In dental hospital, the actual condition of sharps related injuries and the associating factor

산업보건학과 산업보건학전공/석사[한글] 이 연구는 치과 진료 영역에서의 주사침 등의 손상의 발생과 영향요인을 알아보기 위해, 2007년 서울시의 두 개 대학부속 치과병원의 치과의사와 임상실습 경험이 있는 학생을 중심으로 510명으로부터 설문조사를 통하여 자료를 수집하여 시행된 단면적 조사 연구이다. 최근 1년간의 주사침 등의 손상 경험을 조사하였으며 관련성이 있는 주사바늘 찔림 예방 교육여부, 보호구 착용 습관, 주사침 등의 날카로운 기구의 사용 후 처리, 혈액 매개 감염성 질환에 대한 지식을 중심으로 질문하였다.최종 분석 자료에 포함된 대상자는 355명으로 이중에 1년간 1회 이상의 주사침 등의 손상 경험이 있는 경우는 332명으로 전체 대상자중 93.5%를 차지하였다. 조사된 일반적 특성 중 손상 경험의 횟수에 유의한 차이를 보인 변수는 성별이었으며 여성이 남성보다 손상 횟수가 많았다(P<0.05). 손상부위는 왼손과 손바닥 쪽이 오른손이나 손등 쪽보다 많았고 손상도구로는 치과용 주사기가 많았으며 침윤마취 중에 가장 많이 찔렸다. 하루 평균 근무시간이 짧고 주사침 손상 예방 교육을 받은 그룹에서 주사침 등의 날카로운 기구의 사용 후 처리를 잘했다. 회귀분석 결과 손상횟수의 요인으로 주사바늘 분리수거통이 치료유닛마다 비치되어 있는 경우가 손상횟수가 적었다. 그러나 주사침 등의 손상에 영향을 줄 것으로 생각되는 주사침 손상 예방교육 여부, 보호구의 착용습관, 주사바늘 등의 날카로운 기구의 사용 후 처리, 혈액 매개성 감염성 질환에 대한 지식은 통계적으로 유의한 차이를 보이지는 않았다.주사바늘 찔림 예방교육을 받은 그룹과 교육을 받지 않은 그룹 사이에 주사침 등의 손상 후 대응방법에 있어 유의한 차이를 보였으며, 주사바늘 찔림 예방 교육을 받은 그룹이 손상 후 응급처치 시에 베타딘 소독을 더 많이 하고 있었다. 날카로운 기구 등의 사용 후 처리에서도 주사바늘 찔림 예방 교육을 받은 그룹의 점수가 더 높았다. 주사바늘 찔림 예방교육이 주사침 등 날카로운 기구의 사용 후 처리에 영향을 주는 중요한 요인으로 보인다.이상의 결과로 볼 때, 보호구 착용 습관과 예방 교육이 주사침 등의 손상 예방과 사후 관리에 중요한 것으로 나타나, 이들에 대한 관리가 치과 영역의 산업보건관리에 매우 중요한 것으로 판단된다. [영문]The purpose of this study is to research sharps injuries and the risk factor in dentists and dental students. This cross sectional study was based on the data collected from the questionnaires obtained from 510 dentists, oral and maxillofacial surgeons and experienced dental students were working at two different university dental hospitals located in Seoul, Korea.The questionnaires were about the experience of the sharps injuries, whether they had been participated the injury preventive program, whether they knew how to use the protection equipments, how to manage the used injection syringes and what they knew about the bloodborne infectious disease related to the sharps injuries.As a result, 93.5％ of the respondents (322 among 355) answered positive experience of the sharps injuries more than once a year. Female personnel experienced more often than male personnel(p<0.05). There was no statistical difference among the professional personnel groups about the causes of sharps injuries.The left hands had more sharps injuries than the right hands and the palm side had more injuries than the back of the hand. The most frequent instrument involved in sharps injuries was dental syringes and the most frequent procedure related to sharps injuries was anesthetic injection(infiltration).The personnels who had shorter duty hours or who had taken the preventive education handled better the sharp instruments after use than the others. According to the multiple linear regression, the frequency of sharps injuries was smaller in cases that the needle chambers were placed on the unit-chair.To conclude, the personal preventive device including needle chambers and preventive education were very important to prevent sharps injuries.ope
