Managing Polypharmacy and Deprescribing in Elderly

    Get PDF
The increase in the number of medications used may result many negative consequences for patients and health system. Elderly patients are more likely to encounter these health problems associated with polypharmacy. Deprescribing, the process of tapering, withdrawing, discontinuing, or stopping medications, is important in reducing polypharmacy, adverse drug effects, inappropriate or ineffective medication use, and costs. Deprescribing in elderly patients in accordance with the evidence based guidelines has many positive outcomes in older people such as decrease in the risk of falls, improvement in cognition, and improvement in patients’ global health status. Therefore, each visit of an elderly patient should be considered as an opportunity to evaluate the unnecessary use or harms of the prescribed or nonprescribed medications. Clinicians should decide to deprescription process by individualized care goals in line with current guidelines. Beers Criteria, STOPP/START and The Medication Appropriateness Index-MAI can be used to assit clinicians to identify unnecessary or potentially inappropriate drugs and reduce the number of medications in older patients. But, a balance is required between over and under prescribing. In conclusion, prevention of polypharmacy and withdrawing unneccesary and inappropriate medications may be the best clinical decision for family physicians who follow the elderly in primary care

The purpose of the present study has been to explore the effect on students of an alternative measurement and assessment tool developed for the science unit on “Matter and Heat.” Inspired by the teaching technique of concept development and the alternative measurement tool of the descriptive branched tree, an alternative measurement tool of web design was developed to conduct experimental research with 62 students at a primary school in Samsun. Results showed that, compared to the control group students who were instructed with traditional teaching approaches, the experimental group students who were tested with the web-based measurement tool recorded a statistically significant difference in achievement in the unit on “Matter and Heat.” No significant difference, however, was observed in these students’ attitudes toward the science and technology lessons. It is believed that developing an alternative measurement tool of web design encompassing all of the units of the 6th, 7th and 8th grade science and technology curriculum will be of benefit to students and will make an important contribution to the work of teachersBu çalışmanın amacı, “Madde ve Isı” ünitesi için geliştirilen bir alternatif ölçme ve değerlendirme aracını tanıtmak ve öğrenciler üzerindeki etkinliğini araştırmaktır.

EFFECTS OF ANTIEPILEPTIC DRUGS ON AUTONOMIC FUNCTION TESTS AND QT DISPERSION IN EPILEPSY PATIENTSAdı ve Soyadı: Çiğdem Apaydın KayaDanışman Adı: Prof.Dr. Filiz OnatKabul Tarihi: 24.03.2004Program: Farmakoloji ve Klinik FarmakolojiAnabilim Dalı: Farmakoloji ve Klinik Farmakoloji ADSUMMARY:Sudden unexplained death might occur 40 times more frequently among individuals with epilepsy than among those without epilepsy. This is thought to be related to a dysfunction of autonomic nervous system. However it has not yet been clarified whether it is related to the antiepileptic drugs. The aim of this study is to examine if there is an autonomic dysfunction in cardiovascular regulatory systems of epilepsy patients, and if such a disorder exists to find out its relation with the antiepileptic drugs. For this purpose, autonomic function tests and QT dispersion (QTd) are evaluated in newly diagnosed 17 epilepsy patients before and at least 6 months after antiepileptic drug therapy. Findings are compared with the control group consisting of 17 healthy subjects. In the patient group, measured systolic blood pressure (SBP) and diastolic blood pressure (DBP) in resting state after therapy, were found significantly lower than before therapy. The decrease in the DBP was found related to valproic acid (VPA) administration while the decrease in SBP was related to carbamazepine (CBZ) administration. In the generalised epilepsy group, valsalva ratio was higher than observed partial epilepsy group before therapy. There was no difference between groups before and after therapy in the ratio 30/15, heart rate response to standing. However after therapy ratio was higher in the patient group compared to control group. CBZ administered patients and VPA administered patients did not show difference in 30/15 ratio before and after therapy. When blood pressure increase was evaluated with cold test, there was no difference between groups. With cold test, a significant increase compared to basal readings in heart rate after therapy was observed which was related with CBZ administration. No significant difference was observed when before therapy and after therapy QTd readings were compared. These results imply that epilepsy and antiepileptic drug therapy influence autonomic nervous system functions
