goal of this study is to compare the efficacy of different options of antibiotic(s) in spacers concerning the rate of positive cultures at the second stage. Patients and Methods: We retrospectively evaluated two-stage exchange procedures for infected hip or knee arthroplasty performed between 2012 and 2018 in which adequate (at least four deep tissue samples) culture results in both stages were available. The type of spacer and antibiotics used, in addition to several other patient, infection and treatment-related variables, were registered and correlated to microbiological findings in the second stage. Results: Fifty-eight cases were included with a 19.0 % (11/58) overall rate of positive cultures during reimplantation. With a mean follow-up of 46 months, failure rate was significantly higher at 63.6 % (7/11) in cases with positive cultures at reimplantation compared to 4.3 % (2/47) for those with negative cultures during reimplantation ( p < 0.001). The need for additional surgeries was also significantly higher (odds ratio (OR) 122.67, confidence interval (CI) 95 % 11.30-1331.32, p < 0.001). Multivariable analysis revealed antibiotics in the spacers were the main independent prognostic risk factor associated with positive cultures at the second stage with an advantage for combined antibiotics. Monotherapy is associated with failure with an OR of 16.99. Longer time between surgeries did not have statistical significance ( p = 0.05), and previous surgical treatment for PJI, presence of difficult-to-treat microorganism(s), duration of systemic antibiotic therapy or even treatment within a dedicated septic team were not shown to be independent risk factors. Among combined antibiotic spacers, there were no significant differences between the rate of positive cultures during the second stage, comparing commercially available vancomycin/gentamicin spacers to hand-mixed vancomycin/meropenem manufactured spacers (8.3 % [2/24] vs. 15.0 % [3/20], p = 0.68). Conclusions: Results show that combined antibiotic therapy spacers are advantageous when compared to gentamicin monotherapy as they produce significantly lower rates of subsequent positive cultures during the second stage. Hand-mixed high-dose vancomycin/meropenem spacers seem to perform just as well as prefabricated commercially available vancomycin/gentamicin options. Odontogenic myxoma in a child treated with enucleation and curettage

    Get PDF
Odontogenic myxoma is an aggressive benign odontogenic tumor, accounting for 3-6% of all the odontogenic tumors in adults. The incidence among children is lower. Due to its clinical behavior, there is no consensus on the best treatment. In this paper, the authors report the case of a 9-year-old girl with the diagnosis of odontogenic myxoma. The panoramic X-ray showed an extensive radiolucent lesion involving the left mandibular body causing teeth displacement. The treatment consisted of tumor enucleation followed by vigorous curettage of the bone walls. Both the base of the mandible and the inferior alveolar nerve were preserved. The patient is asymptomatic after 6 months of surgery. The age of the patient and the radiographic features were taken into account when deciding in favor of the conservative treatment

Bibliometric Analysis of Publisher and Journal Instructions to Authors on Generative-AI in Academic and Scientific Publishing

We aim to determine the extent and content of guidance for authors regarding the use of generative-AI (GAI), Generative Pretrained models (GPTs) and Large Language Models (LLMs) powered tools among the top 100 academic publishers and journals in science. The websites of these publishers and journals were screened from between 19th and 20th May 2023. Among the largest 100 publishers, 17% provided guidance on the use of GAI, of which 12 (70.6%) were among the top 25 publishers. Among the top 100 journals, 70% have provided guidance on GAI. Of those with guidance, 94.1% of publishers and 95.7% of journals prohibited the inclusion of GAI as an author. Four journals (5.7%) explicitly prohibit the use of GAI in the generation of a manuscript, while 3 (17.6%) publishers and 15 (21.4%) journals indicated their guidance exclusively applies to the writing process. When disclosing the use of GAI, 42.8% of publishers and 44.3% of journals included specific disclosure criteria. There was variability in guidance of where to disclose the use of GAI, including in the methods, acknowledgments, cover letter, or a new section. There was also variability in how to access GAI guidance and the linking of journal and publisher instructions to authors. There is a lack of guidance by some top publishers and journals on the use of GAI by authors. Among those publishers and journals that provide guidance, there is substantial heterogeneity in the allowable uses of GAI and in how it should be disclosed, with this heterogeneity persisting among affiliated publishers and journals in some instances. The lack of standardization burdens authors and threatens to limit the effectiveness of these regulations. There is a need for standardized guidelines in order to protect the integrity of scientific output as GAI continues to grow in popularity.Comment: Pages 16, 1 figure, 2 table

Desmame

A nostalgia como problema metahistórico: uma introdução

Historians are generally little interested in the phenomenon of nostalgia. Sometimes the term "nostalgic" is employed to mark "conservatives," in a complex movement in which such concepts are employed interchangeably. The following texts, translated by us, seek to invite Brazilian historians to pay due attention to possibilities - not only theoretical, but political - suggested by the reading of Arnold Gehlen's essays, "Happiness evaded. An interpretation of nostalgia (1976) - and Svetlana Boym - Nostalgia and its dislikes (2007).Os historiadores são em geral pouco interessados pelo fenômeno da nostalgia. Por vezes o termo “nostálgicos” é empregado no sentido de marcar os “conservadores”, num movimento complexo no qual tais conceitos são empregados de modo intercambiável. ANALYSIS OF THE RELIABILITY OF TWO FORCE PLATFORMS FOR THE SIMULTANEOUS COLLECTION OF KINETICS VARIABLES

    Get PDF
The bilateral force asymmetry between lower-body limbs is seen as one of the major causes of injury and decreased performance in sport. The diagnosis can be done through many measuring methods, instruments and protocols. One of the most validated methods in literature is the vertical jump with countermovement performed on a force platform. Many studies show measurement protocols of jumps performed on a force platform where the jumps are carried out in a unipodial or bipodial manner and the data collection takes place in an alternated or sequential way. This study intends to assess the reliability of two force platforms measuring (maximum force and impulse) simultaneously. The results have indicated that the method used is reliable for the collection of the mentioned variables

A case report of a patient with pseudo bulbar affect previous treatments included haloperidol (10mg), Inosina pranobex (600mg), clozapine (600mg), olanzapine (20mg), carbamazepine (200mg), paroxetine (20mg), phenobarbital (100mg) and topiramate (50mg), all suspended at August 2016, with current use of quetiapine (700mg) Chlorpromazine (600mg) (+ 200mg on demand of aggression), clonazepam (4 mg), valproate 2500 mg, propranolol (40mg). that was successful treated with off label treatment (dextromethorphan plus quinidine). Previous Brief Psychiatric Rating Scale and Clinical Global Impression- Improvement was applied after and before treatment with dextromethorphan (20mg) plus fluoxetine (20 mg, further increased to 40 mg). Previous Brief Psychiatric Rating Scale BPRS score 56 points and Clinical Global Impression-Severity (CGI-S) Score was 6 (severely ill). The addition of dextromethorphan (20mg) and fluoxetine (20 mg, further increased to 40 mg), allowed clear improvement of pathological crying and outbursts, with BPRS decrease of 8 points and Clinical Global Impression-Improvement (CGI-I) 2 (much improved) – especially pertaining to PBA related symptoms and aggressive behavior. There were no noticeable side-effects. This case report shown an interesting clinical response. It’s could be a great alternative in treatment of pseudobulbar affect symptoms. Even though an only case and a great clinical study be necessary

Tunable kinetic proofreading in a model with molecular frustration

In complex systems, feedback loops can build intricate emergent phenomena, so that a description of the whole system cannot be easily derived from the properties of the individual parts. Here we propose that inter-molecular frustration mechanisms can provide non trivial feedback loops which can develop nontrivial specificity amplification. We show that this mechanism can be seen as a more general form of a kinetic proofreading mechanism, with an interesting new property, namely the ability to tune the specificity amplification by changing the reactants concentrations. This contrasts with the classical kinetic proofreading mechanism in which specificity is a function of only the reaction rate constants involved in a chemical pathway. These results are also interesting because they show that a wide class of frustration models exists that share the same underlining kinetic proofreading mechanisms, with even richer properties. These models can find applications in different areas such as evolutionary biology, immunology and biochemistry
