    Using cognitive behaviour therapy in counselling non-adherent hypertensive patients: a nurse counsellor perspective

    Hypertension is a chronic illness that requires the patient to take medicine in the long-term. In hospitals, non-adherent hypertensive patients may be referred to a counsellor by his/her medical officer. Non-adherence is a multifactorial and multifaceted problem that needs intervention through a multidisciplinary health care approach. Counselling non-adherent patients can be a challenging task for a counsellor. This review discusses cognitive behavioural therapy as one of the counselling theories used in counselling hypertensive patients.     Control System Design course prepares students to design automatic systems, which is indispensable of the engineering curriculum. This paper intends to illustrate how the effective learning techniques of Lu Xun are adapted as activities in the teaching of the course. This is the first time that the content design and delivery of the course are analysed objectively. From a study population of 128 students, Activity 5-9 are found to be the most capable of engaging the students to use the 5-organs while learning the course.     A Phenomenological Investigation of Knowing Events Among Women Headteachers of Government Schools

    The main purpose of this study was to understand the nature of knowing for women headteachers in leadership and decision-making. The study was carried out through three research questions. They include: (1) what are the characteristics of knowing events for women headteachers as experienced by them? (2) what are the influences that shape women headteachers' knowing? (3) what are the specific ways of knowing that women headteachers engage in? These questions aimed to determine the characteristics, factors that influence, and structure of the knowing phenomenon among women headteachers in leadership and decision-making. The phenomenological approach was selected since it was deemed the most appropriate for the study of a phenomenon of this nature. Data were gathered from nine women headteachers in primary and secondary government schools. The participants were selected from excellent and successful women headteachers through purposeful sampling. The data collection methods employed were mainly in-depth interviews with the participants. Each interview was taped recorded, transcribed verbatim and analyzed. In addition, informal observation and review of accessible official documents were also carried out. The validity of the study was ensured through clarifying the researcher's biases at the outset of the study, triangulation, member checks, peer examination and rich, thick description of results. The reliability was furthered ensured through stating the researcher's position, triangulation and maintaining an audit trail. The findings yielded characteristics of knowing events as experienced by the women headteachers, influences that shape their knowing, ways of knowing women headteachers engaged in. The knowing events represent the phases that every woman headteacher in the study had passed through from accession or entry into headship to refocusing of values in the securely established state. The influences that shape knowing began in the formation years and occur throughout headship. In addition it also represents the development of knowing from a dependency on authorities to autonomy of self, knowledge as abstract and objective to being relative and subjective, and the cognitive process of receiving knowledge to one of generating knowledge. The study concludes that the knowing practiced among the women headteachers in leadership and decision making is unique to the participants of this study. The findings are conceptualized into a thematic portrayal of knowing. Specific recommendations for further research are also suggested

    The perception of nurses concerning computerised handover and its influence on the quality of patient care

    Background: Nursing handover communications play an important role in ensuring the quality of care of patients. Aim: To understand the perception of Malaysian nurses concerning computerised handover and its influence on the quality of patient care. Materials and Methods: Focus group discussions (FGDs) were formed to elicit multiple opinions on topics related to computerised nursing handover. Twelve registered nurses from a public hospital in Malaysia were recruited for three groups of FGD. Conventional qualitative content analysis was used to derive the codes and themes directly from the data. The NVivo 10 software package was used to assist in coding, categorising, and constructing themes. Result: Four main themes were identified: (1) decreased nursing workload, (2) increased nursing workload, (3) technical problems related to computer that reduced work efficiency, (4) unpleasant effect from computerised nursing handover that jeopardised patient safety. Conclusion: Computerised nursing handover decreased the workload of nurses in some aspects, but also increased it due to duplicate charting. Technical problems related to computer hindered the smoothness of running a computerised nursing handover. The computerised nursing handover compromised the quality of care of patients when nurses copy and pasted patient clinical information carelessly, which exposed nurses to legal and/or ethical risk. Additionally, the prolonged use of computerised structured contents reduced nurses’ critical thinking skills in constructing and planning patient care. Nursing leadership is required to supervise the administrative process of the computerised nursing handover. Nursing continual education on patient care should be designed periodically on the ward to improve the critical thinking of nurses

    A NOVEL CLASS OF IMMUNOPROTEASOME CATALYTIC SUBUNIT LMP2 INHIBITOR AND ITS THERAPEUTIC POTENTIALS IN CANCER

    The immunoproteasome, known to play an important role in MHC class I antigen processing and presentation, have been linked to neurodegenerative diseases and hematological cancers. However, the pathophysiological functions of the immunoproteasome in these diseases are still not very well established. This can be attributed mainly to the lack of appropriate molecular probes that selectively target the immunoproteasome catalytic subunits. Herein, we report the development of a small molecular inhibitor (AM) that selectively targets the major catalytic subunit, LMP2, of the immunoproteasome. We show that the compound covalently modifies the LMP2 subunit with high specificity in human prostate cancer cell. AM was also shown to selectively inhibit the chymotrypsin-like activity of LMP2 subunit. More importantly, the anti-proliferative activity of AM is more pronounced in prostate cancer cells that highly express LMP2 without inducing toxicity in normal cells. These results implicate an important role of LMP2 in regulating cell growth of malignant tumors that highly express LMP2. Subsequently, the modes of action of AM were investigated. Prostate cancer cells that highly express LMP were shown to induce G2/M cell cycle arrest and apoptosis via PARP cleavage when treated with the compound. Similar to epoxomicin, the treatment of AM induced the accumulation of poly-ubiquitination in prostate cancer cells, which indicates the inhibition of proteolysis. However, unlike epoxomicin, the treatment of AM did not appear to inhibit the activation of inflammation. In conclusion, these results suggest that the LMP2 inhibitor, AM, may induce cytotoxicity prostate cancer cells that highly express LMP2 catalytic subunit in similar modes of action as epoxomicin but it does not involve the inflammatory pathway

In addition, a method is provided for synthesizing inhibitors, which includes PI′-modified analogues.
