KM3NeT/ARCA expectations in view of a novel multimessenger study of starburst galaxies
Starburst galaxies (SBGs) and more in general star-forming galaxies represent a class of galaxies with a high star formation rate (up to 100MoË™/year). Despite their low luminosity, they can be considered as guaranteed "factories"of high energy neutrinos, being "reservoirs"of accelerated cosmic rays and hosting a high density target gas in the central region. In this contribution we present a novel multimessenger study of these sources and the possibility of observing their neutrino signals with the KM3NeT/ARCA telescope. The differential sensitivity for different SBG scenarios is reported considering track-like neutrino events in the 100 GeV-100 PeV energy range
Architecture and performance of the KM3NeT front-end firmware
The KM3NeT infrastructure consists of two deep-sea neutrino telescopes being deployed in the Mediterranean Sea. The telescopes will detect extraterrestrial and atmospheric neutrinos by means of the incident photons induced by the passage of relativistic charged particles through the seawater as a consequence of a neutrino interaction. The telescopes are configured in a three-dimensional grid of digital optical modules, each hosting 31 photomultipliers. The photomultiplier signals produced by the incident Cherenkov photons are converted into digital information consisting of the integrated pulse duration and the time at which it surpasses a chosen threshold. The digitization is done by means of time to digital converters (TDCs) embedded in the field programmable gate array of the central logic board. Subsequently, a state machine formats the acquired data for its transmission to shore. We present the architecture and performance of the front-end firmware consisting of the TDCs and the state machine.</p
The KM3NeT multi-PMT optical module
The optical module of the KM3NeT neutrino telescope is an innovative multi-faceted large area photodetection module. It contains 31 three-inch photomultiplier tubes in a single 0.44 m diameter pressure-resistant glass sphere. The module is a sensory device also comprising calibration instruments and electronics for power, readout and data acquisition. It is capped with a breakout-box with electronics for connection to an electro-optical cable for power and long-distance communication to the onshore control station. The design of the module was qualified for the first time in the deep sea in 2013. Since then, the technology has been further improved to meet requirements of scalability, cost-effectiveness and high reliability. The module features a sub-nanosecond timing accuracy and a dynamic range allowing the measurement of a single photon up to a cascade of thousands of photons, suited for the measurement of the Cherenkov radiation induced in water by secondary particles from interactions of neutrinos with energies in the range of GeV to PeV. A distributed production model has been implemented for the delivery of more than 6000 modules in the coming few years with an average production rate of more than 100 modules per month. In this paper a review is presented of the design of the multi-PMT KM3NeT optical module with a proven effective background suppression and signal recognition and sensitivity to the incoming direction of photons

Health–economic Benefits of Treating Trauma in Psychosis
Background: Co-occurrence of posttraumatic stress disorder (PTSD) in psychosis (estimated as 12%) raises personal suffering and societal costs. Health–economic studies on PTSD treatments in patients with a diagnosis of a psychotic disorder have not yet been conducted, but are needed for guideline development and implementation. This study aims to analyse the cost-effectiveness of guideline PTSD therapies in patients with a psychotic disorder.
Methods: This health–economic evaluation alongside a randomized controlled trial included 155 patients with a psychotic disorder in care as usual (CAU), with comorbid PTSD. Participants received eye movement desensitization and reprocessing (EMDR) (n = 55), prolonged exposure (PE) (n = 53) or waiting list (WL) (n = 47) with masked assessments at baseline (T0) and at the two-month (post-treatment, T2) and six-month follow-up (T6). Costs were calculated using the TiC-P interview for assessing healthcare consumption and productivity losses. Incremental cost-effectiveness ratios and economic acceptability were calculated for quality-adjusted life years (EQ-5D-3L-based QALYs) and PTSD ‘Loss of diagnosis’ (LoD, CAPS).
Results: Compared to WL, costs were lower in EMDR (-€1410) and PE (-€501) per patient per six months. In addition, EMDR (robust SE 0.024, t = 2.14, p = .035) and PE (robust SE 0.024, t = 2.14, p = .035) yielded a 0.052 and 0.051 incremental QALY gain, respectively, as well as 26% greater probability for LoD following EMDR (robust SE = 0.096, z = 2.66, p = .008) and 22% following PE (robust SE 0.098, z = 2.28, p = .023). Acceptability curves indicate high probabilities of PTSD treatments being the better economic choice. Sensitivity analyses corroborated these outcomes.
Conclusion: Adding PTSD treatment to CAU for individuals with psychosis and PTSD seem to yield better health and less PTSD at lower costs, which argues for implementation

Prospective Evaluation of Sleep Apnea as Manifestation of Heart Failure in Children
In adults with heart failure, central sleep apnea (CSA), often manifested as Cheyne–Stokes respiration, is common, and has been associated with adverse outcome. Heart failure in children is commonly caused by dilated cardiomyopathy (DCM). It is unknown whether children with heart failure secondary to DCM have CSA, and whether CSA is related to the severity of heart failure. In this prospective observational study, 37 patients (<18 year) with heart failure secondary to DCM were included. They underwent polysomnography, clinical and laboratory evaluation and echocardiographic assessment. After a median follow-up time of 2 years, eight patients underwent heart transplantation. CSA (apnea–hypopnea index [AHI] ≥1) was found in 19 % of the patients. AHI ranged from 1.2 to 4.5/h. The occurrence of CSA was not related to the severity of heart failure. Three older patients showed a breathing pattern mimicking Cheyne–Stokes respiration, two of whom required heart transplantation. CSA was found in 19 % of the children with heart failure secondary to DCM. No relation was found with the severity of heart failure. Identification and Validation Model for Informative Liquid Biopsy-Based microRNA Biomarkers:Insights from Germ Cell Tumor In Vitro, In Vivo and Patient-Derived Data
Liquid biopsy-based biomarkers, such as microRNAs, represent valuable tools for patient management, but often do not make it to integration in the clinic. We aim to explore issues impeding this transition, in the setting of germ cell tumors, for which novel biomarkers are needed. We describe a model for identifying and validating clinically relevant microRNAs for germ cell tumor patients, using both in vitro, in vivo (mouse model) and patient-derived data. Initial wide screening of candidate microRNAs is performed, followed by targeted profiling of potentially relevant biomarkers. We demonstrate the relevance of appropriate (negative) controls, experimental conditions (proliferation), and issues related to sample origin (serum, plasma, cerebral spinal fluid) and pre-analytical variables (hemolysis, contaminants, temperature), all of which could interfere with liquid biopsy-based studies and their conclusions. Finally, we show the value of our identification model in a specific scenario, contradicting the presumed role of miR-375 as marker of teratoma histology in liquid biopsy setting. Our findings indicate other putative microRNAs (miR-885-5p, miR-448 and miR-197-3p) fulfilling this clinical need. The identification model is informative to identify the best candidate microRNAs to pursue in a clinical setting

water-Cherenkov telescopes for the detection of cosmic neutrinos. The neutrino telescopes will consist of large
volume three-dimensional grids of optical modules to detect the Cherenkov light from charged particles produced
by neutrino-induced interactions. Each optical module houses 31 3-in. photomultiplier tubes, instrumentation for
calibration of the photomultiplier signal and positioning of the optical module, and all associated electronics
boards. By design, the total electrical power consumption of an optical module has been capped at seven
Watts. We present an overview of the front-end and readout electronics system inside the optical module, which
has been designed for a 1-ns synchronization between the clocks of all optical modules in the grid during a life
