    Efficacy of first-line sodium thiosulphate administration in a case of potassium cyanide poisoning

    Cyanide poisoning may occur following accidental fire-smoke inhalation or deliberate ingestion of salts. Hydroxocobalamin represents a first-line life-saving antidote. Although hydroxocobalamin represents a first-line lifesaving antidote, it is still not promptly available in the emergency department. Sodium thiosulfate can be administered in association with hydroxocobalamin whereas the delayed onset of clinical response makes sodium thiosulfate less suitable for emergency use. We describe a case of cyanide intoxication of a 43-year-old man who ingested an unknown amount of potassium cyanide, purchased via the Internet, in an attempted suicide. At admission to the emergency department, the patient presented GCS 3 with severe lactic acidosis. Orotracheal intubation, gastric lavage and oral activated charcoal were applied. Sodium thiosulfate was available in the emergency department and 10 grams were infused over a 30 minute period. Hydroxocobalamin was prescribed by the poison control centre and 5 grams were infused 2 hours after admission. Following sodium thiosulfate administration the patient was arousable and lactate concentration improved. No adverse effects were noted. Metabolic acidosis completely resolved 12 hours later. Cyanide concentration performed on blood samples collected at admission confirmed high cyanide blood levels (15 mg/L). This report highlights as the first-line administration of sodium thiosulfate, in rapid infusion, resulted effective and safe for cyanide poisoning.     Transmission Power Measurements for Wireless Sensor Nodes and their Relationship to the Battery Level

    In this work we focus on the new generation EYESIFXv2 [1] wireless sensor nodes by carrying out experimental measurements on power related quantities. In particular, our aim is to characterize the relationship between the level of the battery and the transmission power radiated by the node. The present results point out the non linear and non trivial effects due to the output potentiometer which can be used to tune the transmission power. It shall be observed that a thorough study of how battery and/or potentiometer settings translate to actual transmitted power levels is crucial to e.g. design correct power control algorithms, which can effectively operate under any operational condition of the wireless sensor device

    Inhibition of Multidrug-Resistant Gram-Positive and Gram-Negative Bacteria by a Photoactivated Porphyrin

    The authors studied the in vitro antibacterial activity of the photo-activated porphyrin meso-tri(N-methyl-pyridyl), mono(N-tetradecyl-pyridyl)porphine (C14) against four multidrug-resistant bacteria: Staphylococcus aureus, Enterococcus faecalis (Gram-positive), Escherichia coli, Pseudomonas aeruginosa (Gram-negative). Using 10 g/ml of porphyrin and 60 sec irradiation we observed the remarkable susceptibility of S. aureus and E. faecalis to treatment while, under the same conditions, E. coli and P. aeruginosa showed very low susceptibility. In a later stage, suspensions of Gram-negative bacteria were processed with EDTA before photo-activation, obtaining a significant decrease in viable counts. In view of the results, if the combination of low porphyrin concentrations and short irradiation times will be effective in vivo also, this approach could be a possible alternative to antibiotics, in particular against localized infections due to multidrug-resistant microorganisms

    Seasonal controls on nearshore dissolved oxygen variability and hypoxia in a coastal embayment

    Declining dissolved oxygen (DO) is emerging as an increasingly important stressor in nearshore ecosystems, and there is a growing need to better understand DO dynamics and hypoxia risk in this highly variable environment. In this study, we collected data from monthly cruises on the inner shelf, continuous nearshore moorings inside and outside a small coastal upwelling embayment (San Luis Obispo Bay in Central California), and weekly phytoplankton measurements inside the bay during the upwelling season. Nearshore DO was generally dominated by low-frequency synoptic variability, with increased DO variance near the surface relative to the bottom and inside the bay compared to outside. Two nearshore hypoxic regimes were identified. In the first regime, which occurred during periods of strong upwelling in the spring across all nearshore sites, the nearshore bottom water temperature-DO (T-DO) relationship was aligned with that found offshore, suggesting hypoxia was driven by the direct advection and cross-shelf exchange of low DO subthermocline waters from the shelf. This period also coincided with minimal water-column stratification, small vertical DO differences, and a diatom-dominated phytoplankton assemblage. In the second regime, which occurred during summer months and was characterized by weaker upwelling, strong stratification, and dinoflagellate-dominated phytoplankton assemblage, the near-bottom T-DO relationship inside the bay deviated significantly from that on the shelf offshore. These hypoxic events inside the bay were likely driven by localized respiration and lack of ventilation of bottom waters due to strong stratification. Collectively, these observations reveal a shift in the strength and magnitude of physical versus biological processes driving nearshore DO dynamics. The high spatiotemporal variability of DO dynamics in upwelling bays means that they are likely to be at the forefront of ecosystem impacts of and adaptions to climate change, and may act as sentinel systems or “canaries on the coast.

    Effects of Acetyl-L-Carnitine on Oxidative Stress in Amyotrophic Lateral Sclerosis Patients: Evaluation on Plasma Markers and Members of the Neurovascular Unit

    Oxidative stress, the alteration of mitochondrial function, and the neurovascular unit (NVU), play a role in Amyotrophic Lateral Sclerosis (ALS) pathogenesis. We aimed to demonstrate the changes in the plasma redox system and nitric oxide (NO) in 32 new ALS-diagnosed patients in treatment with Acetyl-L-Carnitine (ALCAR) compared to healthy controls. We also evaluated the effects of plasma on human umbilical cord-derived endothelial vascular cells (HUVEC) and astrocytes. The analyses were performed at the baseline (T0), after three months (T1), and after six months (T2). In ALS patients at T0/T1, the plasma markers of lipid peroxidation, thiobarbituric acid reactive substances (TBARS) and 4-hydroxy nonenal (4-HNE) were higher, whereas the antioxidants, glutathione (GSH) and the glutathione peroxidase (GPx) activity were lower than in healthy controls. At T2, plasma TBARS and 4-HNE decreased, whereas plasma GSH and the GPx activity increased in ALS patients. As regards NO, the plasma levels were firmly lower at T0–T2 than those of healthy controls. Cell viability, and mitochondrial membrane potential in HUVEC/astrocytes treated with the plasma of ALS patients at T0–T2 were reduced, while the oxidant release increased. Those results, which confirmed the fundamental role of oxidative stress, mitochondrial function, and of the NVU in ALS pathogenesis, can have a double meaning, acting as disease markers at baseline and potential markers of drug effects in clinical practice and during clinical trial

    Vegetation, climate and environmental history of the last 4500 years at lake Shkodra (Albania/Montenegro)

    Three parallel overlapping cores have been taken in the Albanian side of Lake Shkodra (Albania/Montenegro). The chronological frame of the record, spanning approximately the last 4500 years, has been assessed using four radiocarbon dates and four well-known tephra layers of Italian volcanoes. Multidisciplinary analyses turned out to be decisive to understand environmental, climatic changes and human impact. Here, we focus on palynology. The humidity at Shkodra was always enough to allow the developing of a luxuriant arboreal vegetation. The pollen percentage diagram does not record important changes in terrestrial plants percentages. Arboreal pollen (AP) shows only a rather slight decrease, with ‘natural forests’ replaced by intensive cultivation of chestnut and walnut in the last seven/eight centuries. The rather minimal changes in composition and dominance are because of the fact that the pollen rain comes from different vegetation belts, from the Mediterranean to the alpine one. Two major periods of humidity are found, one at the base of the pollen concentration and influx diagram, before 4100 yr BP, the other at 1300 yr BP. Minima in pollen influx and concentration occurred soon before 4000, at ca. 2900 and at ca. 1450 yr BP These minima, interpreted as aridity crises, show a temporal coincidence with the so-called Bond events 1-3 already found in other central and eastern Mediterranean records. The minimum in AP occurring after 500 yr BP could represent the record of the ‘Little Ice Age’, even if it could be the effect of a strong land use

    La presente investigación se lleva a cabo en el cantón Playas, ubicado en la provincia del Guayas, tiene como objetivo determinar la influencia que la gestión y promoción turística tiene en el incremento de la demanda durante las temporadas bajas. Se ha empleado el uso de una investigación descriptiva-correlacional apoyada en el levantamiento de información bibliográfica y trabajo de campo. A través de la aplicación de encuestas y entrevistas, se obtuvo información importante relacionada a la gestión turística de Playas y de la misma manera a la información difundida por los diferentes medios de comunicación. La gestión y la promoción tienen un alto índice de influencia sobre los demandantes al momento de decidir qué destino turístico visitar, permitiéndole hacer frecuentes las visitas a dichos lugares
