Correlations between serum lipids and psychiatric symptoms

의학과/석사[한글] 정신적 스트레스가 혈청 콜레스테롤 농도를 높여 관상동맥질환에 기여한다고 알려졌으나 반면 최근 낮아진 콜레스테롤 농도에 의해 사고, 자살, 폭력에 의한 사망률은 증가된다는 연구결과가 발표되었다. 지금까지 혈청콜레스테롤과 정신과적 증상과의 관련성에 대한 결과는 항상 일치되지 않고 있으며 중성지방, 고밀도지단백콜레스테롤과의 관계에 대해서는 더욱 알려진 바가 적다. 본 연구의 목적은 혈청 콜레스테롤, 중성지방, 고밀도지단백콜레스테롤 농도와 어떤 정신과적 증상이 관련이 있는지를 알아보는 것이다. ICD-10의 신경증성, 스트레스 관련성 및 신체형 장애(불안장애, 강박장애, 심한 스트레스에 대한 반응 및 적응장애, 신체형 장애), 정신병적 증상이 없는 우울증 에피소드, 지속적 기분장애의 진단기준을 만족하면서 정신과 외래에 내원한 61명의 환자들을 대상으로 하였다. 이들에게 자가보고형 평가검사인 SCL-90-R과 한국인 환자들이 흔히 호소하는 증상평가를 추가로 시행하고 혈청 지질을 측정하여 정신과적 증상과 지질과의 상관관계를 알아보았다. 또한 혈청 콜레스테롤 농도가 높은 군과 낮은 군으로 나누어 각 증상차원의 차이를 비교하였다. 연구결과 혈청 콜레스테롤, 중성지방, 고밀도지단백를레스테롤 농도와 SCL-90-R의 총점과는 의미있는 상관관계를 보이지 않았다. 혈청 콜레스테롤과 신체와 증상은 양의 상관관계를 보였으며 이는 나이의 효과를 통제하여도 통계적으로 유의하였다. 다른 증상차원과 혈청 콜레스테롤 농도와는 상관관계를 보이지 않았다. 혈청 중성지방, 고밀도지단백콜레스테롤 농도와 의미있는 상관관계를 보이는 증상차원은 없었다. 혈청 콜레스테롤 농도와 양의 상관관계를 보이는 개별 증상항목을 살펴보면 (1)몸에 힘이 없다 (2)몸에 화끈거리거나 열이 날 때가 있다 (3)눈이 침침하고 피로하다 (4)입맛이 없다 (5)팔다리가 아프다 (6)숨쉬기가 거북하다 (7)어지럽다 와 같은 신체증상과 수면 장애가 있었다. 내면적인 분노와 관련된 증상은 혈청 콜레스테롤 농도와 음의 상관관계를 보였다. 또한 혈청 콜레스테롤 농도가 높은 군과 낮은 군으로 나누어서 증상차원의 차이를 비교한 결과 신체와 증상에서만 두 군간 의미있는 차이를 보였다. 이러한 결과는 정신과적 증상중 신체와 증상이 혈청 콜레스테롤과 관련이 있다는 사실을 시사하며 그 기전에 대한 추가 연구가 필요한 것으로 보인다. [영문] Many studies have found that psychological stress produced significant increases in cholesterol in the blood. However recent studies have suggested that lowering cholesterol increased non-illness mortality. Suicide, depression, violence were related with a low cholesterol level, The purpose of this study was to determine which symptoms are correlated with serum concentrations of total cholesterol, triglyceride, high density lipoprotein cholesterol (HDL-C) and low density lipoprotein(LDL-C). The subjects were 61 Patients with depression, dysthymia, anxiety disorder, and somatoform disorder according to ICD-10 criteria. Their psychiatric symptoms were assessed with SCL-90-R and an additional symptoms check. Their fasting serum concentration of cholesterol, triglyceride and HDL-C were obtained. The corrrelation between seam concentration of lipids and psychiatric symptoms were analyzed using Pearson's correlation test. The results showed a significant positive correlation between somatization symptom 야mension and serum cholesterol concentration, although there were no correlation between total SCL-90-R score and serum cholesterol. Also serum cholesterol concentrations were also negatively correlated with suppressed anger. Serum concentrations of triglyceride and HDL-C were not correlated with any symptom dimensions of SCL-90-R. These results suggest that somatization symptoms have some relationship with serum cholesterol. The future study may be necessary between somatization or suppressed anger and sew cholesterol levels.restrictio

Cortical information transmission during tic suppression in boys with Tourette's syndrome

의학과/박사[한글]연구목적: 뚜렛 장애는 다수의 운동 틱과 음성 틱을 특징으로 하는 만성적인 신경학적인 질환이다. 뚜렛 장애의 중심병리는, 기저핵의 이상과 그와 관련된 피질-기저핵-시상 피질 회로(Cortico-Striatal-Thalamocortical Circuit, CSTC)의 이상으로 알려져 있는데 피질의 역할에 대해서는 잘 알려져 있지 않다. 본 연구는 우리나라 뚜렛 장애 아동을 대상으로 하며 뇌파의 비선형 분석을 통해 뚜렛 장애 아동에서 틱을 억제하도록 하였을 때의 피질 내 정보 전달의 흐름을 알아 보고자 한다연구방법: 연구 대상은 DSM-IV의 투렛 장애의 진단기준을 만족시키는 8세-12세 아동 11명과 정상 대조군 10명이었다. 대상군에게 정신과적 평가 및 임상 평가를 시행하고 휴식시와 틱을 억제하였을 때의 뇌파를 측정하며 비선형 분석을 시행하였다. 비선형 분석은 각 영역간의 정보 전달의 정도와 방향을 측정할 수 있는 엔트로피 전달(Trasfer Entropy:TE)를 사용하였다. 휴식시 뚜렛 장애와 정상 대조군간의 엔트로피 전달 차이를 알아보고 뚜렛 장애에서 틱 억제시 엔트로피 전달 변화를 알아보았다.결과 휴식시 뚜렛 장애 환자군은 정상 대조군과 비교하여 몇몇 뇌파 전극에서 의미있는 엔트로피 전달의 차이를 보였다. 뚜렛 장애 군에서 정보전달의 흐름은 좌우 비대칭적이었으며 전두엽과 측두엽에서 전두엽(Fz, F8)과 측두엽(T3,T4)으로 향하였다. 뚜렛 장애의 경우, 틱 억제시에는 휴식상태에 비해 보다 광범위한 부위에서 좌우 비대칭적인 엔트로피 전달의 의미있는 변화를 확인할 수 있었다. 전두엽(Fp1,F7,F3,F8),중심영역(Cz,Fz), 측두엽(T3,T4)에서 전두엽(Fz,F7,F3), 중심영역(C3,Cz,C4), 측두엽으로(T3,T4) 향하는 엔트로피 전달값은 증가되었으며 전두엽(Fp2,F3,F3), 측두엽(T5), 중심 영역(Cz), 두정엽(P3,P4)에서 좌측 측두엽(T5), 좌측 후두엽(O1), 중심영역(Cz)으로 향하는 엔트로피 전달값은 감소를 보였다.결 론 본 연구는 뚜렛 장애 아동에서 뇌파의 비선형 분석을 이용하여 틱 억제시 대뇌피질 정보전달을 살펴본 첫 연구이다. 틱 억제시에는 광범위한 피질 영역에서 정보전달이 일어났으며 특히 전두엽과 측두엽에서 피질 정보 전달이 두드러졌다. [영문]Background : Tourette Syndrome(TS) is a chronic neuropsychiatric disorder characterized by multiple motor and vocal tics with onset in childhood. Substantial evidences suggest TS as a disorder of Cortico-Striatal-Thalamocortical Circuit(CSTC).Objective : The aim of this study was to investigate the cortical mechanism underlying tic suppression in drug na？ve boys with TS using new nonlinear analysis of EEGs, be called ？ransfer Entropy(TE)？ which can detect the directed exchange of information between two systems.Method : Subjects were 11 drug na？ve boys with DSM-IV diagnosis of TS(mean age？.D=9.64？1.51) and 10 control boys(mean age？ S.D=9.5？1.21 years). Clinical assessment included Kiddie-Schedule for Affective Disorder and Schizophrenia-Present and lifetime version (K-SADS-PL), the Yale Global Tic Severity Scale, Korean Child Behavior Checklist (K-CBCL) and IQ tests(KEDI-WISC-III).EEGs were recorded from 19 scalp loci of the international 10-20 systems. We recorded EEGs of TS patients in resting state for 5 minutes and tic suppressed state for 5 minutes. We also recorded EEG of control subject in resting state for 5 minutes. The mean values of TE in each channels were estimated for this EEG time-series data. Mann Whitney U test and Non-parametrical two-related-samples test were implemented to investigate differences of mean TE values between TS and control in resting state and difference between tic suppression and resting state in TS patients using SPSSWIN13.0.Result : Elevated TE was found from multiple channels to superior temporal area (T3, T4) and frontal area(Fz,F8) in resting state of TS compared with normal control. Elevated TE was found from multiple channels including frontal area to frontal, temporal and central areas( Fz, F7,T3, C3,Cz,C4 and T4), and decreased TE was found from multiple area to left temporal, left occipital and central areas(Cz, T5,O1) during the voluntary suppression of tics in TS.Conclusion : Significant asymmetrical cortical information transmission was observed between multiple EEG channels during tic suppression in TS as well as resting state of TS compared with normal control. This findings suggests that the pathogenesis of tics involved an impaired modulation of neuronal activity in subcortical neural circuit. EEG analysis using TE can be a useful tool to investigation of cortical mechanism of tic suppression.ope
