Long-term behaviour of a dam affected by alkali–silica reaction studied by a multi-scale model

This paper aims to employ a 2D thermo-mechanical multi-scale ASR model for the analysis of a concrete gravity dam in Western Switzerland. Simulation results are compared to the field measurements and observations. Analysis of the results reveals negligible effect of temperature variation on the ASR advancement. The difference in length between the upstream and the downstream faces is identified as the main source for the upstream drift at the level of the crest. Study of the structural effects reveals ASR-related expansion anisotropy and cracks alignment being more pronounced along the upstream part and the foundation. If the former is explained by the transmission of the self-weight, the latter is attributed to the constraining effect of the underlying rock

Influence of the family socialization on the formation of gender features of modern teenagers

The relevance of the investigated problem is due to insufficient knowledge about the impact of parent-child relationship features on the development of the adequate teenager gender identity. The purpose of the article is to study the effect of the family socialization on the formation of gender features of modern teenagers. The leading approach to the study of this problem is the system-activity approach. In the course of empirical study it was found that the formation of masculine features of modern teenagers during the family socialization is associated with a positive attitude on the part of the father, and the formation of feminine traits is due to a positive attitude on the part of the mother and too rigid and inconsistent attitude on the part of the father. Роль проводимого тестирования перед началом обучения базовым лапароскопическим навыкам

Background. The urgency of the problem with training specialists in laparoscopic techniques is due to the wide introduction of minimally invasive technologies into surgical practice. Previously untrained, a surgeon runs considerable difficulties with the coordination of movements and the use of tools, which may lead to both the longer life of surgical aids and damage to the viscera and vascular structures in a patient. Many factors influence the level of manual skills in the audience at the beginning of training, which further affect its duration and hence cost.Objective. To evaluate the effect of initial level of basic manual skills for the duration of the study.Materials and methods. The investigation enrolled 255 listeners: medical students, interns, residents, and practicing surgeons. Manual skills as single successive performance of exercises 1 (peg transfers) and 2 (cutting a circle) in an endotrainer box were tested before training. The correlation of the results of testing for the rate of formation of basic laparoscopic skills was then analyzed.Results. According to the results of the testing, only 20 % of the physicians showed a result of less than 110 s and were ready for being further taught in intracorporeal suturing techniques. The authors consider that all students and interns/residents and most physicians must start their training in laparoscopic technique from a series of basic exercises. The analysis revealed a correlation between the results of doing exercises 1 and 2 and the number of their repetitions to achieve the needed level (r = 0.66).Conclusion. The single performance of peg transfers and cutting a circle exercises before training could objectively measure the performance of laparoscopic skills, define an individual plan, and optimize the costs of the training.Введение. Актуальность проблемы обучения специалистов технике лапароскопических вмешательств связана с широким вне­ дрением малоинвазивных технологий в хирургическую практику. Без предварительного тренинга хирург испытывает значитель­ные трудности в координации движений и навигации инструментов, что может привести как к увеличению продолжительно­ сти оперативного пособия, так и к повреждению внутренних органов и сосудистых структур у пациента. Многие факторы влияют на уровень мануальных навыков курсантов в начале обучения, которые в дальнейшем отражаются на продолжительно­сти и, следовательно, стоимости обучения.Цель работы – оценить влияние начального уровня базовых мануальных навыков на продолжительность обучения.Материалы и методы. В исследовании приняли участие 255 слушателей: студенты медицинских вузов, интерны, ординаторы и пра­ктикующие врачи хирургических специальностей. Перед обучением проводили тестирование мануальных навыков в виде однократно­го последовательного выполнения упражнений 1 («перекладывание предметов») и 2 («вырезание круга») в коробочковом тренажере. Далее был проведен анализ корреляции результатов тестирования на скорость формирования базовых лапароскопических навыков.Результаты. По итогам проведенного тестирования только 20 % врачей показали результат менее 110 с и были готовы к даль­нейшему обучению технике интракорпорального шва. Считаем, что все студенты, интерны/ординаторы и бóльшая часть вра­ чей должны начинать обучение лапароскопической технике с блока базовых упражнений. При анализе была выявлена корреляция между результатами выполнения упражнений 1 и 2 и количеством их повторений для достижения необходимого уровня (r = 0,66).Выводы. Однократное выполнение упражнений «перекладывание предметов» и «вырезание круга» перед обучением позволило объ­ективно оценить уровень владения лапароскопическими навыками, определить индивидуальный план и оптимизировать затраты на проводимое обучение

Лапароскопическая пластика мочеточников у онкологических пациентов

Objective: to improve the results of treatment of cancer patients with strictures and obliteration of the ureters after surgery and/or remote radiation therapy.Materials and methods. The study included 6 men and 12 women. The average age of men was 67 + 8.5 years (53—79years) and 46 + 15.8 years (28—74years) among women. According to the results of X-ray examinations, the average length of cicatricial narrowing of the ureter was determined, after which the tactics of surgical treatment were determined.Results. The duration of surgical treatment rangedfrom 97 to 380 minutes. Intraoperative blood loss in all groups was approximately the same and ranged from 20 to 160 ml. Intraoperative complications occurred in one patient (5.5 %), injury of the intestine. The described damage was repaired by the intracorporeal manual suture. All operations were completed laparoscopically. A generalized indicator of postoperative complications reached 16.7 %. All the complications that occurred had a slight effect on the the rehabilitation period.Conclusion. The results of this work indicate that ureteroplasty in patients after cancer treatment can be performed by laparoscopic access, which significantly reduces the invasiveness of surgery, reduces the length of hospital stay and rehabilitation of patients. Also, the possibilities of laparoscopic surgery allow you to achieve better visualization, to form more precise sutures.Цель исследования — улучшение результатов лечения онкологических пациентов со стриктурами и облитерациями мочеточников после перенесенного оперативного вмешательства и/или дистанционной лучевой терапии.Материалы и методы. В исследование включены 6 мужчин и 12 женщин. Средний возраст мужчин составил 67 + 8,5 (53—79) года, женщин — 46 + 15,8 (28—74) года. По результатам проведенных рентгенурологических обследований оценивали среднюю протяженность рубцового сужения мочеточника, после чего определяли тактику оперативного лечения.Результаты. Продолжительность оперативного лечения составила 97—380мин. Интраоперационный объем кровопотери во всех группах был примерно одинаковый и составил 20—160мл. Необходимости в гемотрансфузии не возникло ни в одном случае. Интраоперационное осложнение (вскрытие просвета тонкой кишки) зафиксировано у 1 (5,5 %) пациентки и устранено интракор-поральным мануальным швом. Все операции завершены лапароскопически без конверсии доступа. Обобщенный показатель послеоперационных осложнений достиг 16,7 %. При этом все возникшие осложнения незначительно повлияли на среднее число койко-дней и период реабилитации.Заключение. Результаты свидетельствуют о том, что пластику мочеточника у пациентов после лечения онкологических заболеваний можно выполнять лапароскопическим доступом, который значительно снижает травматичность хирургического вмешательства, сокращает длительность пребывания в стационаре и реабилитации больных. Также возможности лапароскопической хирургии позволяют добиться лучшей визуализации, сформировать более прецизионные швы

The first experience of hand-assisted laparoscopic donor nephrectomy is described in the article. A new technique of the operation and it’s results are discussed in details. Advantages of this type of the operation in comparison with open and full laparoscopic techniques are presented. В статье приведен первый в Российской Федерации опыт выполнения мануально-ассистированной лапа- роскопической донорской нефрэктомии с последующей успешной трансплантацией почки родственному реципиенту. Подробно описана техника операции и ближайшие результаты. Обсуждаются преимущества данной методики перед традиционными «открытыми» и полностью лапароскопическими операциями.

СРАВНИТЕЛЬНЫЙ АНАЛИЗ РЕЗУЛЬТАТОВ ДОНОРСКОЙ НЕФРЭКТОМИИ, ВЫПОЛНЕННОЙ ОТКРЫТЫМ И РАЗЛИЧНЫМИ МОДИФИКАЦИЯМИ ЛАПАРОСКОПИЧЕСКОГО ДОСТУПА

The aim of this study was to evaluate our experience on laparoscopic living donor nephrectomy (LDN). During the last 1,5 years we have gradually expanded the use of minimally invasive LDN with various techniques: strictly laparoscopic and hand-assisted laparoscopic. This comparative study is based on 250 conventional open LDN and 61 laparoscopic LDN. Minimally invasive hand-assisted technique was shown to be associated with a significantly lower risk of major complications and intraoperative incidents, as well as reduced warm ische- mia and operative time. There was no negative impact on recipient’s results. In our opinion, the introduction of hand-assisted technique is probably the most significant single factor for improved results, although accumulated experience and developments in equipment will contribute. Improvements in surgical outcomes following donor nephrectomy may enhance living donor programmes. В статье приводится опыт лапароскопических нефрэктомий, выполненных у живых доноров почки. В последние 1,5 года в нашем Центре используются минимально-инвазивные техники донорской не- фрэктомии: полностью лапароскопическая и мануально-ассистированная лапароскопическая. Проведен сравнительный анализ 250 открытых нефрэктомий и 61 нефрэктомии, выполненной лапароскопическим доступом. Показано, что мануально-ассистированная лапароскопическая нефрэктомия характеризуется минимальным риском серьезных осложнений, сокращает время тепловой ишемии и продолжительность операции, не оказывает отрицательного влияния на результат операций у реципиента. Мы считаем, что мануально-ассистированная лапароскопическая нефрэктомия у донора является методом выбора при родственной трансплантации почки. Внедрение этой технологии может способствовать привлечению до- норов и развитию программ родственной трансплантации почки.

Study on the internal crack network of the ASR-affected concrete by the tomography-based numerical model

In this paper, we present a novel finite element model to simulate alkali-silica reaction in a realistic concrete meso-structure. Application of the internal ASR loading leads to the evolution of multiple deviated cracks and corresponding macroscopic expansion. A particular crack-extension algorithm and a solution scheme provide numerical stability and allow to model complicated crack patterns while preserving the physics. The predictive validity of the model is demonstrated by matching an analytical solution of a loaded penny-shaped crack. The model is applied to the experimental dataset comprising the time-evolving X-ray tomograms of the ASR-affected concrete. Similar to the tomography data, the model results in an expanded concrete sample with a developed crack network. Two hypotheses on the crack loading and extension mechanisms are tested by comparing the crack statistics. Simulations with varying number of ASR sites and application of the uniaxial loading bring interesting insights.
