(The) analysis of the current status of medical accidents and disputes researched in the Korean web sites

치의학과/석사[한글]의료분쟁 증가는 하나의 사회현상으로 특히, 인터넷을 통한 의료사고와 관련된 정보의 생산과 유통이 급속도로 증가되고 있는 현상을 간과해서는 안 된다.본 연구는 2006년 3월 기준으로 인터넷 야후 포털 사이트에서 ‘의료사고’를 검색어로 하여 검색된 의료사고 관련 정보를 제공하는 웹사이트 28개를 사이트 개설자별로 분류하고 사이트 구성내용을 분석하여 의료사고 관련 사이트들의 현황과 개선되어야 할 문제점들을 비교 점검한 후 올바른 의료분쟁 해결을 위한 발전 방향을 모색하는 것을 연구목적으로 하여 분류항목별 전체 개수에 대한 비율을 조사한 결과 다음과 같은 결과를 얻었다.1. 용어의 선호도에서 의료인, 일반인, 법조인 모두 ‘의료사고’라는 용어에 가장 익숙하거나 이를 선호하는 것으로 나타났다.2. ‘의료사고’ 검색어로 검색된 개설자별 사이트 개수를 비교한 결과 28개 사이트 중 의료인 4개, 일반인 7개, 법조인 17개였다. 의료인 중 치과의사가 개설한 사이트는 단지 1개였다.3. 진료기록부 분석원을 따로 둔 일반인과 법조인 개설 사이트의 비율은 높았으며 치과관련 부분은 전무하였다.4. 일반인은 의료사고 예방법에, 법조인은 의료사고 발생 이후 처리 과정에 주된 관심을 갖고 있는 것으로 보였으며, 이에 반해 의료사고 대책에 관해서 의료인 개설 사이트는 비중이 적은 것을 관찰할 수 있었다.5. 일반인은 의료사고 발생시 대책으로 정부가 주도하는 공정한 제3자의 개입을 희망하는 것으로 조사되었다.6. 개설자별 비교에서 의료인 개설 사이트는 의료사고에 대한 실례를 다른 개설 사이트에 비해 적게 다루는 것으로 나타났다.7. 의료사고 상담글에서 치과 관련 내용이 많은 것에 비해 실제 판례 소개는 미미한 것으로 나타났다.8. 국내 치과 판례 중 관혈적인 치과 치료에 대한 판례글이 많았던 반면 국내 공개 상담글은 비보험관련 치과 치료에 대한 상담글이 많았다.9. 개설자별 의료사고 관련 정보 제공 게시판 글의 비교에서 일반인은 관련 용어, 의료인은 관련 의학지식, 법조인은 관련 법률에 대한 내용이 많았다.10. 의료인, 일반인, 법조인 개설 사이트 모두 국내 의료사고 현황을 제공하는 형태로 언론보도 자료를 주로 사용하였으며, 일반인 개설 사이트 중 특히, 시민단체에서는 의료사고 관련 통계자료를 비중있게 다루고 있었다.11. 적기는 하지만 의료사고 관련 사건을 수집하는 배너가 존재하는 사이트도 있었다.이상의 결과를 볼 때 범람하는 정보들 속에서 제3자의 그릇된 정보로 의료분쟁 발생률이 증가될 소지가 크지만 의료인은 의료사고에 대해 가장 소극적인 것으로 나타나 이에 대해 향후 법조인-의료인, 환자-의료인, 일반인-의료인간의 상호대화와 정보교류를 통한 올바른 이해를 바탕으로 의료분쟁을 보다 적극적이고 능동적으로 조정, 해결하려는 노력을 보여야 할 것으로 사료된다. [영문]The increasing tendency of medical disputes is one of the remarkable social phenomena. Especially we must not overlook the phenomenon that production and circulation of information related to medical accidents is increasing rapidly through the internet.In this research, we evaluated the web sites which provide the information related to medical accidents using the keyword "medical accidents" in March 2006, and classified the 28 web sites according to the kinds of establishers. We also analyzed the contents of the sites, and checked and compared the current status of the web sites and problems that have to be improved. Finally, we suggested the possible solutions to prevent medical accidents. The detailed results were listed below.1. Medical practitioners, general public, and lawyers were all familiar with and prefer the term "medical accidents" mainly.2. In the number of sites searched by the keyword "medical accidents", y lawyer had the most sites and medical practitioners had the least ones.3. Many sites by general public and lawyers had their own medical record analysts but there was little professional analysts for dentistry.4. General public were more interested in the prevention of medical accidents but the lawyers were more interested in the process after medical accidents. The sites by medical practitioners dealt with the least remedies of medical accidents, compared with other sites.5. General public wanted the third party such as government intervention into the disputes including the medical dispute arbitration law or/and the establishment of independent medical dispute judgment institution.6. In the comparison among the establishers of web sites, medical practitioners dealt with the least examples of medical accidents.7. The suggestion of cases in counseling articles related to dental accidents were considered less importantly than the reality.8. Whereas there were many articles about domestic cases related to the bloody dental treatment, in the open counseling articles the number of dental treatment regarding to non insurance treatment was large.9. In comparing offered information of medical accidents based on the establishers, general public offered vocabularies, lawyers offered related laws and medical practitioners offered medical knowledge relatively.10. They all cited the news pressed by the media to offer the current status of domestic medical accidents. Especially among the web sites by general public, NGOs provided the plentiful statistical data related to medical accidents.11. The web sites that collect the medical accidents were only two.As a result of our research, we found out that, in the flood of information, medical disputes can be occurred by the wrong information from third party, and the medical practitioners have the most passive attitudes on the medical accidents. Thus, it is crucial to have the mutual interchange and exchange of information between lawyer, patients and medical practitioners, so that based on clear mutual comprehension we can solve the accidents and disputes more positively and actively.ope

Dept. of Dental Science/박사Many patients want to make their hypertrophic masseter muscle slim to reduce a prominentmandibular angle. Surgical reduction has become known widely in the past, but bilateralbotulinum toxin injection in masseter muscle which offers a less invasive approach thansurgical reduction becomes the center of public interest in an alternative treatment forhypertrophic masseter muscle. The aim of this study is to measure the volume and thicknesschange in unilateral hypertrophic masseter muscle with unilateral botulinum toxin type Ainjection using 3D laser scanning unlike former bilateral injection treatment for bilateral masseter muscle hypertrophy. The volume and thickness of lower facial contour of 10 unilateral hypertrophic masseter muscle patients were measured with 3D laser scan before 25 units of botulinum toxin type A injected unilaterally. In same way, the volume and thickness of lower facial contour was measured 4, 8, 12 weeks after injection and superposed 3D laser scan of before injection, and then changed volume and thickness were determined. While not injected side showed little change, mean values of the volume and thickness of injection side at each time point diminished obviously between preinjection and 4, 8, and 12 weeks after postinjection and showed statistically significant difference. Unilateral Botulinum toxin type A injection diminished the volume and thickness of unilateral hypertrophic masseter muscle significantly. Consequently it is considered that unilateral botulinum toxin type A injection is very helpful to the esthetic treatment for muscle induced facial asymmetry patients.ope

부모사별 청소년의 적응에 관한 연구 : 적응유연성 관점으로

부모사별 청소년의 적응에 관한 연구 : 적응유연성 관점으로 본 연구에서는 부모사별을 경험한 청소년들의 심리, 행동, 학업 세 가지 영역의 적응 결과와 이에 영향을 미치는 요인 및 그 기제를 적응유연성 관점을 적용하여 이해하고자 하였다. 이를 위해 첫째, 부모사별 청소년의 적응 결과에 영향을 줄 수 있는 위험요인과 보호요인들의 주효과를 확인하였다. 둘째, 위험요인의 영향을 완충하는 보호요인의 조절효과를 확인하고, 셋째, 이러한 조절효과가 사별특성에 따라 다르게 나타나는지 살펴보았다. 이와 같은 연구 목적을 달성하기 위해 부모님 중 한 분을 사별한 192명의 청소년들로부터 수거된 설문지를 분석하였으며, 주효과와 조절효과를 밝히기 위해 다중회귀분석과 조절회귀분석을 실시하였다. 본 논문의 연구 결과를 요약하면 다음과 같다. 첫째, 심리, 행동, 학업적응에 대한 위험요인과 보호요인의 주효과를 살펴본 결과, 위험요인 중 개인차원의 정서적 대처기제와 가족차원의 낮은 가족적응성, 환경차원의 부정적 생활사건이 심리, 행동, 학업적응에 부분적으로 부(-)적인 영향을 미치는 것으로 나타났다. 반면 사별 부모와의 낮은 애착관계의 경우, 심리, 학업적응에 정(+)적인 영향을 미치는 것으로 나타나 국외 연구 결과와는 반대 양상을 보였다. 보호요인 중 개인차원의 자아존중감, 영적 안녕감, 가족차원의 부모자녀 의사소통, 부모자녀관계, 환경차원의 사회적 지지가 심리, 행동, 학업적응에 부분적으로 정(+)적인 영향을 미치는 것으로 나타났다. 특히 위험요인 중 정서적 대처기제는 세 가지 적응 영역에 공통적으로 부(-)적인 영향을 미치는 것으로 나타났고, 영적 안녕감은 세 가지 적응 영역에 공통적으로 정(+)적인 영향을 미치는 것으로 나타나 이에 대한 개입이 필요함을 알 수 있다. 둘째, 위험요인이 심리, 행동, 학업적응에 미치는 영향에 대한 보호요인의 조절효과는 다음과 같다. 심리적응의 경우, 정서적 대처기제의 영향을 자아존중감, 영적안녕감, 부모자녀 의사소통이 조절하고, 낮은 가족적응성의 영향을 영적 안녕감, 부모자녀 의사소통, 부모자녀관계가 조절하며, 부정적 생활사건의 영향을 부모자녀관계가 조절하는 것으로 나타났다. 행동적응의 경우, 정서적 대처기제의 영향을 영적 안녕감, 부모자녀 의사소통, 부모자녀관계, 사회적 지지가 조절하고, 사별 부모와의 낮은 애착관계의 영향을 부모자녀 의사소통, 부모자녀관계, 사회적 지지가 조절하는 것으로 확인되었다. 낮은 가족적응성의 영향을 개인, 가족, 환경차원의 보호요인이 조절하고, 부정적 생활사건의 영향을 자아존중감, 부모자녀의사소통, 부모자녀관계, 사회적 지지가 조절하는 것으로 나타났다. 특히 학업적응의 경우, 개인, 가족차원 위험요인의 영향을 영적 안녕감과 사회적 지지가 조절하는 것으로 나타나 부모사별 청소년들의 학교적응 수준 향상을 위해서는 학교사회복지제도가 더욱 활성화될 필요가 있음을 알 수 있다. 셋째, 앞서 살펴본 조절효과에 사별특성에 따른 차이가 있음을 확인한 결과, 자녀 성별, 사별부모 성별, 사망 원인에 따라 위험요인이 심리, 행동, 학업적응에 미치는 영향에 대한 보호요인의 조절효과가 서로 다르게 나타났다. 그러나 심리 적응의 경우, 자녀 성별에 관계없이 부모자녀 의사소통이 정서적 대처기제, 낮은 가족 적응성, 부정적 생활사건의 영향력을 상쇄시키는 것으로 확인되었다. 학업 적응의 경우, 사망 원인에 관계없이 영적 안녕감이 정서적 대처기제의 영향을 완충하는 것으로 나타났으며, 자녀 성별에 관계없이 사회적 지지가 정서적 대처기제, 낮은 가족적응성, 부정적 생활사건의 영향을 완충하였다. 본 연구는 지금까지 진행되었던 적응유연성 관련 연구에서 적응유연성에 대한 개념이 혼란스럽게 정의되고 측정되었던 점을 보완하여, 종속변수를 적응으로 측정하고, 부모사별을 경험한 이후 긍정적인 적응을 보이는 과정을 적응유연성 기제로 개념화하였다는 점에서 이론적으로 기여한 점이 있다고 판단된다. 실천적으로는 부모사별 청소년의 적응 영역에 영향을 미치는 위험요인과 보호요인들의 주효과를 확인하고, 위험요인이 부모사별 청소년의 적응에 미치는 영향력을 보호요인이 완충시키는 조절효과를 확인하였으며, 조절효과가 사별 특성에 따라 다르게 나타나는 것을 검증하였다는 점에서 의의가 있다. 본 연구를 통해 다양한 상황에 처해 있는 부모사별 청소년들에 대한 사회복지 실천 현장의 민감성을 높이고, 다양한 개입 방안을 마련할 수 있을 것으로 기대된다. 본 연구가 부모사별 청소년의 적응유연성 연구에서 이론적, 실천적 함의를 갖지만 결과를 일반화하기 어렵고, 소규모 표본으로 인해 측정오차가 발생할 수 있으며, 횡단자료를 사용하여 변수들간의 인과관계를 정확히 설명하기 어렵다는 점은 연구의 한계로 남는다. 주요어 : 부모사별 청소년, 적응유연성, 위험요인, 보호요인, 주효과, 조절효과 학 번 : 2005-30079Docto
