Analysis of contributing factors of the breast cancer mortality rate in Hungary between 2002 and 2014
Early tumour detection is very important in breast cancer treatment and in clinical outcomes. As breast cancer mortality depends on the stage of the diagnosis and oncological outcomes are generally more favourable during early stages of disease before symptoms appear in symptomatic diseases. Despite mammography screening, with the aim of mortality reduction, the incidence of breast cancer mortality in Hungary has not changed significantly since 2002.
Answer of the 2.1. question of the thesis: According to the evaluation of institutional breast cancer care according to EUSOMA quality indicators Hungarian women with breast cancer represented higher tumour stage compared to western countries. The percentage of early stage breast cancer cases was lower compared to US and Sweden. This quality representative cross-sectional view of stage based breast cancer cases suggests that Hungarian relatively higher mortality rate could be associated to higher tumour stages at diagnosis.
Answer of the 2.2. and 2.3. questions of the thesis: According to the comparison of SCR and SYM groups patients with non-palpable early stage breast cancers diagnosed via population-based breast screening had not better survival rates than those with symptomatic cancers. The reason why there were no differences in OS and DFS remains unclear. The potential drawback of symptomatic early stage tumours compared to non-palpable early stage tumours could be equalized by modern breast cancer molecular subtype-based personalized multimodality oncology treatment. There is a trend of the DFS in favour of the SCR group but it has not reached the statistical significance. The amount of distant metastases was higher in SYM group compared to SCR group and as the metastatic breast cancer incurable disease probably a longer follow-up period might show the statistical significance of DFS and/or OS in favour of the SCR group. Further investigations and longer follow-up are needed to answer these questions.
The thesis does not suggest that mammography screening does not reduce breast cancer mortality, but support the evidence that mammography screening reduces the rate of advanced breast cancers. As breast cancer deaths are caused mostly by advanced disease that has already spread to the lymph nodes or distant organs, important public health goal is to increase adherence rate on mammography screening. Increased knowledge regarding the barriers to mammography screening provides information to extend our knowledge of breast cancer screening and effective treatment in Central-Eastern European countries.
Answer of the 2.4. question of the thesis: Our findings highlight that the main reasons for women to not attend OMSP included work absenteeism, fear of painful examination, lack of information, and false beliefs regarding mammography screenings. To reduce barriers such as work absenteeism, a paid day off may increase women’s willingness to attend the OMSP. In order to increase screening compliance, environment and facility staff is very important factors associated with the women’s satisfaction and therefore their participation in future screenings. A patient navigator system employing prevention nurses could help to organize an effective screening program by offering education on breast awareness, screening invitations, and managing appointments for biennial mammograms. Married women more regularly receive breast screening compared to single women; marital status seems to be a protective factor against barriers like embarrassment and financial difficulties that may prevent travel to the DSC. Single women and rural female residents require special attention because of their lower participation rates for mammography exams. In these populations, patient navigation, free public transportation, specialized round-trip bus lines, or mobile breast screening units may help to decrease these screening barriers.
Answer of the 2.5.question of the thesis: In the following using applicable information channels as the internet for future screening target population and the patient navigation system, employing prevention nurses, general practitioners or other specialties could provide feasible breast cancer awareness for actual screening target population in conclusion of increased breast screening participation
Azonnali, postmastectomiás emlőrekonstrukciókkal szerzett tapasztalatok. Száz eset klinikopatológiai utánkövetése és a kozmetikai eredmények felmérése | Evaluation of clinicopathological findings and cosmetic outcome of 100 immediate postmastectomy breast reconstruction cases
Absztrakt
Bevezetés: Az azonnali emlőhelyreállító műtétek – biztosítva az
onkológiai radikalitást és a megfelelő kozmetikai értéket – nem késleltetik az
adjuváns kezeléseket, lehetővé teszik a képalkotó kontrollt, hosszabb műtéti
időt igényelve. Célkitűzés: Hazai betegek körében először, az
azonnali, postmastectomiás emlőhelyreállító műtétek eredményeinek felmérése és
összehasonlítása a nemzetközi irodalom adataival. Módszer: A
szerzők 2011. május és 2014. szeptember között 100 betegen végzett 121 terápiás
és kockázatcsökkentő profilaktikus, postmastectomiás, azonnali emlőhelyreállító
műtét klinikopatológiai adatainak retrospektív feldolgozását végezték. A műtétek
onkológiai, műtéttechnikai és kozmetikai adatait statisztikai módszerekkel
elemezték. Eredmények: A betegek átlagéletkora 42,6 év, az
utánkövetési idő 29,4 hónap, a műtéti idő 132 perc volt. Leggyakrabban
bőrtakarékos mastectomiát végeztek (64%). Az emlőhelyreállítás 70%-ban
submuscularis szöveti expanderrel történt. Korai posztoperatív szövődményt 18
esetben, lokoregionális kiújulást egy betegnél észleltek. Az adjuváns kezelések
megkezdésének átlagideje 4,8 hét volt. A kozmetikai végeredménnyel a betegek
89%-a volt elégedett. Következtetések: Az azonnali
emlőhelyreállítás technikái biztonságosan és eredményesen végezhetők, megfelelve

Abstract
Introduction: Immediate breast reconstruction provides
oncological safety, requires longer operation time. It does not influence the
initiation of adjuvant therapy and radiological control, and results in
favourable cosmetic outcome. Aim: Assessing the Hungarian data
of immediate postmastectomy breast reconstructions, and comparing them to
international findings. Method: Between May, 2011 and
September, 2014 121 therapeutic and prophylactic, postmastectomy immediate
breast reconstructions were performed in 100 patients. The clinico-pathological
findings were assessed retrospectively, and surgical, oncological and cosmetic
outcomes were evaluated statistically. Results: The mean age of
patients was 42.6 years, the follow up time was 29.4 months, and the duration of
operation was 132 minutes. Skin-sparing mastectomy was performed most commonly
(64%) with submuscular tissue expander placement (70%). Early postoperative
complication was identified in 18 patients, and loco-regional recurrence in 1
patient. Most patients (89%) were satisfied with the cosmetic outcome. The
average initiation time of adjuvant therapy was 4.8 weeks.
Conclusions: Immediate breast reconstruction is a safe and

Bevezetés: A European Society of Breast Cancer Specialists
létrehozta az emlőrákellátást végző onkológiai központok és szakorvosok európai
minőségbiztosítási feltételrendszerét, hogy egységes minimumstandardok
bevezetésével csökkentse az emlőrák mortalitását a kontinensen.
Célkitűzés: Jelen vizsgálat célja, hogy az Országos
Onkológiai Intézet Emlő- és Lágyrészsebészeti Osztályán operált, illetve az
intézetben multidiszciplinárisan kezelt emlődaganatok komplex onkológiai
ellátását a European Society of Breast Cancer Specialists minőségbiztosítási
kívánalmai szerint elemezze és a nagy esetszámú, reprezentatív minta révén a
magyar emlőrákos populációról részletes klinikopatológiai adatokat szolgáltasson
az onkológiai ellátórendszer számára. Módszer: 2011. június 1.
és 2012. május 31. között multidiszciplinárisan kezelt emlődaganatok részletes
klinikopatológiai retrospektív adatfeldolgozását végezték a European Society of
Breast Cancer Specialists egységes nemzetközi kritériumrendszere szerint.
Eredmények: A vizsgált időszakban 906 betegnél történt
malignus, illetve jóindulatú emlődaganat miatt műtét. Az EUSOMA kötelező
minőségbiztosítási minimumelvárásainak az Országos Onkológiai Intézet eredményei
mindenben megfeleltek. Következtetések: Az Országos Onkológiai
Intézet emlődiagnosztikával és multidiszciplináris onkológiai kezelésével
foglalkozó emlőrák-ellátási szervezeti egysége a European Society of Breast
Cancer Specialists által meghatározott minimumesetszámot, személyi és tárgyi

Abstract
Introduction: The European Society of Breast Cancer Specialists
has created quality indicators for breast units to establish minimum standards
and to ensure specialist multimodality care with the conscious aim of improving
outcomes and decreasing breast cancer mortality. Aim: The aim
of this study was to analyse the breast cancer care in the National Institute of
Oncology according to the European Society of Breast Cancer Specialists
requirements and in a large number of cases in order to present representative
clinico-pathological data on the incidence of breast cancer in Hungary.
Method: According to the European Society of Breast Cancer
Specialists uniformed criteria clinico-pathological data of multimodality
treated breast cancer cases were retrospectively analysed between June 1, 2011
and May 31, 2012. Results: During the period of interest 906
patients underwent breast surgery for malignant or benign lesions. According to
the European Society of Breast Cancer Specialists quality indicators the breast
cancer care of the National Institute of Oncology is eligible.
Conclusions: The diagnostic modalities and multimodality
care of breast cancer of the National Institute of Oncology breast unit meets
the critical mass and minimum standards of the European Society of Breast Cancer

Második emlőmegtartó műtét és szövetközi sugárkezelés az emlődaganat lokális kiújulásának kezelésére. Ötéves eredmények = Second breast conserving surgery and interstitial radiotherapy for the treatment of breast tumor local recurrences. Five-year results

Bevezetés és célkitűzés: Második emlőmegtartó műtét és nagy
dózisteljesítményű szövetközi sugárkezelés eredményeinek bemutatása azonos
oldali emlődaganat kiújulása miatt kezelt betegeknél. Módszer:
Korai invazív emlőrák előzetes emlőmegtartó kezelése után jelentkező helyi
daganatkiújulás miatt 1999 és 2015 között 33 betegnél végeztünk második
emlőmegtartó műtétet perioperatív szövetközi sugárkezeléssel. A második
emlőmegtartó műtét során a tumorágyba átlagosan 8 (tartomány: 4–24) darab
flexibilis katétert ültettünk be. A perioperatív időszakban a tumorágy és annak
1–2 cm-es biztonsági zónájának területére adott összdózis 22 Gy volt (5 × 4,4
Gy, 3 nap alatt). Adjuváns szisztémás kezelésként 24 beteg (73%) egyedüli
endokrin kezelésben, 6 beteg (18%) pedig kemoterápiában részesült. A túlélési
eredményeket a Kaplan–Meier-módszerrel elemeztük. A késői mellékhatásokat és a
kozmetikai eredményeket feljegyeztük. Eredmények: A második
emlőmegtartó kezeléstől számított követési idő középértéke 61 hónap (tartomány:
26–189 hónap) volt. A követési idő alatt 4 betegnél (12,1%) alakult ki második
lokális recidíva. A második helyi daganatkiújulás, a regionális daganatkiújulás
és a távoli áttétképződés ötéves valószínűsége 6,3%, 6,1% és 14,9% volt, azonos
sorrendben. Az ötéves betegségmentes, daganatspecifikus és teljes túlélés 76,2%,
92,4% és 89,2% volt. Kiváló, jó, megfelelő és rossz kozmetikai eredményt 4
(12%), 19 (58%), 4 (12%) és 6 (18%) betegnél állapítottunk meg. Grade 2-es és
3-as fibrosis 9 (27%) és 1 (3%) betegnél alakult ki. Tünetmentes zsírnekrózist 7
(21%) betegnél figyeltünk meg. Következtetés: A második
emlőmegtartó műtét perioperatív szövetközi sugárkezeléssel biztonságos lehetőség
az emlődaganat helyi kiújulásának kezelésére. A szövetközi sugárkezelés
elfogadható kozmetikai eredmények és kevés késői mellékhatás mellett
csökkentheti a második lokális kiújulás valószínűségét, így válogatott esetekben

Abstract:
Introduction and aim: To report the clinical outcomes of second
breast-conserving therapy with perioperative interstitial radiotherapy for the
treatment of ipsilateral breast tumor recurrences. Method:
Between 1999 and 2015, 33 patients, presenting with an ipsilateral breast tumor
recurrence after previous breast conserving therapy, were salvaged by
re-excision and perioperative high-dose-rate interstitial brachytherapy. A
median of 8 (range: 4–24) catheters were implanted into the tumor bed
intraoperatively. A total dose of 22 Gy in 5 fractions of 4.4 Gy was delivered
to the tumor bed with a margin of 1–2 cm, on 3 consecutive days. The adjuvant
systemic treatments consisted of hormonal therapy for 24 patients (73%) and
chemotherapy for 6 patients (18%). The survival results were estimated by the
Kaplan–Meier method. Late side effects and cosmetic results were also
registered. Results: The median follow-up time following the
second breast conserving therapy was 61 months (range: 26–189 months). During
the follow-up, 4 patients (12.1%) developed second local recurrence. The
five-year actuarial rates of the second local, regional and distant recurrence
were 6.3%, 6.1%, and 14.9%, respectively. The five-year probabilities of
disease-free, cancer-specific and overall survival were 76.2%, 92.4%, and 89.2%,
respectively. Four (12%), 19 (58%), 4 (12%) and 6 (18%) patients had excellent,
good, fair and poor cosmetic results, respectively. Grade 2 and 3 fibrosis
developed in 9 (27%) and 1 (3%) patients. Asymptomatic fat necrosis was detected
in 7 (21%) women. Conclusion: Second breast conserving therapy
with perioperative high-dose-rate interstitial brachytherapy is a safe and
feasible option for the management of ipsilateral breast tumor recurrences.
Interstitial brachytherapy may decrease the risk of second local relapse with
acceptable cosmetic results and low rate of late side effects. Hence, in
selected cases it can provide a feasible alternative to salvage mastectomy. Orv

A módosított Regnault „B” emlőmegtartó műtét mint II. szintű standard onkoplasztikus emlősebészeti technika
Bevezetés és célkitűzés: A szerzők a Regnault „B” típusú emlőfelvarrás sebészi technikájának módosításával végzett onkoplasztikus műtéti technikát és az új emlősebészeti módszer alkalmazásához kapcsolódó retrospektív klinikopatológiai vizsgálat eredményeit mutatják be. Módszer: 2012. április és 2018. október között, emlőrák miatt, módosított Regnault „B” technikával operált 215 nőbeteg klinikopatológiai adatait prospektíven vezetett adatbázis alapján retrospektív módon vizsgáltuk. A betegek életminőségét validált kérdőív segítségével mértük fel, míg a műtétek esztétikai eredményét a szintén validált Breast Cancer Conservative Treatment (BCCT.core) számítógépes program és az 5 pontos Likert-skála alapján értékeltük. Eredmények: A betegek átlagéletkora 53 év (szórás: 29–81 év) volt. A medián utánkövetési idő 47 hónap (szórás: 7–85 hónap) volt. Az átlag műtéti idő 47 perc (szórás: 35–85 perc) volt, míg a patológiai tumorméret átlagosan 33 mm-nek (szórás: 18–58 mm) bizonyult. Pozitív sebészi szél miatt 13 (6%) esetben irányított reexcisióra, míg 3 (1,4%) esetben mastectomiára kényszerültünk. Az összesített szövődményarány 7,4% (n = 16) volt. Az esztétikai eredmények Likert-skála szerinti átlagértéke 4,2 (szórás: 2–5), míg a BCCT.core program alapján 1,3 pont (szórás: 1–4 pont) volt. Az életminőséggel kapcsolatos kérdőívek eredményei magas betegelégedettséget igazoltak. Következtetés: A módosított Regnault „B” emlőmegtartó technika biztonságos és hatékony standard ’level II.’ onkoplasztikus emlősebészeti technika. A műtét magas betegelégedettséggel, illetve kozmetikai eredménnyel képes a közepes vagy nagyobb térfogatú emlők külső, illetve külső-felső negyedeinek T1–T3-as tumorai miatt az emlők akár 20–50%-ának eltávolítására, majd egyidejű parenchymaáthelyezéssel történő rekonstrukciójára. A módszer előnye, hogy nem igényel ellenoldali szimmetrizációs műtétet, míg hátránya, hogy az emlő bőrpalástján vezetett metszések komplettáló mastectomia esetén az azonnali rekonstrukciót nehezítik
A magyar emlőrákos betegek igényei a korszerű onkoplasztikus emlősebészeti ellátásra
The significant need for breast reconstruction resulting from the spread of oncoplastic breast surgery raises a number of systemic issues. Clarification and regulation of the indications are needed for aesthetic changes of the reconstructed breast due to oncotherapy treatments, ageing and technical problems of implants; a number of operations, targeted aesthetic goals as well as surgical capacities and financial background should also be determined.Our aim was to conduct a survey on the opinions and needs of the Hungarian breast cancer population about a modern breast reconstruction system.A study was conducted enrolling 500 patients who underwent mastectomy with immediate or delayed reconstruction. A structured questionnaire containing eleven questions was used to measure the attitude for loss and reconstruction of breast, the expectation of cosmetic outcome and qualification of the operating surgeon and the needs relating to the health system and funding.The median age was 47 years (min.-max.: 26-73), 59% (n = 294) was married and 52% (n = 260) had graduated in university. The majority of women (70%; n = 348) would like to have nakedly also similar breasts after the reconstruction process. To achieve this, 43% (n = 217) and 37% (n = 184) would undergo maximum two or four procedures, respectively, supported by the national health insurance company. 86% (n = 430) would like to choose qualified breast surgeon for her treatment.The modern oncoplastic treatment raises complex, systemic issues. Women with breast cancer would like to have qualified breast surgeons restoring their breasts by two operations, all funded by the national health insurance company. Evaluation of Patient Knowledge, Desire, and Psychosocial Background regarding Postmastectomy Breast Reconstruction in Hungary: A Questionnaire Study of 500 Cases
Background According to European guidelines, breast cancer patients requiring mastectomy should be informed about available options regarding breast reconstruction. There are clear differences in the quality standards of oncoplastic care throughout Europe, with slight improvements in Central European countries like Hungary. The aim of the present investigation was to evaluate patients' knowledge and demand for postmastectomy breast reconstruction, as well as their psychosocial background regarding decision-making. Material and Methods A questionnaire containing 15 structured questions was given to 500 breast cancer patients on the day before undergoing mastectomy. The questions focused on the emotional impact of the malignant disease and the loss of a breast; the importance of environmental conditions; the desire for breast reconstruction; and patients' knowledge and sources of information about the procedure. All answers were statistically analyzed in the context of patient age, marital status, educational level, and place of residence. Results Descriptive statistical results of the answers to all questions, as well as associations of the different aspects of the decision-making process, are presented. Conclusions Hungarian breast cancer patients have very limited knowledge regarding breast reconstruction. We confirmed that patients scheduled for mastectomy have a great degree of anxiety due to their disease and breast loss. Almost 50% of the responders declared their desire for postmastectomy breast reconstruction. Patient's age, residence, educational level, marital status, and profession were confirmed as predictive factors in the decision-making process for breast reconstruction
Ajánlás a B-, a C- és a D-vírus hepatitisek diagnosztikájára és antivirális kezelésére = Hungarian Consensus Guideline for the Diagnosis and Treatment of B, C, and D Viral Hepatitis
More than 1% of the Hungarian population is infected with hepatitis B, C, or D viruses. Since 2006 the diagnostics and therapy of these infections are carried out in treatment centers according to national guidelines – since 2010 according to financial protocols. The consensus-based guidelines for 2012 are published in this paper. The guidelines stress the importance of quick and detailed virologic evaluations, the applicability of transient elastography as an acceptable alternative of liver biopsy in this regard, as well as the relevance of appropriate consistent follow up schedule for viral response during therapy. The first choice of therapy in chronic hepatitis B infection is pegylated interferon for 48 weeks or continuous entecavir therapy. The later must be continued for at least 6 months after hepatitis B surface antigen (HBsAg) seroconversion. Tenofovir disoproxil fumarat is not yet reimbursed by the National Health Insurance Fund. Adefovir dipivoxil is recommended mainly in combination therapy. Lamivudine is no longer a first choice; patients currently taking lamivudine must switch if response is inadequate. Appropriate treatment of patients taking immunosuppressive medications is highly recommended. Pegylated interferon based therapy is recommended for the treatment of concomitant hepatitis D infection. Treatment naive chronic hepatitis C patients should initially receive pegylated interferon and ribavirin dual combination therapy. In genotype 1 infection if response is insufficient at 4 or 12 weeks one of the two new direct acting antivirals (boceprevir or telaprevir) should be added. The length of treatment is usually 48 weeks; in cases of extended early viral response shorter courses are recommended. Previous treatment failure patients with genotype 1 infection should receive a protease inhibitor backed triple combination therapy, mostly for 48 weeks. However, relapsers without cirrhosis and with extended rapid viral response, shorter telaprevir based combination therapy is sufficient. Drug-drug interactions as well as emergence of viral resistance are of particular importance. For genotype 2 or 3 HCV infections 24 weeks, for genotype 4 infections 24, 48 or 72 weeks of pegylated interferon plus ribavirin therapy is recommended in general.
