    The practice of constructing hypnotic realities

    This dissertation presents a constructivist discussion of the experiences of the author in doing hypnosis. It explores the practice of hypnosis as a progression of behavioral changes by the hypnotis in facilitating the initiation, expansion and coherence of the hypnotic domain. These changes include punctuating and redefining everyday experiences and environmental events as hypnotic, and engaging participants in discussions based on the hypnotic logic. Hypnosis is defined as social and cognitive domain that is specified in language, and maintained by the embodied descriptions of participants. It is described how the hypnotic system evolves in terms of complexity, distinctness and functional integrity, and how this reflects the structurally determined fit between its members. In this system the hypnotist facilitates the development of the hypnotic description as a viable domain of existence that is experienced as real and all-inclusive. In conclusion some constructivist ideas for research, treatment and training are presented before constructivist thinking is used to contextualize the dissertation itself.PsychologyM.A. (Clinical Psychology

    Cardiac myosin binding protein C, adrenergic stimulation and cardiac contractility

    Myosin binding protein C remained a perplexing although integral component of the sarcomeric thick filament until the discovery that genetic defects in its corresponding gene is a frequent cause of hypertrophic cardiomyopathy. Basic science investigation subsequently revealed that it is one of the most potent regulators of cardiac contractility. Phosphorylation of its N-terminus upon adrenergic stimulation, causes increased order in myosin heads as well as increased ATPase activity, Fmax and Ca2+-sensitivity of contraction, while its dephosphorylation upon cholinergic stimulation or during low flow ischaemia leads to changes in the sarcomeric thick filament that diminish interaction between myosin heads and actin. This dynamic flux in phosphorylation upon adrenergic stimulation is not only crucial to normal cardiac function and structure, but also vital for protection against ischaemic injury. Genetically-driven deficiency or inadequacy in cMyBPC leads to severe cardiac dysfunction and structural changes, including cardiac hypertrophy and dilation, and particularly attenuates the adaptive increase in left ventricular contractility that follows on β-adrenergic stimulation or pressure overload, resulting in decreased systolic function, and reduced cardiac output

    Mental health service users are one of the most socially excluded groups in society. Surveys indicate that younger people have less understanding of their own and others’ mental health. In this article we discuss attempts to promote a psychosocial understanding of mental health in schools and describe a brief classroom-based intervention in a secondary school. It emphasises the importance of psychologists and service users working together to challenge discrimination against people with mental health difficulties

    The aim of this study is to investigate the extent to which selected management accounting techniques are practised by small-medium enterprises (SMEs) in Nelson Mandela Bay. Management accounting techniques can be described as the information and methods management use for decision-making in order to operate the business more effectively. Management accounting research has focused on large companies over the years. Limited research exists with regards to the practises of management accounting techniques within SMEs and the extent to which it is used. It is widely accepted that SMEs are an important part of most economies and contribute significantly in economic terms. This study outlines the selected management accounting techniques, and highlights the importance of management accounting within SMEs. This study appropriately considered a quantitative method, by using a self-administered questionnaire for the primary objective of the study. The designated population, who was surveyed, included the financial manager, management accountant or skilled accounting staff members within SMEs. Purposive sampling was considered, which consisted of the database of Nelson Mandela Bay Chamber of Commerce, Business Partners and Transnet, all of which work closely with SMEs in Nelson Mandela Bay. The study obtained 53 responses from 126 SMEs targeted. However, only 50 responses were usable. The main findings of the study reveal that SMEs are neutral with regards to the usage of the selected management accounting techniques, which means that to some extent the selected management accounting techniques are used, and to another extent it is not used. Conclusions with regard to the extent to which SMEs use the selected management accounting techniques, cannot be generalised, and further research is recommended. However, this research can be used as a basis for future research

    The presence of pain in community-dwelling South African manual wheelchair users with spinal cord injury

    BACKGROUND: Pain after spinal cord injury (SCI) is common and is likely to continue throughout life with varying levels of severity. OBJECTIVE: To determine the presence of pain, the sociodemographic and injury profile of community-dwelling manual wheelchair users. METHOD: This quantitative correlational study used a sociodemographic and injury profile sheet and the Douleur Neuropathique 4 Questions (DN4) questionnaire to document demographic, SCI profiles as well as pain characteristics. Pain severity was determined using the Numeric Rating Scale. Data were analysed using the Statistical Package for the Social Sciences (SPSS) v27 at 0.05 level of significance. RESULTS: The pain rate was 104; 85% of 122 participants and mainly in those with complete SCI (77.9%). Neuropathic pain was more common (76; 62.5%) and significantly associated (p < 0.05) with higher pain severity. Pain was mainly in one area of the body (59; 48.4%) but occurring in up to five areas. The most painful area had a mean severity of 6.7/10; was more common in the lower limbs below the injury level (48; 39.4%); and was burning in nature (40; 32.7%). CONCLUSIONS: Pain after SCI is as problematic in the South African context as it is globally. With the rising SCI prevalence in the country, understanding pain and its presentation is important for holistic management of a person with SCI.     On the Equivalence of Quadratic Optimization Problems Commonly Used in Portfolio Theory

    In the paper, we consider three quadratic optimization problems which are frequently applied in portfolio theory, i.e, the Markowitz mean-variance problem as well as the problems based on the mean-variance utility function and the quadratic utility.Conditions are derived under which the solutions of these three optimization procedures coincide and are lying on the efficient frontier, the set of mean-variance optimal portfolios. It is shown that the solutions of the Markowitz optimization problem and the quadratic utility problem are not always mean-variance efficient. The conditions for the mean-variance efficiency of the solutions depend on the unknown parameters of the asset returns. We deal with the problem of parameter uncertainty in detail and derive the probabilities that the estimated solutions of the Markowitz problem and the quadratic utility problem are mean-variance efficient. Because these probabilities deviate from one the above mentioned quadratic optimization problems are not stochastically equivalent.     Assessment of scapular position in patients suffering from shoulder dysfunction

    Shoulder dysfunction inhibits 80% of competitive swimmers from performing optimally. The most common contributing factor to shoulder dysfunction is an altered scapula position. A standard for the distance between T4 and the medial border of the scapula is lacking. Measurements of the distance between T4 and the root of the scapula were made and the relationship of the distance and the function of trapezius middle fibres at resting length was tested. A Vernier caliper® (ICC 0.94) was used to measure the distance from T4 to the medial border of the scapula. Exercises and stretches to retrain and strengthen the middle fibres of the trapezius specifically were performed twice a week, scheduled around the swimmers’ regular training and strengthening sessions, for six weeks. The results showed that palpation of the scapula to measure the distance between T4 and the spinal root is reliable and valid. A normalisation index should be used to adjust for body build and posture. Furthermore, the distance between the thoracic spine and the scapula did not change, regardless of the statistical improvement in the function of trapezius middle fibres (p < 0.05). The research results are of importance for clinical practice, evaluation and treatment programmes for physiotherapists.     Myomegalin is a novel A-kinase anchoring protein involved in the phosphorylation of cardiac myosin binding protein C

    <p>Abstract</p> <p>Background</p> <p>Cardiac contractility is regulated by dynamic phosphorylation of sarcomeric proteins by kinases such as cAMP-activated protein kinase A (PKA). Efficient phosphorylation requires that PKA be anchored close to its targets by A-kinase anchoring proteins (AKAPs). Cardiac Myosin Binding Protein-C (cMyBPC) and cardiac troponin I (cTNI) are hypertrophic cardiomyopathy (HCM)-causing sarcomeric proteins which regulate contractility in response to PKA phosphorylation.</p> <p>Results</p> <p>During a yeast 2-hybrid (Y2H) library screen using a trisphosphorylation mimic of the C1-C2 region of cMyBPC, we identified isoform 4 of myomegalin (MMGL) as an interactor of this N-terminal cMyBPC region. As MMGL has previously been shown to interact with phosphodiesterase 4D, we speculated that it may be a PKA-anchoring protein (AKAP).</p> <p>To investigate this possibility, we assessed the ability of MMGL isoform 4 to interact with PKA regulatory subunits R1A and R2A using Y2H-based direct protein-protein interaction assays. Additionally, to further elucidate the function of MMGL, we used it as bait to screen a cardiac cDNA library. Other PKA targets, viz. CARP, COMMD4, ENO1, ENO3 and cTNI were identified as putative interactors, with cTNI being the most frequent interactor.</p> <p>We further assessed and confirmed these interactions by fluorescent 3D-co-localization in differentiated H9C2 cells as well as by <it>in vivo </it>co-immunoprecipitation. We also showed that quantitatively more interaction occurs between MMGL and cTNI under β-adrenergic stress. Moreover, siRNA-mediated knockdown of MMGL leads to reduction of cMyBPC levels under conditions of adrenergic stress, indicating that MMGL-assisted phosphorylation is requisite for protection of cMyBPC against proteolytic cleavage.</p> <p>Conclusions</p> <p>This study ascribes a novel function to MMGL isoform 4: it meets all criteria for classification as an AKAP, and we show that is involved in the phosphorylation of cMyBPC as well as cTNI, hence MMGL is an important regulator of cardiac contractility. This has further implications for understanding the patho-aetiology of HCM-causing mutations in the genes encoding cMyBPC and cTNI, and raises the question of whether MMGL might itself be considered a candidate HCM-causing or modifying factor.</p

    Management of spinal cord injury-related pain using complementary and alternative medicine : a scoping review protocol

    OBJECTIVE : This scoping review will identify complementary and alternative medicine methods used to manage spinal cord injury–related pain. INTRODUCTION : Spinal cord injury–related pain is common, with a third of individuals experiencing severe pain. Conventional interventions are well documented, however, pain relief remains elusive for people with spinal cord injuries. Although complementary and alternative medicine is available to alleviate various health problems, little is known about the complementary and alternative medicine methods used to manage pain in people with spinal cord injuries. INCLUSION CRITERIA : This review will consider all studies on complementary and alternative medicine treatment methods used by adults with spinal cord injury to treat their associated pain. The concept of interest in this study is complementary and alternative medicine. Quantitative, qualitative, and mixed methods studies, text and opinion papers, as well as systematic reviews will be included in this review. METHODS : A three-step search strategy, consisting of an initial limited search, a full search, and a screening of the reference lists of all included articles will be undertaken. Key information sources to be searched include CINAHL, Cochrane Library, JBI Evidence Synthesis, MEDLINE, Scopus, Web of Science, DynaMed, Natural Medicines, HerbMed, Open Dissertations, and OpenGrey. All titles and abstracts of identified citations will be screened and then uploaded to a reference management program.
