Problématique clinique pour la psychose

La cure analytique d'un psychotique suppose une problématique globale sur les causes de la psychose, sa structure, son déclenchement et les étapes de son développement. La psychose est ici envisagée dans sa dimension proprement psychique où la théorie lacanienne, à laquelle l'auteur se rattache, situe la cause de la maladie : la forclusion des Noms-du-père. Cette formule indique l'absence de la fonction paternelle. Ce manque s'origine dans le fait qu'un dysfonctionnement de l'ordre symbolique dans la structure de la famille préside à la naissance d'un sujet. Le repérage des éléments (les signifiants) d'un tel dysfonctionnement du symbolique est essentiel puisque le délire cherche à réparer ce défaut dans l'ordre de la représentation. Contre ce défaut, le sujet a d'abord construit des montages imaginaires, qui lui permettent une articulation provisoire et satisfaisante pour lui, à autrui et à la réalité sociale. Le déclenchement de la psychose va entraîner l'écroulement de tels échafaudages imaginaires. Puis après un temps de sidération et de retrait psychique, tout l'effort du psychotique va être la reconstitution d'un nouvel ordre de représentation. Le travail de l'analyse consiste à partir des signifiants mis en scène par le délire de tenter d'assister le psychotique dans sa restitution de l'ordre symbolique. Le délire est la voie d'accès à ce travail dans la psychose, comme le rêve est la voie royale vers l'inconscient dans la névrose. The work of the analysis consistants of attempting, on the basis of the signifiers put forward by the delusion, to assist the psychotic in his reconstitution of symbolic order. The delusion is the access road to this work in psychosis just as the dream is the royal road to the unconscious in the neurosis. More than ever we have to think of health and mental health in particular in terms of a social dynamics where the participation of social groups and individuals in the responsibility for collective health has priority over the structures of state and institutional interventions. The responsibilities of the state, the institutions and professionals are therefore displaced and redefined while new rights emerge and with them the need for more information and control for the users who pay for health services with their taxes. The concern to adapt a system now anachronistic can only increases the problems of a society responsible for its obsolescence. The social and human costs of the radical changes needed, will in the short term, be socially less burdensome than the consequences of illusory adaptations. In this area, we can expect that nothing will be effective without the mobilisation by the state of the collective responsabilities for a social involvement in public health
