    Haematopoietic stem cell transplantation and oral complications

    De ontwikkeling van nieuwe procedures heeft ertoe geleid dat een hematopoëtische stamceltransplantatie tegenwoordig ook kan worden toegepast bij patiënten die vroeger niet hiervoor in aanmerking kwamen, zoals ouderen. Tevens leiden deze ontwikkelingen tot verschuivingen in het spectrum van complicaties als gevolg van hematopoëtische stamceltransplantatie. In dit artikel komen de belangrijkste principes van hematopoëtische stamceltransplantatie aan de orde en de verschillende orale complicaties die hierbij kunnen optreden: mucositis, infecties, bloedingen, graft-versus-hostziekte, xerostomie, hyposialie, smaakverandering, secundaire tumoren, osteoporose, osteonecrose en groei- en ontwikkelingsstoornissen. Tot slot wordt aandacht besteed aan de rol van mondzorgverleners bij een hematopoëtische stamceltransplantatie.New haematopoietic stem cell transplantation procedures make the treatment available to patients who previously did not qualify, such as the elderly. This article is a review of the main principles of haematopoietic stem cell transplantation and provides information on oral complications which may develop, such as mucositis, infections, bleeding, graft-versus-host disease, xerostomia, hyposalivation, altered taste, secondary tumors, osteoporosis, osteonecrosis and growing and developing disturbancies. Finally, the role of dental care providers in cases of haematopoietic stem cell transplantation is addressed.</p

    Persistent hematologic and immunologic disturbances in 8-year-old Dutch children associated with perinatal dioxin exposure.

    Perinatal exposure to Dutch "background" dioxin levels in 1990 was high, but comparable with that of other industrialized Western European countries. Exposure during the sensitive perinatal period may cause permanent disturbances. Therefore, we assessed the health status and various hematologic and immunologic parameters among our longitudinal cohort. A medical history was taken and venipuncture performed in a longitudinal cohort of 27 healthy 8-year-old children who had documented perinatal dioxin exposure. Linear regression revealed a decrease in allergy in relation to prenatal (p = 0.02) and postnatal (p = 0.03) dioxin exposure. Increases in CD4+ T-helper cells (p = 0.006) and in CD45RA+ cells (p = 0.02) were seen in relation to postnatal exposure. A persistently decreased platelet count (p = 0.04) and increased thrombopoietin concentration (p = 0.03) were seen in relation to postnatal exposure. This follow-up has shown a decrease in allergy, persistently decreased thrombocytes, increased thrombopoietin, and increased CD4+ T-helper and increased CD45RA+ cell counts. This study provides indications of effects at the stem cell level of perinatal dioxin exposure, persisting until minimally 8 years after birth

    Archival Film: New Opportunities for Case Study Development and Presentation?

    The potential opportunities and limitations of utilising archival film as a primary data source have received very little attention from business historians. Archival film can be a rich source of oral and visual material for the development and presentation of historical case study material, but it can also be utilised as a powerful research tool. The paper draws on the experiences of the author, who produced two films during a study of the history of the South Coast Labour Council (SCLC). The SCLC is the peak union body for the Illawarra region of NSW. During the study access to one of the region’s local television newsreel archives provided a rare opportunity to work with primary data that significantly extended the range of possibilities for rich case study development and presentation. The resulting artefacts included 1) a 15 minutes documentary on the 75 year history of the SCLC and; 2) a two hour set of selected historical excerpts. The presentation explores first, a range of essential processes that require consideration when working with this form of data. Issues explored include: 1) access, 2) equipment and 3) production processes. Second, the paper explores a range of research methods that allowed a deeper exploration of the history of the organisation post production.
