Post-operative wound infection in developing country - A prospective survey at the Q.E.C.H., Blantyre, Malawi

A prospective survey of post-operative wound infection rates was undertaken at the QueenElizabeth Central Hospital, Blantyre, Malawi over a three-month period from April to June, 1985. The methods whereby wound sepsis data were obtained are presented. The results show an overall infection rate of 25.8% and that for clean wounds of 14.8%. These figures are relatively unfavourable and an attempt is made to explain, this. The limitations of this study are discussed, as is the importance of continued sepsis surveillance with particular reference to the use of antibiotic prophylaxis

Surgical Observations on Atelectasis of the Middle Ear in Children

Atelectasis of the middle ear, synonymous with that of the lung, is a variable collapse of the middle ear space, associated with retraction pockets formed by extreme atrophy of the tympanic membrane. It is a common condition in the paediatric patient population presenting to the Sophia Children’s Hospital and though much controversy still surrounds the aetiology and progression, there is a clear association with the development of cholesteatoma. There is however almost always an associated otitis media with effusion (OME), though this may have cleared up by the time the patient is seen. Observations and careful follow-up of the patients, though not the optimal method for elucidating the progression, does, in the absence of a suitable animal model, allow cautious estimates of possible aetiological developments

Monitoring gesloten voermest kringlopen : achtergronddocument bij 'Kwalitatieve monitorsysteeminnovaties verduurzaming landbouw'

Dit onderzoek richt zich op de percepties over de ontwikkeling van de systeeminnovatie ‘Gesloten voer-mest kringlopen’ als één van de graadmeters voor de verduurzaming van de veehouderij. Hiertoe zijn interviews gehouden met respondenten uit beleid, onderzoek en ketens. Uit de interviews komt naar voren dat de ontwikkeling van de gesloten voer-mest kringlopen in de voorontwikkelingsfase zit, èn dat een grote diversiteit omtrent de percepties heerst. Een belangrijk knelpunt is de economische haalbaarheid van mestverwerking. Als belangrijkste drijvende krachten achter het veranderingsproces worden genoemd de toenemende fosfaatschaarste op lange termijn, alsmede toenemende energieprijzen. Verder gaven respondenten aan dat voor het kunnen beoordelen van de vooruitgang meer feiten en cijfers beschikbaar zouden moeten zijn

Recreant, patiënt en leerling; de stedeling aan het woord over het Land van Wijk en Wouden

In 1999 hebben enkele boeren en burgers uit het Land van Wijk en Wouden aandacht gevraagd voor de ontwikkelingen in het gebied. Dit heeft geresulteerd in het ontstaan van een gebiedscommissie. De gebiedscommissie heeft in april 2001 een integrale gebiedsvisie gepresenteerd. In het vervolg op de gebiedsvisie wordt gewerkt aan een uitvoeringsprogramma. In opdracht van de provincie Zuid-Holland is onderzoek gedaan in het Land van Wijk en Wouden bij bezoekers uit de stad en de inwoners van het gebied naar de wensen als het gaat om recreatie en ontspanning. Ook is aan zorg- en onderwijsinstellingen gevraagd in hoeverre zij geïnteresseerd zijn in producten en diensten afkomstig uit het Land van Wijk en Wouden

Variable Growth Across Species and Life Stages in Caribbean Reef Octocorals

Growth rates often measured as linear extension rates can serve as a proxy for organismal performance across environments, yet few estimates of these growth rates exist, and fewer are reported from multi-year monitoring. We studied the effect of species, depth, exposure, and life stages on the linear extension rates of common octocorals inhabiting Caribbean reefs. We monitored 16 species from the families Briareidae, Gorgoniidae, and Plexauridae, including eight genera: Antillogorgia, Briareum, Eunicea, Gorgonia, Muricea, Muriceopsis, Plexaura, and Plexaurella. We surveyed over 2,000 colonies across eight reefs in Southwest Puerto Rico from 2003 to 2006. Our surveys include reefs along a gradient of water motion from inside protected reefs to offshore unprotected reefs and from shallow (5 m) forereefs to deeper environments reaching 25 m. We observed that linear extension rates vary across species and largely follow branch thickness variation and colony architecture, with thin plume-like shapes growing faster. We also found that in 7 out of the 16 species, colonies from shallow habitats with brighter and warmer environments have higher growth rates than colonies in deep, low energy, and darker areas. Importantly, small/juvenile corals of most species (\u3c10 cm) grew faster than adult colonies. Our data highlight the possibility that, as climate change continues to deteriorate Caribbean reefs by decimating scleractinian corals and opening new habitat for less vulnerable species, plume-like corals with fast growth rates such as species within the genus Antillogorgia may become the most abundant benthic cnidarians on Caribbean shallow forereefs

Ward Round-A rare tumor of the kidney resulting in hypertension, renal failure and a cerebrovascular accident in a young female

A 17-year-old girl was admitted to our hospital with severe refractory hypertension evolving over approximately 4 years. Despite not having the resources to identify plasma-renin levels and using standard imaging techniques, a juxtaglomerular cell tumor was suspected and was histologically confirmed after surgical excision. This is a potentially lethal condition if left untreated and surgical excision is curative. The benign nature of the tumor is emphasized and its chemical, radiological and microscopic appearance discussed according to the literature. To the best of our knowledge, this is the first reported case of a patient surviving a cerebrovascular accident associated with a juxtaglomerular cell tumor
